Les hépatopathies sont rares au cours de la grossesse, mais peuvent avoir des conséquences dramatiques pour la mère et l'enfant si elles ne sont pas diagnostiquées à temps. On différencie principalement les hépatopathies spécifiquement secondaires à la grossesse des intercurrentes. Parmi les premières, on peut citer les manifestations hépatiques de l'hyperemesis gravidarum, la cholestase intrahépatique gravidique, les atteintes hépatiques lors d'une (pré-)éclampsie, y compris le syndrome HELLP, et la stéatose hépatique aiguë gravidique. Le diagnostic différentiel est basé sur l'anamnèse (stade de la grossesse), la clinique, quelques examens de laboratoire et l'échographie comme imagerie de première intention. Le traitement d'une cholestase intrahépatique gravidique par acide ursodésoxycholique améliore le prurit et les tests hépatiques maternels. Une surveillance rapprochée de la grossesse reste cependant indispensable. Lors d'un syndrome HELLP ou d'une stéatose hépatique aiguë gravidique, il faut procéder à l'accouchement le plus vite possible. Toutes les hépatopathies déjà connues nécessitent un suivi strict durant la grossesse. While liver diseases are a rare occurrence in pregnancy, they may have dramatic implications for mother and child if not detected in good time. A distinction is drawn between pregnancy-specific liver diseases and intercurrent liver diseases during pregnancy. SUMMARY : The traditional medical advice for pregnant women has been to reduce their physical activity (PA) levels. The advice was based on concerns that exercise could affect pregnancy outcomes by increasing core body temperature, by increasing the risk of maternal musculoskeletal injury and by altering the transplacental transport of oxygen and nutrients to maternal skeletal muscle rather than to the developing foetus. In the meantime, several studies have provided new information on adaptation of the pregnant woman and her foetus to moderate PA. New investigations have shown no adverse maternal or neonatal outcomes, abnormal foetal growth, increase in early pregnancy loss, or late pregnancy complications. Moreover, enrolment in moderate PA has proven to result in marked health benefits including improved maternal cardiovascular function, reduction of excessive weight gain and fat retention, less complicated labour, improved foetal stress tolerance and neurobehavioral maturation. In view of the beneficial effects, current recommendations encourage healthy pregnant women to engage in 30 minutes of moderate PA on most, if not all, days of the week. This thesis work addressed several questions. Firstly, it examined whether compliance with the recommended levels of PA during pregnancy results in better preparedness for the sudden physical exertion of labour and delivery. Secondly, it measured PA during pregnancy as compared to postpartum. Lastly, it assessed the influence of pre-pregnancy body mass index on gestational resting metabolic rate. Data collection was conducted on healthy women living in Switzerland during the third trimester of pregnancy and postpartum. Total and activity energy expenditure was assessed through 24-hour heart rate and accelerations recordings, and cardiovascular fitness through an individual step-test. Information related to pregnancy, labour and delivery was collected from medical records. The results indicate that a minimum 30 min of moderate PA per day during pregnancy are associated with better cardiovascular fitness and lower risk of operative delivery with no negative effects on maternal and foetal conditions (study 1). Despite these benefits, a substantial proportion of pregnant women (39%) living in Switzerland do not meet the PA recommendations. The decrease in activity related energy expenditure during pregnancy compared to postpartum was measured to be around 100 kcal/day (~13%), whereas the total energy expenditure was found to increase by 300 kcal/day (study 2). Thus, the energy cost of late pregnancy in Switzerland corresponds to 200 kca/day. These findings are based on average values of the study group. It should be noted, however, that large variations in individual energy expenditure may occur depending on the pre-pregnancy body mass index (study 3). When adjusted to body weight, gestational resting metabolic rate is significantly lower among women of high pre-pregnancy body mass index compared to women of normal or low pre-pregnancy body mass index. This can be explained by the fact that resting metabolic rate is primarily a function of fat-free mass, and when expressed per kg body weight, it decreases as the percentage of body fat increases. If energy intake is not modified appropriately in order to match lower energy cost per kg body weight in overweight and obese women it will result in positive energy balance, thus contributing to the current trend towards increasing adiposity in affluent society. The results of these studies go beyond the current state of knowledge on PA and pregnancy (study 4) and provide valid evidence to guide clinical practice. In view of the current epidemic of sedentary behaviour and obesity related pathology, the findings contribute new and reliable information to public health policies regarding the effects of PA in pregnancy, an important period of life for both mother and infant.

