A new device for the analyses of nurses' satisfaction has been developed and validated on two types of general and intensive treatments at the University Hospital in Vaudois, Switzerland. A questionnaire has been elaborated for identifying the variables linked with characteristics of the nurse's work, as well as personal variables of the employer which could have an influence on the level of satisfaction. In identifying the sources of satisfaction and dissatisfaction, it has been possible to propose recommendations and corrective measures in order to improve the level of global satisfaction of the nursing team.

Ce rapport de recherche a été commandé par la Commission externe d'évaluation des politiques publiques du Canton de Genève (Suisse) dans le cadre d'une évaluation de la politique de formation de la police dans ce canton. L'étude montre que, d'une manière générale, les policiers/ères récemment assermenté-e-s jugent positivement leur formation, en particulier certains éléments du dispositif pédagogique (durée de la formation, charge de travail, formateurs/trices) et se sentent bien préparé-e-s à gérer les contacts avec leurs différents publics (en particulier les personnes agressives et les victimes de violences domestiques). Néanmoins, la plupart des policiers/ères interrogé-e-s perçoivent le travail effectué au sein des services comme étant éloigné des prescriptions scolaires. Ils estiment que ces dernières ne sont pas celles qui sont appliquées par leurs collègues expérimenté-e-s. Les matières physico-techniques sont, d'une manière générale, considérées comme plus utiles et plus importantes que les matières relationnelles. Dans cet ordre d'idée, un certain nombre de policiers/ères réclament moins de psychologie. Il s'agit cependant de la matière relationnelle à propos de laquelle les jugements sont les plus nuancés, en particulier à propos des savoirs permettant de gérer les personnes présentant des troubles mentaux, d'améliorer la communication et de gérer les conflits (au détriment de ceux visant à l'introspection). Les enseignements de police de proximité ont quant à eux tendance à être associés à une spécialisation propre à la gendarmerie et inutile pour les nouveaux/elles entrant-e-s, en particulier les inspecteurs/trices. L'éthique et les droits de l'Homme concentrent par contre les jugements négatifs : ces cours sont perçus comme diffusant des « bonnes paroles » prêchées par des formateurs/trices extérieurs à la profession (si ce n'est réellement, du moins symboliquement), éloignées de la réalité du métier et échouant à combattre les préjugés, en particulier ceux basés sur l'origine ethnico-nationale. L'étude montre en outre que les compétences relationnelles et les rapports aux différents publics sont centrales dans les appréciations portées sur l'adéquation entre formation et pratique. À ce sujet, ont été repérés néanmoins des points qui posent problèmes (aux yeux des policiers/ères interrogé-e-s ou à nos yeux) : peu de réflexivité en ce qui concerne les risques associés au «délit de faciès», problèmes avec les « maghrébins » et les jeunes en groupe (liés en particulier à la remise en cause de l'autorité policière), imprévisibilité des consommateurs/trices de drogue et vision pessimiste (parfois très peu empathique) de la toxicomanie, sentiment de difficulté et d'inconfort associé aux tâches visant à prendre en charge la souffrance d'autrui, relégation de la plupart des situations de violence domestique au rang de tâches indues, difficultés associées aux contacts avec les personnes atteintes de maladies psychiques graves, et la mention en priorité des cours de TTI pour la gestion des personnes agressives (au détriment des matières relationnelles).

Aberrant blood vessels enable tumor growth, provide a barrier to immune infiltration, and serve as a source of protumorigenic signals. Targeting tumor blood vessels for destruction, or tumor vascular disruption therapy, can therefore provide significant therapeutic benefit. Here, we describe the ability of chimeric antigen receptor (CAR)-bearing T cells to recognize human prostate-specific membrane antigen (hPSMA) on endothelial targets in vitro as well as in vivo. CAR T cells were generated using the anti-PSMA scFv, J591, and the intracellular signaling domains: CD3ζ, CD28, and/or CD137/4-1BB. We found that all anti-hPSMA CAR T cells recognized and eliminated PSMA(+) endothelial targets in vitro, regardless of the signaling domain. T cells bearing the third-generation anti-hPSMA CAR, P28BBζ, were able to recognize and kill primary human endothelial cells isolated from gynecologic cancers. In addition, the P28BBζ CAR T cells mediated regression of hPSMA-expressing vascular neoplasms in mice. Finally, in murine models of ovarian cancers populated by murine vessels expressing hPSMA, the P28BBζ CAR T cells were able to ablate PSMA(+) vessels, cause secondary depletion of tumor cells, and reduce tumor burden. Taken together, these results provide a strong rationale for the use of CAR T cells as agents of tumor vascular disruption, specifically those targeting PSMA. The aim of this retrospective