BACKGROUND/AIMS: Brivudin is licensed in several European countries for the treatment of herpetic infections, and is considered safe (approximately 1% of patients with transient elevation of liver enzymes) in large multicenter trials. METHODS: We report a case of acute brivudin hepatitis documented with a liver biopsy in detail. RESULTS: Liver biopsy demonstrated acute liver injury with a predominant cytolytic pattern and features suggestive of a drug-induced immunoallergic hepatitis. Elevated ALT levels returned to normal within weeks. CONCLUSIONS: This is the first published case of acute immunoallergic hepatitis due to brivudin.

Purpose: Letrozole (LET) has recently been shown to be superior to tamoxifen for postmenopausal patients (pts). In addition, LET radiosensitizes breast cancer cells in vitro. We conducted a phase II randomized study to evaluate concurrent and sequential radiotherapy (RT)-LET in the adjuvant setting. We present here clinical results with a minimum follow-up of 24 months. Patients and Methods: Postmenopausal pts with early-stage breast cancer were randomized after conservative surgery to either: A) concurrent RT-LET (LET started 3 weeks before the first day of RT) or B) sequential RT-LET (LET started 3 weeks after the end of RT). Whole breast RT was delivered to a total dose of 50 Gy. A 10-16 Gy boost was allowed according to age and pathological prognostic factors. Pts were stratified by center, adjuvant chemotherapy, boost, and radiation-induced CD8 apoptosis (RILA). RILA was performed before RT as previously published (Ozsahin et al. Clin Cancer Res, 2005). An independent monitoring committee reviewed individual safety data. Skin toxicities were evaluated by two different clinicians at each medical visit (CTCAE v3.0). Lung CT-scan and functional pulmonary tests were performed regularly. DNA samples were screened for SNPs in candidate genes as recently published (Azria et al., Clin Cancer Res, 2008). Results: A total of 150 pts were randomized between 01/05 and 02/07. Median follow-up is 26 months (range, 3-40 months). No statistical differences were identified between the two arms in terms of mean age; initial TNM; median surgical bed volume; post surgical breast volume. Chemotherapy and RT boost were delivered in 19% and 38% of pts, respectively. Nodes received 50 Gy in 23% of patients without differences between both arms. During RT and within the first 6 weeks after RT, 10 patients (6.7%) presented grade 3 acute skin dermatitis during RT but no differences were observed between both arms (4 and 6 patients in arm A and B, respectively). At 26 month of follow-up, grade 2 and more radiation-induced subcutaneous fibrosis (RISCF) was present in 4 patients (3%) without any difference between arm A (n = 2) and B (n = 2), p=0.93. In both arms, all patients that presented a RICSF had a RILA lower than 16%. Sensitivity and specificity were 100% and 39%, respectively.No acute lung toxicities were observed and quality of life was good to excellent for all patients.SNPs analyses are still on-going (Pr Rosenstein, NY). Conclusion: Acute and early late grade 2 dermatitis were similar in both arms. The only factor that influenced RISCF was a low radiation-induced lymphocyte apoptosis yield. We confirmed prospectively the capacity of RILA for identifying hypersensitive patients to radiation. Indeed, patients with RILA superior to 16% did not present late effects to radiation and confirmed the first prospective trial we published in 2005 (Ozsahin et al., Clin Cancer Res).

Airborne microbial products have been reported to promote immune responses that suppress asthma, yet how these beneficial effects take place remains controversial and poorly understood. We have found that pulmonary exposure with the bacterium Escherichia coli leads to a suppression of allergic airway inflammation, characterized by reduced airway-hyperresponsiveness, eosinophilia and cytokine production by T cells in the lung. This immune modulation was neither mediated by the induction of a Th1 response nor regulatory T cells; was dependent on TLR-4 but did not involve TLR-desensitization. Dendritic cell migration to the draining lymph nodes and subsequent activation of T cells was unaffected by prior exposure to E.coli indicating that the immunomodulation was limited to the lung environment. In non-treated control mice ovalbumin was primarily presented by airway CD11b+ CD11c+ DCs expressing high levels of MHC class II molecules whilst the DCs in E.coli-treated mice displayed a less activated phenotype and had impaired antigen presentation capacity. Consequently, in situ Th2 cytokine production by ovalbuminspecific effector T cells recruited to the airways was significantly reduced. The suppression of airways hyper responsiveness was mediated through the recruitment of IL-17-producing gd-T cells; however, the suppression of dendritic cells and T cells was mediated through a distinct mechanism that could not be overcome by the local administration of activated dendritic cells, or by the in vivo administration of TNF-alpha. Taken together, these data reveal a novel multi-component immunoregulatory pathway that acts to protect the airways from allergic inflammation.

