What do we know about the effectiveness of various treatments of alcoholism? This review of literature shows that lack--or weaknesses--of published studies make it impossible to draw definite conclusions. Rigorous controlled studies show high rates of spontaneous remission and important uncertainties about specialised treatments of alcoholism. However, except for severe dependence that may well require a different approach, brief interventions conducted by non-specialists have proved highly effective for at-risk alcohol drinkers: based on minimal medical advice, they increase the chances of lowering alcohol consumption. General practitioners may thus represent on important link in the therapeutic chain.

Flexible intramedullary nailing (FIN) is the gold standard treatment for femur fracture in school-aged children. It has been performed successfully in younger children, although Spica cast immobilisation (SCI) has been the most widely used strategy to date. METHOD: A retrospective analysis was performed between two comparable groups of children aged 1-4 years with a femoral shaft fracture. Two University hospitals, each using specific treatment guidelines, participated in the study: SCI in Group I (Basel, Switzerland) and FIN in Group II (Lausanne, Switzerland). RESULTS: Group I included 19 children with a median age of 26 months (range 12-46 months). Median hospital stay was 1 day (range 0-5 days) and casts were retained for a median duration of 21 days (range 12-29 days). General anaesthesia was used in six children and sedation in four. Skin breakdown secondary to cast irritation occurred in two children (10.5%). The median follow-up was 114 months (range 37-171 months). No significant malunion was noted. Group II included 27 children with a median age of 38.4 months (range 18.7-46.7 months). Median hospital stay was 4 days (range 1-13 days). All children required general anaesthesia for insertion and removal of the nails. Free mobilisation and full weight bearing were allowed at a median of 2 days (range 1-10 days) and 7 days (range 1-30 days), respectively, postoperatively. Nail exteriorisation was noted in three children (11%). The median follow-up was 16.5 months (range 8-172 months). No significant malunion was reported. CONCLUSIONS: Young children with a femoral shaft fracture treated by SCI or FIN had similarly favourable outcomes and complication rates. FIN allowed earlier mobilisation and full weight bearing. Compared to SCI, a greater number of children required general anaesthesia. In a pre-school child with a femoral shaft fracture, immediate SCI applied by a paediatric orthopaedic team following specific guidelines allowed early discharge from hospital with few complications.

The 1st federal transplant law was enforced in July 2007 with the obligation to promote quality and efficiency in the procedures for organ and tissue donation for transplantation. The Latin organ donation programme (LODP) created in 2008 aims to develop organ donation in 17 public hospitals in 7 Latin cantons, covering 2.2 million people; 29% of the Swiss population. The implementation of various effective measures by the LODP enabled the increase in the number of donors by 70% between 2008 and 2010, with four organs procured per donor; greatly exceeding the European average of three. The results show that LODP has successfully professionalised the system and we can only hope that similar organisations will be put into place throughout Switzerland.

Objective: Enhanced Recovery After Surgery (ERAS) clinical pathways in¦colorectal surgery are known to reduce postoperative complications leading¦to shortened hospital stay. However, the implementation of such an ERAS¦pathway requires time and financial investment. This study evaluates whether¦the savings related to the reduction in the length of stay (LOS) outweigh the¦costs of implementing an ERAS pathway.¦Methods: An ERAS pathway was implemented in our institution for colorectal¦surgery. The first 50 consecutive patients subjected to this ERAS pathway¦(ERAS group) were compared to 50 consecutive patients that were operated one¦year before its introduction (control group). Primary LOS, readmission within¦30 days, and total costs based on costs per day were compared. The mean costs¦per day were: 3,263 CHF for intensive care, 1,152 CHF for intermediate care,¦and 728 CHF for basic care.¦Results: Primary LOS was shorter in the ERAS group than in the control¦group: median 7 (interquartile range 5-12·25) versus 10 (7-18) days (P =¦0·0025). The readmission within 30 postoperative days was similar in both¦groups (2 patients each). In the ERAS group, the added primary LOS was¦485 days (379 in basic care, 99 in intermediate care, 7 in intensive care) compared¦to 706 days in the control group (533 in basic care, 146 in intermediate care,¦27 in intensive care). The total costs were significantly lower for the 50 ERAS¦patients compared to the control group: 412,801 CHF versus 644,317 CHF (P <¦0·01). Investments required for the 50 first ERAS patients were approximately¦83,544 CHF, including 348 working hours as well a full-time ERAS dedicated¦nurse. The overall cost saving was approximately 2,959 CHF per patient.¦Conclusion: Implementation of an ERAS pathway significantly reduced LOS¦after colorectal surgery. The financial investment to introduce and maintain¦such a pathway is clearly inferior to the cost-saving of reduced hospital stay.

