L'objet de ce cahier est de décrire la méthode de construction d'un système de "Case MiX" qui, en se fondant sur les DRG, ne décrit plus seulement la clientèle hospitalière en fonction des diagnostics principaux mais aussi des comorbidités ou complications recensées et des interventions chirurgicales subies.

PURPOSE: To assess objective response rate (ORR) after two cycles of temozolomide in combination with topotecan (TOTEM) in children with refractory or relapsed neuroblastoma. PATIENTS AND METHODS: This multicenter, non-randomised, phase II study included children with neuroblastoma according to a two-stage Simon design. Eligibility criteria included relapsed or refractory, measurable or metaiodobenzylguanidine (mIBG) evaluable disease, no more than two lines of prior treatment. Temozolomide was administered orally at 150mg/m(2) followed by topotecan at 0.75mg/m(2) intravenously for five consecutive days every 28days. Tumour response was assessed every two cycles according to International Neuroblastoma Response Criteria (INRC), and reviewed independently. RESULTS: Thirty-eight patients were enroled and treated in 15 European centres with a median age of 5.4years. Partial tumour response after two cycles was observed in 7 out of 38 evaluable patients [ORR 18%, 95% confidence interval (CI) 8-34%]. The best ORR whatever the time of evaluation was 24% (95% CI, 11-40%) with a median response duration of 8.5months. Tumour control rate (complete response (CR)+partial response (PR)+mixed response (MR)+stable disease (SD)) was 68% (95% CI, 63-90%). The 12-months Progression-Free and Overall Survival were 42% and 58% respectively. Among 213 treatment cycles (median 4, range 1-12 per patient) the most common treatment-related toxicities were haematologic. Grade 3/4 neutropenia occurred in 62% of courses in 89% of patients, grade 3/4 thrombocytopenia in 47% of courses in 71% of patients; three patients (8%) had febrile neutropenia. CONCLUSION: Temozolomide-Topotecan combination results in very encouraging ORR and tumour control in children with heavily pretreated recurrent and refractory neuroblastoma with favourable toxicity profile.

L'article se propose de discuter la question des modalités de la référence aux évangiles au cinéma en s'attachant à l'étude de la composante verbale et de ses liens avec la représentation de la figure christique dans deux films: le premier, Ben-Hur (Fred Niblo, 1925), est une superproduction produite par Hollywood à l'époque de l'apogée du muet; le second, Golgotha (1935), est réalisé par le cinéaste français Julien Duvivier durant les premières années de la généralisation du parlant. D'un côté l'écrit (sous forme d'intertitres), de l'autre la profération orale d'un texte. Le rapprochement proposé entre ces deux films tient à la façon dont la citation néotestamentaire participe à la construction par le film de la figure de Jésus, ou plutôt à la mise à distance de celle-ci, constamment rejetée dans une forme de hors-champ.

Objective: to assess the between and within-device reproducibility, as well as within-day variability of body fat measurements. Methods: body fat percentage (%BF) was measured twice on seventeen female students aged between 18 and 20 with a body mass index of 21.9 22.6 kg/m2 (mean SD) using seven bipolar bioelectrical impedance devices (BF-306) according to the manufacturer's recommendations. Each student was also measured each hour between 7:00 and 22:00. Statistical analysis was conducted using a general linear model for repeated measurements. Results: the correlation between first and second measurements was very high (Pearson r between 0.985 and 1.000, p<0.001), as well as the correlation between devices (Pearson r between 0.986 and 0.999, all p<0.001). Repeated measurements analysis showed no differences were between devices (F test=0.83, p=0.59) or readings (first vs. second: F test=0.12, p=0.74). Conversely, significant differences were found between assessment periods throughout the day, measurements made in the morning being lower than those made in the afternoon. Assuming an overall daily average of 100 (based on all measurements), the values were 95.8 3.2 (mean SD) at 8:00 versus 101.3 3.0 at 20:00, corresponding to a mean change of 2.2 1.1 in %BF (F test for repeated values=6.58, p<0.001). Conclusions: the between and within-device reproducibility for measuring body fat is high, enabling the use of multiple devices in a single study. Conversely, small but significant changes in body fat measurements occur during the day, urging body fat measurements to be performed at fixed times.

Sponsored by the Health Administrations of nine cantons, this study was conducted by the University Institute of Social and Preventive Medicine in Lausanne in order to assess how DRGs could be used within the Swiss context. A data base mainly provided by the Swiss VESKA statistics was used. The first step provided the transformation of Swiss diagnostic and intervention codes into US codes, allowing direct use of the Yale Grouper for DRG. The second step showed that the overall performance of DRG in terms of variability reduction of the length of stay was similar to the one observed in US; there are, however, problems when the homogeneity of medicotechnical procedures for DRG is considered. The third steps showed how DRG could be used as an account unit in hospital, and how costs per DRG could be estimated. Other examples of applications of DRG were examined, for example comparison of Casemix or length of stay between hospitals.

Gene expression data from microarrays are being applied to predict preclinical and clinical endpoints, but the reliability of these predictions has not been established. In the MAQC-II project, 36 independent teams analyzed six microarray data sets to generate predictive models for classifying a sample with respect to one of 13 endpoints indicative of lung or liver toxicity in rodents, or of breast cancer, multiple myeloma or neuroblastoma in humans. In total, >30,000 models were built using many combinations of analytical methods. The teams generated predictive models without knowing the biological meaning of some of the endpoints and, to mimic clinical reality, tested the models on data that had not been used for training. We found that model performance depended largely on the endpoint and team proficiency and that different approaches generated models of similar performance. The conclusions and recommendations from MAQC-II should be useful for regulatory agencies, study committees and independent investigators that evaluate methods for global gene expression analysis.

Im Auftrag der Dienststelle für Gesundheitswesen hat das Walliser Gesundheitsobservatorium (WGO) den vierten Bericht über den Gesundheitszustand der Walliser Bevölkerung veröffentlicht. Der Bericht zeigt einen guten Gesundheitszustand der Walliserinnen und Walliser. Herz- Kreislauf- Krankheiten. Krebserkrankungen und Lungenerkrankungen bleiben weiterhin für rund zwei Drittel der Todesfälle verantwortlich. Um das Ausmass dieser Krankheiten zu reduzieren wird eine aktive und gezielte Gesundheitsförderungs- und Präventionspolitik verfolgt. Der Staatsrat hat ein entsprechendes Rahmenprogramm angenommen. Der Bericht über den Gesundheitszustand der Walliser Bevölkerung wurde vom Walliser Gesundheitsobservatorium in Zusammenarbeit mit dem Institut für Sozial- und Präventivmedizin der Universität Lausanne (IUMSP) erarbeitet. Der Bericht dient der Regierung als Referenzdokument, um die Stossrichtung ihrer Präventions- und Gesundheitsförderungspolitik 2011-2014 vorzugeben.
