Although interpersonal continuity is commonly assumed to be essential for care, some patients prefer to attend a university outpatient clinic where physicians change regularly and interpersonal continuity of care is not ensured. The aim of this exploratory study was to evaluate the differences between patients attending a university outpatient clinic and patients frequenting a private practice, explore their patterns of care-seeking and their understanding of continued care. We conducted a cross-sectional study of patients attending the university medical outpatient clinic (OC) in Lausanne, Switzerland and ten randomly selected private general practices (PP). Eligible patients were >30 years, Swiss nationals or long term residents, with one or more chronic conditions and attending the same practice for >3 years. They were asked to complete a questionnaire on sociodemographic data, use of medical resources and reasons for choosing and remaining at the same practice. Semi-structured interviews were conducted with a randomly selected subset of 26 patients to further explore their preferences. 329 patient questionnaires were completed, 219 by PP and 110 by OC patients. OC patients tended to be of lower socioeconomic status than PP patients. The main reason for choosing a PP were personal recommendation, while a higher percentage of patients chose the OC because they could obtain a first appointment quickly. A higher percentage of PP patients accorded importance to physician communication skills and trust, whereas a higher percentage of OC patients favoured investigation facilities. Qualitative data suggested that although OC and PP patients reported different reasons for consulting, their expectations on the medical and relationship level were similar. Our study suggests that the two groups of patients belong to different social backgrounds, have different patterns of care-seeking and attach importance to different aspects of care continuity. However, patients' expectations and perceptions of the physician-patient relationship are similar.

BACKGROUND: Regional rates of hospitalization for ambulatory care sensitive conditions (ACSC) are used to compare the availability and quality of ambulatory care but the risk adjustment for population health status is often minimal. The objectives of the study was to examine the impact of more extensive risk adjustment on regional comparisons and to investigate the relationship between various area-level factors and the properly adjusted rates. METHODS: Our study is an observational study based on routine data of 2 million anonymous insured in 26 Swiss cantons followed over one or two years. A binomial negative regression was modeled with increasingly detailed information on health status (age and gender only, inpatient diagnoses, outpatient conditions inferred from dispensed drugs and frequency of physician visits). Hospitalizations for ACSC were identified from principal diagnoses detecting 19 conditions, with an updated list of ICD-10 diagnostic codes. Co-morbidities and surgical procedures were used as exclusion criteria to improve the specificity of the detection of potentially avoidable hospitalizations. The impact of the adjustment approaches was measured by changes in the standardized ratios calculated with and without other data besides age and gender. RESULTS: 25% of cases identified by inpatient main diagnoses were removed by applying exclusion criteria. Cantonal ACSC hospitalizations rates varied from to 1.4 to 8.9 per 1,000 insured, per year. Morbidity inferred from diagnoses and drugs dramatically increased the predictive performance, the greatest effect found for conditions linked to an ACSC. More visits were associated with fewer PAH although very high users were at greater risk and subjects who had not consulted at negligible risk. By maximizing health status adjustment, two thirds of the cantons changed their adjusted ratio by more than 10 percent. Cantonal variations remained substantial but unexplained by supply or demand. CONCLUSION: Additional adjustment for health status is required when using ACSC to monitor ambulatory care. Drug-inferred morbidities are a promising approach.