Antigen presentation is a required prime event before T-cell activation can occur. Cells which constitutively express major histocompatibility antigen class I or II are responsible for presenting antigens. These are essentially alveolar macrophages (AM) residing mostly in the air spaces, and dendritic cells (DC), which create a tight surveillance network just below the epithelial cells of the airways and in the loose connective tissue around the vessels or in the pleura. AM are poor antigen presenting cells compared to DC. AM when encountering foreign particles or organisms may, however, influence the degree of activity or maturation of neighbouring DC, by releasing cytokines. Thus, we will describe how the innate immune processes may influence specific immunity and perhaps Th1 and Th2 differentiation. Following the description of the differences in phenotype and functions of AM and DC, we will provide data showing that in some pathological conditions, such as sarcoidosis, AM can acquire some specificities of DC.

BACKGROUND & AIMS: Despite the proven ability of immunization to reduce Helicobacter infection in mouse models, the precise mechanism of protection has remained elusive. This study explores the possibility that interleukin (IL)-17 plays a role in the reduction of Helicobacter infection following vaccination of wild-type animals or in spontaneous reduction of bacterial infection in IL-10-deficient mice. METHODS: In mice, reducing Helicobacter infection, the levels and source of IL-17 were determined and the role of IL-17 in reduction of Helicobacter infection was probed by neutralizing antibodies. RESULTS: Gastric IL-17 levels were strongly increased in mice mucosally immunized with urease plus cholera toxin and challenged with Helicobacter felis as compared with controls (654 +/- 455 and 34 +/- 84 relative units for IL-17 messenger RNA expression [P < .01] and 6.9 +/- 8.4 and 0.02 +/- 0.04 pg for IL-17 protein concentration [P < .01], respectively). Flow cytometry analysis showed that a peak of CD4(+)IL-17(+) T cells infiltrating the gastric mucosa occurred in immunized mice in contrast to control mice (4.7% +/- 0.3% and 1.4% +/- 0.3% [P < .01], respectively). Gastric mucosa-infiltrating CD4(+)IL-17(+) T cells were also observed in IL-10-deficient mice that spontaneously reduced H felis infection (4.3% +/- 2.3% and 2% +/- 0.6% [P < .01], for infected and noninfected IL-10-deficient mice, respectively). In wild-type immunized mice, intraperitoneal injection of anti-IL-17 antibodies significantly inhibited inflammation and the reduction of Helicobacter infection in comparison with control antibodies (1 of 12 mice vs 9 of 12 mice reduced Helicobacter infection [P < .01], respectively). CONCLUSIONS: IL-17 plays a critical role in the immunization-induced reduction of Helicobacter infection from the gastric mucosa.

In AKI, dying renal cells release intracellular molecules that stimulate immune cells to secrete proinflammatory cytokines, which trigger leukocyte recruitment and renal inflammation. Whether the release of histones, specifically, from dying cells contributes to the inflammation of AKI is unknown. In this study, we found that dying tubular epithelial cells released histones into the extracellular space, which directly interacted with Toll-like receptor (TLR)-2 (TLR2) and TLR4 to induce MyD88, NF-κB, and mitogen activated protein kinase signaling. Extracellular histones also had directly toxic effects on renal endothelial cells and tubular epithelial cells in vitro. In addition, direct injection of histones into the renal arteries of mice demonstrated that histones induce leukocyte recruitment, microvascular vascular leakage, renal inflammation, and structural features of AKI in a TLR2/TLR4-dependent manner. Antihistone IgG, which neutralizes the immunostimulatory effects of histones, suppressed intrarenal inflammation, neutrophil infiltration, and tubular cell necrosis and improved excretory renal function. In summary, the release of histones from dying cells aggravates AKI via both its direct toxicity to renal cells and its proinflammatory effects. Because the induction of proinflammatory cytokines in dendritic cells requires TLR2 and TLR4, these results support the concept that renal damage triggers an innate immune response, which contributes to the pathogenesis of AKI.

