Objective: Existing VADs are single-ventricle pumps needing anticoagulation. We developed a bi ventricular external assist device that reproduces the physiological heart muscle movement completely avoiding anticoagulants. Methods: The device has a carbon fibre skeleton fitting a 30-40 kg patient's heart, to which a Nitinol based artificial muscle is connected. The artificial muscle wraps both ventricles. The strength of the Nitinol fibres is amplified by a pivot articulation in contact with the ventricle wall. The fibres are electrically driven and a dedicated control unit has been developed. We assessed hemodynamic performances of this device using a previously described dedicated bench test. Volume ejected and pressure gradient has been measured with afterload ranging from 25 to 50mmHg. Results: With anafterload of 50mmHg the system has an ejection fraction (EF) of 10% on the right side and 8% on the left side. The system is able to generate a systolic ejection of 5,5 ml on the right side and 4,4 ml on the left side. With anafterload of 25mmHg the results are reduced of about 20%. The activation frequency is 80/minute resulting in a total volume displacement of 440 ml/minute on the right side and 352 ml/minute on the left side. Conclusions: The artificial muscle follows Starling's law as the ejected volume increases when afterload increases. These preliminary studies confirmed the possibility of improving the EF of a failing heart using artificial muscle for external cardiac compression. This device could be helpful in weaning CPB and/or for short-term cardio-circulatory support in paediatric population with cardiac failure.

Approaches exploiting trait distribution extremes may be used to identify loci associated with common traits, but it is unknown whether these loci are generalizable to the broader population. In a genome-wide search for loci associated with the upper versus the lower 5th percentiles of body mass index, height and waist-to-hip ratio, as well as clinical classes of obesity, including up to 263,407 individuals of European ancestry, we identified 4 new loci (IGFBP4, H6PD, RSRC1 and PPP2R2A) influencing height detected in the distribution tails and 7 new loci (HNF4G, RPTOR, GNAT2, MRPS33P4, ADCY9, HS6ST3 and ZZZ3) for clinical classes of obesity. Further, we find a large overlap in genetic structure and the distribution of variants between traits based on extremes and the general population and little etiological heterogeneity between obesity subgroups.

Introduction: « Osteo-Mobile Vaud » is a mobile osteoporosis (OP) screening program. The women > 60 years living in the region Vaud will be offered OP screening with new equipment installed in a bus. The main goal is to evaluate the fracture risk with the combination of clinical risk factors (CRF) and informations extracted by a single DXA: bone mineral density (BMD), vertebral fracture assessment (VFA), and micro-architecture (MA) evaluation. MA is yet evaluable in daily practice by the Trabecular Bone Score (TBS) measure. TBS is a novel grey-level texture measurement reflecting bone MA based on the use of experimental variograms of 2D projection images. TBS is very simple to obtain, by reanalyzing a lumbar DXA-scan. TBS has proven to have diagnosis and prognosis value, partially independent of CRF and BMD. A 55-years follow- up is planned. Method: The Osteo-Mobile Vaud cohort (1500 women, > 60 years, living in the region Vaud) started in July 2010. CRF for OP, lumbar spine and hip BMD, VFA by DXA and MA evaluation by TBS are recorded. Preliminary results are reported. Results: In July 31th, we evaluated 510 women: mean age 67 years, BMI 26 kg/m². 72 women had one or more fragility fractures, 39 had vertebral fracture (VFx) grade 2/3. TBS decreases with age (-0.005 / year, p<0.001), and with BMI (-0.011 per kg/m², p<0.001). Correlation between BMD and site matched TBS is low (r=0.4, p<0.001). For the lowest T-score BMD, odds ratio (OR, 95% CI) for VFx grade 2/3 and clinical OP Fx are 1.8 (1.1-2.9) and 2.3 (1.5-3.4). For TBS, age-, BMI- and BMD adjusted ORs (per SD decrease) for VFx grade 2/3 and clinical OP Fx are 1.9 (1.2-3.0) and 1.8 (1.2-2.7). The TBS added value was independent of lumbar spine BMD or the lowest T-score (femoral neck, total hip or lumbar spine). Conclusion: As in the already published studies, these preliminary results confirm the partial independence between BMD and TBS. More importantly, a combination of TBS and BMD may increase significantly the identification of women with prevalent OP Fx. For the first time we are able to have complementary information about fracture (VFA), density (BMD), and micro-architecture (TBS) from a simple, low ionizing radiation and cheap device: DXA. The value of such informations in a screening program will be evaluated.

