The prevalence of cardiovascular diseases is high in the old age. These conditions have a negative impact on the quality of life and are associated with a high risk of disability. A marked increase in the number of affected individuals is likely, in coming decades, with population aging. Primary cardiovascular prevention, but also an early recognition of subclinical heart diseases and secondary and tertiary prevention will be of up most importance for individuals (quality of life) and for societies (burden of functional impairments) as the baby-boom generation reaches retirement age.

Background¦Erythrocyte MCV might be used as an inexpensive marker to predict and¦optimize the efficacy and tolerability of thiopurine therapy in IBD patients.¦Aim and methods¦This retrospective observational study aimed to assess the monitoring¦performances of MCV in patients under 3 months or more thiopurine treatment followed up¦in the Swiss IBD Cohort Study. All available MCV, white blood cells (WBC) and 6¦thioguanine nucleotide (6TGN) measurements, among others, were recorded. An IBD¦"flare" was defined as a composite outcome encompassing treatment change,¦colonoscopy, histology, CT scan or MRI reports showing active IBD lesions, occurrence of¦intestinal surgery and IBD-related hospitalisations. Whether MCV measurements predicted¦efficacy of thiopurine treatment was investigated by assessing the statistical association¦between the occurrence of IBD "flares", and the current or recent MCV values, taking into¦account the patient clustering and longitudinal aspect of data.¦Results¦140 patients (77 women), mean age 38 years (17-74), 104 diagnosed with¦Crohn's disease, 36 with ulcerative colitis, mean disease duration 8 years (0.25-36),¦receiving either azathioprine (n=125) or 6-mercaptopurine (n=15) were included, most of¦them over 3-year follow up.¦Thiopurines increased mean patient MCV by an average 5.8±5.2 fL, while¦patientsfluctuated by ±4.3 fL around their individual mean (p<0.001). They decreased¦WBC by an average of 2.4+/- 2.6 G/L (p<0.001).¦Significant associations were observed between the probability of flare occurrence and low¦current MVC (p=0.017) or high current WBC (p=0.009) and, with a relative risk of 3.7% for¦every fL of MCV decrease or 8% for every G/L of WBC increase. Both markers revealed¦some memory effect.¦Despite this, the performance of MCV and WBC to predict IBD "flare" remained rather¦limited, as it is less accurate than the 6-TGN-level , although only determined in a¦subgroup of patients in this study.¦Conclusion¦MCV and WBC deserve to be observed to check and monitor therapeutic¦exposure to thiopurine agents in IBD patients. Unfortunately, their predictive performance¦precludes their privileged use for optimization of therapy. Further prospective studies¦should suitably include the systematic measurement of metabolite concentration.

De nombreuses maladies métaboliques peuvent atteindre la cheville et le tarse postérieur. Dans la phase aiguë, la goutte peut toucher l'arrière-pied, la cheville, le médio-tarse ou le tendon calcanéen. Une rougeur intense des tissus souscutanés du dos du pied peut être en rapport avec une inflammation liée à des microtophus sous-cutanés. Un diagnostic de certitude se fait par la mise en évidence de cristaux d'urate de sodium dans le liquide de ponction articulaire ou dans les tissus. L'imagerie par tomodensitométrie ou par échographie peut orienter de façon pratiquement certaine le diagnostic. Le traitement de la goutte de l'arrière-pied fait appel aux antiinflammatoires, aux anti-inflammatoires non stéroïdiens (AINS) et à la colchicine. Dans la phase chronique, un traitement hypo-uricémiant au long terme est à instaurer. L'hémochromatose se manifeste principalement sous forme d'une arthrose, liée souvent à une chondrocalcinose de la cheville et du tarse postérieur. L'enthésopathie hyperostosante diffuse peut causer des talalgies ou des douleurs du fascia plantaire liées à des exostoses. L'hypercholestérolémie familiale provoque souvent des xanthomes tendineux des tendons calcanéens. Des calcifications apatitiques de la région du talon peuvent s'observer, notamment chez des patients en hémodialyse chronique. Numerous metabolic diseases can affect the ankle and the hind-foot. In the acute phase, gout can affect the rear of the foot, the ankle, the mid-foot and the calcaneal (Achilles) tendon. Intense redness of the subcutaneous tissue of the back of the foot can be present in conjunction with inflammation associated with subcutaneous micro-tophaceous deposits. A definitive diagnosis is made by confirming the existence of sodium urate crystals in joint puncture fluid or in tissue. CT scan or ultrasonography images can also be used to provide a fairly definitive diagnosis. Treatment of gout of the rear of the foot requires the use of anti-inflammatory medication, NSAIDs and colchicine. In the chronic phase, long-term hypouricemic therapy is to be used. Haemochromatosis mainly shows in the form of arthritis, often associated with chondrocalcinosis of the ankle and hind-foot. A diffuse hyperostosis enthesopathy can cause talalgia or pain to the plantar fascia associated with exostoses. Familial hypercholesterolaemia often leads to tendinous xanthoma on the calcaneal tendons. Apatitic calcifications to the heel can also be observed, especially undergoing chronic haemodialysis.