The Munc13 gene family encodes molecules located at the synaptic active zone that regulate the reliability of synapses to encode information over a wide range of frequencies in response to action potentials. In the CNS, proteins of the Munc13 family are critical in regulating neurotransmitter release and synaptic plasticity. Although Munc13-1 is essential for synaptic transmission, it is paradoxical that Munc13-2 and Munc13-3 are functionally dispensable at some synapses, although their loss in other synapses leads to increases in frequency-dependent facilitation. We addressed this issue at the calyx of Held synapse, a giant glutamatergic synapse that we found to express all these Munc13 isoforms. We studied their roles in the regulation of synaptic transmission and their impact on the reliability of information transfer. Through detailed electrophysiological analyses of Munc13-2, Munc13-3, and Munc13-2-3 knock-out and wild-type mice, we report that the combined loss of Munc13-2 and Munc13-3 led to an increase in the rate of calcium-dependent recovery and a change in kinetics of release of the readily releasable pool. Furthermore, viral-mediated overexpression of a dominant-negative form of Munc13-1 at the calyx demonstrated that these effects are Munc13-1 dependent. Quantitative immunohistochemistry using Munc13-fluorescent protein knock-in mice revealed that Munc13-1 is the most highly expressed Munc13 isoform at the calyx and the only one highly colocalized with Bassoon at the active zone. Based on these data, we conclude that Munc13-2 and Munc13-3 isoforms limit the ability of Munc13-1 to regulate calcium-dependent replenishment of readily releasable pool and slow pool to fast pool conversion in central synapses.

The hydrological and biogeochemical processes that operate in catchments influence the ecological quality of freshwater systems through delivery of fine sediment, nutrients and organic matter. Most models that seek to characterise the delivery of diffuse pollutants from land to water are reductionist. The multitude of processes that are parameterised in such models to ensure generic applicability make them complex and difficult to test on available data. Here, we outline an alternative - data-driven - inverse approach. We apply SCIMAP, a parsimonious risk based model that has an explicit treatment of hydrological connectivity. we take a Bayesian approach to the inverse problem of determining the risk that must be assigned to different land uses in a catchment in order to explain the spatial patterns of measured in-stream nutrient concentrations. We apply the model to identify the key sources of nitrogen (N) and phosphorus (P) diffuse pollution risk in eleven UK catchments covering a range of landscapes. The model results show that: 1) some land use generates a consistently high or low risk of diffuse nutrient pollution; but 2) the risks associated with different land uses vary both between catchments and between nutrients; and 3) that the dominant sources of P and N risk in the catchment are often a function of the spatial configuration of land uses. Taken on a case-by-case basis, this type of inverse approach may be used to help prioritise the focus of interventions to reduce diffuse pollution risk for freshwater ecosystems. MOTIVATION: Comparative analyses of gene expression data from different species have become an important component of the study of molecular evolution. Thus methods are needed to estimate evolutionary distances between expression profiles, as well as a neutral reference to estimate selective pressure. Divergence between expression profiles of homologous genes is often calculated with Pearson's or Euclidean distance. Neutral divergence is usually inferred from randomized data. Despite being widely used, neither of these two steps has been well studied. Here, we analyze these methods formally and on real data, highlight their limitations and propose improvements. RESULTS: It has been demonstrated that Pearson's distance, in contrast to Euclidean distance, leads to underestimation of the expression similarity between homologous genes with a conserved uniform pattern of expression. Here, we first extend this study to genes with conserved, but specific pattern of expression. Surprisingly, we find that both Pearson's and Euclidean distances used as a measure of expression similarity between genes depend on the expression specificity of those genes. We also show that the Euclidean distance depends strongly on data normalization. Next, we show that the randomization procedure that is widely used to estimate the rate of neutral evolution is biased when broadly expressed genes are abundant in the data. To overcome this problem, we propose a novel randomization procedure that is unbiased with respect to expression profiles present in the datasets. Applying our method to the mouse and human gene expression data suggests significant gene expression conservation between these species. L?objectif de ce travail de recherche était de décrypter l?