Resumo:
A key feature differentiating cooperative animal societies Is the apportionment of reproduction among individuals. Only recently have studies started to focus on intraspecific variability in the distribution of reproduction within animal societies, and the available data suggest that this variability might be greater than previously suspected. How can one account for intra-and interspecific variability in partitioning of reproduction? This Is one of the most intriguing problems in the study of social behaviour, and understanding the factors underlying this variability is one of the keys to understanding the properties of complex animal societies.

It is well documented that reducing blood pressure (BP) in hypertensive individuals reduces the risk of cardiovascular (CV) events. Despite this, many patients with hypertension remain untreated or inadequately treated, and fail to reach the recommended BP goals. Suboptimal BP control, whilst arising from multiple causes, is often due to poor patient compliance and/or persistence, and results in a significant health and economic burden on society. The use of fixed-dose combinations (FDCs) for the treatment of hypertension has the potential to increase patient compliance and persistence. When compared with antihypertensive monotherapies, FDCs may also offer equivalent or better efficacy, and the same or improved tolerability. As a result, FDCs have the potential to reduce both the CV event rates and the non-drug healthcare costs associated with hypertension. When FDCs are adopted for the treatment of hypertension, issues relating to copayment, formulary restrictions and therapeutic reference pricing must be addressed.

We investigate the coevolution between philopatry and altruism in island-model populations when kin recognition occurs through phenotype matching. In saturated environments, a good discrimination ability is a necessary prerequisite for the emergence of sociality. Discrimination decreases not only with the average phenotypic similarity between immigrants and residents (i.e., with environmental homogeneity and past gene flow) but also with the sampling variance of similarity distributions (a negative function of the number of traits sampled). Whether discrimination should rely on genetically or environmentally determined traits depends on the apportionment of phenotypic variance and, in particular, on the relative values of e (the among-group component of environmental variance) and r (the among-group component of genetic variance, which also measures relatedness among group members). If r exceeds e, highly heritable cues do better. Discrimination and altruism, however, remain low unless philopatry is enforced by ecological constraints. If e exceeds r, by contrast, nonheritable traits do better. High e values improve discrimination drastically and thus have the potential to drive sociality, even in the absence of ecological constraints. The emergence of sociality thus can be facilitated by enhancing e, which we argue is the main purpose of cue standardization within groups, as observed in many social insects, birds, and mammals, including humans.

Introduction The benefit of stress ulcer prophylaxis (SUP) in noncriticallyill patients has not been proved. Over-prescription of SUP isnot devoided of risks (i.e. drug-drug interactions, adverse events).This prospective study aimed to evaluate the use of proton pumpinhibitors (PPIs) for SUP in a visceral surgery ward.Materials & Methods Data collection was performed prospectivelyduring a 8-week period on patients hospitalized in a visceral surgeryward (58 beds). Patients with a PPI prescription for the treatment ofulcers, gastroesophageal reflux disease, esophagitis or epigastralgiawere excluded as well as patients hospitalized twice during the studyperiod. The American Society of Health-System Pharmacists guidelineson SUP were used to assess the appropriateness of de novo PPIprescriptions.Results Among 255 patients in the study, 138 (54.1%) received aprophylaxis with PPI, of which 86 (62.3%) were de novo PPI prescriptions.93.5% of patients received esomeprazole (according to thehospital drug formulary) mainly orally at 40 mg qd. 79.1% of patientshad no risk factors for SUP. 17.9% and 3.0% had one and two riskfactors, respectively. 95% of the patients with PPI were not hospitalizedin the intensive care unit (ICU) before their stay in the visceralsurgery ward. At discharge, PPI therapy was continued in 34.2% ofpatients with a de novo PPI prescription.Discussion & Conclusion This study highlights the over-utilizationof PPIs in non-ICU patients and the inappropriate continuation of PPIprescriptions at discharge. The PPI dosage prescribed for prophylaxiswas probably too high compared with the data of the literature.Treatment recommendations for SUP are needed to restrict PPIuse for justified indications.

Introduction Medication errors in hospitalsmay occur at any step of the medication process including prescription, transcription, preparation and administration, and may originate with any of the actors involved. Neonatal intensive care units (NICU) take care of extremely frail patients in whom errors could have dramatic consequences. Our objective was to assess the frequency and nature of medication errors in the NICU of a university hospital in order to propose measures for improvement.Materials & Methods The design was that of an observational prospective study over 4 consecutivemonths. All patients receiving C 3drugs were included. For each patient, observations during the different stages were compiled in a computer formulary and compared with the litterature. Setting: The 11-bed NICU of our university hospital.Main outcome measures:(a) Frequency and nature of medication errors in prescription,transcription, preparation and administration.(b) Drugs affected by errors.Results 83 patients were included. 505 prescriptions and transcriptions, 447 preparations and 464 administrations were analyzed. 220 medications errors were observed: 102 (46.4%) at prescription, 25 (11.4%) at transcription, 19 (8.6%) at preparation and 73 (33.2%) at administration. Uncomplete/ambiguous orders (24; 23.5%) were the most common errors observed at prescription, followed by wrong name (21; 20.6%), wrong dose (17; 16.7%) and omission (15; 14.7%). Wrong time (33; 45.2%) and wrong administration technique (31; 42.5%) were the most important medication errors during administration. According to the ATC classification, systemic antibacterials (53; 24.1%) were the most implicated, followed by perfusion solutions (40; 18.2%), respiratory system products (30; 13.6%), and mineral supplements and antithrombotic agents (20; 9.1%).Discussions, Conclusion Proposed recommendations: ? Better teaching of neonatal prescription to medical interns;? Improved prescription form to avoid omissions and ambiguities;? Development of a neonatal drug formulary, including prescription,preparation and administration modalities to reduce errors at different stages;? The present article examines the final part of the regnal formulas in the Book of Kings, i.e. the epilogue formulary. Most reports of the kings of Israel and Judah end with an epilogue containing formulaic statements about the death of the king and his succession. Typically, the epilogue formula is introduced by the phrase (source reference): 'Now the rest of the acts of PN1, are they not written in the Book of the Chronicles of the Kings of Judah (of the Chronicles of the Kings of Israel)?' and consists of three elements: 1. Dynastic notice ('PN1 slept with his fathers'); 2. Statement of burial; 3. Statement of succession. Sometimes one, two or all three of these elements are altered or lacking. The epilogue formulae of the Judean kings are more consistent than those of the Israelite kings; the latter often lacks a burial notice. Interestingly, the accounts of the deported (arrested) kings (Hoshea: 2 Kgs 17:6, Jehoahaz: 23:34, Zedekiah:25:7 and Jehoiachin: 25:27-30) do not contain an epilogue at all, nor the accounts of the reigns of Ahaziah and Athaliah which mark an episode of disruption in the history of the Davidic kingdom. For all these kings even the phrase 'Now the rest of the acts of PN1, are they not written in the Book of the Chronicles of... ?' is lacking. The absence of an epilogue in these cases is probably due to the fact that the king's succession (cf. element 3) is considered a constitutive and indispensable component of the epilogue. In the first instance, the approach of this study is descriptive and philological; it aims to contribute to the understanding of the notices. Secondly, the study addresses the question how the irregularities and variations within the formulae are to be explained. In particular, the study will pay attention to differences between the epilogue formulae concerning the kings of Israel and those concerning the Judean kings. At the end, questions concerning the epilogues formula's provenance and its formation date and concerning further redactional developments will be considered.
