INTRODUCTION: It has been known for a long time that the efficiency and toxicity of drugs change during a 24-h period. However, the molecular mechanisms involved in these processes have started to emerge only recently. AREAS COVERED: This review aims to highlight recent discoveries showing the direct role of the molecular circadian clock in xenobiotic metabolism at the transcriptional and post-transcriptional levels in the liver and intestine, and the different ways of elimination of these metabolized drugs via biliary and urine excretions. Most of the related literature focuses on transcriptional regulation by the circadian clock of xenobiotic metabolism in the liver; however, the role of this timing system in the excretion of metabolized drugs and the importance of the kidney in this phenomenon are generally neglected. The goal of this review is to describe the molecular mechanisms involved in rhythmic drug metabolism and excretion. EXPERT OPINION: Chronopharmacology is used to analyze the metabolism of drugs in mammals according to the time of day. The circadian timing system plays a key role in the changes of toxicity of drugs by influencing their metabolisms in the liver and intestine in addition to their excretion via bile flow and urine.

Systemic autoinflammatory diseases are caused by abnormal activation of the cells that mediate innate immunity. In the past two decades, single-gene defects in different pathways, driving clinically distinct autoinflammatory syndromes, have been identified. Studies of these aberrant pathways have substantially advanced understanding of the cellular mechanisms that contribute to mounting effective and balanced innate immune responses. For example, mutations affecting the function of cytosolic immune sensors known as inflammasomes and the IL-1 signalling pathway can trigger excessive inflammation. A surge in discovery of new genes associated with autoinflammation has pointed to other mechanisms of disease linking innate immune responses to a number of basic cellular pathways, such as maintenance of protein homeostasis (proteostasis), protein misfolding and clearance, endoplasmic reticulum stress and mitochondrial stress, metabolic stress, autophagy and abnormalities in differentiation and development of myeloid cells. Although the spectrum of autoinflammatory diseases has been steadily expanding, a substantial number of patients remain undiagnosed. Next-generation sequencing technologies will be instrumental in finding disease-causing mutations in as yet uncharacterized diseases. As more patients are reported to have clinical features of autoinflammation and immunodeficiency or autoimmunity, the complex interactions between the innate and adaptive immune systems are unveiled.

Verbal auditory hallucinations can have a strong impact on the social and professional functioning of individuals diagnosed with schizophrenia. The safety-seeking behaviours used to reduce the threat associated with voices play a significant role in explaining the functional consequences of auditory hallucinations. Nevertheless, these safety-seeking behaviours have been little studied. Twenty-eight patients with schizophrenia and verbal auditory hallucinations were recruited for this study. Hallucinations were evaluated using the Psychotic Symptom Rating Scale and the Belief About Voice Questionnaire and safety behaviours using a modified version of the Safety Behaviour Questionnaire. Our results show that the vast majority of patients relies on safety behaviours to reduce the threat associated with voices. This reliance on safety behaviours is mostly explained by beliefs about origin of voices the omnipotence attributed to hallucinations and the behavioural and emotional reactions to the voices. Safety-seeking behaviours play an important role in maintaining dysfunctional beliefs with respect to voices. They should be better targeted within the cognitive and behavioural therapies for auditory hallucinations.

The peripheral projections of two distinct subpopulations of primary sensory neurons, expressing either calbindin D-28k or substance P, were studied in chick hindlimbs by immunodetecting calbindin D-28k with a rabbit antiserum and substance P with a mouse monoclonal antibody. Calbindin D-28k-immunoreactive axons provided an innervation restricted to specific mechanoreceptors such as muscle spindles, Herbst and Merkel corpuscles, or collars of feather follicles but were absent from Golgi tendon organs. In contrast, substance P-positive axons spread out diffusely in muscles and skin, formed loose plexuses, and extended free branches to the endomysium, arteries, superficial dermis, or dermal pulp of feather follicles. The present results show that calbindin D-28k- and substance P-immunoreactive primary sensory neurons provide distinct modes of innervation to selective targets in peripheral tissues. The results suggest a possible correlation between CaBP-expressing nerve endings and rapidly adapting mechanoreceptors.

