Les élevages intensifs d'animaux de rente, en particulier ceux de bovins et de porcins sont nombreux en France et sont concentrés dans certaines régions (Bretagne, Normandie, Massif central, Alpes, Pyrénées). Au total, on dénombre environ 20 millions de bovins répartis dans 280 000 exploitations et 25 millions de porcs répartis dans 30 000 exploitations. Ceci représente en moyenne 70 vaches/exploitation et 830 porcs/exploitation. De telles densités d'animaux réunis sur des surfaces de taille minimale, génèrent d'énormes quantités de déchets organiques, notamment de matières fécales qui contiennent une grande diversité de bactéries, pouvant parfois être pathogènes pour l'humain. Une partie de ces bactéries sont des bactéries gram négatif (entérobactéries) dont les parois contiennent des endotoxines (1). Ces endotoxines sont connues pour causer des problèmes respiratoires ou des problèmes toxiques (ODTS) (2), lorsqu'elles sont inhalées (Cole, Todd, Wing ; 2000). Jusqu'à présent, plusieurs études se sont attachées à évaluer l'exposition à ces bioaérosols (3) à l'intérieur des élevages d'animaux (vaches, chevaux, porcs, poules) et ont démontré la présence d'importantes concentrations de bactéries et d'endotoxines aéroportées. Cependant, très peu d'études ont estimé la dispersion de ces bioaérosols à l'extérieur des installations d'élevages. En 2008, une étude américaine avait caractérisé les bactéries aéroportées retrouvées dans,et autour, d'une douzaine d'exploitations porcines. Cette étude avait été discutée dans le cadre d'une note d'actualité scientifique précédente (BVS 9). Aujourd'hui, ces mêmes auteurs nous livrent des résultats concernant l'exposition aux endotoxines dans et autour de ces mêmes élevages de porcs (Ko et al., 2010). Une autre équipe de recherche chinoise a estimé la dispersion d'Escherichia coli dans l'environnement immédiat d'exploitations porcines (Yuan, Chai, Miao ; 2010). Cette bactérie, appelée aussi coliforme, fait partie de la flore intestinale normale de tous les animaux à sang chaud (mammifères et oiseaux). Sa mise en évidence dans certains milieux,notamment l'eau, est utilisée comme indicateur de contamination fécale. Finalement, une autre étude vient de paraître concernant les concentrations en endotoxines au cours de l'année, à proximité de stabulations ouvertes de vaches laitières (Dungan, Leytem, Bjorneberg ; 2010). BACKGROUND: Tracheal intubation may be more difficult in morbidly obese (MO) patients than in the non-obese. The aim of this study was to evaluate clinically if the use of the Video Intubation Unit (VIU), a video-optical intubation stylet, could improve the laryngoscopic view compared with the standard Macintosh laryngoscope in this specific population. METHODS: We studied 40 MO patients (body mass index >35 kg/m(2)) scheduled for bariatric surgery. Each patient had a conventional laryngoscopy and a VIU inspection. The laryngoscopic grades (LG) using the Cormack and Lehane scoring system were noted and compared. Thereafter, the patients were randomised to be intubated with one of the two techniques. In one group, the patients were intubated with the help of the VIU and in the control group, tracheal intubation was performed conventionally. The duration of intubation, as well as the minimal SpO(2) achieved during the procedure, were measured. RESULTS: Patient characteristics were similar in both groups. Seventeen patients had a direct LG of 2 or 3 (no patient had a grade of 4). Out of these 17 patients, the LG systematically improved with the VIU and always attained grade 1 (P<0.0001). The intubation time was shorter within the VIU group, but did not attain significance. There was no difference in the SpO(2) post-intubation. CONCLUSION: In MO patients, the use of the VIU significantly improves the visualisation of the larynx, thereby improving the intubation conditions.

BACKGROUND: Carotid artery stenosis is associated with the occurrence of acute and chronic ischemic lesions that increase with age in the elderly population. Diffusion Imaging and ADC mapping may be an appropriate method to investigate patients with chronic hypoperfusion consecutive to carotid stenosis. This non-invasive technique allows to investigate brain integrity and structure, in particular hypoperfusion induced by carotid stenosis diseases. The aim of this study was to evaluate the impact of a carotid stenosis on the parenchyma using ADC mapping. METHODS: Fifty-nine patients with symptomatic (33) and asymptomatic (26) carotid stenosis were recruited from our multidisciplinary consultation. Both groups demonstrated a similar degree of stenosis. All patients underwent MRI of the brain including diffusion-weighted MR imaging with ADC mapping. Regions of interest were defined in the anterior and posterior paraventricular regions both ipsilateral and contralateral to the stenosis (anterior circulation). The same analysis was performed for the thalamic and occipital regions (posterior circulation). RESULTS: ADC values of the affected vascular territory were significantly higher on the side of the stenosis in the periventricular anterior (P<0.001) and posterior (P<0.01) area. There was no difference between ipsilateral and contralateral ADC values in the thalamic and occipital regions. CONCLUSIONS: We have shown that carotid stenosis is associated with significantly higher ADC values in the anterior circulation, probably reflecting an impact of chronic hypoperfusion on the brain parenchyma in symptomatic and asymptomatic patients. This is consistent with previous data in the literature.

BACKGROUND: Endurance athletes are advised to optimize nutrition prior to races. Little is known about actual athletes' beliefs, knowledge and nutritional behaviour. We monitored nutritional behaviour of amateur ski-mountaineering athletes during 4 days prior to a major competition to compare it with official recommendations and with the athletes' beliefs. METHODS: Participants to the two routes of the 'Patrouille des Glaciers' were recruited (A, 26 km, ascent 1881 m, descent 2341 m, max altitude 3160 m; Z, 53 km, ascent 3994 m, descent 4090 m, max altitude 3650 m). Dietary intake diaries of 40 athletes (21 A, 19 Z) were analysed for energy, carbohydrate, fat, protein and liquid; ten were interviewed about their pre-race nutritional beliefs and behaviour. RESULTS: Despite belief that pre-race carbohydrate, energy and fluid intake should be increased, energy consumption was 2416 ± 696 (mean ± SD) kcal · day(-1), 83 ± 17 % of recommended intake, carbohydrate intake was only 46 ± 13 % of minimal recommended (10 g · kg(-1) · day(-1)) and fluid intake only 2.7 ± 1.0 l · day(-1). CONCLUSIONS: Our sample of endurance athletes did not comply with pre-race nutritional recommendations despite elementary knowledge and belief to be compliant. In these athletes a clear and reflective nutritional strategy was lacking. This suggests a potential for improving knowledge and compliance with recommendations. Alternatively, some recommendations may be unrealistic.