study was to compare the clinical and radiographic results after TKA (PFC, DePuy), performed either by computer assisted navigation (CAS, Brainlab, Johnson&Johnson) or by conventional means. Material and methods: Between May and December 2006 we reviewed 36 conventional TKA performed between 2002 and 2003 (group A) and 37 navigated TKA performed between 2005 and 2006 (group B) by the same experienced surgeon. The mean age in group A was 74 years (range 62-90) and 73 (range 58-85) in group B with a similar age distribution. The preoperative mechanical axes in group A ranged from -13° varus to +13° valgus (mean absolute deviation 6.83°, SD 3.86), in group B from -13° to +16° (mean absolute deviation 5.35, SD 4.29). Patients with a previous tibial osteotomy or revision arthroplasty were excluded from the study. Examination was done by an experienced orthopedic resident independent of the surgeon. All patients had pre- and postoperative long standing radiographs. The IKSS and the WOMAC were utilized to determine the clinical outcome. Patient's degree of satisfaction was assessed on a visual analogous scale (VAS). Results: 32 of the 37 navigated TKAs (86,5%) showed a postoperative mechanical axis within the limits of 3 degrees of valgus or varus deviation compared to only 24 (66%) of the 36 standard TKAs. This difference was significant (p = 0.045). The mean absolute deviation from neutral axis was 3.00° (range -5° to +9°, SD: 1.75) in group A in comparison to 1.54° (range -5° to +4°, SD: 1.41) in group B with a highly significant difference (p = 0.000). Furthermore, both groups showed a significant postoperative improvement of their mean IKSS-values (group A: 89 preoperative to 169 postoperative, group B 88 to 176) without a significant difference between the two groups. Neither the WOMAC nor the patient's degree of satisfaction - as assessed by VAS - showed significant differences. Operation time was significantly higher in group B (mean 119.9 min.) than in group A (mean 99.6 min., p <0.000). Conclusion: Our study showed consistent significant improvement of postoperative frontal alignment in TKA by computer assisted navigation (CAS) compared to standard methods, even in the hands of a surgeon well experienced in standard TKA implantation. However, the follow-up time of this study was not long enough to judge differences in clinical outcome. Thus, the relevance of computer navigation for clinical outcome and survival of TKA remains to be proved in long term studies to justify the longer operation time. References 1 Stulberg SD. Rapport de synthèse Enjeux et contexte: L'hyponatrémie est un trouble électrolytique fréquent et associé à un pronostic défavorable dans de nombreuses affections card iovascu lai res (1-5), pour lesquelles il est un marqueur de l'activation neurohumorale (6). Sa valeur pronostique chez les patients se présentant avec une emboîie pulmonaire était jusque là inconnue ; elle fait l'objet de la présente étude.Objectifs: Examiner chez les patients hospitalisés pour une embolie pulmonaire, les associations entre hyponatrémie et mortalité ainsi qu'avec le taux de réhospitalisation. Méthodes: Nous avons étudié les données de 13728 patients avec un diagnostic principal d'embolie pulmonaire provenant de 185 hôpitaux en Pennsylvanie (janvier 2000 à novembre 2002.) Nous avons utilisé un modèle de régression logistique afin d'établir l'association indépendante entre le niveau de sodium lors de la présentation aux urgences et la mortalité ainsi que le taux de ^hospitalisation durant 30 jours. Nous avons ajusté pour les caractéristiques du patient (race, assurance, sévérité de la maladie, usage de la thrombolyse) et de l'hôpital (région, taille, avec ou sans médecins en formation.)Résultats principaux: Une hyponatrémie (sodium £ 135 mmol/l) était présente chez 2907 patients (21.1%). Les patients avec un sodium >135, 130-135, et <130 mmol/l avaient une mortalité cumulée à 30 jours de 8.0%, 13.6%, et 28.5% (P <0.001), et un taux de réadmission de 11.8%, 15.6%, et 19.3% (P <0.001), respectivement. Comparés aux patients avec un sodium >135 mmol/l, les odd ratios ajustés concernant la mortalité étaient significativement plus important pour les patients avec un sodium compris entre 130 et 135 mmol/l (OR 1.53, 95% Cl: 1.33-1.76) ou <130 mmol/l (OR 3.26, 95% Cl: 2.48-4.29). Les odd ratios ajustés concernant la réhospitalisation étaient également augmentés pour les patients présentant un sodium entre 130 et 135 mmol/l (OR 1.28, 95% Cl: 1.12-1.46) ou <130 mmol/l (OR 1.44, 95% Cl: 1.02-2.02). Conclusions et perspectives: L'hyponatrémie est fréquente chez les patients se présentant avec une embolie pulmonaire, de plus elle est un prédicateur indépendant de la mortalité à court terme, ainsi que du taux de réhospitalisation. La natrémie est une information généralement disponible lors de l'établissement d'un pronostic. Bien que cette association soit compatible avec une activation neurohumorale, nous ne pouvons pas attester des mécanismes impliqués, du fait que notre étude ne donne pas d'informations sur d'autres étapes de la physiologie de cette association.