évolution géodynamique de la Péninsule de Biga (Turquie du N-O), à travers l?analyse de deux régions géologiques peu connues, le mélange de Çetmi et la zone d?Ezine (i.e. le Groupe d?Ezine et l?ophiolite de Denizgören). Une étude complète et détaillée de terrain (cartographie et échantillonnage) ainsi qu?une approche multidisciplinaire (sédimentologie de faciès, pétrographie sédimentaire et magmatique, micropaléontologie, datations absolues, géochimie sur roche totale, cristallinité de l?illite) ont permis d?obtenir de nouveaux éléments d?information sur la région considérée. ? Le mélange de Çetmi, de type mélange d?accrétion, affleure au nord et au sud de la Péninsule de Biga ; les principaux résultats de son étude peuvent se résumer comme suit: - Son aspect structural actuel (nature des contacts, organisation tectonique) est principalement dû au régime extensif Tertiaire présent dans la région. - Il est constitué de blocs de différentes natures : rares calcaires Scythien-Ladinien dans le faciès Han Bulog, blocs hectométriques de calcaires d?âge Norien-Rhaetien de rampe carbonatée, nombreux blocs décamétriques de radiolarites rouges d?âge Bajocien- Aptien, blocs/écailles de roches magmatiques de type spilites (basaltes à andésite), ayant des signatures géochimiques d?arcs ou intra-plaques. - La matrice du mélange est constituée d?une association greywacke-argilites dont l?âge Albien inférieur à moyen a été déterminé par palynologie. - L?activité du mélange s?est terminée avant le Cénomanien (discordance Cénomanienne au sommet du mélange, pas de bloc plus jeune que la matrice). - Du point de vue de ses corrélations latérales, le mélange de Çetmi partage plus de traits communs avec les mélanges se trouvant dans les nappes allochtones du Rhodope (nord de la Grèce et sud-ouest de la Bulgarie) qu?avec ceux de la suture Izmir-Ankara (Turquie); il apparaît finalement que sa mise en place s?est faite dans une logique balkanique (chevauchements vers le nord d?âge anté-Cénomanien). ? Le Groupe d?Ezine et l?ophiolite sus-jacente de Denizgören affleurent dans la partie ouest de la Péninsule de Biga. Le Groupe d?Ezine est une épaisse séquence sédimentaire continue (3000 m), subdivisée en trois formations, caractérisée chacune par un type de sédimentation spécifique, relatif à un environnement de dépôt particulier. De par ses caractéristiques (grande épaisseur, variations latérales de faciès et d?épaisseur dans les formations, érosion de matériel provenant de l?amont du bassin), le groupe d?Ezine est interprétée comme un dépôt syn-rift d?âge Permien moyen-Trias inférieur. Il pourrait représenter une partie de la future marge passive sud Rhodopienne à la suite de l?ouverture de l?océan Maliac/Méliata. L?ophiolite de Denizgören sus-jacente repose sur le Groupe d?Ezine par l?intermédiaire d?une semelle métamorphique à gradient inverse, du faciès amphibolite à schiste vert. L?âge du faciès amphibolite suggère une initiation de l?obduction au Barrémien (125 Ma, âge Ar/Ar); cet âge est unique dans le domaine égéen, mais il peut là aussi être relié à une logique balkanique, sur la base de comparaison avec le domaine Rhodopien. ? Toutes les unités précédentes (mélange de Çetmi, Groupe d?Ezine et ophiolite de Denizgören) ont passivement subi trois phases extensives pendant le Tertiaire. Dans la région d?Ezine et du mélange nord, les micaschistes HP sous-jacents ont été exhumés avant l?Eocène moyen. Dans le cas du mélange sud, cette exhumation Eocene est en partie enregistrée dans les mylonites séparant le mélange du dôme métamorphique sous-jacent du Kazda?. Le mélange sud est dans tous les cas fortement érodé à la suite de la double surrection du dôme du Kazda?, près de la lim ite Oligocène/Miocene et pendant le Plio- Quaternaire. Dans le premier cas, ce soulèvement est caractérisé par le développement d?une faille de détachement à faible pendage, qui contrôle à la fois l?exhumation du massif, et la formation d?un bassin sédimentaire syntectonique, de type bassin supradétachement; quant à la phase extensive la plus récente, elle est contrôlée par le jeu de failles normales à forts pendages qui remanient l?ensemble des structures héritées, et dictent la géomorphologie actuelle de la région. ? Il est possible de proposer un scénario pour l?