The Tax protein of the human T-cell leukemia virus type 1 (HTLV-1) has been implicated in human T-cell immortalization. The primary function of Tax is to transcriptionally activate the HTLV-1 promoter, but Tax is also known to stimulate expression of cellular genes. It has been reported to associate with several transcription factors, as well as proteins not involved in transcription. To better characterize potential cellular targets of Tax present in infected cells, a Saccharomyces cerevisiae two-hybrid screening was performed with a cDNA library constructed from the HTLV-1-infected MT2 cell line. From this study, we found 158 positive clones representing seven different cDNAs. We focused our attention on the cDNA encoding the transcription factor CREB-2. CREB-2 is an unconventional member of the ATF/CREB family in that it lacks a protein kinase A (PKA) phosphorylation site and has been reported to negatively regulate transcription from the cyclic AMP response element of the human enkephalin promoter. In this study, we demonstrate that CREB-2 cooperates with Tax to enhance viral transcription and that its basic-leucine zipper C-terminal domain is required for both in vitro and in vivo interactions with Tax. Our results confirm that the activation of the HTLV-1 promoter through Tax and factors of the ATF/CREB family is PKA independent.

New data confirm that antidepressants have a global protective effect on the risk of suicide even if they very moderately and transiently increase suicide attempts, particularly in young people. We also present recent data about the following topics. (1) The main results of the STAR*D study, an outpatient study about depression treatment following a very complex algorithm. Nearly all the head to head comparisons between treatments showed a lack of significant difference, and the rates of clinical remission for patients non responders to their first two treatments were low. (2) The reintroduction of sertindole, an atypical antipsychotic: cardiac monitoring is necessary, but should not prevent its use as a second line drug. (3) The potential efficacy of dawn simulation and high density negative air ionization in seasonal depression.

Rapport de synthèse : Enjeux et contexte Etant le seul agent hypnotique à avoir des effets hémodynamiques négligeables, l'étomidate est souvent considéré comme l'agent anesthésique d'induction de choix lors de l'anesthésie de patients hémodynamiquement instables Cependant, une dose unique de cet hypnotique imidazolé inhibe la 11-ß hydroxylase, une enzyme mitochondriale de la phase terminale de la synthèse du cortisol. Au Centre Hospitalier Universitaire Vaudois, tout patient candidat à une chirurgie cardiaque reçoit de l'étomidate à l'induction de l'anesthésie. L'objectif de cette étude a été de déterminer l'incidence de dysfonction surrénalienne chez les patients subissant une chirurgie cardiaque et présentant un état de choc postopératoire nécessitant de hautes doses de noradrénaline. t Méthodes Une étude rétrospective et descriptive a été réalisée dans le service de médecine intensive adulte du Centre Hospitalier Universitaire Vaudois. Soixante-trois patients admis en réanimation après chirurgie cardiaque nécessitant plus de 0.2 νg/kg/min de noradrénaline au cours des premières 48 heures postopératoires ont été étudiés. L'insuffisance surrénalienne absolue a été définie par un cortisol basal inférieur à 414mmo/L, l'insuffisance surrénalienne relative par un cortisol basal entre 414mmo/L et 938nmo/L avec augmentation de la cortisolémie (à 60 minute .après un test de stimulation par 250Ng of corticotropine de synthèse) inférieure à 250mmo/L. Statistiques Les résultats ont été rapportés en médianes et extrêmes. Les comparaisons entre les groupes ont été réalisées, lorsque cela était indiqué, par un test Chi 2 ou un test de Wilcoxon. Pour les comparaisons répétées des taux de cortisol et des doses de noradrénaline, une correction de Bonferroni a été appliquée. Une valeur de p<0,05 a été considérée comme significative. Résultats : Quatorze patients [22%] ont présenté une fonction surrénalienne normale, 10 [16%] une insuffisance surrénalienne absolue et 39 [62%] une insuffisance surrénalienne relative. Tous les patients ont reçu une substitution stéroïdienne, sans aucune différence d'évolution clinique entre les différents groupes. Conclusions : L'incidence d'insuffisance surrénalienne chez les patients qui ont reçu un bolus d'étomidate à l'induction, subi une chirurgie cardiaque avec circulation extra-corporelle et présenté une défaillance circulatoire postopératoire est élevée. Il convient probablement de garder à l'esprit qu'une telle insuffisance peut contribuer à l'état de choc du patient. Dans de telles circonstances, l'exploration de la fonction surrénalienne et la suppléance par stéroïdes d'une éventuelle insuffisance surrénalienne en attendant les résultats des examens endocrinologiques devraient être discutés.