OBJECTIVE: Virologic failure of HIV-positive patients is of special concern during pregnancy. We compared virologic failure and the frequency of treatment changes in pregnant and non-pregnant women of the Swiss HIV Cohort Study. METHODS: Using data on 372 pregnancies in 324 women we describe antiretroviral therapy during pregnancy. Pregnant women on HAART at conception (n = 131) were matched to 228 non-pregnant women (interindividual comparison) and to a time period of equal length before and after pregnancy (intraindividual comparison). Women starting HAART during pregnancy (n = 145) were compared with 578 non-pregnant women starting HAART. FINDINGS: The median age at conception was 31 years, 16% (n = 50) were infected through injecting drug use and the median CD4 cell count was 489 cells/microl. In the majority of pregnancies (n = 220, 59%), women had started ART before conception. When ART was started during pregnancy (n = 145, 39%), it was mainly during the second trimester (n = 100, 69%). Two thirds (n = 26) of 35 women starting in the third trimester were diagnosed with HIV during pregnancy. The risk of virologic failure tended to be lower in pregnant than in non-pregnant women [adjusted odds ratio 0.52 (95% confidence interval 0.25-1.09, P = 0.08)], but was similar in the intraindividual comparison (adjusted odds ratio 1.04, 95% confidence interval 0.48-2.28). Women starting HAART during pregnancy changed the treatment less often than non-pregnant women. CONCLUSION: Despite the physiological changes occurring during pregnancy, HIV infected pregnant women are not at higher risk of virologic failure.

Lebererkrankungen treten in der Schwangerschaft selten auf, können jedoch für Mutter und Kind dramatische Folgen haben, wenn sie nicht rechtzeitig erkannt werden. Prinzipiell unterscheidet man schwangerschaftsspezifische Lebererkrankungen von interkurrierenden Lebererkrankungen während der Schwangerschaft. Zu ersteren gehören die hepatischen Manifestationen der Hyperemesis gravidarum, die intrahepatische Schwangerschaftscholestase, die Leberbeteiligung bei Präeklampsie bzw. Eklampsie inkl. HELLP-Syndrom und die akute Schwangerschaftsfettleber. Die Differentialdiagnose schwangerschaftsassoziierter Lebererkrankungen basiert auf der Anamnese (Stadium der Schwangerschaft), der Klinik, wenigen Laboruntersuchungen und einer Ultrasonographie als primärem bildgebendem Verfahren. Die Behandlung der intrahepatischen Schwangerschaftscholestase mit Ursodeoxycholsäure verbessert den Pruritus und die mütterlichen Leberwerte. Eine engmaschige Überwachung der Schwangerschaft bleibt jedoch unabdingbar. Beim HELLP-Syndrom und der akuten Schwangerschaftsfettleber ist die rasche Entbindung anzustreben. Vorbestehende Lebererkrankungen bedürfen in der Schwangerschaft einer intensivierten Kontrolle. Two contrasted father-mother-infant interactions are observed longitudinally during trilogue play. They illustrate the contribution of recent research to the exploration of triangulation in infancy: namely, the infant's capacity to handle triangular interactions and share her affects with her two parents, and the way that this capacity is recruited in functional versus problematic alliances. It is likely that an infant under stress when interacting with one parent will protest at that parent and also at the other. Such is the case when, for example, the father acts intrusively while playing with his baby. The infant is then driven to avert and turns to the mother. The regulation of this dyadic intrusion-avoidance pattern at family level depends on the family alliance. When coparenting is supportive, the mother validates the infant's bid for help without interfering with the father. Thus, the problematic pattern is contained in the dyad, and the infant's triangular capacities remain in the service of her own developmental goals. But when coparenting is hostile-competitive, the mother ignores the infant's bid or engages with her in a way that interferes with her play with her father. In this case, the infant's triangular capacities are used to relieve the tension between the parents. The importance of tracing family process back to infancy for family therapy is discussed.