évolution géodynamique de la Péninsule de Biga, basé sur l?ensemble des résultats précédents et sur les données de la géologie régionale ; ses points principaux sont: - La Péninsule de Biga fait partie de la marge Rhodopienne. - Le Groupe d?Ezine est un témoin de la marge passive nord Maliac/Méliata. - L?ophiolite de Denizgören et le mélange de Çetmi ont été mis en place tous deux vers le nord sur la marge précédente, respectivement au Barrémien et à l?Albien terminal- Cénomanien inférieur. - Une forte composante décrochante durant l?emplacement est suggérée par la préservation de fragments de la marge passive et l?absence de métamorphisme dans la plaque inférieure. - Tous les évènements précédents ont été largement affectés par le régime d?extension Tertiaire.<br/><br/>The purpose of this study is to unravel the geodynamic evolution of the Biga Peninsula (NW Turkey) through the detailed study of two poorly known areas, the Çetmi mélange and the Ezine zone (i.e. the Ezine Group and the Denizgören ophiolite). The methodology was based on a detailed field work and a multidisciplinary approach. ? The accretion-related Çetmi mélange is mainly cropping out north and south of the Biga Peninsula; the main results of its study can be summarized as follows: -Its present-day structural aspect (type of contacts, tectonic organisation) is largely inherited from the Tertiary extensional regime in the region. -It is made of blocks of various natures: Han Bulog limestones with a Scythian to Ladinian age, common carbonate ramp Norian-Rhaetian limestones (biggest blocks of the mélange), red radolarite with a Bajocian to Aptian age; the most common lithology of the mélange is made by block/slices of spilitic magmatic rocks (basalt to andesite); they have volcanic arc or within plate basalt geochemical signatures. -The matrix of the mélange is made of a greywacke-shale association of Early-Middle Albian age. - The mélange stopped its activity before the Cenomanian (no younger blocks than the matrix, and Cenomanian unconformity). - If compared to the regional geology, the Çetmi mélange shares some characteristics with the Izmir-Ankara mélanges (less), and with the mélanges from allochthonous nappes found in eastern Rhodope (more); it appears finally that its emplacement is related to a Balkanic logic (ante-Cenomanian northward thrusting). ? The Ezine Group and the overlying Denizgören ophiolite are cropping out in the western part of the Biga Peninsula. The Ezine Group is a thick sedimentary sequence interpreted as a syn-rift deposit of Middle Permian-Early Triassic age. It represents a part of the south Rhodopian passive margin, following the opening of the Maliac/Meliata oceanic domain. The Denizgören ophiolite has been emplaced northward on the Ezine Group in the Barremian (125 Ma, age of the amphibolitic sole); this age is unique in the Aegean domain, but here again, it may be related to a Balkan logic. ? All the previous units (Çetmi mélange, Ezine Group and Denizgören ophiolite) have passively suffered two extensional regimes during the Tertiary. In the Ezine and northern Çetmi mélange area, the underlying HP Çamlýca micaschists were exhumed before the Middle Eocene. As for the southern mélange, it was strongly eroded following the Late Oligocene to Quaternary uplift of the underlying Kazda? Massif. This uplift was characterized by the development of a low-angle detachment fault controlling a part of the exhumation, as well as the development of a supra-detachment basin. ? Based on the previous results, and on the data from the regional geology, one can propose a scenario for the geodynamic evolution of the Biga Peninsula. Its key points are:- The Biga Peninsula is belonging to the Rhodope margin. - The Ezine Group is a remnant of the northern Maliac/Meliata passive margin. - Both the Denizgören ophiolite and the Çetmi mélange have been emplaced northward on the previous margin, respectively in the Barremian and in the Late Albian-Early Cenomanian times. - The preservation of the remnants of the Rhodope margin, as well as the absence of metamorphism in the lower plate suggest a strong strike-slip component during the emplacements. - All the previous events are (at least) partly obliterated by the Tertiary extensional regime.