Spontaneous CD4(+) T-cell responses to the tumor-specific antigen NY-ESO-1 (ESO) are frequently found in patients with epithelial ovarian cancer (EOC). If these responses are of effector or/and Treg type, however, has remained unclear. Here, we have used functional approaches together with recently developed MHC class II/ESO tetramers to assess the frequency, phenotype and function of ESO-specific cells in circulating lymphocytes from EOC patients. We found that circulating ESO-specific CD4(+) T cells in EOC patients with spontaneous immune responses to the antigen are prevalently T(H)1 type cells secreting IFN-γ but no IL-17 or IL-10 and are not suppressive. We detected tetramer(+) cells ex vivo, at an average frequency of 1:25,000 memory cells, that is, significantly lower than in patients immunized with an ESO vaccine. ESO tetramer(+) cells were mostly effector memory cells at advanced stages of differentiation and were not detected in circulating CD25(+)FOXP3(+)Treg. Thus, spontaneous CD4(+) T-cell responses to ESO in cancer patients are prevalently of T(H)1 type and not Treg. Their relatively low frequency and advanced differentiation stage, however, may limit their efficacy, that may be boosted by immunogenic ESO vaccines.

Objective. Collaborative quality improvement programs have been successfully used to manage chronic diseases in adults and acute lung complications in premature infants. Their effectiveness to improve pain management in acute care hospitals is currently unknown. The purpose of this study was to determine whether a collaborative quality improvement program implemented at hospital level could improve pain management and overall pain relief. Design.To assess the effectiveness of the program, we performed a before-after trial comparing patient's self-reported pain management and experience before and after program implementation. We included all adult patients hospitalized for more than 24 hours and discharged either to their home or to a nursing facility, between March 1, 2001 and March 31, 2001 (before program implementation) and between September 15, 2005 and October 15, 2005 (after program implementation). Setting.A teaching hospital of 2,096 beds in Geneva, Switzerland. Patients.All adult patients hospitalized for more than 24 hours and discharged between 1 to 31 March 2001 (before program) and 15 September to 15 October 2005 (after program implementation). Interventions.Implementation of a collaborative quality improvement program using multifaceted interventions (staff education, opinion leaders, patient education, audit, and feedback) to improve pain management at hospital level. Outcome Measures.Patient-reported pain experience, pain management, and overall hospital experience based on the Picker Patient Experience questionnaire, perceived health (SF-36 Health survey). Results.After implementation of the program only 2.3% of the patients reported having no pain relief during their hospital stay (vs 4.5% in 2001, P = 0.05). Among nonsurgical patients, improvements were observed for pain assessment (42.3% vs 27.9% of the patients had pain intensity measured with a visual analog scale, P = 0.012), pain management (staff did everything they could to help in 78.9% vs 67.9% of cases P = 0.003), and pain relief (70.4% vs 57.3% of patients reported full pain relief P = 0.008). In surgical patients, pain assessment also improved (53.7.3% vs 37.6%) as well as pain treatment. More patients received treatments to relieve pain regularly or intermittently after program implementation (95.1% vs 91.9% P = 0.046). Conclusion.Implementation of a collaborative quality improvement program at hospital level improved both pain management and pain relief in patients. Further studies are needed to determine the overall cost-effectiveness of such programs.

