BACKGROUND: Cone-beam computed tomography (CBCT) image-guided radiotherapy (IGRT) systems are widely used tools to verify and correct the target position before each fraction, allowing to maximize treatment accuracy and precision. In this study, we evaluate automatic three-dimensional intensity-based rigid registration (RR) methods for prostate setup correction using CBCT scans and study the impact of rectal distension on registration quality. METHODS: We retrospectively analyzed 115 CBCT scans of 10 prostate patients. CT-to-CBCT registration was performed using (a) global RR, (b) bony RR, or (c) bony RR refined by a local prostate RR using the CT clinical target volume (CTV) expanded with 1-to-20-mm varying margins. After propagation of the manual CT contours, automatic CBCT contours were generated. For evaluation, a radiation oncologist manually delineated the CTV on the CBCT scans. The propagated and manual CBCT contours were compared using the Dice similarity and a measure based on the bidirectional local distance (BLD). We also conducted a blind visual assessment of the quality of the propagated segmentations. Moreover, we automatically quantified rectal distension between the CT and CBCT scans without using the manual CBCT contours and we investigated its correlation with the registration failures. To improve the registration quality, the air in the rectum was replaced with soft tissue using a filter. The results with and without filtering were compared. RESULTS: The statistical analysis of the Dice coefficients and the BLD values resulted in highly significant differences (p<10(-6)) for the 5-mm and 8-mm local RRs vs the global, bony and 1-mm local RRs. The 8-mm local RR provided the best compromise between accuracy and robustness (Dice median of 0.814 and 97% of success with filtering the air in the rectum). We observed that all failures were due to high rectal distension. Moreover, the visual assessment confirmed the superiority of the 8-mm local RR over the bony RR. CONCLUSION: The most successful CT-to-CBCT RR method proved to be the 8-mm local RR. We have shown the correlation between its registration failures and rectal distension. Furthermore, we have provided a simple (easily applicable in routine) and automatic method to quantify rectal distension and to predict registration failure using only the manual CT contours.

La tesi è imperniata sulle modalità di rappresentazione letteraria del tempo e délia memoria (dunque sulla relazione fra tempo e narratività) nella produzione narrativa ad ampio respiro délia scrittrice Giovanna Zangrandi.¦Il canone delle opere prese in esame comprende, in particolar modo, i romanzi I Brusaz (1954) e Orsola nelle stagioni (1957), la narrazione a carattere autobiografico de II campo rosso (1959) e il diario resistenziale Igiorni Veri (1963).¦Il percorso di indagine si è awalso di strumenti teorici e analitici interdisciplinari: alla critica e alla prassi letteraria è stata affiancata una strumentazione di matrice storico-filosofica, sociologica e, in misura minore, psicanalitica.¦La tesi si articola in diverse sezioni. Nella prima parte del lavoro vengono esplorati alcuni nessi fondanti del lavoro: il rapporto fra storia e memoria, la relazione fra memoria individuale e memoria collettiva, la dialettica fra ricordo e oblio e quella fra memoria e narrazione, ossia ira contenuti mnestici individuali e loro configurazione letteraria; successivamente la tesi si focalizza sul nesso individuabile, soprattutto nelle opere più scopertamente autobiografiche di Giovanna Zangrandi, tra testo letterario e meccanismi individuali del ricordo, analizzando la dialettica tra pulsione autobiografica e sua rielaborazione tematica, tra piani memoriali soggettivi e loro traduzione narrativa.¦Nell'ultima parte del percorso critico la tesi si sofferma, in particolare, sulle complesse relazioni instaurate daU'autrice, nel diario I giorni veri, fra dimensioni storico-memoriali, piani autobiografïci e sistemi letterari. A taie fine, il punto di riferimento teorico fondamentale è costituito dalla prospettiva linguististica di Harald Weinrich, applicata alio studio dei tempi verbali, in base all'assunto secondo il quale il tempo verbale non esprime necessariamente una dimensione cronologica, quanto piuttosto una condizione psicologica délia soggettività narrante.¦Soffermandosi su un aspetto flnora inesplorato nell'ambito délia produzione narrativa di Giovanna Zangrandi, il lavoro evidenzia come la dialettica memoria/oblio e la corrispondente dicotomia tra passato e présente tendano a trasformarsi nella direttrice tematica fondamentale - nonchè in uno dei tratti più interessanti e significativi - dell'opera zangrandiana.

A generic LC-MS approach for the absolute quantification of undigested peptides in plasma at mid-picomolar levels is described. Nine human peptides namely, brain natriuretic peptide (BNP), substance P (SubP), parathyroid hormone 1-34 (PTH), C-peptide, orexines A and B (Orex-A and -B), oxytocin (Oxy), gonadoliberin-1 (gonadothropin releasing-hormone or luteinizing hormone-releasing hormone, LHRH) and α-melanotropin (α-MSH) were targeted. Plasma samples were extracted via a 2-step procedure: protein precipitation using 1vol of acetonitrile followed by ultrafiltration of supernatants on membranes with a MW cut-off of 30 kDa. By applying a specific LC-MS setup, large volumes of filtrates (e.g., 2×750 μL) were injected and the peptides were trapped on a 1mm i.d.×10 mm length C8 column using a 10× on-line dilution. Then, the peptides were back-flushed and a second on-line dilution (2×) was applied during the transfer step. The refocalized peptides were resolved on a 0.3mm i.d. C18 analytical column. Extraction recovery, matrix effect and limits of detection were evaluated. Our comprehensive protocol demonstrates a simple and efficient sample preparation procedure followed by the analysis of peptides with limits of detection in the mid-picomolar range. This generic approach can be applied for the determination of most therapeutic peptides and possibly for endogenous peptides with latest state-of-the-art instruments.