<br/><br/>Le géologue est comme un «historien» de la Terre, qui porte un intérêt particulier à l?étude du passé de notre planète; ce dernier, très ancien, se mesure en dizaines ou centaines de millions d?années (Ma). Or le visage de la terre a constamment évolué au cours des ces millions d?années écoulés, car les plaques (continentales et océaniques) qui composent son enveloppe superficielle ne restent pas immobiles, mais se déplacent continuellement à sa surface, à une vitesse de l?ordre du cm/an (théorie de la tectonique des plaques); c?est ainsi, par exemple, que des océans naissent, grandissent, puis finissent par se refermer. On appelle sutures océaniques, les zones, aujourd?hui sur la terre ferme, où l?on retrouve les restes d?océans disparus. Ces sutures sont caractérisées par deux associations distinctes de roches, que l?on appelle les mélanges et les ophiolites; ces mélanges et ophiolites sont donc les témoins de l?activité passée d?un océan aujourd?hui refermé. L?équipe de recherche dans laquelle ce travail à été réalisé s?intéresse à un vaste domaine océanique fossile: l?océan Néotéthys. Cet océan, de plusieurs milliers de kilomètres de large, séparait alors l?Europe et l?Asie au nord, de l?Afrique, l?Inde et l?Australie au sud. De cet océan, il n?en subsiste aujourd?hui qu?une infime partie, qui se confond avec notre mer Méditerranée actuelle. Or, tout comme l?océan Pacifique est bordé de mers plus étroites (Mer de Chine, du Japon, etc?), l?océan Néotéthys était bordé au nord de mers marginales. C?est dans ce cadre que s?est inscrit mon travail de thèse, puisqu?il a consisté en l?étude d?une suture océanique (mélange plus ophiolite), témoin d?une des mers qui bordait l?océan Néotéthys sur sa marge nord. L?objectif était de préciser de quelle suture il s?agissait, puis de déterminer quand et comment elle avait fonctionné (i.e son évolution géologique). Les roches qui composent cette suture affleurent aujourd?hui en Turquie nord occidentale dans la Péninsule de Biga. Au nord et au sud de la péninsule se trouvent les zones géologique du mélange de Çetmi, et à l?ouest, le Groupe d?Ezine et l?ophiolite susjacente, dite ophiolite de Denizgören. Une étude complète et détaillée de terrain (cartographie, échantillonnage), suivie de diverses analyses en laboratoire (détermination de leur âge, de leur condition de formation, etc?), ont permis d?aboutir aux principaux résultats suivants : - Mise en évidence dans le mélange de Çetmi des témoins (1) de l?océan Lycien disparu (ancienne mer marginale de la Néotéthys), et (2) de la marge continentale qui le bordait au nord. - Fin de l?activité du mélange de Çetmi il y a environ 105 Ma (Albien). - Le mélange de Çetmi est difficilement corrélable dans le temps avec les unités semblables affleurant dans la région d?étude (unicité du mélange), ce qui implique des conditions particulière de formation. - L?ophiolite de Denizgören est un morceau d?océan Lycien posé sur un reste préservé de sa marge continentale nord. - Cette dernière est représentée sur le terrain par une succession de roches caractéristiques, le Groupe d?Ezine. Celui-ci est lui-même un témoin de l?ouverture d?un océan marginal de la Néotethys antérieur au Lycien, l?océan Maliac, qui s?est ouvert il y a 245 Ma (Permien-Trias). - La mise en place de l?ophiolite de Denizgören sur le Groupe d?Ezine (125 Ma, Barrémien) est antérieure à la mise en place du mélange de Çetmi. - Il apparaît que ces deux mises en place sont contemporaines de la formation de la chaîne des Balkans, terminée avant le Cénomanien (100 Ma). - L?évolution dans le temps des objets précédents (océans, marges continentales) montre de grands mouvements latéraux est-ouest entre ces objets (translation). Ce qui implique que les roches que l?on retrouve aujourd?hui sur un transect nord-sud ne l?étaient pas nécessairement auparavant. - Enfin, il s?avère que le mélange de Çetmi, l?ophiolite de Denizgören, et le Groupe d?Ezine ont subi par la suite des déformations extensives importantes qui ont considérablement perturbé le schéma post-mise en place.

