Beyond Centre and Margin: (Self-)translation and the Ecopoetics of Space in Geetanjali Shree's "Mai"

Croisant les concepts de traduction culturelle et une approche « queer » de l'identité, notre article propose une lecture de l'utilisation de tropes végétaux ou organiques dans le roman de Geetanjali Shree, Mai, comme critique de la logique binaire du centre et de la marge qui caractérise autant l'orientalisme que le système patriarcal. Ecrit à la première personne, le roman invente un nouvel espace d'énonciation en narrant l'enfance et la jeunesse d'une jeune femme et la constitution de son identité à travers la relation complexe qu'elle entretient avec sa mère, son milieu familial issu de la classe moyenne du Nord de l'Inde, et la société indienne contemporaine aux prises avec la globalisation. Toutefois, ce cercle ou centre est en constante évolution puisque le contexte postcolonial dans lequel ces identités féminines se situent nous amène à considérer d'autres modes d'intervention (agency) qui opèrent non seulement à travers la prise de parole mais aussi à travers l'usage stratégique du silence. Fleurissant entre l'anglais et le hindi, ces identités hybrides nous poussent à revoir nos cartographies critiques et à investiguer et investir des lieux liminaux dans lesquels des subjectivités traversent les frontières et transgressent les limites imposées par l'ordre patriarcal et les cartographies imposées par les centres de pouvoir.

RESUME DE LA THESE Introduction. Les traitements de la phase aiguë d'un accident vasculaire cérébral (AVC) ischémique ne peuvent être proposés actuellement qu'à une minorité de patients. Nous avons évalué l'accès à la thrombolyse des patients admis pour un AVC aigu dans un hôpital régional de Suisse romande (Neuchâtel) et l'avons comparé avec celui d'un centre universitaire de référence, distant de 75 km, administrant directement ce type de traitement (CHUV, Lausanne). Méthodes. Tous les cas admis à l'hôpital à Neuchâtel pour une suspicion d'AVC aigu sur une période de 2 ans ont été analysés rétrospectivement au niveau de la phase pré-hospitalière et hospitalière par rapport à la possibilité d'une thrombolyse. Résultats. Chez 120 patients (54%), le diagnostic d'AVC ischémique a été confirmé. Si l'on fait abstraction du facteur temps, 46 patients (38.3%) auraient été éligibles pour une thrombolyse sur des critères cliniques et radiologiques. Trois patients (2.5%) ont finalement bénéficié d'une thrombolyse intraveineuse (iv) dans un intervalle de 3 heures après transfert au CHUV. En comparaison, 33 patients (7%) ont été traités au CHUV durant la même période. Conclusion. Grâce notamment à l'avènement de la télémédecine, la pratique de la thrombolyse iv dans des centres régionaux sélectionnés pourrait contribuer à rendre ce traitement plus accessible.

Introduction : Il essentiel pour les patients, mais aussi pour l'utilisation appropriée des ressources médicales et financières, que les centres pédiatriques oncologiques de moindre envergure répondent de manière adéquate aux normes de soins et atteignent des résultats comparables à ceux des grands centres. Étant donné que l'impact économique des soins de santé est devenu un important modulateur des décisions politiques, il est urgent que les institutions abordent le sujet de la qualité en chirurgie pédiatrique oncologique, et fournissent des éléments pour appuyer ou modifier les lignes directrices existantes Patients et méthode : Par cette étude monocentrique rétrospective de 13 ans, nous avons analysé la qualité de la chirurgie pédiatrique de tumeurs abdominales ou thoraciques du Département Médico-Chirurgical du CHUV pratiquée sur 48 enfants. Afin d'y parvenir, nous avons choisi et analysé deux indicateurs de la qualité de ces soins. Le premier est la conformité de la pratique de ce service, avec les directives des protocoles de traitements internationaux. Le second paramètre étudié est la mesure du taux d'événements indésirables liés à la chirurgie du cancer observés dans notre population de patients. Résultats : La conformité avec les protocoles de traitement est très élevée avec un taux de 96%. L'incidence des événements indésirables liés à la chirurgie est de 34 patients sur 48 (66%). Les complications graves sont rares, avec 85% ne nécessitant pas plus qu'un traitement médical simple. Aucune mortalité chirurgicale n'a été identifiée. Conclusion : Nos données suggèrent que la planification méticuleuse de la chirurgie et l'exécution de celle-ci par des chirurgiens dédiés à l'oncologie pédiatrique, ainsi qu'une étroite collaboration avec des oncologues pédiatres sont des conditions essentielles à la qualité des soins apportés à ces patients. Dans ces conditions, même un centre d'oncologie pédiatrique de moindre envergure peut répondre aux normes établies par les grands centres internationaux.

