The vibration syndrome is an often unrecognized occupational neurovascular disease with a prevalence of more than 70% in certain high-risk occupations. Early recognition is crucial because continued exposure to vibration can lead to irreversible ischemic injury and loss of digits. Digital ischemia due to the vibration syndrome may be due to a vasospastic phenomenon, an organic microangiopathy or arterial thrombosis. Demyelinating neuropathy and carpal tunnel syndrome are often associated. Many pathophysiological mechanisms are implicated: hyperactivity of the central sympathetic nervous system, release of plasma endothelin-1 and loss of calcitonin-gene-related-peptide vasoregulation. Investigation tests, treatment and the European Community Directive for the protection of workers are also presented.

In-vitro fertilization: advantage and disadvantage of covering the costs of IVF/CSI by the health insurance in Switzerland The reimbursement of certain infertility treatments (stimulation with/without insemination) whereas IVF/ICSI is not leads patients with an indication of IVF to prefer treatments of low efficacy. The costs of multiple pregnancies issued by reimbursed or non-reimbursed fertility treatments are paid by the society. There should be measures to reduce these costs and to take the money used today to pay the complications of infertility treatments to reimburse IVF. The efficacy of such a system (single embryo transfer) has been proven in Belgium since several years. The dangers of complete reimbursement (IVF treatment in cases without any chances of success, only because it is for free) can be avoided by an Efficacy and Safety Board.

Objective: The main objective of the study is to identify practical and cultural factors influencing the mental health of mothers of children with an orofacial cleft in Benin and to compare it with a sample of Swiss mothers in the same conditions. Method: Thirty-six mothers of children with an orofacial cleft in Benin and 40 mothers of children with an orofacial cleft in Switzerland were interviewed about practical and emotional aspects concerning their child and their own lives. Then, they completed the Perinatal Postraumatic Stress Questionnaire and the Beck Depression Inventory. Results: Mothers in Benin had significantly higher posttraumatic stress and depression symptoms compared with mothers in Switzerland. Depression symptoms were higher in Beninese mothers coming from urban areas, in Beninese mothers with few or no other children, and in Beninese mothers whose child was operated on at a more advanced age. Discussion: This study stressed the importance of cultural differences in perceptions of orofacial clefts in order to provide appropriate care to patients and their families. In particular, wide campaigns of information should help parents to understand the cleft origin and the medical staff in small dispensaries to provide adequate support and care. This may diminish anxiety concerning the child's short- and long-term prognosis. Creation of a Beninese parental support group for children with clefts and their families could be another way to provide information and support where multidisciplinary care is not available.

Fitness to drive in elderly drivers is most commonly discussed with a focus on cognitive impairment. Therefore, this article is focussing on mental illness and the use of psychotropic drugs in elderly drivers, which can both interfere with fitness to drive. Based on a detailed literature review and on clinical judgement, we propose signposts and "red flags" to judge the individual risks. Health professionals dealing with elderly patients should in particular be aware of the dangers related to cumulative risks and need to inform the patients appropriately. For medico-legal reasons the information provided to patients must be written down in the medical record. Individual counselling is important as fitness to drive is a complex topic.

Wilson disease (WD) is an inherited disorder of hepatic copper excretion leading to toxic accumulation of copper in the liver as well as the brain, cornea, and other organs. The defect is due to mutations of the copper-transporting ATPase ATP7B. Clinical manifestations are highly variable and comprise acute liver failure, chronic hepatitis and cirrhosis as well as neurological or psychiatric symptoms. The Kayser-Fleischer corneal ring is pathognomonic but absent in about 50% of patients with hepatic manifestations alone. A high index of suspicion in clinically compatible situations is key, with a combination of laboratory tests allowing the diagnosis of WD. Treatment is based on the use of chelating agents, D-penicillamine or trientine. Liver transplantation should be considered for patients with acute liver failure or advanced cirrhosis.