PURPOSE: The aim of this study was to investigate the effect of a single intravitreal (i.v.t.) injection of vasoactive intestinal peptide (VIP) loaded in rhodamine-conjugated liposomes (VIP-Rh-Lip) on experimental autoimmune uveoretinitis (EAU). METHODS: An i.v.t. injection of VIP-Rh-Lip, saline, VIP, or empty-(E)-Rh-Lip was performed simultaneously, either 6 or 12 days after footpad immunization with retinal S-antigen in Lewis rats. Clinical and histologic scores were determined. Immunohistochemistry and cytokine quantification by multiplex enzyme-linked immunosorbent assay were performed in ocular tissues. Systemic immune response was determined at day 20 postimmunization by measuring proliferation and cytokine secretion of cells from inguinal lymph nodes (ILNs) draining the immunization site, specific delayed-type hypersensitivity (DTH), and the serum concentration of cytokines. Ocular and systemic biodistribution of VIP-Rh-Lip was studied in normal and EAU rats by immunofluorescence. RESULTS: The i.v.t. injection of VIP-Rh-Lip performed during the afferent, but not the efferent, phase of the disease reduced clinical EAU and protected against retinal damage. No effect was observed after saline, E-Rh-Lip, or VIP injection. VIP-Rh-Lip and VIP were detected in intraocular macrophages and in lymphoid organs. In VIP-Rh-Lip-treated eyes, macrophages expressed transforming growth factor-beta2, low levels of major histocompatibility complex class II, and nitric oxide synthase-2. T-cells showed activated caspase-3 with the preservation of photoreceptors. Intraocular levels of interleukin (IL)-2, interferon-gamma (IFN-gamma), IL-17, IL-4, GRO/KC, and CCL5 were reduced with increased IL-13. At the systemic level, treatment reduced retinal soluble autoantigen lymphocyte proliferation, decreased IL-2, and increased IL-10 in ILN cells, and diminished specific DTH and serum concentration of IL-12 and IFN-gamma. CONCLUSIONS: An i.v.t. injection of VIP-Rh-Lip, performed during the afferent stage of immune response, reduced EAU pathology through the immunomodulation of intraocular macrophages and deviant stimulation of T-cells in ILN. Thus, the encapsulation of VIP within liposomes appears as an effective strategy to deliver VIP into the eye and is an efficient means of the prevention of EAU severity.

Less-lethal weapons are used in law enforcement to neutralize combative individuals and to disperse riot crowds. Local police recently used such an impact weapon, the Flash-Ball, in two different situations. This gun fires large rubber bullets with kinetic energies around 200 J. Although it is designed to avoid skin penetration, impacts at such energies may still create major trauma with associated severe injuries to internal organs. This is a report of 2 patients shot with the Flash-Ball who required medical attention. One could be discharged quickly, but the other required hospitalization for heart and lung contusion. Both patients required advanced investigations including computed tomography (CT) scan. The medical literature on injuries induced by less-lethal impact weapons is reviewed. Impacts from the Flash-Ball can cause significant injury to internal organs, even without penetration. Investigations as for other high-energy blunt traumas are called for in these cases.

