BACKGROUND. Glomerular hyperfiltration (GHF) is a well-recognized early renal alteration in diabetic patients. As the prevalence of GHF is largely unknown in populations in the African region with respect to normal fasting glucose (NFG), impaired fasting glucose (IFG) and type 2 diabetes [diabetes mellitus (DM)], we conducted a cross-sectional study in the Seychelles islands among families including at least one member with hypertension. METHODS. The glomerular filtration rate (GFR), effective renal plasma flow (ERPF) and proximal tubular sodium reabsorption were measured using inulin, p-aminohippurate (PAH) and endogenous lithium clearance, respectively. Twenty-four-hour urine was collected on the preceding day. RESULTS. Of the 363 participants (mean age 44.7 years), 6.6% had IFG, 9.9% had DM and 63.3% had hypertension. The prevalence of GHF, defined as a GFR >140 ml/min, was 17.2%, 29.2% and 52.8% in NFG, IFG and DM, respectively (P trend <0.001). Compared to NFG, the adjusted odds ratio for GHF was 1.99 [95% confidence interval (CI) 0.73-5.44] for IFG and 5.88 (2.39-14.45) for DM. Lithium clearance and fractional excretion of lithium were lower in DM and IFG than NFG (P < 0.001). CONCLUSION. In this population of African descent, subjects with impaired fasting glucose or type 2 diabetes had a high prevalence of GHF and enhanced proximal sodium reabsorption. These findings provide further insight on the elevated incidence of nephropathy reported among African diabetic individuals.

The case of a man exposed during 25 years to vibration while maneuvering a heavy earth moving tractor is reported. The first clinical manifestation of hand-arm vibration syndrome was a bilateral Raynaud's phenomenon followed five years later by digital necrosis. The arteriography revealed a proximal and bilateral ulnar artery occlusion. Bilateral median nerve conduction abnormalities were also present. Vibration exposure level was much higher than the threshold level proposed by the European Commission. Laboratory examinations for vasculitis and other vascular diseases were all negative. The purpose of this report is to show that vibration can be responsible for proximal occlusion of a medium sized artery and severe neurovascular abnormalities which must be distinguished from the usual vasospastic Raynaud's phenomenon and the classical hypothenar hammer syndrome. An early and correct diagnosis is crucial because continued repetitive trauma can result in irreversible ischemic injury and loss of digits.

Stoppa-Vaucher S, Ayabe T, Paquette J, Patey N, Francoeur D, Vuissoz J-M, Deladoëy J, Samuels ME, Ogata T, Deal CL. 46, XY gonadal dysgenesis: new SRY point mutation in two siblings with paternal germ line mosaicism. Familial recurrence risks are poorly understood in cases of de novo mutations. In the event of parental germ line mosaicism, recurrence risks can be higher than generally appreciated, with implications for genetic counseling and clinical practice. In the course of treating a female with pubertal delay and hypergonadotropic hypogonadism, we identified a new missense mutation in the SRY gene, leading to somatic feminization of this karyotypically normal XY individual. We tested a younger sister despite a normal onset of puberty, who also possessed an XY karyotype and the same SRY mutation. Imaging studies in the sister revealed an ovarian tumor, which was removed. DNA from the father's blood possessed the wild type SRY sequence, and paternity testing was consistent with the given family structure. A brother was 46, XY with a wild type SRY sequence strongly suggesting paternal Y-chromosome germline mosaicism for the mutation. In disorders of sexual development (DSDs), early diagnosis is critical for optimal psychological development of the affected patients. In this case, preventive karyotypic screening allowed early diagnosis of a gonadal tumor in the sibling prior to the age of normal puberty. Our results suggest that cytological or molecular diagnosis should be applied for siblings of an affected DSD individual.