BACKGROUND: Uveal metastasis is the most common intraocular malignancy. METHODS: This was a retrospective study of all patients with uveal metastases referred to the Liverpool Ocular Oncology Centre between January 2007 and December 2012. Biopsy was performed as a primary investigation if the clinical examination suggested metastasis with no evidence of any extraocular metastases. RESULTS: Ninety-six patients (109 eyes) were included. Breast and lung carcinomas were the most common primary malignancies, affecting 41 and 27 patients, respectively. The median time interval between detection of primary cancer and uveal metastasis was 24 months (range 1-288 months). Thirty-nine patients underwent ocular biopsy, confirming the diagnosis in all patients. The biopsy indicated the site of origin in 24 out of the 27 without a known primary tumour. In 7 of these 27 cases, previous systemic investigations had failed to identify the primary tumour. Seventy-three patients received external beam irradiation; two patients received photodynamic therapy; and two patients had Ru-106 plaque radiotherapy. The visual acuity was stable or improved in 75.5% of the cases. CONCLUSIONS: Immediate biopsy provides a quick diagnosis that may expedite treatment and improve any opportunities for conserving vision while facilitating the general oncologic management on these patients.
Adenocarcinoma of the pancreas: Comparative single centre analysis between ductal and mucinous type.

Background¦Adenocarcinomas of the pancreas are exocrine tumors, originate from ductal system, including two morphologically distinct entities: the ductal adenocarcinoma and mucinous adenocarcinoma. Ductal adenocarcinoma is by far the most frequent malignant tumor in the pancreas, representing at least about 90% of all pancreas cancers. It is associated with very poor prognosis, due to the fact that actually there are no any biological markers or diagnostic tools for identification of the disease at an early stage. Most of the time the disease is extensive with vascular and nerves involvement or with metastatic spread at the time of diagnosis (1). The median survival is less than 5% at 5 years, placing it, at the fifth leading cause of death by cancer in the world (2). The mucinous form of pancreatic adenocarcinoma is less frequent, and seems to have a better prognosis with about 57% survival at 5 years (1)(3)(4).¦Each morphologic type of pancreatic adenocarcinoma is associated with particular preneoplastic lesions. Two types of preneoplastic lesions are described: firstly, pancreatic intra-epithelial neoplasia (PanIN) which affects the small and peripheral pancreatic ducts, and the intraductal papillary-mucinous neoplasm (IPMN) interested the main pancreatic ducts and its principal branches. Both of preneoplastic lesions lead by different mechanisms to the pancreatic adenocarcinoma (1)(2)(3)(4)(5)(6)(7)(8)(9)(10).¦The purpose of our study consists in a retrospective analysis of various clinical and histo-morphological parameters in order to assess a difference in survival between these two morphological types of pancreatic adenocarcinomas.¦1.2 Material and methods¦We conducted a retrospective analysis including 35 patients, (20 men and 15 women), beneficed the surgical treatment for pancreas adenocarcinoma at the Surgical Department of University Hospital in Lausanne. The patients involved in our study have been treated between 2003 and 2008, permitting at least 5-years mean follow up. For each patient the following parameters were analysed: age, gender, type of operation, type of preneoplastic lesions, TNM stage, histological grade of the tumor, vascular invasion, lymphatic and perineural invasion, resection margins, and adjuvant treatment.¦The results from these observations were included in a univariate and multivariate statistical analysis and compared with overall survival, as well as specific survival for each morphologic subtype of adenocarcinoma.¦As a low number of mucinous adenocarcinomas (n=5) was insufficient to conduct a pertinent statistical analysis, we compared the data obtained from adenocarcinomas developed on PanIN with adenocarcinomas developed on IPMN including both, ductal or mucinous types.¦1.3 Result¦Our results show that adenocarcinomas developed on pre-existing IPMN including both morphologic types (ductal and mucinous form) are associated with a better survival and prognosis than adenocarciomas developed on PanIN.¦1.4 Conclusion¦This study reflects that the most relevant parameter in survival in pancreatic adenocarcinoma seems to be the type of preneoplastic lesion. The significant difference in survival was noted between adenocarcinomas developing on PanIN as compared to adenocarcinomas developed on IPMN precursor lesions. Ductal adenocarcinomas developped on IPMN present significantly longer survival than those developed on PanIN lesions (P value= 0,01). Therefore we can suggest that the histological type of preneoplastic lesion rather than the histological type of adenocarcinoma should be the determinant prognosis factor in survival of pancreatic adenocarcinoma.