PURPOSE: Platelet-derived growth factor receptor-alpha (PDGFRA) mutations are found in approximately 5% to 7% of advanced gastrointestinal stromal tumors (GIST). We sought to extensively assess the activity of imatinib in this subgroup. EXPERIMENTAL DESIGN: We conducted an international survey among GIST referral centers to collect clinical data on patients with advanced PDGFRA-mutant GISTs treated with imatinib for advanced disease. RESULTS: Fifty-eight patients were included, 34 were male (59%), and median age at treatment initiation was 61 (range, 19-83) years. The primary tumor was gastric in 40 cases (69%). Thirty-two patients (55%) had PDGFRA-D842V substitutions whereas 17 (29%) had mutations affecting other codons of exon 18, and nine patients (16%) had mutation in other exons. Fifty-seven patients were evaluable for response, two (4%) had a complete response, eight (14%) had a partial response, and 23 (40%) had stable disease. None of 31 evaluable patients with D842V substitution had a response, whereas 21 of 31 (68%) had progression as their best response. Median progression-free survival was 2.8 [95% confidence interval (CI), 2.6-3.2] months for patients with D842V substitution and 28.5 months (95% CI, 5.4-51.6) for patients with other PDGFRA mutations. With 46 months of follow-up, median overall survival was 14.7 months for patients with D842V substitutions and was not reached for patients with non-D842V mutations. CONCLUSIONS: This study is the largest reported to date on patients with advanced PDGFRA-mutant GISTs treated with imatinib. Our data confirm that imatinib has little efficacy in the subgroup of patients with D842V substitution in exon 18, whereas other mutations appear to be sensitive to imatinib. The interest in reactive electrophile species (RES) stems largely from the fact that they can have powerful biological activities. RES stimulate the expression of cell survival genes as well many other genes commonly upregulated in environmental stress and pathogenesis. RES levels must be carefully controlled in healthy cells but their formation and destruction during stress is of great interest. Unlike many 'classical' signals and hormones, RES can potentially affect gene expression at all levels by chemically reacting with nucleic acids, proteins and small molecules as well as by indirectly lowering pools of cellular reductants. Recent works involving genetic approaches have begun to provide compelling evidence that, although excess RES production can lead to cell damage, lower levels of RES may modulate the expression of cell survival genes and may actually contribute to survival during severe stress.

PURPOSE: Tumor-associated TIE-2-expressing monocytes (TEM) are highly proangiogenic cells critical for tumor vascularization. We previously showed that, in human breast cancer, TIE-2 and VEGFR pathways control proangiogenic activity of TEMs. Here, we examine the contribution of these pathways to immunosuppressive activity of TEMs. EXPERIMENTAL DESIGN: We investigated the changes in immunosuppressive activity of TEMs and gene expression in response to specific kinase inhibitors of TIE-2 and VEGFR. The ability of tumor TEMs to suppress tumor-specific T-cell response mediated by tumor dendritic cells (DC) was measured in vitro. Characterization of TEM and DC phenotype in addition to their interaction with T cells was done using confocal microscopic images analysis of breast carcinomas. RESULTS: TEMs from breast tumors are able to suppress tumor-specific immune responses. Importantly, proangiogenic and suppressive functions of TEMs are similarly driven by TIE-2 and VEGFR kinase activity. Furthermore, we show that tumor TEMs can function as antigen-presenting cells and elicit a weak proliferation of T cells. Blocking TIE-2 and VEGFR kinase activity induced TEMs to change their phenotype into cells with features of myeloid dendritic cells. We show that immunosuppressive activity of TEMs is associated with high CD86 surface expression and extensive engagement of T regulatory cells in breast tumors. TIE-2 and VEGFR kinase activity was also necessary to maintain high CD86 surface expression levels and to convert T cells into regulatory cells. CONCLUSIONS: These results suggest that TEMs are plastic cells that can be reverted from suppressive, proangiogenic cells into cells that are able to mediate an antitumoral immune response. Antibody-drug conjugates (ADC) are emerging as powerful treatment strategies with outstanding target-specificity and high therapeutic activity in patients with cancer. Brentuximab vedotin represents a first-in-class ADC directed against CD30(+) malignancies. We hypothesized that its sustained clinical responses could be related to the stimulation of an anticancer immune response. In this study, we demonstrate that the dolastatin family of microtubule inhibitors, from which the cytotoxic component of brentuximab vedotin is derived, comprises potent inducers of phenotypic and functional dendritic cell (DC) maturation. In addition to the direct cytotoxic effect on tumor cells, dolastatins efficiently promoted antigen uptake and migration of tumor-resident DCs to the tumor-draining lymph nodes. Exposure of murine and human DCs to dolastatins significantly increased their capacity to prime T cells. Underlining the requirement of an intact host immune system for the full therapeutic benefit of dolastatins, the antitumor effect was far less pronounced in immunocompromised mice. We observed substantial therapeutic synergies when combining dolastatins with tumor antigen-specific vaccination or blockade of the PD-1-PD-L1 and CTLA-4 coinhibitory pathways. Ultimately, treatment with ADCs using dolastatins induces DC homing and activates cellular antitumor immune responses in patients. Our data reveal a novel mechanism of action for dolastatins and provide a strong rationale for clinical treatment regimens combining dolastatin-based therapies, such as brentuximab vedotin, with immune-based therapies.