Ce document présente un plan de prévention pour le Canton de Vaud (version octobre 2007). Ce plan concentre ses efforts sur la prévention de quelques maladies (infarctus du myocarde, accident vasculaire cérébral, diabète, cancer colorectal, cancer du sein, cancer du poumon, dépression, fracture ostéoporotique), à l'aide d'un nombre restreint d'interventions (améliorer l'alimentation et augmenter l'activité physique, lutte contre le tabagisme, prévention primaire et secondaire de la maladie athéromateuse, prévention secondaire du cancer colorectal, prévention secondaire du cancer du sein, prévention secondaire de la dépression). Après la présentation de quelques enjeux de ce plan, le rapport conclut sur trois recommandations portant sur les structures de la prévention des maladies et de promotion de la santé, le monitorage des activités de prévention des maladies et de promotion de la santé et enfin sur le mandat et l'organisation de la Commission cantonale de prévention.

Autoimmune glomerulopathies are an important cause of chronic kidney disease. Conventional treatments based on steroids, antiproliferative and cytotoxic agents are efficacious, but highly toxic. Because of their central role in the pathogenesis of autoimmunity, B cells have become an attractive therapeutic target. Rituximab is a monoclonal antibody directed against CD20 expressed on the surface of B cells, inducing profound depletion of B cells in the peripheral blood. In spite of encouraging results regarding the off-label use of Rituximab in membranous nephropathy, systemic lupus erythematosus and small vessel vasculitis, controlled, long-term data, and data with specific renal endpoints are currently lacking.

Controversies regarding the pathogenesis of cardiovascular diseases in HIV patients Since the introduction of HAART (Highly active anti-retroviral therapy), the incidence of cardiovascular events has risen in patients infected with HIV. This development is mainly due to the increased survival in these patients. Nonetheless, the pathogenic effects of HIV on the principal components of haemostasis (endothelium, platelets and the clotting cascade) are the subject of numerous ongoing research studies, and are becoming an argument for starting HAART or for modifying the components of an established therapy. The aim of this article is to raise clinician awareness regarding the issue of cardiovascular disease in the HIV-infected patient.

Les POCT (point of care tests) ont un grand potentiel d'utilisation en médecine infectieuse ambulatoire grâce à leur rapidité d'exécution, leur impact sur l'administration d'antibiotiques et sur le diagnostic de certaines maladies transmissibles. Certains tests sont utilisés depuis plusieurs années (détection de Streptococcus pyogenes lors d'angine, anticorps anti-VIH, antigène urinaire de S. pneumoniae, antigène de Plasmodium falciparum). De nouvelles indications concernent les infections respiratoires, les diarrhées infantiles (rotavirus, E. coli entérohémorragique) et les infections sexuellement transmissibles. Des POCT, basés sur la détection d'acides nucléiques, viennent d'être introduits (streptocoque du groupe B chez la femme enceinte avant l'accouchement et la détection du portage de staphylocoque doré résistant à la méticilline). POCT have a great potential in ambulatory infectious diseases diagnosis, due to their impact on antibiotic administration and on communicable diseases prevention. Some are in use for long (S. pyogenes antigen, HIV antibodies) or short time (S. pneumoniae antigen, P. falciparum). The additional major indications will be community-acquired lower respiratory tract infections, infectious diarrhoea in children (rotavirus, enterotoxigenic E. coli), and hopefully sexually transmitted infections. Easy to use, these tests based on antigen-antibody reaction allow a rapid diagnosis in less than one hour; the new generation of POCT relying on nucleic acid detection are just introduced in practice (detection of GBS in pregnant women, carriage of MRSA), and will be extended to many pathogens

Fecal calprotectin (FC) is a valid biomarker to discriminate with a good sensitivity and specificity the presence of mucosal lesions of the gastrointestinal tube (e.g. ulcers in the context of inflammatory bowel disease (IBD)) from functional disorders (e.g. irritable bowel syndrome). FC is not specific for IBD and can be elevated also in gastrointestinal infections, ischemic colitis or neoplasia. An elevated FC should stimulate further investigations, notably an endoscopic workup. The level of FC correlates with the endoscopic score in Crohn's disease and ulcerative colitis. The correlation of FC and the endoscopic severity is better than the one of CRP or blood leukocytes. Thus, FC can also be used in the follow-up of IBD patients.