OBJECTIVE: To determine if the results of resin-dentin microtensile bond strength (µTBS) is correlated with the outcome parameters of clinical studies on non-retentive Class V restorations. METHODS: Resin-dentin µTBS data were obtained from one test center; the in vitro tests were all performed by the same operator. The µTBS testing was performed 8h after bonding and after 6 months of storing the specimens in water. Pre-test failures (PTFs) of specimens were included in the analysis, attributing them a value of 1MPa. Prospective clinical studies on cervical restorations (Class V) with an observation period of at least 18 months were searched in the literature. The clinical outcome variables were retention loss, marginal discoloration and marginal integrity. Furthermore, an index was formulated to be better able to compare the laboratory and clinical results. Estimates of adhesive effects in a linear mixed model were used to summarize the clinical performance of each adhesive between 12 and 36 months. Spearman correlations between these clinical performances and the µTBS values were calculated subsequently. RESULTS: Thirty-six clinical studies with 15 adhesive/restorative systems for which µTBS data were also available were included in the statistical analysis. In general 3-step and 2-step etch-and-rinse systems showed higher bond strength values than the 2-step/3-step self-etching systems, which, however, produced higher values than the 1-step self-etching and the resin modified glass ionomer systems. Prolonged water storage of specimens resulted in a significant decrease of the mean bond strength values in 5 adhesive systems (Wilcoxon, p<0.05). There was a significant correlation between µTBS values both after 8h and 6 months of storage and marginal discoloration (r=0.54 and r=0.67, respectively). However, the same correlation was not found between µTBS values and the retention rate, clinical index or marginal integrity. SIGNIFICANCE: As µTBS data of adhesive systems, especially after water storage for 6 months, showed a good correlation with marginal discoloration in short-term clinical Class V restorations, longitudinal clinical trials should explore whether early marginal staining is predictive for future retention loss in non-carious cervical restorations.

Introductionþ: L'insulinothérapie intraveineuse est la mieux adaptée pour obtenirun contrôle glycémique rapidement efficace ou lors de besoins en insulinechangeants de façon peu prévisible, mais son emploi hors des soins intensifs seheurte souvent aux manque de formation et réticences des soignants. L'inclusionL'inclusiondu contrôle glycémique rapide dans nos standards institutionnels de priseen charge de l'AVC aigu a suscité une demande de protocole thérapeutiqueadapté aux besoins de l'Unité cérébrovasculaire.Patients et méthodesþ: Le protocole d'insulinothérapie a été dérivé d'algorithmespubliés intégrant glycémie actuelle, cinétique glycémique et sensibilité àl'insuline du patient. Aux repas, une augmentation du débit d'insuline iv. pendant1 h a été ajoutée. Les objectifs glycémiques étaient 4-6þmmol/l en préprandialetþ< 8þmmol/l en postprandial. L'implémentation s'est faite à travers unprocessus de co-construction (outils de gestion, documents et activités de formation)avec les responsables médico-infirmiers du service.Résultatsþ: Les données des 90 premiers patients ont été analysées (diabète connuþ:38, hyperglycémie nouvelleþ: 52, 2715h de traitement cumulées). Les duréesd'insulinothérapie iv. étaient de 34,5 h [interquartile 24-39] et 26,5 h [21-36,3] respectivement(pþ=þ0,03), les délais d'atteinte de l'objectif de 5 h [4.0-8.25] et 7 h[4.0-9.75] (pþ=þns.). Pendant les 24 h suivantes, les taux de glycémies dans la cibleétaient de 70,4þ%/81,3þ% (90,3þ%/94,6þ% entre 4-8þmmol/l), avec un faible tauxd'hypoglycémies (3,9þ%/3,1þ%þ< 4,0þmmol/l, 0,4þ%/0,2þ%þ<þ3,3þmmol/l) et un contrôleglycémique postprandial comparable (excursions +2,6þmmol/l [0,7-3,9] et+1,7þmmol/l [0,6-3,7]þ; Nþ=þ75þ; pþ=þns.).Conclusionþ: L'insulinothérapie intraveineuse hors des soins intensifs est faisable,hautement sûre et efficace, même avec des objectifs glycémiques particulièrementstricts. Outre la fiabilité perçue de l'outil de gestion, la démarche departenariat adoptée avec les soignants, permettant la prise en compte de leurspréoccupations à chaque étape du processus, a été un facteur de succès importantpour son implémentation.