Ces trente dernières années, on note en Suisse une augmentation significative de l'incidence du syndrome de détresse respiratoire (SDR) chez le nouveau-né (NN), touchant particulièrement les enfants avec un poids de naissance >2500 g. En même temps, le taux des césariennes (CS) s'est aussi accru. Une explication pour une éventuelle corrélation entre les deux évolutions est une augmentation en particulier des CS électives qui ont tendance à être planifiées à un terme précoce pour éviter la mise en travail spontanée. Suite à cela, le foetus est privé de différents mécanismes qui favorisent l'adaptation pulmonaire périnatale. Les bénéfices réels de la CS sur la morbidité tant foetale que maternelle ne doivent pas faire oublier que la CS est un facteur de risque pour le SDR du NN. Ce risque peut être diminué efficacement en planifiant une CS élective après 39 semaines révolues. In Switzerland, the rate of respiratory distress in neonates needing hospitalization has doubled over the last thirty years, concerning in particular babies weighing more than 2500 g. In the same time, the rate of Caesarean section (CS) has also multiplied. We suppose that a link between the two evolutions might be the increase of elective CS. They tend to be planned early at term to avoid the onset of spontaneous labour As a consequence, the foetus is deprived of different mechanisms helping pulmonary transition around birth. The potential benefits of CS regarding morbidity of foetus and mother should not overshadow that CS is a significant risk factor for respiratory problems of the neonate. This risk could be dramatically decreased by planning elective CS only after completed 39 weeks of gestation

Codeine is commonly used in North America in the postpartum period for pain associated with episotomyand caesarean section. Analgesic properties of codeine are mainly due to its metabolisation intomorphine (5-10%) via CYP2D6. This enzyme is subject to genetic variability, which can alter theamount of active narcotic excreted into breastmilk. A recent case report highlighted this issue, reportingfatal consequences in a newborn whose mother was taking codeine for episiotomy-related pain (1-2). New-born's blood (post-mortem) and mother's milk showed high morphine concentrations. Genotypeanalysis classified the mother as a CYP2D6 ultrarapid metabolizer, a genotype associated withenhanced formation of morphine from codeine. The authors concluded "clinical and laboratory picturewas consistent with opioid toxicity leading to neonatal death". Subsequent comments expressed reasonnabledoubts on this conclusion, though (3-4). Since, anxiety increased about the safety of codeineduring breastfeeding and genetic screening was proposed as a prevention strategy.STIS position:? Codeine with paracetamol is not a usual pain prescription in the postpartum period in Switzerland.This markedly reduces codeine use during lactation in our country, and may partly explain why webarely collected 3 codeine exposures through breastmilk in 15 years at the STIS (all reported afterabove case's publication and without side effects).? Other centrally acting analgesics are not considered safer (5) than codeine during lactation andrequire close observation for somnolence in both the mother and the infant in case of repeated maternaldosage. A lack of monitoring was salient in the case reported above (1).? If the incidence of CYP2D6 polymorphism (1-10% of individuals in Western Europe) (6) can beconsidered of clinical significance, it is not the exclusive predisposing factor to toxic effects. Healthynewborns can be particularly sensitive to even usual doses of narcotic analgesics because of immaturedrug disposition (7). Conditions leading to impaired clearance or increased susceptibility inthe infant (e.g. preterm birth, metabolic diseases) represent further risk factors for opioid toxicity,regardless of the molecule.In conclusion, when prescribed on a large scale, codein can be rarely associated with adverse drugreactions in breastfed infants (8-9). However, other central acting analgesics cannot be considered asinvariably safer. Therefore, paracetamol and well documented NSAID should be used in 1st choiceduring lactation. In case of severe pain, codeine (with paracetamol) remains an acceptable choice butrequires close monitoring, and breastfeeding mothers should be educated regarding risks related toaccumulation in the newborn. Finally, it is doubtful whether CYP2D6 genetic screening would preventall toxic effects, as other risk factors exist for opioids toxicity

