Idiopathic hypogonadotropic hypogonadism (IHH) is defined by absent or incomplete puberty and characterised biochemically by low levels of sex steroids, with low or inappropriately normal gonadotropin hormones. IHH is frequently accompanied by non-reproductive abnormalities, most commonly anosmia, which is present in 50-60% of cases and defines Kallmann syndrome. The understanding of IHH has undergone rapid evolution, both in respect of genetics and breadth of phenotype. Once considered in monogenic Mendelian terms, it is now more coherently understood as a complex genetic condition. Oligogenic and complex genetic-environmental interactions have now been identified, with physiological and environmental factors interacting in genetically susceptible individuals to alter the clinical course and phenotype. These potentially link IHH to ancient evolutionary pressures on the ancestral human genome.

Background: Publications from the International Breast Screening Network (IBSN) have shown that varying definitions create hurdles for comparison of screening performance. Interval breast cancer rates are particularly affected. Objective: to test whether variations in definition of interval cancer rates (ICR) affect comparisons of international ICR, specific to a comparison of ICR in Norway and North Carolina (NC). Methods: An interval cancer (IC) was defined as a cancer diagnosed following a negative screening mammogram in a defined follow-up period. ICR was calculated for women ages 50-69, at subsequent screening in Norway and NC, during the time period 1996 - 2002. ICR was defined using three different denominators (negative screens, negative final assessments and all screens) and three different numerators (DCIS, invasive cancer and all cancers). ICR was then calculated with two methods: 1) number of ICs divided by the number of screens, and ICs divided by the number of women-years at risk for IC. Results: There were no differences in ICR depending on the definition used. In the 1-12 month follow up period ICR (based on number of screens) were: 0.53, 0.54, and 0.54 for Norway; and 1.20, 1.25 and 1.17 for NC, for negative screens, negative final assessment and all screens, respectively: The same trend was seen for 13-24 and 1-24 months follow-up. Using women-years for the analysis did not change the trend. ICR was higher in NC compared to Norway under all definitions and in all follow-up time periods, regardless of calculation method. Conclusion: The ICR within or between Norway and NC did not differ by definition used. ICR were higher in NC than Norway. There are many potential explanations for the difference.

This paper introduces a nonlinear measure of dependence between random variables in the context of remote sensing data analysis. The Hilbert-Schmidt Independence Criterion (HSIC) is a kernel method for evaluating statistical dependence. HSIC is based on computing the Hilbert-Schmidt norm of the cross-covariance operator of mapped samples in the corresponding Hilbert spaces. The HSIC empirical estimator is very easy to compute and has good theoretical and practical properties. We exploit the capabilities of HSIC to explain nonlinear dependences in two remote sensing problems: temperature estimation and chlorophyll concentration prediction from spectra. Results show that, when the relationship between random variables is nonlinear or when few data are available, the HSIC criterion outperforms other standard methods, such as the linear correlation or mutual information.

BACKGROUND: Only 25% of IVF transfer cycles lead to a clinical pregnancy, calling for continued technical progress but also more in depth analysis of patients' individual characteristics. The interleukin-1 (IL-1) system and matrix metalloproteinases (MMPs) are strongly implicated in embryo implantation. The genes coding for IL-1Ra (gene symbol IL-1RN), IL-1beta, MMP2 and MMP9 bear functional polymorphisms. We analysed the maternal genetic profile at these polymorphic sites in IVF patients, to determine possible correlations with IVF outcome. METHODS: One hundred and sixty women undergoing an IVF cycle were enrolled and a buccal smear was obtained. The presence of IL-1RN variable number of tandem repeats and IL-1B + 3953, MMP2-1306 and MMP9-1562 single nucleotide substitutions were determined. Patients were divided into pregnancy failures (119), biochemical pregnancies (8) and clinical pregnancies (33). RESULTS: There was a 40% decrease in IL-1RN*2 allele frequency (P = 0.024) and a 45% decrease in IL-1RN*2 carrier status in the clinical pregnancy group as compared to the pregnancy failure group (P = 0.017). This decrease was still statistically significant after a multivariate logistic regression analysis. The likelihood of a clinical pregnancy was decreased accordingly in IL-1RN*2 carriers: odds ratio = 0.349, 95% confidence interval = 0.2-0.8, P = 0.017. The IL-1B, MMP2 and MMP9 polymorphisms showed no correlation with IVF outcome. CONCLUSIONS: IL-1RN*2 allele carriage is associated with a poor prognosis of achieving a pregnancy after IVF.