BACKGROUND: Segmental handling of sodium along the proximal and distal nephron might be heritable and different between black and white participants. METHODS: We randomly recruited 95 nuclear families of black South African ancestry and 103 nuclear families of white Belgian ancestry. We measured the (FENa) and estimated the fractional renal sodium reabsorption in the proximal (RNaprox) and distal (RNadist) tubules from the clearances of endogenous lithium and creatinine. In multivariable analyses, we studied the relation of RNaprox and RNadist with FENa and estimated the heritability (h) of RNaprox and RNadist. RESULTS: Independent of urinary sodium excretion, South Africans (n = 240) had higher RNaprox (unadjusted median, 93.9% vs. 81.0%; P < 0.001) than Belgians (n = 737), but lower RNadist (91.2% vs. 95.1%; P < 0.001). The slope of RNaprox on FENa was steeper in Belgians than in South Africans (-5.40 +/- 0.58 vs. -0.78 +/- 0.58 units; P < 0.001), whereas the opposite was true for the slope of RNadist on FENa (-3.84 +/- 0.19 vs. -13.71 +/- 1.30 units; P < 0.001). h of RNaprox and RNadist was high and significant (P < 0.001) in both countries. h was higher in South Africans than in Belgians for RNaprox (0.82 vs. 0.56; P < 0.001), but was similar for RNadist (0.68 vs. 0.50; P = 0.17). Of the filtered sodium load, black participants reabsorb more than white participants in the proximal nephron and less postproximally. CONCLUSION: Segmental sodium reabsorption along the nephron is highly heritable, but the capacity for regulation in the proximal and postproximal tubules differs between whites and blacks.

INTRODUCTION: Primary venous aneurysm is a rare, but essential consideration in the differential diagnosis of an inguinal and femoral hernia. METHODS: We report a case of a 43-year-old man who was referred for evaluation and treatment of a femoral hernia. RESULTS: The patient presented with a 3-month history of an asymptomatic tumor on his right upper inner thigh. Physical examination noted a non-tender, non-indurated tumor. CONCLUSION: Surgical exploration demonstrated a primary venous aneurysm of the proximal saphenous vein.

Dans la schizophrénie, la religion a surtout été étudiée sous son aspect pathologique. Or, de nombreux patients s'appuient sur leurs ressources spirituelles pour faire face à leur maladie. Nous avons développé une grille d'entretien pour évaluer les relations entre spiritualité, pratiques religieuses et coping. Cet instrument a démontré que la religion a des effets positifs pour 71% et négatifs pour 14% des 115 patients évalués. La religion influence l'image de soi, les symptômes, l'adaptation psychosociale, la toxicomanie, les tentatives de suicide et l'adhérence au traitement. Elle devrait donc être systématiquement évaluée par le praticien. Notre instrument a démontré une bonne fidélité inter-juge et une validité théorique. Il ne nécessite pas de formation préalable. C'est pourquoi, nous le mettons à disposition des praticiens.