Despite evidence for the important role of oestrogens in the aetiology and pathophysiology of chronic immune/inflammatory diseases, the previous view of an unequivocal beneficial effect of oestrogens on RA compared with a detrimental effect on SLE has to be reconsidered. Likewise, the long-held belief that RA remits in the majority of pregnant patients has been challenged, and shows that only half of the patients experience significant improvement when objective disease activity measurements are applied. Pregnancies in patients with SLE are mostly successful when well planned and monitored interdisciplinarily, whereas a small proportion of women with APS still have adverse pregnancy outcomes in spite of the standard treatment. New prospective studies indicate better outcomes for pregnancies in women with rare diseases such as SSc and vasculitis. Fertility problems are not uncommon in patients with rheumatic disease and need to be considered in both genders. Necessary therapy, shortly before or during the pregnancy, demands taking into account the health of both mother and fetus. Long-term effects of drugs on offspring exposed in utero or during lactation is a new area under study as well as late effects of maternal rheumatic disease on children.

Anophthalmia and microphthalmia are important birth defects, but their pathogenesis remains incompletely understood. We studied a patient with severe unilateral microphthalmia who had a 2.7 Mb deletion at chromosome 18q22.1 that was inherited from his mother. In-situ hybridization showed that one of the deleted genes, TMX3, was expressed in the retinal neuroepithelium and lens epithelium in the developing murine eye. We re-sequenced TMX3 in 162 patients with anophthalmia or microphthalmia, and found two missense substitutions in unrelated patients: c.116G>A, predicting p.Arg39Gln, in a male with unilateral microphthalmia and retinal coloboma, and c.322G>A, predicting p.Asp108Asn, in a female with unilateral microphthalmia and severe micrognathia. We used two antisense morpholinos targeted against the zebrafish TMX3 orthologue, zgc:110025, to examine the effects of reduced gene expression in eye development. We noted that the morphant larvae resulting from both morpholinos had significantly smaller eye sizes and reduced labeling with islet-1 antibody directed against retinal ganglion cells at 2 days post fertilization. Co-injection of human wild type TMX3 mRNA rescued the small eye phenotype obtained with both morpholinos, whereas co-injection of human TMX3(p.Arg39Gln) mutant mRNA, analogous to the mutation in the patient with microphthalmia and coloboma, did not rescue the small eye phenotype. Our results show that haploinsufficiency for TMX3 results in a small eye phenotype and represents a novel genetic cause of microphthalmia and coloboma. Future experiments to determine if other thioredoxins are important in eye morphogenesis and to clarify the mechanism of function of TMX3 in eye development are warranted.

Rheumatoid arthritis occurs frequently in women in childbearing years. With the improvement of the treatments, more patients with rheumatoid arthritis consider a pregnancy. Close co-operation between the physician and the obstetrician caring for the mother and the foetus is necessary. The disease should be well controlled at the time of the conception, although an amelioration of rheumatoid arthritis occurs in about 75% of pregnancies, in the first trimester. Some medications can be used during pregnancy and lactation. There is no indication of any adverse effects of rheumatoid arthritis on pregnancy outcome. The mother needs to be followed up regularly after delivery because of the high risk of post-partum flare.

In the present article on intergenerational transmission of attachment representations, we use mothers' and fathers' Adult Attachment Interview classifications to predict a 3-year-old's responses to the Attachment Story Completion Task (ASCT). We present a Q-sort coding procedure for the ASCT, which was developed for children as young as three. The Q-sort yields scores on four attachment dimensions (security, deactivation, hyperactivation, and disorganization). One-way ANOVAs revealed significant mother-child associations for each dimension, although results for the hyperactivation and disorganization dimensions were significant only according to contrast tests. Conversely, no father-child association was found, regardless of the dimension considered. Findings are discussed in terms of the respective part played by each parent in their children's emotional development.