Antigenic recognition by naive CD4+ T cells induces their proliferation and differentiation into functionally distinct T helper (Th) cell. Each CD4+ Th cell subset expresses specific transcription factors and produces signature cytokines that coordinate immune responses against encountered pathogens. Among the factors influencing CD4+ Th cell differentiation, Notch signaling pathway has been reported to play a role in the differentiation and function of multiple CD4+Thcell subsets. Notch signaling is an evolutionarily conserved cell-to-cell signaling cascade involved in many cell fate decision processes. How Notch signaling modulates the differentiation of CD4+ Th cell subsets and whether Notch signaling alone is sufficient or not for the differentiation of CD4+ Th cells is still a matter of debate. Th17 cells are a distinct subset of CD4+ Th cells. They play a role in the control of extracellular bacterial and fungal infections and may lead to inflammatory and autoimmune diseases if not properly regulated. Th17 cells are defined by the expression of RAR-related orphan receptor (ROR)a and RORyT transcription factors and their secretion of IL-17A, IL-17F cytokines. The involvement of Notch signaling in Th17 cell differentiation has mostly been studied in vitro. However, neither the experimental conditions when Notch signaling might be involved in Th17 cell differentiation in vitro and in vivo nor the precise role of Notch in this process remain clear. To better define how Notch signaling impacts Th17 differentiation, we used mice with T cell specific ablation of Notchl and Notch2 (N1 N2ACD4Cre) or of Notch transcriptional repressor RBP- JK (RBP-J ACD4Cre). We show that impaired Notch signaling in T cells, when TCR activating signal were reduced, increased RORyT and IL-17 mRNA levels during in vitro Th17 cell differentiation. Following immunization with OVA in CFA, an adjuvant that induces mostly Th17 cell response, increased IL-17A mRNA and intracellular IL-17A levels were observed in draining lymph nodes of Notch-deficient CD4+T cells. Our data suggest that Notch limited Th17 cell differentiation. Despite high levels of IL-17 mRNA and intracellular IL-17 proteins observed in Notch-deficient T cells, their release of Th17 cytokines ex vivo was markedly decreased, indicating a role for Notch signaling. During the second part of this thesis, we observed that the impact of Notch on Th17 cell differentiation and effector functions was context-dependent using different in vivo experimental models, in which Th17 cells and IL-17A were reported to contribute in the disease development. Collectively, our data reveal that Notch signaling controls the fine-tuning of Th17 cell differentiation and effector functions by limiting their differentiation but promoting selectively cytokine release through Notch-dependent mechanisms that still need to be defined. -- Lors d'une réponse immunitaire et grâce à la reconnaissance antigénique, les lymphocytes CD4+ T naïfs prolifèrent, puis se différencient en CD4+ T auxiliaires ("T helper" ou Th) fonctionnellement distincts. Chaque sous-population de lymphocytes CD4+ T auxiliaires exprime des facteurs de transcription et des cytokines spécifiques qui coordonnent la réponse immunitaire contre les pathogènes rencontrés. Parmi les facteurs influençant la différenciation des lymphocytes CD4+ T auxiliaires, la voie de signalisation Notch a été identifiée comme ayant un rôle dans la différenciation et la fonction des différents sous-types de cellules CD4+ T auxiliaires. La voie de signalisation Notch est une voie évolutivement conservée, qui est impliquée dans la signalisation entre les cellules et dans de nombreux processus de décisions cellulaires. La manière dont la voie de signalisation Notch régule la différenciation des lymphocytes CD4+ T en sous-types de cellules CD4+ auxiliaires, mais également la question de savoir si la voie de signalisation Notch est capable ou non d'induire la différenciation des cellules CD4+T auxiliaires, restent à débattre. Les cellules T auxiliaires 17 (Th17) sont un sous-type distinct de cellules CD4+T. Elles jouent un rôle important dans la défense immunitaire contre des pathogènes tels que les bactéries extracellulaires et les champignons. Une dérégulation de la réponse des cellules Th17 peut conduire à des inflammations mais également à des maladies auto-immunes. Les cellules Th17 sont définies par l'expression de leurs facteurs de transcription RAR-related orphan receptor (ROR)a, RORyT