Progresses in pediatric oncology over the last decades have been dramatic and allow current cure rates above 80%. There are mainly due to multicentre clinical trials aiming at optimizing chemotherapy protocols as well as local therapies in a stepwise approach. Most of the new anticancer drugs currently in development are based on targeted therapies, directed to specific targets present only in or on tumor cells, like growth factor receptors, mechanisms involved in proliferation, DNA repair, apoptosis, tumor invasion or angiogenesis. Concerning bone marrow transplantation also, new strategic approaches are in advanced development. They aim at reducing treatment induced toxicity and enhancing efficacy at the same time. This short paper would like to point out these new technologies, which should be known by the general practitioner.

Introduction : Il essentiel pour les patients, mais aussi pour l'utilisation appropriée des ressources médicales et financières, que les centres pédiatriques oncologiques de moindre envergure répondent de manière adéquate aux normes de soins et atteignent des résultats comparables à ceux des grands centres. Étant donné que l'impact économique des soins de santé est devenu un important modulateur des décisions politiques, il est urgent que les institutions abordent le sujet de la qualité en chirurgie pédiatrique oncologique, et fournissent des éléments pour appuyer ou modifier les lignes directrices existantes Patients et méthode : Par cette étude monocentrique rétrospective de 13 ans, nous avons analysé la qualité de la chirurgie pédiatrique de tumeurs abdominales ou thoraciques du Département Médico-Chirurgical du CHUV pratiquée sur 48 enfants. Afin d'y parvenir, nous avons choisi et analysé deux indicateurs de la qualité de ces soins. Le premier est la conformité de la pratique de ce service, avec les directives des protocoles de traitements internationaux. Le second paramètre étudié est la mesure du taux d'événements indésirables liés à la chirurgie du cancer observés dans notre population de patients. Résultats : La conformité avec les protocoles de traitement est très élevée avec un taux de 96%. L'incidence des événements indésirables liés à la chirurgie est de 34 patients sur 48 (66%). Les complications graves sont rares, avec 85% ne nécessitant pas plus qu'un traitement médical simple. Aucune mortalité chirurgicale n'a été identifiée. Conclusion : Nos données suggèrent que la planification méticuleuse de la chirurgie et l'exécution de celle-ci par des chirurgiens dédiés à l'oncologie pédiatrique, ainsi qu'une étroite collaboration avec des oncologues pédiatres sont des conditions essentielles à la qualité des soins apportés à ces patients. Dans ces conditions, même un centre d'oncologie pédiatrique de moindre envergure peut répondre aux normes établies par les grands centres internationaux.

Neuroblastoma (NB) is the most common extracranial malignant tumor in young children and arises at any site of the sympathetic nervous system. The disease exhibits a remarkable phenotypic diversity ranging from spontaneous regression to fatal disease. Poor outcome results from a rapidly progressive, metastatic and drug-resistant disease. Recent studies have suggested that solid tumors may arise from a minor population of cancer stem cells (CSCs) with stem cell markers and typical properties such as self-renewal ability, asymmetric division and drug resistance. In this model, CSCs possess the exclusive ability to initiate and maintain the tumor, and to produce distant metastases. Tumor cell subpopulations with stem-like phenotypes have indeed been identified in several cancer including leukemia, breast, brain and colon cancers. CSC hypothesis still needs to be validated in the other cancers including NB.NB originates from neural crest-derived malignant sympatho-adrenal cells. We have identified rare cells that express markers in conformity with neural crest stem cells and their derived lineages within primary NB tissue and cell lines, leading us to postulate the existence of CSCs in NB tumors.In the absence of specific markers to isolate CSCs, we adapted to NB tumor cells the sphere functional assay, based on the ability of stem cells to grow as spheres in non-adherent conditions. By serial passages of spheres from bone marrow NB metastases, a subset of cells was gradually selected and its specific gene expression profile identified by micro-array time-course analysis. The differentially expressed genes in spheres are enriched in genes implicated in development including CD133, ABC-transporters, WNT and NOTCH genes, identified in others solid cancers as CSCs markers, and other new markers, all referred by us as the Neurosphere Expression Profile (NEP). We confirmed the presence of a cell subpopulation expressing a combination of the NEP markers within a few primary NB samples.The tumorigenic potential of NB spheres was assayed by in vivo tumor growth analyses using orthotopic (adrenal glands) implantations of tumor cells into immune-compromised mice. Tumors derived from the sphere cells were significantly more frequent and were detected earlier compared to whole tumor cells. However, NB cells expressing the neurosphere-associated genes and isolated from the bulk tumors did not recapitulate the CSC-like phenotype in the orthotopic model. In addition, the NB sphere cells lost their higher tumorigenic potential when implanted in a subcutaneous heterotopic in vivo model.These results highlighted the complex behavior of CSC functions and led us to consider the stem-like NB cells as a dynamic and heterogeneous cell population influenced by microenvironment signals.Our approach identified for the first time candidate genes that may be associated with NB self-renewal and tumorigenicity and therefore would establish specific functional targets for more effective therapies in aggressive NB.