BACKGROUND: Insulin-like growth factor-I (IGF-I) and C-reactive protein (CRP) may be positively associated with the risk of epithelial ovarian cancer (EOC) but no previous studies have investigated their associations with non-epithelial ovarian cancers (NEOC). METHODS: A case-control study was nested within the Finnish Maternity Cohort. Case subjects were 58 women diagnosed with sex cord-stromal tumors (SCST) and 30 with germ cell tumors (GCT) after recruitment. Control subjects (144 for SCST and 74 for GCT) were matched for age, parity, and date of blood donation of the index case. RESULTS: Doubling of IGF-I concentration was not related to maternal risk of either SCST (OR 0.97, 95% CI 0.58-1.62) or GCT (OR 1.13, 95% CI 0.51-2.51). Similarly, doubling of CRP concentrations was not related to maternal risk of either SCST (OR 1.10, 95% CI 0.85-1.43) or GCT (OR 0.93, 95% CI 0.68-1.28). CONCLUSIONS: Pre-diagnostic IGF-I and CRP concentrations during the first trimester of pregnancy were not associated with increased risk of NEOC in the mother. Risk factors for NEOC may differ from those of EOC.

BACKGROUND: European Surveillance of Congenital Anomalies (EUROCAT) is a network of population-based congenital anomaly registries in Europe surveying more than 1 million births per year, or 25% of the births in the European Union. This paper describes the potential of the EUROCAT collaboration for pharmacoepidemiology and drug safety surveillance. METHODS: The 34 full members and 6 associate members of the EUROCAT network were sent a questionnaire about their data sources on drug exposure and on drug coding. Available data on drug exposure during the first trimester available in the central EUROCAT database for the years 1996-2000 was summarised for 15 out of 25 responding full members. RESULTS: Of the 40 registries, 29 returned questionnaires (25 full and 4 associate members). Four of these registries do not collect data on maternal drug use. Of the full members, 15 registries use the EUROCAT drug code, 4 use the international ATC drug code, 3 registries use another coding system and 7 use a combination of these coding systems. Obstetric records are the most frequently used sources of drug information for the registries, followed by interviews with the mother. Only one registry uses pharmacy data. Percentages of cases with drug exposure (excluding vitamins/minerals) varied from 4.4% to 26.0% among different registries. The categories of drugs recorded varied widely between registries. CONCLUSIONS: Practices vary widely between registries regarding recording drug exposure information. EUROCAT has the potential to be an effective collaborative framework to contribute to post-marketing drug surveillance in relation to teratogenic effects, but work is needed to implement ATC drug coding more widely, and to diversify the sources of information used to determine drug exposure in each registry.

