This article offers a comprehensive approach to the health of lesbian, gay, bisexual and transgender (LGBT) people, where respect for diversity and non judgemental care play a central role. It calls for a health and medical vision that goes beyond HIV risk. For those who never had to question their own sexual orientation or gender identity, it is certainly difficult to understand how the discovery of one's identity trait in childhood or early adolescence can be transformed under social pressure into a burden which often remains invisible but is associated with considerable emotional and medical morbidity. This article raises the following question: How many LGBT patients go unnoticed every week, leaving the physician's office without an opportunity to receive appropriate listening, support and care?

Cet article propose une approche globale de la santé des personnes lesbiennes, gays, bisexuelles, et transgenres (LGBT), où respect des singularités et non-jugement occupent une position centrale. Il invite à dépasser une vision centrée sur les risques liés au VIH. Pour qui n'a pas connu de questionnement en lien avec son orientation sexuelle ou son identité de genre, il est difficile de concevoir comment la découverte d'une caractéristique identitaire pendant l'enfance peut se transformer sous le regard des autres en un fardeau souvent invisible mais fréquemment associé avec une morbidité émotionnelle et médicale considérable. Cet article pose la question suivante : combien de personnes LGBT ressortent chaque semaine d'une consultation médicale sans avoir eu l'opportunité de bénéficier d'une écoute, d'un soutien et de soins adaptés ? On estime à un(e) sur dix les élèves gays, lesbiennes, bisexuels(les), transgenres (LGBT) ou en questionnement sur leur orientation sexuelle ou leur identité de genre. Cette population, qui présente des vulnérabilités spécifiques en termes de santé, est cependant peu, voire pas visible au sein des établissements scolaires. Les infirmières scolaires en santé communautaire peuvent leur apporter écoute et soutien.

Contexte: L'intervention "Break The Chains" 2012 de l'OFSP a l'intention des gays visait à informer sur la primo-infection et a inciter a un test de dépistage VIH. Design: L'évaluation a recouru a un design Pre-Post avec 2 dispositifs: A) enquête par questionnaire Internet avec une vague avant la campagne et une après (transversal repete); B) 1) relevé centré sur le canton de Zurich du nombre de test effectues par des gays durant le premier semestre 2012 utilisant des données récoltées par Sentinella, par les centres de test utilisant l'application BerDa et auprès des praticiens de HIV-Prakt; et 2) recueil d'information par le questionnaire internet post-intervention des intentions d'effectuer un test VIH. Résultats: 366 HSH ont répondu au questionnaire de la phase pré-, et 964 à la phase post-intervention. 69.8% ont entendu parler de la campagne. Parmi les répondants ayant identifie le message, 48.6% se sont sentis concernes. Avant la campagne, 34.2% des répondants identifiaient les 4 symptômes de la primo-infection, 46.4% se sentaient bien informes et 79.2% connaissaient le délai minimum pour qu'un test informe sur une absence d'infection; après la campagne, ces proportions étaient 39.6%, 62.0% et 85.2%. 17.1% des répondants ont déclaré avoir fait/avoir l'intention de faire, un test VIH suite à la campagne. Le nombre de tests effectues par les répondants Internet montre effectivement un pic dans le 2e trimestre 2012; les données BerDa montrent une stabilité du nombre total de tests. Les données épidémiologiques montrent une augmentation de la proportion et du nombre d'infections récentes détectées durant ce 2e trimestre, mais une diminution des infections anciennes. Conclusions: L'intervention BTC a amélioré légèrement les connaissances relatives à la phase de primo-infection du VIH et a la nécessité d'effectuer un test VIH. Elle semble avoir incite le public cible à effectuer un test VIH qui n'aurait pas été réalisé autrement et a peut-être ainsi contribuer a la détection d'infections récentes.

Most research on sexual orientation and alcohol use in the United States has found higher rates of alcohol use and abuse among gay men and lesbians. Studies from other countries have found smaller or no differences between sexual minority and heterosexual women and men. The present study used general population survey data from 14 countries to examine high-volume and risky single-occasion drinking by sexual orientation. Data from 248 gay men and lesbians and 3720 heterosexuals were analyzed in a case-control design. In several countries partnered or recently partnered gay men and lesbians had no greater risk of heavy drinking or engaging in heavy drinking than heterosexual controls. Only lesbians in North America showed higher risk for both indicators. Future general population health research should include larger samples of gays and lesbians and use more comprehensive measures of sexual orientation for investigating the prevalence of health risk factors.

The present article contributes to the ongoing academic debate on migrants' appropriation of artistic and political spaces in Germany. Cologne, one of the largest cities in Germany, is an interesting example of the tension between political discourse centred around multiculturalism and cultural segregation processes. The 'no fool is illegal' carnival organised by asylum seekers shows their capacity to act, as they reinvent an old local tradition by reinterpreting medieval rituals. Today, different groups and associations appropriate this festive art space: migrants, gays and lesbians, feminists and far-left groups either organise their own parties or take part in the official parties and parades as separate groups. As a result, the celebration of diversity figures on the local political agenda and becomes part of the official carnival festivities. This leads to a blurring of boundaries, whereby mainstream popular culture becomes more and more influenced by multicultural elements.