Introduction. - En Suisse, la prescription de produits biologiqueschez les patients souffrant de polyarthrite rhumatoïde (PR) n'est nilimitée à des centres hospitaliers de rhumatologie, ni soumise à desdirectives strictes éditées par les autorités sanitaires sur le type oule nombre de traitements de fond préalables. La notion d'échec auxtraitements de fond n'est pas non plus précisément définie, et enparticulier l'activité de la maladie ne doit pas répondre à des critèresstricts, notamment en terme de valeurs de DAS, et ce contrairementà de nombreux autres pays où l'impact de ces restrictions aété publié récemment (1, 2).Le registre SCQM peut être considéré comme un bon reflet de lapopulation suisse avec PR, aussi bien pour la population suivie pardes centres hospitaliers que par les practiciens en cabinet privé, eton estime qu'environ 30 % des patients avec PR recevant des traitementsbiologiques en Suisse sont inclus dans ce registre.L'objectif primaire de cette étude est de comparer les caractéristiquesdes patients de notre registre à l'initiation et après un an de traitementbiologique avec celles de registres du même type dans des pays avecun accès plus restreint aux traitements biologiques. Les objectifssecondaires sont de comparer les patients traités en milieu hospitalieret ceux pris en charge en cabinet privé, et aussi d'examiner s'il existedes tendances temporelles (avant et après 2005).Patients et Méthodes. - Les données sont extraites du registre suissede PR (SCQM) qui comprend 4 500 patients inclus entre 1997 et2011. 2 715 patients bénéficient d'un traitement biologique, dont2 427 avec des données à l'introduction du traitement : DAS28/VS,DAS28/CRP, HAQ, durée de la maladie, nbre de tttt préalables, comorbidités,etc. Les principales données démographiques sont : âgemoyen 55 ans, 77 % de femmes 72 % FR+, médecins prescripteurs :62 % en cabinet, 21 % en centre hospitaliers et 16 % en centres universitaires.Nous avons calculés les moyennes (+/- écart type) pourdifférents paramètres de l'activité de la maladie.Résultats. - La moyenne du DAS28/VS à l'introduction du traitement(4,4 +/- 1,3) est nettement inférieur aux valeurs publiées pard'autres registres européens ou canadien (5,3 < > 6,6 ; 1,2). Il en ende même pour le HAQ (1 versus 1,4). Les biologiques sont introduitsaprès en moyenne 1,1 +/- 1 DMARD préalable contre > 3 en Suède,au Danemark ou au Canada.Les caractéristiques démographiques, le degré d'activité et les traitementsprodigués sont similaires entre les patients traités encabinet privé ou en milieu hospitalier, hormis pour une proportionmoindre de traitements iv en cabinet (20 % versus 40 %). Après2005, les traitements biologiques sont introduits beaucoup plusprécocemment, avec une durée médiane de maladie avant l'introductionde thérapies biologiques diminuant de 96 à 51 mois. Onnote également une répartition entre les divers produits biologiquesqui se diversifie. Même si les traitements sont introduits à undegré d'activité similaire (DAS28/VS moyen à 4,4 +/- 1,3) onobserve de meilleurs résultats à 1 an avec un DAS moyen à 1 an :3,5 +/- 1,4 avant 2005 contre 3,1 +/- 1,3 après 2005 (p = 0.0001).Conclusion. - Les données du registre suisse des PR (SCQM) suggèrentque, en l'absence de critères restrictifs d'accès aux traitements biologiques,ceux-ci sont prescrits à des scores d'activité de la maladie(DAS et HAQ) inférieurs, et plus précocemement en terme de nombrede DMARD préalables. Cette tendance se confirme dans le temps, etse retrouve aussi bien en milieu hospitalier qu'en cabinet.En terme de résultats, après 2005, plus de 50 % des patients atteignentun bas degré d'activité de la maladie en terme de DAS aprèsun an de traitement, chiffre qui semble justifier ce type de systèmepeu restrictif favorisant certainement une approche thérapeutiqueplus proche des nouveaux paragidmes de traitement avec une stratégiede type « treat to target ».Références[1] Curtis J R et al. Sex differences in cognition have been largely investigated. The most consistent sex differences favoring females are observed in object location memory involving the left hemisphere whereas the most consistent sex differences favoring males are observed in tasks that require mental rotation involving the right hemisphere. Here we used a task involving these two abilities to see the impact of mental rotation on object location memory. To that end we used a combination of behavioral and event-related potential (ERP) electroencephalography (EEG) measures.A computer screen displayed a square frame of 4 pairs of images (a "teddy" bear, a shoe, an umbrella and a lamp) randomly arranged around a central fixation cross. After a 10-second interval for memorization, images disappeared and were replaced by a test frame with no image but a random pair of two locations marked in black. In addition, this test frame was randomly displayed either in the original orientation (0° rotation) or in the rotated one (90° clockwise - CW - or 90° counterclockwise - CCW). Preceding the test frame, an arrow indicating the presence or the absence of rotation of the frame was displayed on the screen. The task of the participants (15 females and 15 males) was to determine if two marked locations corresponded or not to a pair of identical images. Each response was followed by feedback.Findings showed no significant sex differences in the performance of the original orientation. In comparison with this position, the rotation of the frame produced an equal decrease of male and female performance. In addition, this decrease was significantly higher when the rotation of the frame was in a CCW direction. We further assessed the ERP when the arrow indicated the direction of rotation as stimulus-onset, during four time windows representing major components C1, P1, N1 and N2. Although no sex differences were observed in performance, brain activities differed according to sex. Enhanced amplitudes were found for the CCW compared to CW rotation over the right posterior areas for the P1, N1 and N2 components for men as well as for women. Major topographical differences related to sex were measured for the CW rotation condition as marked lateralized amplitude: left-hemisphere amplitude larger than right one was measured during P1 time range for men. These similar patterns prolonged from P1 to N1 for women. Early distinctions were found in interaction with sex between CCW and CW waveform amplitudes, expressing over anterior electrode sites during C1 time range (0-50 ms post-stimulus).In conclusion (i) women do not outperform men in object location memory in this study (absence of rotation condition); (ii) mental rotation, in particular the direction of rotation, influences performance on object location memory; (iii) CCW rotation is associated with activity in the right parietal hemisphere whereas the CW rotation involves the left parietal hemisphere; (iv) this last effect is less pronounced in males, which could explain why greater involvement of right parietal areas in men and of bilateral posterior areas in women is generally reported in mental rotation tasks; and (v) the early distinctions between both directions of rotation located over anterior sites could be related to sex differences in their respective involvement of control mechanisms.