Les cellules CD8? T cytolytiques (CTL) sont les principaux effecteurs du système immunitaire adaptatif contre les infections et les tumeurs. La récente identification d?antigènes tumoraux humains reconnus par des cellules T cytolytiques est la base pour le, développement des vaccins antigène spécifiques contre le cancer. Le nombre d?antigènes tumoraux reconnus par des CTL que puisse être utilisé comme cible pour la vaccination des patients atteints du cancer est encore limité. Une nouvelle technique, simple et rapide, vient d?être proposée pour l?identification d?antigènes reconnus par des CTL. Elle se base sur l?utilisation de librairies combinatoriales de peptides arrangées en un format de "scanning" ou balayage par position (PS-SCL). La première partie de cette étude a consisté à valider cette nouvelle technique par une analyse détaillée de la reconnaissance des PS-SCL par différents clones de CTL spécifiques pour des antigènes associés à la tumeur (TAA) connus ainsi que par des clones de spécificité inconnue. Les résultats de ces analyses révèlent que pour tous les clones, la plupart des acides aminés qui composent la séquence du peptide antigénique naturel ont été identifiés par l?utilisation des PS-SCL. Les résultats obtenus ont permis d?identifier des peptides analogues ayant une antigènicité augmentée par rapport au peptide naturel, ainsi que des peptides comportant de multiples modifications de séquence, mais présentant la même réactivité que le peptide naturel. La deuxième partie de cette étude a consisté à effectuer des analyses biométriques des résultats complexes générés par la PS-SCL. Cette approche a permis l?identification des séquences correspondant aux épitopes naturels à partir de bases de données de peptides publiques. Parmi des milliers de peptides, les séquences naturelles se trouvent comprises dans les 30 séquences ayant les scores potentiels de stimulation les plus élevés pour chaque TAA étudié. Mais plus important encore, l?utilisation des PS-SCL avec un clone réactif contre des cellules tumorales mais de spécificité inconnue nous a permis d?identifier I?epitope reconnu par ce clone. Les données présentées ici encouragent l?utilisation des PS-SCL pour l?identification et l?optimisation d?épitopes pour des CTL réactifs anti-tumoraux, ainsi que pour l?étude de la reconnaissance dégénérée d?antigènes par les CTL.<br/><br/>CD8+ cytolytic T lymphocytes (CTL) are the main effector cells of the adaptive immune system against infection and tumors. The recent identification of moleculariy defined human tumor Ags recognized by autologous CTL has opened new opportunities for the development of Ag-specific cancer vaccines. Despite extensive work, however, the number of CTL-defined tumor Ags that are suitable targets for the vaccination of cancer patients is still limited, especially because of the laborious and time consuming nature of the procedures currentiy used for their identification. The use of combinatorial peptide libraries in positionai scanning format (Positional Scanning Synthetic Combinatorial Libraries, PS-SCL)' has recently been proposed as an alternative approach for the identification of these epitopes. To validate this approach, we analyzed in detail the recognition of PS-SCL by tumor-reactive CTL clones specific for multiple well-defined tumor-associated Ags (TAA) as well as by tumor-reactive CTL clones of unknown specificity. The results of these analyses revealed that for all the TAA-specific clones studied most of the amino acids composing the native antigenic peptide sequences could be identified through the use of PS-SCL. Based on the data obtained from the screening of PS-SCL, we could design peptide analogs of increased antigenicity as well as cross-reactive analog peptides containing multiple amino acid substitutions. In addition, the resuits of PS-SCL-screening combined with a recently developed biometric data analysis (PS-SCL-based biometric database analysis) allowed the identification of the native peptides in public protein databases among the 30 most active sequences, and this was the case for all the TAA studied. More importantiy, the screening of PS- SCL with a tumor-reactive CTL clone of unknown specificity resulted in the identification of the actual epitope. Overall, these data encourage the use of PS-SCL not oniy for the identification and optimization of tumor-associated CTL epitopes, but also for the analysis of degeneracy in T lymphocyte receptor (TCR) recognition of tumor Ags.