et par la sécrétion de cytokines comme IL-17A, IL-17F. Le rôle de la voie de signalisation Notch dans la différenciation des cellules Th17 a principalement été démontré in vitro. Malgré tout, ni les conditions expérimentales dans lesquelles cette voie pourrait être impliquée dans la différenciation des cellules Th17 in vitro et in vivo, mais également ni la fonction exacte de Notch dans ces processus, ne sont des questions résolues. Afin de mieux définir comment la voie de signalisation Notch est impliquée dans la différenciation des cellules Th17, nous avons utilisé des souris avec une déficience spécifique dans les cellules T des récepteurs Notchl et Notch2 (N1N2ACD4Cre) ou du répresseur transcriptionnel de Notch RBP-JK (RBP-J ACD4Cre). Nous avons montré que lorsque la voie de signalisation Notch est déficiente, les niveaux d'ARN messager (ARNm) de RORyT et de IL-17A sont augmentés dans les cellules Th17 pendant la différenciation in vitro, en présence de niveaux réduits des signaux activant les cellules T CD4+. Une augmentation dans les niveaux d'ARNm de IL-17A et de IL-17A intracellulaire au niveau protéinique a été observée dans les cellules T CD4+ Notch déficientes, au niveau des ganglions drainants après immunisation avec l'OVA dans le CFA, un adjuvant induisant une réponse des cellules Th17. Nos résultats suggèrent que Notch pourrait réguler négativement l'expression de IL-17A au niveau transcriptionnel mais également protéinique. Malgré une augmentation de IL-17A au niveau de l'ARNm et protéinique dans les cellules CD4+ T Notch déficientes, paradoxalement la sécrétion de IL-17A mais également de cytokines associées aux fonctions effectrices des cellules Th17 sont profondément diminuées 6X vivo, suggérant un rôle de la voie de signalisation Notch dans ce processus. Dans la deuxième partie de ce travail de thèse, nous avons observé que l'impact de Notch dans la différenciation des cellules Th17 et dans leurs fonctions effectrices était dépendant du contexte dans d'autres modèles expérimentaux in vivo, où les cellules Th17 et l'IL-17A ont été identifiées comme ar-.riCociêSM dans le développement ds la pathologie. En résumé, nous avons montré que la voie de la signalisation Notch contrôle la régulation précise de la différenciation des cellules Th17 en limitant leur différenciation, mais en promouvant sélectivement leur relâchement en cytokines associés aux cellules Th17 par l'intermédiaire de mécanismes dépendant de Notch, qui restent toujours à déterminer. -- Lors d'une réponse immunitaire et grâce à la reconnaissance antigénique, les lymphocytes CD4+ T naïfs prolifèrent, puis se différencient en CD4+ T auxiliaires ("T helper" ou Th) fonctionnellement distincts. Chaque sous-population de lymphocytes T auxiliaires exprime des facteurs de transcription et des cytokines spécifiques qui coordonnent une réponse immunitaire contre différents pathogènes. Les mécanismes liés à la différenciation des lymphocytes CD4+ T auxiliaires sont complexes et régulés. Après immunisation avec un adjuvant qui induit principalement une réponse des cellules Th17, nous avons observé que les niveaux de ARNm de IL-17A et aussi de IL-17A intracellulaire au niveau protéinique étaient augmentés dans les ganglions drainants dans les cellules CD4+ Notch déficientes. Ces résultats suggèrent que Notch pourrait réguler négativement l'expression de IL- 17 au niveau transcriptionnel mais également protéinique. Malgré des niveaux plus élevés de IL- 17 ARNm et aussi IL-17A intracellulaire dans les cellules T Notch déficientes, le relâchement en cytokines Th17 est profondément diminué indiquant un rôle de la voie de signalisation Notch dans ces processus de sécrétion. Dans la deuxième partie de cette thèse, nous avons observé que le rôle de Notch dans ia différenciation dss cellules Ti,17 et dans leurs fonctions effectrices était dépendant du contexte dans d'autres modèles expérimentaux, qui ont été rapportés comme une réponse induisant des cellules Th17. En résumé, nos données montrent que la voie de la signalisation Notch contrôle la régulation précise de la différenciation des cellules Th17 en limitant leur différenciation mais en promouvant sélectivement le relâchement en cytokines associées aux cellules Th17 par des mécanismes dépendant de Notch qui restent toujours à déterminer. Par conséquent, l'inhibition de la voie de signalisation Notch pourrait être utilisée dans des situations inflammatoires ou d'auto-immunité où la réponse des cellules Th17 est exacerbée.