Le rétinoblastome représente 11% de tous les cancers apparaissant pendant la première année de vie. Pour éviter les effets secondaires de la chimiothérapie systémique ou de la radiothérapie externe, de nouveaux médicaments et de nouvelles techniques de traitement focalisé ont été développés. Des voies d'administration telles la voie périoculaire (topotécan) ou la voie artérielle ophtalmique directe (carboplatine) sont utilisées aujourd'hui dans certains cas résistants. La radiothérapie actuelle, appliquée exclusivement en deuxième intention, fait appel également à des techniques ciblées, permettant d'éviter les tissus sains et de réduire le risque de cancers radio-induits non oculaires. La radiothérapie stéréotaxique conformationnelle ou conformale et la proton-thérapie font ainsi partie du nouvel arsenal thérapeutique du rétinoblastome. Retinoblastoma represents 11% of all cancers during the first year of life. New drugs and focal treatments have been developed in order to avoid the side effects of systemic chemotherapy and external radiotherapy. New targeted and local administration strategies such as periocular chemotherapy (topotecan) or direct ophthalmic artery delivery (carboplatin), are already used today in selected resistant cases. Radiotherapy, presently indicated only as a second-line treatment, is also subject to new techniques, targeting tumors more closely to avoid involving healthy tissue and reduce the risk of radio-induced nonocular tumors. Stereotactic conformal radiotherapy and proton therapy may thus be included in the new range of treatment methods in retinoblastoma

AIMS OF THE STUDY: Analysis of indications and results of paediatric renal transplantation in a single centre, before and after the introduction of cyclosporine A (CSA). METHODS: Historical retrospective study. RESULTS: 19 transplantations were performed in 14 patients (5 second grafts) between 1971 and 1987 (group I). 13 patients were transplanted between 1988 and 1998 (no second transplant) (group II). In group II, all the patients had immunosuppression with CSA, but none in group I. Group II, with CSA, showed better renal survival than patients without CSA. In group I, obstructive uropathies (posterior urethral valves, pyelo-ureteral junction stenosis, vesico-ureteral reflux) represent a common cause (35%) of terminal chronic renal failure (TCRF), whereas in group II they represent only 15% of the causes and chronic glomerulonephritis is the most common cause (69%) of TCRF. Acute and chronic graft rejections were the cause of 9 and 1 graft losses in group I and II respectively. Living related donors account for 14% of all renal transplantations in group I and 46% in group II. CONCLUSIONS: The incidence of paediatric patients referred to Lausanne for TCRF is stable. We have observed a constant and steady decrease in obstructive uropathies leading to TCRF and renal transplantations, whereas glomerulonephritis are increasingly frequent. Graft survival has much improved since the introduction of cyclosporine A, without an increase in morbidity. In carefully selected cases, intrafamilial renal transplantation provides good results and helps to shorten the time spent on dialysis.

Chaque année en Suisse, 15000 personnes meurent du cancer, ce qui en fait la deuxième cause de mortalité [1]. Il est donc nécessaire d'introduire sur le marché de nouveaux traitements plus efficaces. Ces derniers doivent passer par une période d'essais cliniques qui comporte plusieurs phases, dont la phase I. Celle-ci sert principalement à déterminer la dose maximale tolérable d'un médicament, en exposant le patient à des doses croissantes. Déterminer les motivations des patients à participer à la phase I d'un essai clinique, alors que les bénéfices personnels sont relativement faibles [3].

Tumoral calcinosis (Ct) is a rare pathology of unknown origin.We present the clinical, imaging ( including CT and MRI), surgical and pathology findings of Ct in a 9 year-old boy who presented with an incidental finding of a large elbow mass. The MR aspect of Ct has been reported only once and this case is the second description in a child. The association of Ct and dermatomyositis, as reported hereby, has also been described only once. With respect to treatment alternatives, we believe that it is important for radiologists to recognise this rare pathology in pediatric patients.