<br/><br/>Les cellules T CD8? cytolytiques font partie des globules blancs du sang et sont les principales responsables de la lutte contre les infections et les tumeurs. Les immunologistes cherchent depuis des années à identifier des molécules exprimées et présentées à la surface des tumeurs qui puissent être reconnues par des cellules T CD8? cytolytiques capables ensuite de tuer ces tumeurs de façon spécifique. Ce type de molécules représente la base pour le développement de vaccins contre le cancer puisqu?elles pourraient être injectées aux patients afin d?induire une réponse anti- tumorale. A présent, il y a très peu de molécules capables de stimuler le système immunitaire contre les tumeurs qui sont connues parce que les techniques développées à ce jour pour leur identification sont complexes et longues. Une nouvelle technique vient d?être proposée pour l?identification de ce type de molécules qui se base sur l?utilisation de librairies de peptides. Ces librairies représentent toutes les combinaisons possibles des composants de base des molécules recherchées. La première partie de cette étude a consisté à valider cette nouvelle technique en utilisant des cellules T CD8? cytolytiques capables de tuer des cellules tumorales en reconnaissant une molécule connue présente à leur surface. On a démontré que l?utilisation des librairies permet d?identifier la plupart des composants de base de la molécule reconnue par les cellules T CD8? cytolytiques utilisées. La deuxième partie de cette étude a consisté à effectuer une recherche des molécules potentiellement actives dans des protéines présentes dans des bases des données en utilisant un programme informatique qui permet de classer les molécules sur la base de leur activité biologique. Parmi des milliers de molécules de la base de données, celles reconnues par nos cellules T CD8? cytolytiques ont été trouvées parmi les plus actives. Plus intéressant encore, la combinaison de ces deux techniques nous a permis d?identifier la molécule reconnue par une population de cellules T CD8? cytolytiques ayant une activité anti-tumorale, mais pour laquelle on ne connaissait pas la spécificité. Nos résultats encouragent l?utilisation des librairies pour trouver et optimiser des molécules reconnues spécifiquement par des cellules T CD8? cytolytiques capables de tuer des tumeurs.

PURPOSE: To combine weighted iterative reconstruction with self-navigated free-breathing coronary magnetic resonance angiography for retrospective reduction of respiratory motion artifacts. METHODS: One-dimensional self-navigation was improved for robust respiratory motion detection and the consistency of the acquired data was estimated on the detected motion. Based on the data consistency, the data fidelity term of iterative reconstruction was weighted to reduce the effects of respiratory motion. In vivo experiments were performed in 14 healthy volunteers and the resulting image quality of the proposed method was compared to a navigator-gated reference in terms of acquisition time, vessel length, and sharpness. RESULT: Although the sampling pattern of the proposed method contained 60% more samples with respect to the reference, the scan efficiency was improved from 39.5 ± 10.1% to 55.1 ± 9.1%. The improved self-navigation showed a high correlation to the standard navigator signal and the described weighting efficiently reduced respiratory motion artifacts. Overall, the average image quality of the proposed method was comparable to the navigator-gated reference. CONCLUSION: Self-navigated coronary magnetic resonance angiography was successfully combined with weighted iterative reconstruction to reduce the total acquisition time and efficiently suppress respiratory motion artifacts. Adult-onset Still's disease is a rare and difficult to diagnose multisystemic disorder considered as a multigenic autoinflammatory syndrome. Its immunopathogenesis seems to be at the crossroads between inflammasomopathies and hemophagocytic lymphohistiocytosis, the most severe manifestation of the disease. According to recent insights in the pathophysiology and thanks to cohort studies and therapeutic trials, two phenotypes of adult-onset Still's disease may be distinguished: a systemic pattern, initially highly symptomatic and with a higher risk to exhibit life-threatening complications such as reactive hemophagocytic lymphohistiocytosis, where interleukin-1 blockade seems to be very effective, a chronic articular pattern, more indolent with arthritis in the foreground and less severe systemic manifestations, which would threat functional outcome and where interleukin-6 blockade seems to be more effective. This review focuses on these data.