Question: When multiple observers record the same spatial units of alpine vegetation, how much variation is there in the records and what are the consequences of this variation for monitoring schemes to detect change? Location: One test summit in Switzerland (Alps) and one test summit in Scotland (Cairngorm Mountains). Method: Eight observers used the GLORIA protocols for species composition and visual cover estimates in percent on large summit sections (>100 m2) and species composition and frequency in nested quadrats (1 m2). Results: The multiple records from the same spatial unit for species composition and species cover showed considerable variation in the two countries. Estimates of pseudoturnover of composition and coefficients of variation of cover estimates for vascular plant species in 1m x 1m quadrats showed less variation than in previously published reports whereas our results in larger sections were broadly in line with previous reports. In Scotland, estimates for bryophytes and lichens were more variable than for vascular plants. Conclusions: Statistical power calculations indicated that, unless large numbers of plots were used, changes in cover or frequency were only likely to be detected for abundant species (exceeding 10% cover) or if relative changes were large (50% or more). Lower variation could be reached with the point methods and with larger numbers of small plots. However, as summits often strongly differ from each other, supplementary summits cannot be considered as a way of increasing statistical power without introducing a supplementary component of variance into the analysis and hence the power calculations.

The authors compared radial steady-state free precession (SSFP) coronary magnetic resonance (MR) angiography, cartesian k-space sampling SSFP coronary MR angiography, and gradient-echo coronary MR angiography in 16 healthy adults and four pilot study patients. Standard gradient-echo MR imaging with a T2 preparatory pulse and cartesian k-space sampling was the reference technique. Image quality was compared by using subjective motion artifact level and objective contrast-to-noise ratio and vessel sharpness. Radial SSFP, compared with cartesian SSFP and gradient-echo MR angiography, resulted in reduced motion artifacts and superior vessel sharpness. Cartesian SSFP resulted in increased motion artifacts (P <.05). Contrast-to-noise ratio with radial SSFP was lower than that with cartesian SSFP and similar to that with the reference technique. Radial SSFP coronary MR angiography appears preferable because of improved definition of vessel borders.

