RESUMECette recherche empirique porte sur les emplois temporaires subventionnés (ETS) instaurés dans le cadre du chômage et de l'aide sociale en Suisse depuis une dizaine d'années. La mise en place de politiques d'activation dans le cadre de la protection sociale met l'accent sur les liens explicites, souvent réglementaires, qu'entretiennent actuellement la protection sociale, les politiques de l'emploi et le marché du travail dans les pays industrialisés. Ces transformations ont largement contribué au développement d'activités exercées en marge du marché de l'emploi. Dans le cadre du chômage, comme dans celui de l'aide sociale, une mise au travail peut être exigée en contrepartie du versement des indemnités ; en Suisse, on nomme ce procédé l'assignation au travail. L'assignation est le processus par lequel un-e conseiller?ère en placement peut contraindre, sous peine de sanction (suppression des indemnités pour un temps déterminé) une personne au chômage (inscrit-e auprès d'un office régional de placement) à souscrire à une mesure du marché du travail (MMT), particulièrement les ETS.Cette recherche propose une analyse de l'assignation à un programme d'emploi temporaire sous l'éclairage de la sociologie du travail. Elle adopte une perspective compréhensive attentive aux tensions que vivent les individus pris dans une situation de travail hybride et inédite qui les place aux frontières des différentes catégories administratives de chômage, d'inactivité et de population occupée. Partant d'une étude empirique auprès de personnes assignées, cette recherche mène une analyse qualitative des conditions et de l'organisation du travail en ETS, des modalités contractuelles et statutaires des personnes assignées à une activité de travail contrainte et matériellement non reconnue, puisque non salariée. Elle s'attache également à cerner le vécu de l'assignation au travail, ainsi que le sens que les personnes lui attribuent dans leur trajectoire biographique et professionnelle.SUMMARYThis research investigates a specific and new form of labor, namely «temporary subsidized jobs» {emplois temporaires subventionnés, ETS) that have been developed since the late 1990s in Switzerland in the context of unemployment and social assistance benefits. Although ETS are specific to Switzerland, they echo similar «workfare» measures imposed on unemployed and welfare recipients introduced in recent years in almost all industrialized countries. Indeed, the evolution of public policies and the generalization of «active labour-market policies» {politiques d'activation) have become central to social protection in the majority of Western countries and have emerged in parallel to the expansion of work activities situated at the margins of traditional wage- labor.My analysis of the ETS phenomenon is informed by labor sociology and discusses the pertinence of a classical approach in grasping this hybrid and new work situation, which sets individual persons at the border between various administrative categories such as «unemployed», «inactive» and «at work». I investigate in particular the issue of contractual and statutory modalities imposed on persons who have been assigned to a form of activity that is both constraining as well as materially non-recognized (as it remains outside of traditional wage-labor forms). In order to understand ETS conditions and labor organization, my fieldwork consist of interviews of persons who have been assigned to it and observations. I investigate their personal experience, as well as the meaning that these individuals attribute to the ETS experience in the context of their biographical and professional trajectory.

