Syndrome métabolique, diabète sucré et vulnérabilité: une approche "syndémique" de la maladie chronique [Metabolic syndrome, diabetes mellitus and vulnerability: a syndemic approach of chronic diseases].

The pandemic metabolic syndrome is generally attributed to our lifestyle. The current therapeutic strategies are centered on the behavioral changes and pharmacotherapy. A deeply analysis reveals the importance of the socio-cultural determinants with a "dose-responses effect according to the socio-economic level. The "syndemic" theory, which puts at the same level the socio-cultural environment, the behaviors and biomedecine, suggests a more holistic approach. This theory suggests introducing other partners of care, such cultural-mediators and welfare workers trained in the care, to have finally an approach centered on the roots of the causes. The healthcare networks centered on the management of the costs of health should not forget the socio-cultural dimension, unless wanting to select the good cases.

Performer par le dispositif ? Un retour critique sur la théorie de la performativité.

L'objectif de cet article est de revenir sur un point saillant de la théorie de la performativité des énoncés économiques, c'est-à-dire l'importance qu'elle accorde aux dispositifs techniques dans l'explication de l'influence des théories économiques sur le monde social. Cette démarche débouche sur le refus de mobiliser les catégories sociales (institution, convention, norme sociale, etc.) dans les explications qu'elle produit. On soulignera ici l'origine de cette posture épistémologique, la théorie de l'acteur-réseau, avant d'en faire la critique. On montrera principalement que penser les dispositifs techniques nécessite impérativement de les penser au sein d'un contexte conventionnel particulier. Dès lors, le rejet des catégories sociales est épistémologiquement contre-productif dans les études de performativité

Influence des conditions de travail sur la santé des employés de production

Une intervention ergonomique a été réalisée dans la maison-mère d'un grand groupe suisse constructeur d'équipements. Les situations de travail qui ont été étudiées sont des ateliers de tôleries, de mécanique et de soudure. La demande d'intervention à l'IST (Institut universitaire romand de santé au travail) est partie du constat par la hiérarchie et par l'infirmière du travail qu'un nombre croissant d'employés de production souffraient de maux de dos et de stress et qu'il était nécessaire d'entreprendre des améliorations du travail afin de réduire ces symptômes. Dysfonction métabolique et stress chronique: un nouveau regard sur la pandémie de "diabésité" [Metabolic dysfunction and chronic stress: a new sight at "diabesity" pandemic]

Chronic stress in Western society can activate the autonomus, neuroendocrine and inflammatory/immunlogic systems. Chronic exposure to stressors can indeed stimulate the hypothalamic-pituitary-adrenal axis and induce a disbalance between anabolic and catabolic hormones, responsible of an increased in visceral fat and of insulin resistance. These metabolic consequences can lead to pre-diabetes. Exposure to chronic stress results in allostatic load and its pathophysiologic consequences. The knowledge of this mecanisms and the cardiovascular and metabolic risk related, should influence our way of thinking about patient care. To decrease allostatic load, practitioners can rely on therapeutic relation. Therapeutic education is one of the skill that can be use to create therapeutic relation.