Dans le contexte d'un climat de plus en plus chaud, une étude « géosystémique » de la répartition du pergélisol dans l'ensemble d'un versant périglaciaire alpin, de la paroi rocheuse jusqu'au glacier rocheux, s'avère primordiale. S'insérant dans cette problématique, ce travail de thèse vise comme objectif général l'étude des versants d'éboulis situés à l'intérieur de la ceinture du pergélisol discontinu selon deux volets de recherche différents : une étude de la stratigraphie et de la répartition du pergélisol dans les éboulis de haute altitude et des processus qui lui sont associés ; une reconstitution de l'histoire paléoenvironnementale du domaine périglaciaire alpin pendant le Tardiglaciaire et l'Holocène. La stratigraphie et la répartition spatiale du pergélisol a été étudiée dans cinq éboulis des Alpes Valaisannes (Suisse), dont trois ont fait l'objet de forages profonds, grâce à la prospection géophysique de détail effectuée à l'aide de méthodes thermiques, de résistivité, sismiques et nucléaires. Les mesures effectuées ont permis de mettre en évidence que, dans les cinq éboulis étudiés, la répartition du pergélisol est discontinue et aucun des versants n'est intégralement occupé par du pergélisol. En particulier, il a été possible de prouver de manière directe que, dans un éboulis, le pergélisol est présent dans les parties inférieures du versant et absent dans les parties supérieures. Trois facteurs de contrôle principaux de la répartition du pergélisol déterminée au sein des éboulis étudiés ont été individualisés, pouvant agir seuls ou de manière combinée : la ventilation ascendante, l'augmentation de la granulométrie en direction de l'aval et la redistribution de la neige par le vent et les avalanches. Parmi ceux-ci, la relation ventilation-granulométrie semble être le facteur de contrôle principal permettant d'expliquer la présence de pergélisol dans les parties inférieures d'un éboulis et son absence dans les parties supérieures. Enfin, l'analyse de la structure des éboulis périglaciaires de haute altitude a permis de montrer que la stratigraphie du pergélisol peut être un élément important pour l'interprétation de la signification paléoclimatique de ce type de formes. Pour le deuxième volet de la recherche, grâce aux datations relatives effectuées à l'aide de l'utilisation conjointe de la méthode paléogéographique et du marteau de Schmidt, il a été possible de définir la chrono-stratigraphie du retrait glaciaire et du développement des glaciers rocheux et des versants d'éboulis des quatre régions des Alpes suisses étudiées (régions du Mont Gelé - Mont Fort, des Fontanesses et de Chamosentse, dans les Alpes Valaisannes, et Massif de la Cima di Gana Bianca, dans les Alpes Tessinoises). La compilation de toutes les datations effectuées a permis de montrer que la plupart des glaciers rocheux actifs étudiés se seraient développés soit juste avant et/ou pendant l'Optimum Climatique Holocène de 9.5-6.3 ka cal BP, soit au plus tard juste après cet évènement climatique majeur du dernier interglaciaire. Parmi les glaciers rocheux fossiles datés, la plupart aurait commencé à se former dans la deuxième moitié du Tardiglaciaire et se serait inactivé dans la première partie de l'Optimum Climatique Holocène. Pour les éboulis étudiés, les datations effectuées ont permis d'observer que leur surface date de la période entre le Boréal et l'Atlantique récent, indiquant que les taux d'éboulisation après la fin de l'Optimum Climatique Holocène ont dû être faibles, et que l'intervalle entre l'âge maximal et l'âge minimal est dans la plupart des cas relativement court (4-6 millénaires), indiquant que les taux d'éboulisation durant la période de formation des éboulis ont dû être importants. Grâce au calcul des taux d'érosion des parois rocheuses sur la base du volume de matériaux rocheux pour quatre des éboulis étudiés, il a été possible mettre en évidence l'existence d'une « éboulisation parapériglaciaire » liée à la dégradation du pergélisol dans les parois rocheuses, fonctionnant principalement durant les périodes de réchauffement climatique rapide comme cela a été le cas au début du Bølling, du Préboréal à la fin de l'Atlantique récent et, peut-être, à partir des années 1980. - In the context of a warmer climate, a « geosystemical » study of the permafrost distribution in a whole alpine periglacial hillslope, from the rockwall to the rockglacier, is of great importance. With respect to this problem, the general objective of this PhD thesis is the global study of talus slopes located within the alpine periglacial belt following two different research axes: the analysis of the internal structure and of the permafrost distribution of high altitude talus slopes and of the related processes; the reconstruction of the palaeoenvironmental history of the alpine periglacial belt during the Lateglacial and the Holocene. The stratigraphy and the permafrost distribution were studied in five talus slopes of the Valais Alps (Switzerland) with the analysis of borehole data (on three of the five talus slopes) and other methods of permafrost prospecting: Electrical Resistivity Tomography (ERT), Refraction Seismic Tomography (RST) and nuclear well logging. The collected data shows that, in all of the studied talus slopes, permafrost distribution is discontinuous and that neither of the hillslopes is integrally characterised by permafrost. In particular, this data proves by direct investigations that, in talus slopes, permafrost is present in the lower parts of the hillslope, whereas it is absent in the upper parts. Permafrost distribution in alpine talus slopes is depending of the combination of almost three controlling factors, whose respective importance is variable: the chimney effect, the increase of grain size downslope and the redistribution of snow by avalanches. Depending on the size of the talus and on topographical and geomorphological heterogeneities, various cases are possible: one dominant controlling factor or the combination of various factors. Nevertheless, it would be an error to consider each controlling factor independently, without considering their relationships. Between these controlling factors, the relationship chimney effect/grain size seems to be the most important factor controlling the presence of permafrost in the lowest part of periglacial talus slopes, and its absence in the upper parts. Finally, the analysis of the talus structure shows that the permafrost stratigraphy may be an important element of interpretation of the palaeoclimatic significance of an alpine talus slope. The second research axe focused on the establishment of a chronology of the Lateglacial glacier retreat and the dating of rockglaciers and talus slopes development in four studied regions of the Swiss Alps (Mont Gelé - Mont Fort, Fontanesses and Chamosentse regions, in the Valais Alps, and the Cima di Gana Bianca Massif, in the Ticino Alps). The compilation of the dates acquired through the combination of the palaeogeographical method and of the Schmidt hammer indicates that most of the investigated active rockglaciers started to evolve during the early phases of the Holocene or, at the latest, after the early-to-mid Holocene Climatic Optimum (ending around 6.3 ka cal BP). For the dated relict rockglaciers, most of them started to evolve in the second half of the Lateglacial, and probably became inactive at the beginning of the Holocene Climatic Optimum. For the investigated talus slopes, the relative dating carried out allowed to show that their surface date from the period included between the Boreal and the end of the Atlantic, pointing out that the rockwall retreat after the end of the Holocene Climatic Optimum was weak, and that the interval between maximal and minimal ages is in most cases relatively short (4-6 millennia). Therefore, the rockwall retreat during the development period of the talus slopes must has been considerable. Thanks to the calculation of rockwall erosion rates based on the volume of talus accumulations for four of the investigated hillslopes, it was possible to find evidences of the existence of "paraperiglacial rockfall phases" related to the permafrost degradation in rockwalls. These phases coincide with rapid climate warming periods, as at the beginning of the Bølling, during the Preboreal or, maybe, since 1980.