Sexual selection in lek-breeding species might drastically lower male effective population size, with potentially important consequences for evolutionary and conservation biology. Using field-monitoring and parental-assignment methods, we analyzed sex-specific variances in breeding success in a population of European treefrogs, to (1) help understanding the dynamics of genetic variance at sex-specific loci, and (2) better quantify the risk posed by genetic drift in this species locally endangered by habitat fragmentation. The variance in male mating success turned out to be markedly lower than values obtained from other amphibian species with polygamous mating systems. The ratio of effective breeding size to census breeding size was only slightly lower in males (0.44) than in females (0.57), in line with the patterns of genetic diversity previously reported from H. arborea sex chromosomes. Combining our results with data on age at maturity and adult survival, we show that the negative effect of the mating system is furthermore compensated by the effect of delayed maturity, so that the estimated instantaneous effective size broadly corresponded to census breeding size. We conclude that the lek-breeding system of treefrogs impacts only weakly the patterns of genetic diversity on sex-linked genes and the ability of natural populations to resist genetic drift.

Résumé¦L'Embrochage Centro-Médullaire Elastique Stable (ECMES) est le traitement de choix des fractures du fémur chez l'enfant en âge scolaire. Il est pratiqué avec succès chez le jeune enfant, alors que l'Immobilisation par Plâtre (IP) était la technique la plus largement utilisée jusque-là.¦Méthode : Une analyse rétrospective comparant deux groupes d'enfants âgés de 1 à 4 ans avec des fractures diaphysaires du fémur a été effectuée. Deux hôpitaux universitaires, utilisant chacun une méthode de traitement spécifique, ont participé à cette étude : l'IP dans le groupe I (Bâle, Suisse) et l'ECMES dans le groupe II (Lausanne, Suisse).¦Résultats : Le groupe I inclue 19 enfants avec un âge médian de 26 mois (12-46 mois). La médiane du séjour hospitalier est de 1 jour (0-5 jours) et le plâtre est laissé en place pour une durée médiane de 21 jours (12-29 jours). Une anesthésie générale a été nécessaire chez 6 enfants et une sédation chez 4. Des lésions cutanées secondaires au plâtre sont apparues chez 2 enfants (10.5%). La médiane de la durée du suivi est de 114 mois (37-171 mois). Aucun défaut de consolidation n'est à déplorer. Le groupe II inclue 27 enfants avec un âge médian de 38.4 mois (18.7-46.7 mois). La médiane du séjour hospitalier est de 4 jours (1-13 jours). Tous les enfants ont nécessité une anesthésie générale pour la mise en place et pour le retrait des broches. La mobilisation et la mise en charge complète du membre ont été permises respectivement à une médiane de 2 jours (1-10 jours) et 7 jours (1-30 jours) postopératoires. Une complication sous la forme d'une extériorisation à la peau d'une broche a été notée chez 3 enfants (11%). La médiane de la durée du suivi et de 16.5 mois (8-172 mois). Aucun défaut de consolidation n'est à déplorer.¦Conclusion : Les jeunes enfants présentant une fracture diaphysaire du fémur, traité pas IP ou ECMES, ont des résultats favorables et des taux de complications similaires. L'ECMES permet une mobilisation et une charge complète sur le membre fracturé plus rapide. Mais comparé à l'IP, l'ECMES requiert un plus grand nombre d'anesthésies générales. Chez un enfant d'âge préscolaire présentant une fracture diaphysaire du fémur, l'application immédiate d'un plâtre par une équipe orthopédique pédiatrique entraînée à la mise en place de plâtre chez l'enfant, permet un retour à domicile rapide et un taux de complication bas.