Le Programme de dépistage du cancer du sein BEJUNE a démarré en mai 2005 dans le canton du Jura, s'est étendu au canton de Neuchâtel en mai 2007 et englobe le Jura bernois depuis janvier 2009. Ses buts sont de promouvoir, organiser, gérer et mener à bien une action de dépistage auprès de la population féminine dans la tranche d'âge de 50 à 69 ans révolus (environ 42'000 femmes). [...] Ce rapport constitue, après 5 ans et demi d'activité, la première évaluation épidémiologique du Programme de dépistage du cancer du sein BEJUNE. Cette évaluation a été réalisée par l'Unité d'épidémiologie du cancer (UEC) de l'Institut universitaire de médecine sociale et préventive de Lausanne (IUMSP) qui jouit d'une longue expérience dans le domaine. Dans ce rapport, on analyse le fonctionnement, la participation, la qualité et l'efficacité précoce du programme. Les indicateurs de performance retenus suivent les recommandations européennes et les directives émises par la Fédération suisse des programmes de dépistage du cancer du sein. Entre mai 2005 et novembre 2010, plus de 37'000 mammographies ont été réalisées dans 11 Centres/Instituts de radiologie. Les 6750 examens effectués annuellement correspondent à un volume de 420 mammographies par radiologue: 380 pour les 1e lecteurs et 1300 pour les 2e lecteurs (auxquels s'ajoutent les lectures réalisées dans le cadre d'un autre programme cantonal). BACKGROUND: Clinical practice does not always reflect best practice and evidence, partly because of unconscious acts of omission, information overload, or inaccessible information. Reminders may help clinicians overcome these problems by prompting the doctor to recall information that they already know or would be expected to know and by providing information or guidance in a more accessible and relevant format, at a particularly appropriate time. OBJECTIVES: To evaluate the effects of reminders automatically generated through a computerized system and delivered on paper to healthcare professionals on processes of care (related to healthcare professionals' practice) and outcomes of care (related to patients' health condition). SEARCH METHODS: For this update the EPOC Trials Search Co-ordinator searched the following databases between June 11-19, 2012: The Cochrane Central Register of Controlled Trials (CENTRAL) and Cochrane Library (Economics, Methods, and Health Technology Assessment sections), Issue 6, 2012; MEDLINE, OVID (1946- ), Daily Update, and In-process; EMBASE, Ovid (1947- ); CINAHL, EbscoHost (1980- ); EPOC Specialised Register, Reference Manager, and INSPEC, Engineering Village. The authors reviewed reference lists of related reviews and studies.  SELECTION CRITERIA: We included individual or cluster-randomized controlled trials (RCTs) and non-randomized controlled trials (NRCTs) that evaluated the impact of computer-generated reminders delivered on paper to healthcare professionals on processes and/or outcomes of care. DATA COLLECTION AND ANALYSIS: Review authors working in pairs independently screened studies for eligibility and abstracted data. We contacted authors to obtain important missing information for studies that were published within the last 10 years. For each study, we extracted the primary outcome when it was defined or calculated the median effect size across all reported outcomes. We then calculated the median absolute improvement and interquartile range (IQR) in process adherence across included studies using the primary outcome or median outcome as representative outcome. MAIN RESULTS: In the 32 included studies, computer-generated reminders delivered on paper to healthcare professionals achieved moderate improvement in professional practices, with a median improvement of processes of care of 7.0% (IQR: 3.9% to 16.4%). Implementing reminders alone improved care by 11.2% (IQR 6.5% to 19.6%) compared with usual care, while implementing reminders in addition to another intervention improved care by 4.0% only (IQR 3.0% to 6.0%) compared with the other intervention. The quality of evidence for these comparisons was rated as moderate according to the GRADE approach. Two reminder features were associated with larger effect sizes: providing space on the reminder for provider to enter a response (median 13.7% versus 4.3% for no response, P value = 0.01) and providing an explanation of the content or advice on the reminder (median 12.0% versus 4.2% for no explanation, P value = 0.02). Median improvement in processes of care also differed according to the behaviour the reminder targeted: for instance, reminders to vaccinate improved processes of care by 13.1% (IQR 12.2% to 20.7%) compared with other targeted behaviours. In the only study that had sufficient power to detect a clinically significant effect on outcomes of care, reminders were not associated with significant improvements. AUTHORS' CONCLUSIONS: There is moderate quality evidence that computer-generated reminders delivered on paper to healthcare professionals achieve moderate improvement in process of care. Two characteristics emerged as significant predictors of improvement: providing space on the reminder for a response from the clinician and providing an explanation of the reminder's content or advice. The heterogeneity of the reminder interventions included in this review also suggests that reminders can improve care in various settings under various conditions.