The present paper studies the probability of ruin of an insurer, if excess of loss reinsurance with reinstatements is applied. In the setting of the classical Cramer-Lundberg risk model, piecewise deterministic Markov processes are used to describe the free surplus process in this more general situation. It is shown that the finite-time ruin probability is both the solution of a partial integro-differential equation and the fixed point of a contractive integral operator. We exploit the latter representation to develop and implement a recursive algorithm for numerical approximation of the ruin probability that involves high-dimensional integration. Furthermore we study the behavior of the finite-time ruin probability under various levels of initial surplus and security loadings and compare the efficiency of the numerical algorithm with the computational alternative of stochastic simulation of the risk process. Abstract Animal behaviours or structures are used by senders as signals to try to increase their fitness by altering the behaviour of receivers. A large fraction of studies on sexual selection have focussed on male ornaments and have demonstrated that these ornaments signal the quality of their owner and are used by female for mate choice. Although females can also exhibit conspicuous traits, studies on female ornaments are markedly lacking. In chapter 1, we show that female starlings are showier on chest whiteness than males and that females' whiteness may potentially indicate female condition at the start of breeding and provide fitness advantages to breeding birds. Furthermore we point out that feather density and abrasion are important factors shaping the expression of chest whiteness. This suggests that further understanding of the evolution of chest whiteness in Starlings requires to examine the environmental and physiological factors that shape feather condition. Plumage may suffer from damage through abrasion and bacterial activity. In chapter 2, we focus on factors that influence feather-degrading bacterial communities. Within the hypothesis that parental care can be trade-off against the demands of self-maintenance, we show that a brood size manipulation modifies the structure of feather-degrading bacterial communities and the density of free- living bacteria. Thus we have pointed out a potentially poorly known cost of reproduction. In the same context of a trade-off between reproductive activities and individual self-maintenance, chapter 3 shows that at a proximate level in females but not in males, the individual variation in time and/or energy allocated in reproductive activities is associated with prolactin hormone levels. Our study provides evidence for the existence of a sex related difference in the relationship between brood size and prolactin levels. Birds have evolved sanitation behaviours and preen gland secretions to preserve the condition of their plumage. In chapter 4, we describe a method that allows to measure preen gland in situ. Then we use this method to characterize a number of phenotypic and ecological factors that explain variation in preen gland size in free-living individuals. In parent-offspring interactions, parents use offspring signals to provision their brood. In chapter 5, we demonstrate that nestling flanges and body skin reflect in the ultra-violet (UV) wavelengths ant that parents use this UV reflectance in food allocation decisions. Résumé Certains comportements et structures chez les animaux agissent, pour ceux qui les émettent, comme des signaux permettant d'augmenter leur fitness en altérant les comportements de ceux qui les perçoivent. Une grande partie des études sur la sélection sexuelle s'est focalisée sur les ornements mâles. Ces études ont démontré que ces ornements pouvaient signaler la qualité de celui qui les porte et influencer le choix des femelles. Bien que les femelles puissent aussi présenter des traits voyants, les études sur leurs ornements font défaut. Dans le chapitre 1 de ce travail, nous montrons que les étourneaux femelles sont plus voyantes que les mâles sur la base de la blancheur de la poitrine. De plus la blancheur des femelles peut signaler leur condition au début de la saison de reproduction et ainsi être corrélée avec leur fitness. Nous mettons aussi en évidence que la densité et l'abrasion des plumes sont des facteurs importants, contrôlant l'expression de la blancheur de la poitrine. Ceci suggère que des études futures pourraient examiner le rôle des facteurs environnementaux et physiologiques qui influencent la condition des plumes pour mieux comprendre l'évolution de la blancheur chez les étourneaux. Le plumage subit des dommages à travers l'abrasion et probablement aussi par l'activité de dégradation de bactéries. Dans le chapitre 2 de ce travail, nous nous intéressons aux facteurs qui influencent les communautés de bactéries dégradant les plumes. Nous basant sur l'hypothèse selon laquelle il existe un compromis entre les soins parentaux et la maintenance corporelle, nous montrons qu'une manipulation de la taille de nichée modifie la structure des communautés de bactéries dégradant les plumes ainsi que les densités de bactéries libres présentes sur le plumage. Ainsi nous mettons en évidence un coût encore peu connu des activités de reproduction. Dans le même contexte, nous montrons, dans le chapitre 3, que des variations individuelles dans l'énergie et/ou le temps alloué dans les activités de reproduction sont associés, chez les femelles, à un niveau proximal à l'hormone prolactine. Cette relation n'est pas présente chez les mâles. Cette étude montre que la relation entre la taille de nichée et les niveaux de prolactine diffère avec le sexe des individus. Les oiseaux utilisent des comportements de nettoyage associés aux sécrétions de la glande uropygiale afin de préserver la condition de leurs plumes. Dans le chapitre 4 de ce travail, nous décrivons une méthode qui permet de mesurer la taille de la glande in situ. Puis nous caractérisons certains facteurs écologiques et physiologiques qui expliquent les variations de la taille de la glande chez des individus capturés dans leur environnement. Les parents nourrissent leur progéniture en réponse à des signaux émis par ceux-ci. Dans le chapitre 5 de ce travail, nous démontrons que les commissures et la peau sur le corps des oisillons reflètent la lumière dans l'ultraviolet. Nous montrons que les parents utilisent cette réflexion dans l'ultraviolet lors de l'allocation de nourriture pour leurs jeunes.