Le passage de la vie solitaire à la vie sociale représente une des principales transitions évolutives. La socialité a évolué au sein de plusieurs taxons du règne animal et notamment chez les insectes sociaux qui ont atteint son niveau le plus élevé : l'eusocialité. Les colonies d'insectes sociaux se composent d'une reine, qui monopolise la reproduction, et d'ouvrières, non-reproductrices ou parfois stériles, qui aident à élever la descendance de la reine. Selon la théorie de la sélection de parentèle, les ouvrières augmentent leur fitness (succès reproducteur) non pas à travers leur propre progéniture, mais en aidant des individus apparentés (leur reine) à produire davantage de descendants. Cette théorie prédit ainsi que les ouvrières ont un intérêt à rester fidèles à leur nid natal. Toutefois, chez la guêpe tropicale Polistes canadensis, de nombreuse ouvrières visitent d'autres nids que leur nid natal : un phénomène appelé « dérive des ouvrières ». Le but de ce doctorat est ainsi de mieux comprendre les mécanismes impliqués dans ce comportement particulier des ouvrières ainsi que ces implications pour la théorie de la sélection de parentèle. Nous avons examiné le comportement de dérive des ouvrières à travers une étude des dynamiques sociales chez la guêpe tropicale P. canadensis. Mes résultats montrent que les populations de P. canadensis se composent en différentes agrégations de nids. Malgré de précédentes suggestions, on n'observe qu'une faible viscosité génétique au sein des populations de P. canadensis étudiées. On retrouve toutefois un degré d'apparentement entre nids d'une même agrégation. Ceci suggère que les ouvrières dériveuses sont susceptibles de bénéficier de fitness indirect en aidant les nids proches géographiquement. De plus, ces échanges d'ouvrières ne semblent pas accidentels puisque l'on constate des variations de taux de dérive et puisque les déplacements observés entre nids persistent sur plusieurs périodes de temps. La charge de travail, qui correspond aux différences d'effort de fourragement entre nid visités et natals, est décrite dans notre étude comme potentiel facteur expliquant le comportement de dérive des ouvrières chez P. canadensis. Nos expériences de retrait d'ouvrières et de couvain ont révélées que les dériveuses ne semblent pas répondre aux changements de besoins en aide des nids visités. Les ouvrières dériveuses biaisent leur effort en aidant leur propre nid, par lequel elles bénéficient le plus en termes de fitness indirect, avant de se consacrer à tout autre nid. Dans l'ensemble, ces résultats sur la dérive des ouvrières chez P. canadensis sont cohérents et suggèrent que ce comportement est une importante stratégie de reproduction alternative chez cette espèce qui contribue à la fitness indirecte de ces ouvrières non-reproductrices. De plus, ce doctorat apporte des informations sur la structure génétique des populations de guêpes Polistes et décrit le rôle des ouvrières inactives. Celles-ci semblent servir de réserve en ouvrières apportant du support à la colonie dans l'éventualité d'une perte d'individus. Plus généralement, ce travail met l'accent sur l'organisation complexe et l'adaptabilité des individus dans les sociétés d'insectes. - One major transition in evolution is the shift from solitary to social life. Sociality has evolved in a few taxa of the animal kingdom, most notably in the social insects which have achieved the highest level of sociality: eusociality. Colonies of social insects are formed by a reproductive queen, and many non-reproductive or sterile workers who help raise their mother queen's offspring. Kin selection theory explains worker behaviour in terms of the indirect fitness they gain from raising non-offspring kin. It therefore predicts that workers should stay faithful to their natal nests, to which they are the more related. However, in the tropical paper wasps Polistes canadensis, high levels of nest-drifting, whereby workers spend time on other neighbouring nests, has been reported. This PhD aimed at understanding the mechanisms involved in this peculiar behaviour as well as its implications for kin selection theory. I examined nest-drifting through the study of the social dynamics of the tropical paper wasp P. canadensis. My results showed that populations of this species of paper wasps are composed of different aggregations of nests. The studied populations showed little limited dispersal (viscosity), despite previous suggestion, but nests within these aggregations were more related to each other than nests outside of aggregations. This suggested that drifters may benefit from indirect fitness when helping on neighbouring nests. Drifting was unlikely to be accidental since we found drifting patterns at various rates and consistently over several time periods during monitoring. Workload (differences in colony-level foraging effort) was also a potential factor explaining nest-drifting in P. canadensis. Worker and brood removal experiments revealed that drifters do not respond to any changes in the need for help in the non-natal nests they visit. Drifters thus bias their help in their natal nests, from which they may benefit the most in terms of indirect fitness, before investing in others. Altogether, these results on nest-drifting in P. canadensis are consistent and suggest that nest-drifting is an important alternative reproductive strategy, contributing to the indirect fitness benefits gained by non-reproductive wasps. Additionally, this PhD provides information on the genetic structure of paper wasps' populations and demonstrates the role of inactive or lazy wasps as a "reserve worker force", which provides resilience to the colony in the event of worker mortality. More generally, this work further highlights the complex organization and adaptability of individuals in insect societies.