En Suisse, l'offre en matière de jeu est très importante. Que ce soit pour les loteries ou les casinos, les revenus bruts des jeux (RBJ) ont augmenté considérablement au cours de ces dix dernières années. Une offre de prévention et de traitement s'est développée en Suisse romande et le nombre de joueurs qui consultent ne cesse d'augmenter. Toutefois, il n'en reste pas moins qu'il s'agit là d'une petite proportion des joueurs problématiques qui font appel aux services d'aide. Le Programme intercantonal de lutte contre la dépendance au jeu (PILDJ) vise en premier lieu à sensibiliser la population au problème du jeu excessif. Il prévoit pour cela, notamment, de faciliter l'accès à l'information et au traitement. Par ailleurs, il a aussi pour objectif de sensibiliser les professionnels, afin d'améliorer la détection et la prise en charge des joueurs pathologiques. Le PILDJ entend finalement centraliser les informations relatives aux actions menées dans les cantons afin d'améliorer l'échange d'expériences. La coordination de ce Programme a été confiée au Groupement romand d'études des addictions (GREA). Le PILDJ comprend la mise sur pied de 4 modules parmi lesquels figure l'Etude romande sur le jeu. Ce rapport présente les résultats des volets couverts par l'Etude romande sur le jeu. [résumé, p. 5]

La remédiation cognitive est devenue en quelques années un outil thérapeutique important dans le domaine des maladies psychiatriques, et plus particulièrement dans celui des troubles schizophréniques. Parmi les programmes utilisés, RECOS est l'un des seuls à proposer un entraînement qui tient compte du profil cognitif individuel, permettant ainsi de répondre de manière ciblée à la grande hétérogénéité des déficits observés. Cet ouvrage constitue le support de base indispensable à la formation délivrée aux futurs thérapeutes RECOS. Il se divise en deux parties. La première partie présente les données scientifiques actuelles sur les troubles cognitifs de la schizophrénie et les moyens d'y remédier. Le lien entre les performances cognitives et les capacités fonctionnelles permet de comprendre comment et pourquoi la remédiation cognitive favorise la réinsertion sociale et professionnelle. La deuxième partie fait office de manuel d'utilisation pour tous les thérapeutes (psychologues, psychiatres, infirmiers, ergothérapeutes) souhaitant utiliser RECOS. L'ouvrage décrit les pathologies psychiatriques visées par le programme, la schizophrénie n'étant pas la seule concernée. Il aborde ensuite les différentes étapes du traitement, en consacrant une place importante à 1 'évaluation cognitive et clinique ainsi qu'aux exercices de remédiation. Des cas cliniques illustrent la manière d'adapter le travail thérapeutique au profil cognitif de chaque participant. Afin que le lecteur puisse bénéficier d'un maximum d'informations et de documents pratiques, plusieurs outils nécessaires à 1 'utilisation du programme figurent en annexe de l'ouvrage.

RÉSUMÉ Cette thèse porte sur le développement de méthodes algorithmiques pour découvrir automatiquement la structure morphologique des mots d'un corpus. On considère en particulier le cas des langues s'approchant du type introflexionnel, comme l'arabe ou l'hébreu. La tradition linguistique décrit la morphologie de ces langues en termes d'unités discontinues : les racines consonantiques et les schèmes vocaliques. Ce genre de structure constitue un défi pour les systèmes actuels d'apprentissage automatique, qui opèrent généralement avec des unités continues. La stratégie adoptée ici consiste à traiter le problème comme une séquence de deux sous-problèmes. Le premier est d'ordre phonologique : il s'agit de diviser les symboles (phonèmes, lettres) du corpus en deux groupes correspondant autant que possible aux consonnes et voyelles phonétiques. Le second est de nature morphologique et repose sur les résultats du premier : il s'agit d'établir l'inventaire des racines et schèmes du corpus et de déterminer leurs règles de combinaison. On examine la portée et les limites d'une approche basée sur deux hypothèses : (i) la distinction entre consonnes et voyelles peut être inférée sur la base de leur tendance à alterner dans la chaîne parlée; (ii) les racines et les schèmes peuvent être identifiés respectivement aux séquences de consonnes et voyelles découvertes précédemment. L'algorithme proposé utilise une méthode purement distributionnelle pour partitionner les symboles du corpus. Puis il applique des principes analogiques pour identifier un ensemble de candidats sérieux au titre de racine ou de schème, et pour élargir progressivement cet ensemble. Cette extension est soumise à une procédure d'évaluation basée sur le principe de la longueur de description minimale, dans- l'esprit de LINGUISTICA (Goldsmith, 2001). L'algorithme est implémenté sous la forme d'un programme informatique nommé ARABICA, et évalué sur un corpus de noms arabes, du point de vue de sa capacité à décrire le système du pluriel. Cette étude montre que des structures linguistiques complexes peuvent être découvertes en ne faisant qu'un minimum d'hypothèses a priori sur les phénomènes considérés. Elle illustre la synergie possible entre des mécanismes d'apprentissage portant sur des niveaux