Executive Summary The unifying theme of this thesis is the pursuit of a satisfactory ways to quantify the riskureward trade-off in financial economics. First in the context of a general asset pricing model, then across models and finally across country borders. The guiding principle in that pursuit was to seek innovative solutions by combining ideas from different fields in economics and broad scientific research. For example, in the first part of this thesis we sought a fruitful application of strong existence results in utility theory to topics in asset pricing. In the second part we implement an idea from the field of fuzzy set theory to the optimal portfolio selection problem, while the third part of this thesis is to the best of our knowledge, the first empirical application of some general results in asset pricing in incomplete markets to the important topic of measurement of financial integration. While the first two parts of this thesis effectively combine well-known ways to quantify the risk-reward trade-offs the third one can be viewed as an empirical verification of the usefulness of the so-called "good deal bounds" theory in designing risk-sensitive pricing bounds. Chapter 1 develops a discrete-time asset pricing model, based on a novel ordinally equivalent representation of recursive utility. To the best of our knowledge, we are the first to use a member of a novel class of recursive utility generators to construct a representative agent model to address some long-lasting issues in asset pricing. Applying strong representation results allows us to show that the model features countercyclical risk premia, for both consumption and financial risk, together with low and procyclical risk free rate. As the recursive utility used nests as a special case the well-known time-state separable utility, all results nest the corresponding ones from the standard model and thus shed light on its well-known shortcomings. The empirical investigation to support these theoretical results, however, showed that as long as one resorts to econometric methods based on approximating conditional moments with unconditional ones, it is not possible to distinguish the model we propose from the standard one. Chapter 2 is a join work with Sergei Sontchik. There we provide theoretical and empirical motivation for aggregation of performance measures. The main idea is that as it makes sense to apply several performance measures ex-post, it also makes sense to base optimal portfolio selection on ex-ante maximization of as many possible performance measures as desired. We thus offer a concrete algorithm for optimal portfolio selection via ex-ante optimization over different horizons of several risk-return trade-offs simultaneously. An empirical application of that algorithm, using seven popular performance measures, suggests that realized returns feature better distributional characteristics relative to those of realized returns from portfolio strategies optimal with respect to single performance measures. When comparing the distributions of realized returns we used two partial risk-reward orderings first and second order stochastic dominance. We first used the Kolmogorov Smirnov test to determine if the two distributions are indeed different, which combined with a visual inspection allowed us to demonstrate that the way we propose to aggregate performance measures leads to portfolio realized returns that first order stochastically dominate the ones that result from optimization only with respect to, for example, Treynor ratio and Jensen's alpha. We checked for second order stochastic dominance via point wise comparison of the so-called absolute Lorenz curve, or the sequence of expected shortfalls for a range of quantiles. As soon as the plot of the absolute Lorenz curve for the aggregated performance measures was above the one corresponding to each individual measure, we were tempted to conclude that the algorithm we propose leads to portfolio returns distribution that second order stochastically dominates virtually all performance measures considered. Chapter 3 proposes a measure of financial integration, based on recent advances in asset pricing in incomplete markets. Given a base market (a set of traded assets) and an index of another market, we propose to measure financial integration through time by the size of the spread between the pricing bounds of the market index, relative to the base market. The bigger the spread around country index A, viewed from market B, the less integrated markets A and B are. We investigate the presence of structural breaks in the size of the spread for EMU member country indices before and after the introduction of the Euro. We find evidence that both the level and the volatility of our financial integration measure increased after the introduction of the Euro. That counterintuitive result suggests the presence of an inherent weakness in the attempt to measure financial integration independently of economic fundamentals. Nevertheless, the results about the bounds on the risk free rate appear plausible from the view point of existing economic theory about the impact of integration on interest rates.

Isolated nonperfused rabbit renal proximal tubules were used to investigate the basolateral step of transport of the organic cation N1-methylnicotinamide (NMN). NMN accumulation was highest and saturable in S2 and S3 segments, but lowest and nonsaturable in S1 segments. In S1 segments, accumulation of [3H]-NMN (0.5-8 microM in the bath) resulted in an average tubular water/medium concentration ratio (T/M) of 8.2, whereas in S2 and S3 segments T/M averaged 19.5 and 18.6, respectively. At these concentrations, about 30% of the label was attached in all segments to a metabolite comigrating with nicotinamide. KCN (10(-2) M) or ouabain (10(-4) M) reduced T/M to about 8 for all segments. NMN accumulation was inhibited (to a T/M of about 3 with mepiperphenidol) by other organic cations (10(-5)-10(-3) M) with the potency sequence mepiperphenidol greater than tetraethylammonium = quinine greater than morphine, these organic cations having no effect on p-aminohippurate accumulation, except for the highest concentration of quinine (10(-3) M). After correction for metabolism, NMN accumulation could be accounted for by simple electrochemical equilibrium across the basolateral membrane. The basolateral step of NMN transport appears therefore to be a carrier-mediated diffusion, in opposition to the active basolateral accumulation described for tetraethylammonium.