Recent research has highlighted the existence of a social bias in the extent to which children have access to childcare. In general, children living in higher income households are more likely to be cared for in childcare centres. While the existence of a social bias in access to childcare services has been clearly demonstrated, we currently lack a clear explanation as to why this is the case. This paper uses a unique dataset based on survey data collected specifically to study patterns of childcare use in the Swiss canton of Vaud (N = 875). The paper exploits the variation in the way childcare is organised within the canton. Childcare is a municipal policy, as a result of which there are twenty-nine different systems in operation. Fees are progressive everywhere, but variation is substantial. Availability is also very different. This peculiar institutional setup provides an ideal situation to examine the determinants of childcare use by different income groups. Our findings suggest that differences in the fees charged to low-income households, as well as the degree of progressivity of the fee structure, are significant predictors of use, while availability seems to matter less.

The purpose of this paper is to examine the CSR practices and their implementation in the context of French professional sports clubs. In doing so, it analyses the link between the governance of sports clubs and CSR, which is viewed as a component of governance expanded to stakeholders and contributing to the creation of shared value. Drawing on interview data with key stakeholders of four professional sport clubs (football and basketball) and secondary material, the study sheds light on the determinants, the implementation as well as the impact of CSR on the governance of the professional clubs under examination.

This chapter presents possible uses and examples of Monte Carlo methods for the evaluation of uncertainties in the field of radionuclide metrology. The method is already well documented in GUM supplement 1, but here we present a more restrictive approach, where the quantities of interest calculated by the Monte Carlo method are estimators of the expectation and standard deviation of the measurand, and the Monte Carlo method is used to propagate the uncertainties of the input parameters through the measurement model. This approach is illustrated by an example of the activity calibration of a 103Pd source by liquid scintillation counting and the calculation of a linear regression on experimental data points. An electronic supplement presents some algorithms which may be used to generate random numbers with various statistical distributions, for the implementation of this Monte Carlo calculation method.

BACKGROUND & AIMS: Protein and energy requirements in critically ill children are currently based on insufficient data. Moreover, longitudinal measurements of both total urinary nitrogen (TUN) and resting energy expenditure (REE) are lacking. The aim of this study was to investigate how much protein and energy are needed to equilibrate nitrogen and energy balances in ventilated critically ill children on the basis of daily measurements of TUN, REE and protein and energy intakes. Comparisons were made with the guidelines of the American Society for Parenteral and Enteral Nutrition and the Dietary Reference Intakes. METHODS: Children with an expected duration of mechanical ventilation ≥72 h were prospectively recruited. TUN was measured by chemiluminescence, and REE was measured by indirect calorimetry. Generalised linear models for longitudinal data were used to study the relation between protein intake and nitrogen balance and to calculate the minimum intake of protein needed to achieve nitrogen equilibrium. A similar approach was used for energy. Results were compared to the recommended values. RESULTS: Based on 402 measurements performed in 74 children (median age: 21 months), the mean TUN was high at 0.20 (95% CI: 0.20, 0.22) g/kg/d and the REE was 55 (95% CI: 54, 57) kcal/kg/d. Nitrogen and energy balances were achieved with 1.5 (95% CI: 1.4, 1.6) g/kg/d of protein and 58 (95% CI: 53, 63) kcal/kg/d for the entire group, but there were differences among children of different ages. Children required more protein and less energy than the Dietary Reference Intakes. CONCLUSIONS: In critically ill children, TUN was elevated and REE was reduced during the entire period of mechanical ventilation. Minimum intakes of 1.5 g/kg/d of protein and 58 kcal/kg/d can equilibrate nitrogen and energy balances in children up to 4 years old. Older children require more protein.

Much like the first volume published in 2011, the second volume of this anthology series gathers a selection of analyses which are empirically based on the data of the Swiss panel study TREE (Transitions from Education to Employment). The contributions of this volume carry on the investigation of the critical transitions during youth and young adulthood, drawing on sociological, economic, psychological and pedagogical research questions and thus highlighting the analytic and pluri-disciplinary research potential of the TREE data. One of the topical foci is the long-term influence of social origin on education and labour market pathways, particularly with regard to access to higher education.