PURPOSE: To determine whether a mono-, bi- or tri-exponential model best fits the intravoxel incoherent motion (IVIM) diffusion-weighted imaging (DWI) signal of normal livers. MATERIALS AND METHODS: The pilot and validation studies were conducted in 38 and 36 patients with normal livers, respectively. The DWI sequence was performed using single-shot echoplanar imaging with 11 (pilot study) and 16 (validation study) b values. In each study, data from all patients were used to model the IVIM signal of normal liver. Diffusion coefficients (Di ± standard deviations) and their fractions (fi ± standard deviations) were determined from each model. The models were compared using the extra sum-of-squares test and information criteria. RESULTS: The tri-exponential model provided a better fit than both the bi- and mono-exponential models. The tri-exponential IVIM model determined three diffusion compartments: a slow (D1 = 1.35 ± 0.03 × 10(-3) mm(2)/s; f1 = 72.7 ± 0.9 %), a fast (D2 = 26.50 ± 2.49 × 10(-3) mm(2)/s; f2 = 13.7 ± 0.6 %) and a very fast (D3 = 404.00 ± 43.7 × 10(-3) mm(2)/s; f3 = 13.5 ± 0.8 %) diffusion compartment [results from the validation study]. The very fast compartment contributed to the IVIM signal only for b values ≤15 s/mm(2) CONCLUSION: The tri-exponential model provided the best fit for IVIM signal decay in the liver over the 0-800 s/mm(2) range. In IVIM analysis of normal liver, a third very fast (pseudo)diffusion component might be relevant. KEY POINTS: ? For normal liver, tri-exponential IVIM model might be superior to bi-exponential ? A very fast compartment (D = 404.00 ± 43.7 × 10 (-3)  mm (2) /s; f = 13.5 ± 0.8 %) is determined from the tri-exponential model ? The compartment contributes to the IVIM signal only for b ≤ 15 s/mm (2.)

In this paper, we show how business model modelling can be connected to IT infrastructure, drawing parallels from enterprise architecture models such as ArchiMate. We then show how the proposed visualization based on enterprise architecture, with a strong focus on business model strategy, can help IT alignment, at both the business model and the IT infrastructure level.

La station valaisanne de Crans-Montana est richement représentée par la photographie, la peinture, les affiches et l'architecture. Cette thèse de doctorat s'emploie à réunir un large corpus de photographies et de représentations : peintures, affiches, cartes postales et reproductions de bâtiments emblématiques (voir le corpus illustré et documentaire annexé). Les questions liées à l'identité du territoire et son image sont les fils conducteurs de ce travail qui a débuté en 2008. Un premier ensemble visuel a été réuni par le Dr Théodore Stephani (1868-1951), un acteur fondamental pour l'histoire de la naissance de la station. Médecin, mais également photographe, il réalise une collection de plus de 1300 clichés, réunie en six albums, sur une période de trente-sept ans (1899-1936). Les photographies du médecin, originaire de Genève, fondateur de ce lieu désormais touristique sont le point de départ de cette recherche et son fil rouge. Celle-ci tentera d'articuler des représentations sur l'évolution du paysage et l'urbanisation de la station autour d'acteurs illustres, tels que les peintres Ferdinand Hodler (1853-1918) et Albert Muret (1874-1955), l'écrivain Charles-Ferdinand Ramuz (1878-1947) et les nombreux hôteliers ou médecins qui ont marqué l'histoire de la naissance du Haut-Plateau. Les représentations débutent en 1896 car c'est à ce moment-là que le Dr Stephani s'établit à Montana. Les architectes les plus connus de la première période sont François-Casimir Besson (1869-1944), Markus Burgener (1878-1953), suivi de la deuxième génération autour de Jean-Marie Ellenberger (1913-1988), André Perraudin (1915-2014) et André Gaillard (1921-2010). Parallèlement ou avant eux, les peintres déjà cités, Ferdinand Hodler et Albert Muret, - suivis de René Auberjonois (1872-1957), Henri-Edouard Bercher (1877-1970), Charles-Clos Olsommer (1883-1966), Oskar Kokoschka (1886-1980), Albert Chavaz (1907¬1990), Paul Monnier (1907-1982) et Hans Emi (1909-2015) - qui appartiennent tous à l'histoire culturelle de la région. Quant aux écrivains qui ont résidé dans la région, nous citons Elizabeth von Arnim (1866-1941), sa cousine Katherine Mansfield (1888-1923) alors que l'oeuvre de Charles-Ferdinand Ramuz est largement développée par une interprétation de son oeuvre Le Règne de l'esprit malin (1917) et un clin d'oeil pour Igor Stravinsky (1882¬1971). Nous présenterons aussi les films de trois cinéastes qui se sont inspirés des oeuvres écrites par Ramuz lors de son passage à Lens, à savoir Dimitri Kirsanoff (1899-1957), Claude Goretta (1929) et Francis Reusser (1942). Le concept du « village » est abordé depuis l'exposition nationale suisse (1896) jusqu'au projet des investisseurs russes, à Aminona. Ce « village » est le deuxième mégaprojet de Suisse, après celui d'Andermatt. Si le projet se réalise, l'image de la station s'en trouvera profondément transformée. En 1998, la publication de Au bord de la falaise. L'histoire entre certitudes et inquiétudes amène une grande visibilité aux propositions de Roger Chartier, qui lie l'étude des textes aux objets matériels et les usages qu'ils engendrent dans la société. Il définit l'histoire culturelle comme "une histoire culturelle du social" alors que pour Pascal Ory, une histoire culturelle est "comme une forme d'histoire sociale", ce qui revient presque au même, mais nous choisirons celle d'Ory pour une histoire sociale du paysage et de l'architecture. Ce travail adopte ainsi plusieurs points de vue : l'histoire sociale, basée sur les interviews de nombreux protagonistes de l'histoire locale, et l'histoire de l'art qui permet une sélection d'objets emblématiques ; l'histoire culturelle offre ainsi une méthode transversale pour lire et relier ces différents regards ou points de vue entre les paysages, les arts visuels, l'architecture, la littérature et le cinéma.