de description linguistique distincts, et cherche à déterminer quand et pourquoi cette coopération échoue. Elle conclut que la tension entre l'universalité de la distinction consonnes-voyelles et la spécificité de la structuration racine-schème est cruciale pour expliquer les forces et les faiblesses d'une telle approche. ABSTRACT This dissertation is concerned with the development of algorithmic methods for the unsupervised learning of natural language morphology, using a symbolically transcribed wordlist. It focuses on the case of languages approaching the introflectional type, such as Arabic or Hebrew. The morphology of such languages is traditionally described in terms of discontinuous units: consonantal roots and vocalic patterns. Inferring this kind of structure is a challenging task for current unsupervised learning systems, which generally operate with continuous units. In this study, the problem of learning root-and-pattern morphology is divided into a phonological and a morphological subproblem. The phonological component of the analysis seeks to partition the symbols of a corpus (phonemes, letters) into two subsets that correspond well with the phonetic definition of consonants and vowels; building around this result, the morphological component attempts to establish the list of roots and patterns in the corpus, and to infer the rules that govern their combinations. We assess the extent to which this can be done on the basis of two hypotheses: (i) the distinction between consonants and vowels can be learned by observing their tendency to alternate in speech; (ii) roots and patterns can be identified as sequences of the previously discovered consonants and vowels respectively. The proposed algorithm uses a purely distributional method for partitioning symbols. Then it applies analogical principles to identify a preliminary set of reliable roots and patterns, and gradually enlarge it. This extension process is guided by an evaluation procedure based on the minimum description length principle, in line with the approach to morphological learning embodied in LINGUISTICA (Goldsmith, 2001). The algorithm is implemented as a computer program named ARABICA; it is evaluated with regard to its ability to account for the system of plural formation in a corpus of Arabic nouns. This thesis shows that complex linguistic structures can be discovered without recourse to a rich set of a priori hypotheses about the phenomena under consideration. It illustrates the possible synergy between learning mechanisms operating at distinct levels of linguistic description, and attempts to determine where and why such a cooperation fails. It concludes that the tension between the universality of the consonant-vowel distinction and the specificity of root-and-pattern structure is crucial for understanding the advantages and weaknesses of this approach.

Background Medication adherence is a complex, dynamic and changing behaviour that is affected by a variety of factors, including the patient's beliefs and life circumstances. Studies have highlighted barriers to medication adherence (e.g., unmanaged side effects or a lack of social support), as well as facilitators of medication adherence (e.g., technical simplicity of treatment and psychological acceptance of the disease). Since August 2004, in Lausanne (Switzerland), physicians have referred patients who are either experiencing or are at risk of experiencing problems with their HIV antiretroviral treatment (ART) to a routine interdisciplinary ART adherence programme. This programme consists of multifactorial intervention including electronic drug monitoring (MEMS(TM)). Objective This study's objective was to identify the barriers and facilitators encountered by HIV patients with suboptimal medication adherence (≤90 % adherence over the study period). Setting The community pharmacy of the Department of Ambulatory Care and Community Medicine in Lausanne (Switzerland). Method The study consisted of a retrospective, qualitative, thematic content analysis of pharmacists' notes that were taken during semi-structured interviews with patients and conducted as part of the ART adherence programme between August 2004 and May 2008. Main outcome measure Barriers and facilitators encountered by HIV patients. Results Barriers to and facilitators of adherence were identified for the 17 included patients. These factors fell into three main categories: (1) cognitive, emotional and motivational; (2) environmental, organisational and social; and (3) treatment and disease. Conclusion The pharmacists' notes revealed that diverse barriers and facilitators were discussed during medication adherence interviews. Indeed, the results showed that the 17 non-adherent patients encountered barriers and benefited from facilitators. Therefore, pharmacists should inquire about all factors, regardless of whether they have a negative or a positive impact on medication adherence, and should consider all dimensions of patient adherence. The simultaneous strengthening of facilitators and better management of barriers may allow healthcare providers to tailor care to a patient's specific needs and support each individual patient in improving his medication-related behaviour.