Lactate may represent a supplemental fuel for the brain. We examined cerebral lactate metabolism during prolonged brain glucose depletion (GD) in acute brain injury (ABI) patients monitored with cerebral microdialysis (CMD). Sixty episodes of GD (defined as spontaneous decreases of CMD glucose from normal to low [<1.0 mmol/L] for at least 2 h) were identified among 26 patients. During GD, we found a significant increase of CMD lactate (from 4±2.3 to 5.4±2.9 mmol/L), pyruvate (126.9±65.1 to 172.3±74.1 μmol/L), and lactate/pyruvate ratio (LPR; 27±6 to 35±9; all, p<0.005), while brain oxygen and blood lactate remained normal. Dynamics of lactate and glucose supply during GD were further studied by analyzing the relationships between blood and CMD samples. There was a strong correlation between blood and brain lactate when LPR was normal (r=0.56; p<0.0001), while an inverse correlation (r=-0.11; p=0.04) was observed at elevated LPR >25. The correlation between blood and brain glucose also decreased from r=0.62 to r=0.45. These findings in ABI patients suggest increased cerebral lactate delivery in the absence of brain hypoxia when glucose availability is limited and support the concept that lactate acts as alternative fuel.

Background: Bronchopulmonary dysplasia (BPD) remains the leading cause of chronic pulmonary morbidity among preterm neonates. However, the exact pathophysiology is still unknown. Here we present the first results from a new model inteAbstracts, 25th International Workshop on Surfactant Replacement 400 Neonatology 2010;97:395-400 grating the most common risk factors for BPD (lung immaturity, inflammation, mechanical ventilation (MV), oxygen), which allows long-term outcome evaluation due to a non-traumatic intubation procedure. Objectives: To test the feasibility of a new rat model by investigating effects of MV, inflammation and oxygen applied to immature lungs after a ventilation-free interval. Methods: On day 4, 5, or 6 newborn rats were given an intraperitoneal injection of lipopolysaccharides to induce a systemic inflammation. 24 h later they were anesthetized, endotracheally intubated and ventilated for 8 h with 60% oxygen. After weaning of anesthesia and MV the newborn rats were extubated and returned to their mothers. Two days later they were killed and outcome measurements were performed (histology, quantitative RT-PCR) and compared to animals investigated directly after MV. Results: Directly after MV, histological signs of ventilator-induced lung injury were found. After 48 h, the first signs of early BPD were seen with delayed alveolar formation. Expression of inflammatory genes was only transiently increased. After 48 h genes involved in alveolarization, such as matrix metalloproteinase-9 and tropoelastin, showed a significant change of their expression. Conclusion: For the first time we can evaluate in a newborn rat model the effects of MV after a ventilation-free interval. This allows discrimination between immediate response genes and delayed changes of expression of more structural genes involved in alveolarization.

PURPOSE: To test the efficiency of locally administrated tresperimus in experimental autoimmune uveoretinitis (EAU). METHODS: EAU was induced in Lewis rats by S-antigen (S-Ag) immunization. Three intravitreal injections of tresperimus (prevention or prevention/treatment protocols) were performed at different time points after immunization. The pharmacokinetics of tresperimus was evaluated in the ocular tissues and plasma. The in vitro effect of tresperimus was evaluated on macrophages. EAU was graded clinically and histologically. Blood ocular barrier permeability was evaluated by protein concentration in ocular fluids. Immune response to S-Ag was examined by delayed type hypersensitivity, the expression of inflammatory cytokines in lymph nodes, ocular fluids and serum by multiplex ELISA, and in ocular cells by RT-PCR. RESULTS: In vitro, tresperimus significantly reduced the production of inflammatory cytokines by lipopolysaccharide-stimulated macrophages. In vivo, in the treatment protocol, efficient tresperimus levels were measured in the eye but not in the plasma up to 8 days after the last injection. Tresperimus efficiently reduced inflammation, retinal damage, and blood ocular barrier permeability breakdown. It inhibited nitric oxide synthase-2 and nuclear factor κBp65 expression in ocular macrophages. IL-2 and IL-17 were decreased in ocular media, while IL-18 was increased. By contrast, IL-2 and IL-17 levels were not modified in inguinal lymph nodes draining the immunization site. Moreover, cytokine levels in serum and delayed type hypersensitivity to S-Ag were not different in control and treated rats. In the prevention/treatment protocol, ocular immunosuppressive effects were also observed. CONCLUSIONS: Locally administered tresperimus appears to be a potential immunosuppressive agent in the management of intraocular inflammation.