From practice to research in a couple and family therapy consultation - This article presents a study that took place at a Couples and Family outpatient clinic. Multiple levels are involved in couples and family therapy. The definition of relevant variables allowing the evaluation of a systemic therapeutic intervention's efficiency is thus particularly difficult. The aim of our study is to assess the short- and medium-term evolution on four different levels: (1) individual symptomatology; (2) conjugal satisfaction; (3) parental and co-parental relationship quality, and (4) global family functioning for a brief systemic intervention (BSI). BSI consists in a therapeutic treatment of a maximum of six sessions. The different levels are assessed by validated self-reported questionnaires before and after BSI as well as at a three-month follow-up. The therapeutic alliance is also measured by a self-reported questionnaire (WAI) after each session. The results of a pilot sample of N = 10 couples and families showed the efficiency of BSI on most of the variables measured and a greater therapeutic change for women than for men. No therapeutic effect was observed for men's individual symptomatology or for global family functioning. These results suggest that BSI has a different impact on the four levels measured. The importance of the therapeutic alliance on the therapeutic success is also confirmed.

La manipulation de végétaux ou de débris végétaux dans des espaces fermés (hangar pour le compostage, serres pour les cultures) est fréquemment effectuée en France. Néanmoins, le niveau d'exposition des travailleurs des différentes filières concernées aux bioaérosols dégagés par ces substrats, est peu connu. Dans cette note, deux études ont été choisies pour illustrer l'ampleur de ce type d'exposition dans des espaces fermés. La première publication concerne un centre de compostage et la deuxième publication cible des serres de culture de la tomate et du concombre. L'extrapolation des résultats obtenus à partir de prélèvements d'ambiance à l'exposition professionnelle des travailleurs est tout particulièrement discutée.

This article analyzes the role of the press in direct democratic campaigns. The paper argues the press has a dual role: On news pages, newspapers ought to inform citizens about the issue positions and frames of the pro and con camps in a balanced way. In editorials, newspapers act as political advocates that promote their own issue frames and try to shape public opinion through voting recommendations. Comparing the issue positions and frames in editorials and news reports in the run-up to the vote on the popular initiative "Yes to Europe" in Switzerland, this article shows that newspapers give similar visibility to the pro and con camps regardless of the papers' own editorial position. However, some newspapers favor issue frames that are in line with their editorial perspectives. In conclusion, newspapers are more similar in news report content than in editorial views.

Action research is a useful instrument for the organization health care and the clinical governance of psychiatric institutions. What this type of research offers can be illustrated by the cohort study of migrant patients without health insurance who consulted the Department of Psychiatry of the Vaudois university medical center (CHUV) in 2008. While giving greater visibilty to the psychological suffering and social distress of these patients, the study also enabled the authors to determine which clinical procedures were actually offered to these patients and the amount these procedures cost the department. The small number of cases that were identified as well as their uneven distribution amongst the different services of the department suggest that considerable efforts must still be made to improve access for this population to public psychiatric services.

The basic photosynthetic unit containing the reaction centre and the light-harvesting I complex (RC-LHI) of the purple non-sulphur bacterium Rhodospirillum rubrum was purified and reconstituted into two-dimensional (2D) membrane crystals. Transmission electron microscopy using conventional techniques and cryoelectron microscopy of the purified single particles and of 2D crystals yielded a projection of the RC-LHI complex at a resolution of at least 1.6 nm. In this projection the LHI ring appears to have a square symmetry and packs in a square crystal lattice. The square geometry of the LHI ring was observed also in images of single isolated particles of the RC-LHI complex. However, although the LHI units are packed identically within the crystal lattice, a new rotational analysis developed here showed that the reaction centres take up one of four possible orientations within the ring. This fourfold disorder supports our interpretation of a square ring symmetry and suggests that a hitherto undetected component may be present within the photosynthetic unit.