BACKGROUND: Physician training in smoking cessation counseling has been shown to be effective as a means to increase quit success. We assessed the cost-effectiveness ratio of a smoking cessation counseling training programme. Its effectiveness was previously demonstrated in a cluster randomized, control trial performed in two Swiss university outpatients clinics, in which residents were randomized to receive training in smoking interventions or a control educational intervention. DESIGN AND METHODS: We used a Markov simulation model for effectiveness analysis. This model incorporates the intervention efficacy, the natural quit rate, and the lifetime probability of relapse after 1-year abstinence. We used previously published results in addition to hospital service and outpatient clinic cost data. The time horizon was 1 year, and we opted for a third-party payer perspective. RESULTS: The incremental cost of the intervention amounted to US$2.58 per consultation by a smoker, translating into a cost per life-year saved of US$25.4 for men and 35.2 for women. One-way sensitivity analyses yielded a range of US$4.0-107.1 in men and US$9.7-148.6 in women. Variations in the quit rate of the control intervention, the length of training effectiveness, and the discount rate yielded moderately large effects on the outcome. Variations in the natural cessation rate, the lifetime probability of relapse, the cost of physician training, the counseling time, the cost per hour of physician time, and the cost of the booklets had little effect on the cost-effectiveness ratio. CONCLUSIONS: Training residents in smoking cessation counseling is a very cost-effective intervention and may be more efficient than currently accepted tobacco control interventions.

Introduction: Therapeutic drug monitoring (TDM) aims at optimizing treatment by individualizing dosage regimen based on measurement of blood concentrations. Maintaining concentrations within a target range requires pharmacokinetic and clinical capabilities. Bayesian calculation represents a gold standard in TDM approach but requires computing assistance. In the last decades computer programs have been developed to assist clinicians in this assignment. The aim of this benchmarking was to assess and compare computer tools designed to support TDM clinical activities.¦Method: Literature and Internet search was performed to identify software. All programs were tested on common personal computer. Each program was scored against a standardized grid covering pharmacokinetic relevance, user-friendliness, computing aspects, interfacing, and storage. A weighting factor was applied to each criterion of the grid to consider its relative importance. To assess the robustness of the software, six representative clinical vignettes were also processed through all of them.¦Results: 12 software tools were identified, tested and ranked. It represents a comprehensive review of the available software's characteristics. Numbers of drugs handled vary widely and 8 programs offer the ability to the user to add its own drug model. 10 computer programs are able to compute Bayesian dosage adaptation based on a blood concentration (a posteriori adjustment) while 9 are also able to suggest a priori dosage regimen (prior to any blood concentration measurement), based on individual patient covariates, such as age, gender, weight. Among those applying Bayesian analysis, one uses the non-parametric approach. The top 2 software emerging from this benchmark are MwPharm and TCIWorks. Other programs evaluated have also a good potential but are less sophisticated (e.g. in terms of storage or report generation) or less user-friendly.¦Conclusion: Whereas 2 integrated programs are at the top of the ranked listed, such complex tools would possibly not fit all institutions, and each software tool must be regarded with respect to individual needs of hospitals or clinicians. Interest in computing tool to support therapeutic monitoring is still growing. Although developers put efforts into it the last years, there is still room for improvement, especially in terms of institutional information system interfacing, user-friendliness, capacity of data storage and report generation.
