The prevalence of clonal complex (CC) 398 methicillin-susceptible Staphylococcus aureus (MSSA) was unexpectedly high among bone and joint infections (BJIs) and nasal-colonizing isolates in France, with surprising geographical heterogeneity. With none of the major, most-known staphylococcal virulence genes, MSSA CC398 BJI was associated with lower biological inflammatory syndrome and lower treatment failure rates.

BACKGROUND: The epidemiology of congenital small intestinal atresia (SIA) has not been well studied. This study describes the presence of additional anomalies, pregnancy outcomes, total prevalence and association with maternal age in SIA cases in Europe. METHODS: Cases of SIA delivered during January 1990 to December 2006 notified to 20 EUROCAT registers formed the population-based case series. Prevalence over time was estimated using multilevel Poisson regression, and heterogeneity between registers was evaluated from the random component of the intercept. RESULTS: In total 1133 SIA cases were reported among 5126, 164 registered births. Of 1044 singleton cases, 215 (20.6%) cases were associated with a chromosomal anomaly. Of 829 singleton SIA cases with normal karyotype, 221 (26.7%) were associated with other structural anomalies. Considering cases with normal karyotype, the total prevalence per 10 000 births was 1.6 (95% CI 1.5 to 1.7) for SIA, 0.9 (95% CI 0.8 to 1.0) for duodenal atresia and 0.7 (95% CI 0.7 to 0.8) for jejunoileal atresia (JIA). There was no significant trend in SIA, duodenal atresia or JIA prevalence over time (RR=1.0, 95% credible interval (CrI): 1.0 to 1.0 for each), but SIA and duodenal atresia prevalence varied by geographical location (p=0.03 and p=0.04, respectively). There was weak evidence of an increased risk of SIA in mothers aged less than 20 years compared with mothers aged 20 to 29 years (RR=1.3, 95% CrI: 1.0 to 1.8). CONCLUSION: This study found no evidence of a temporal trend in the prevalence of SIA, duodenal atresia or JIA, although SIA and duodenal atresia prevalence varied significantly between registers.

AimThe study of adaptive radiations provides an evolutionary perspective on the interactions between organisms and their environment, and is necessary to understand global biodiversity. Adaptive radiations can sometimes be replicated over several disjunct geographical entities, but most examples are found on island or in lakes. Here, we investigated the biogeographical history of the clownfishes, a clade of coral reef fish with ranges that now span most of the Indo-Pacific Ocean, in order to explore the geographical structure of an unusual adaptive radiation. LocationIndian Ocean, Indo-Australian Archipelago (IAA) and Central Pacific Ocean. MethodsWe generated DNA sequence data comprising seven nuclear markers for 27 of the 30 clownfish species. We then inferred a Bayesian phylogeny and reconstructed the biogeographical history of the group using three different methods. Finally, we applied a biogeographical model of diversification to assess whether diversification patterns differ between the Indian and Pacific Oceans. ResultsThe phylogenetic tree is highly supported and allows reconstruction of the biogeographical history of the clade. While most species arose in the IAA, one clade colonized the eastern shores of Africa and diversified there. We found that the diversification rate of clownfishes does not differ between the main radiation and the African clade. Main conclusionsThe clownfishes first appeared and diversified in the IAA. Following a colonization event, a geographically independent radiation occurred in the Indian Ocean off East Africa. This rare example of replicated adaptive radiation in the marine realm provides intriguing possibilities for further research on ecological speciation in the sea.

Résumé : Ce travail jette un regard nouveau sur l'information documentaire publique. A travers l'analyse des régulations qui régissent l'archivage, la préservation du patrimoine documentaire et la gestion des documents au niveau national en Suisse, il propose en effet l'application d'un concept d'analyse fédérateur, habituellement exploité pour la compréhension du fonctionnement des res¬sources naturelles. Après avoir exposé que la création, la gestion et la préservation de l'information sont historiquement liées à l'exercice du pouvoir, au système politique en place et au développement des technologies de l'information, ce travail montre que - à la suite des importants changements dus au développe¬ment de la société du savoir et des nouvelles technologies de l'information - les politiques concernées en Suisse sont en train de converger. Ce mouve¬ment, signe de la lente reconnaissance de l'existence d'une ressource unique, va de pair avec le développement d'un système de règles. Cette ressource, désignée par le terme générique d'information, est en train de devenir un enjeu politique et économique fondamental : comme les ressources natu¬relles, elle exige de la part des acteurs publics une prise en charge appropriée afin d'assurer sa durabilité - c'est-à-dire sa capacité de renouvellement - en régulant les rivalités entre usages compétitifs, qui risquent de mettre cette ressource en péril. Ainsi, par l'application d'un modèle d'analyse appelé Régime institutionnel des ressources (RIR), ce travail évalue la durabilité de la ressource information en vérifiant l'existence et la mise en oeuvre de règles pour cha¬cun des usages identifiés de cette ressource. A partir des huit études de cas (consacrées aux archives d'Etat, aux Helvetica, aux données météoro¬logiques et climatologiques, aux données statistiques, aux données d'observation des sols, aux données de la mensuration officielle, aux don¬nées d'état civil et aux données de la taxe sur la valeur ajoutée), notre analyse qualitative montre que, si la durabilité de la ressource est assurée à court terme, elle ne l'est pas sur le long terme. En effet, en dépit des progrès significatifs qui ont été faits ces dernières années, notamment en termes de gestion des documents, le régime institu¬tionnel de la ressource information présente des failles : les principales rivalités existantes ne sont pas contrôlées par des régulations spécifiques. Ainsi, l'identification des informations disponibles, la traçabilité de celles-ci, la sélection de celles qu'il convient de préserver à long terme et, enfin, leur conservation physique continueront à poser problème à l'avenir. Le document se clôt sur l'encouragement à poursuivre les travaux scienti¬fiques et politiques dans ce domaine, dans le but de bénéficier enfin d'une compréhension plus approfondie des mécanismes qui le régissent.

BACKGROUND: Pneumocystis jirovecii dihydropteroate synthase (DHPS) mutations are associated with failure of prophylaxis with sulfa drugs. This retrospective study sought to better understand the geographical variation in the prevalence of these mutations. METHODS: DHPS polymorphisms in 394 clinical specimens from immunosuppressed patients who received a diagnosis of P. jirovecii pneumonia and who were hospitalized in 3 European cities were examined using polymerase chain reaction (PCR) single-strand conformation polymorphism. Demographic and clinical characteristics were obtained from patients' medical charts. RESULTS: Of the 394 patients, 79 (20%) were infected with a P. jirovecii strain harboring one or both of the previously reported DHPS mutations. The prevalence of DHPS mutations was significantly higher in Lyon than in Switzerland (33.0% vs 7.5%; P < .001). The proportion of patients with no evidence of sulfa exposure who harbored a mutant P. jirovecii DHPS genotype was significantly higher in Lyon than in Switzerland (29.7% vs 3.0%; P < .001). During the study period in Lyon, in contrast to the Swiss hospitals, measures to prevent dissemination of P. jirovecii from patients with P. jirovecii pneumonia were generally not implemented, and most patients received suboptimal prophylaxis, the failure of which was strictly associated with mutated P. jirovecii. Thus, nosocomial interhuman transmission of mutated strains directly or indirectly from other individuals in whom selection of mutants occurred may explain the high proportion of mutations without sulfa exposure in Lyon. CONCLUSIONS: Interhuman transmission of P. jirovecii, rather than selection pressure by sulfa prophylaxis, may play a predominant role in the geographical variation in the prevalence in the P. jirovecii DHPS mutations.