Captan and folpet are two fungicides largely used in agriculture, but biomonitoring data are mostly limited to measurements of captan metabolite concentrations in spot urine samples of workers, which complicate interpretation of results in terms of internal dose estimation, daily variations according to tasks performed, and most plausible routes of exposure. This study aimed at performing repeated biological measurements of exposure to captan and folpet in field workers (i) to better assess internal dose along with main routes-of-entry according to tasks and (ii) to establish most appropriate sampling and analysis strategies. The detailed urinary excretion time courses of specific and non-specific biomarkers of exposure to captan and folpet were established in tree farmers (n = 2) and grape growers (n = 3) over a typical workweek (seven consecutive days), including spraying and harvest activities. The impact of the expression of urinary measurements [excretion rate values adjusted or not for creatinine or cumulative amounts over given time periods (8, 12, and 24 h)] was evaluated. Absorbed doses and main routes-of-entry were then estimated from the 24-h cumulative urinary amounts through the use of a kinetic model. The time courses showed that exposure levels were higher during spraying than harvest activities. Model simulations also suggest a limited absorption in the studied workers and an exposure mostly through the dermal route. It further pointed out the advantage of expressing biomarker values in terms of body weight-adjusted amounts in repeated 24-h urine collections as compared to concentrations or excretion rates in spot samples, without the necessity for creatinine corrections.

Hematocrit (Hct) is one of the most critical issues associated with the bioanalytical methods used for dried blood spot (DBS) sample analysis. Because Hct determines the viscosity of blood, it may affect the spreading of blood onto the filter paper. Hence, accurate quantitative data can only be obtained if the size of the paper filter extracted contains a fixed blood volume. We describe for the first time a microfluidic-based sampling procedure to enable accurate blood volume collection on commercially available DBS cards. The system allows the collection of a controlled volume of blood (e.g., 5 or 10 μL) within several seconds. Reproducibility of the sampling volume was examined in vivo on capillary blood by quantifying caffeine and paraxanthine on 5 different extracted DBS spots at two different time points and in vitro with a test compound, Mavoglurant, on 10 different spots at two Hct levels. Entire spots were extracted. In addition, the accuracy and precision (n = 3) data for the Mavoglurant quantitation in blood with Hct levels between 26% and 62% were evaluated. The interspot precision data were below 9.0%, which was equivalent to that of a manually spotted volume with a pipet. No Hct effect was observed in the quantitative results obtained for Hct levels from 26% to 62%. These data indicate that our microfluidic-based sampling procedure is accurate and precise and that the analysis of Mavoglurant is not affected by the Hct values. This provides a simple procedure for DBS sampling with a fixed volume of capillary blood, which could eliminate the recurrent Hct issue linked to DBS sample analysis.

OBJECTIVE: Accuracy studies of Patient Safety Indicators (PSIs) are critical but limited by the large samples required due to low occurrence of most events. We tested a sampling design based on test results (verification-biased sampling [VBS]) that minimizes the number of subjects to be verified. METHODS: We considered 3 real PSIs, whose rates were calculated using 3 years of discharge data from a university hospital and a hypothetical screen of very rare events. Sample size estimates, based on the expected sensitivity and precision, were compared across 4 study designs: random and VBS, with and without constraints on the size of the population to be screened. RESULTS: Over sensitivities ranging from 0.3 to 0.7 and PSI prevalence levels ranging from 0.02 to 0.2, the optimal VBS strategy makes it possible to reduce sample size by up to 60% in comparison with simple random sampling. For PSI prevalence levels below 1%, the minimal sample size required was still over 5000. CONCLUSIONS: Verification-biased sampling permits substantial savings in the required sample size for PSI validation studies. However, sample sizes still need to be very large for many of the rarer PSIs.

Introduction : Multimorbidity (MM) is currently a major health concern for hospitalized patients but little is known about the relative importance of MM in the general population. Accordingly we assessed whether MM could be a good predictor of overall mortality. Method : Data from the population based CoLaus Study: 3239 participants (1731 women, mean age 50+/-9 years) followed for a median time of 5.4 years (range 0.4 to 8.5 years). MM was defined as presenting >=2 morbidities according to Barnett et al. (27 items, measured data). Survival analysis was conducted using Cox regression. Results : During follow-up, 53 (1.6%) participants died. Participants who died had a higher number of morbidities (2.4 +/- 1.6 vs. 1.9 +/- 1.5, p<0.05) and had a higher prevalence of MM (69.8% vs. 55.9%, p<0.05). On bivariate analysis, presence of MM (defined as a yes/no variable) was significantly related with overall mortality: relative risk (RR) of 1.84, 95% confidence interval [1.02; 3.31], p<0.05 (see figure), but this association became non-significant after adjusting for age, gender and smoking: RR=1.68 [0.93; 3.04], p=0.09. Similar results were obtained when using the number of morbidities: RR for an extra morbidity 1.22 [1.05; 1.44], p<0.02; after adjusting for age, gender and smoking, RR=1.16 [0.99; 1.37], p=0.07. Conclusion : During a short 5 year observation period, measured MM in the general population is associated with overall mortality. This association becomes borderline significant after multivariate adjustment. These observations will have to be confirmed during a longer follow-up period. This increased mortality in MM patients may require developing specific strategies of screening and prevention.