Melanoma progression is associated with changes in adhesion receptor expression, in particular upregulation of N-cadherin which promotes melanoma cell survival and invasion. Plasma membrane lipid rafts contribute to the compartmentalization of signaling complexes thereby regulating their function, but how they may affect the properties of adhesion molecules remains elusive. In this study, we addressed the question whether lipid rafts in melanoma cells may contribute to the compartmentalization of N-cadherin. We show that a fraction of N-cadherin in a complex with catenins is associated with cholesterol/sphingolipid-rich membrane microdomains in aggressive melanoma cells in vitro and experimental melanomas in vivo. Partitioning of N-cadherin in membrane rafts is not modulated by growth factors and signaling pathways relevant to melanoma progression, is not necessary for cell-cell junctions' establishment or maintenance, and is not affected by cell-cell junctions' and actin cytoskeleton disruption. These results reveal that two independent pools of N-cadherin exist on melanoma cell surface: one pool is independent of lipid rafts and is engaged in cell-cell junctions, while a second pool is localized in membrane rafts and does not participate in cell-cell adhesions. Targeting to membrane rafts may represent a previously unrecognized mechanism regulating N-cadherin function in melanoma cells.

Noces est un film réalisé par Philippe Béziat. Issu d'une coproduction franco-suisse, il est sorti en France le 8 février 2012 et en Suisse romande le 26 septembre de la même année. Il met en scène, dans le Lavaux, une production contemporaine de l'oeuvre Les Noces, des répétitions à la présentation de la pièce dans son intégralité. La particularité de cette représentation, qui constitue l'aboutissement du film, réside dans le fait qu'elle n'est pas jouée devant un public, et était d'emblée prévue pour être exclusivement filmée1. Cette «cantate-ballet» a été composée par Igor Stravinsky lors de son séjour à Morges et le livret a été adapté en français par Charles-Ferdinand Ramuz à partir de contes et chansons populaires russes. La création du spectacle est encadrée par les Souvenirs sur Igor Strawinsky2, ouvrage dans lequel le poète vaudois évoque sa collaboration avec le compositeur et dépeint le lieu où ce projet a été développé. Constatant la diversité des discours critiques portés sur le film lors de sa sortie, le présent article cherche à identifier les logiques de réception ainsi que les aspects du film qui, parce que susceptibles de produire des lectures plurivoques, peuvent conduire à élaborer des avis opposés. 1Philippe Béziat revendique cette particularité de Noces dans l'interview qui figure dans le dossier de presse du film : « Mais le dispositif de représentation est absolument unique et ne peut être que cinématographique. On ne peut pas imaginer donner ces Noces comme ça dans une salle de concert ou dans une salle d'opéra. On n'entendrait pas ce que le spectateur du film entend, avec un son direct, spatialisé, et toujours du point de vue de la caméra. The reactivity spectrum of five different monoclonal anti-melanoma antibodies cross-reacting with gliomas and neuroblastomas and one monoclonal anti-glioma antibody cross-reacting with melanomas and neuroblastomas was investigated. Comparison of the binding activity of these monoclonal antibodies for 11 melanoma, seven glioma, and three neuroblastoma cell lines showed that each of these clones had a different pattern of cross-reactivity. The results indicated that the antigenic determinants detected by these antibodies were not associated with the same antigen and thus suggested the existence of at least six different antigens common to melanomas, gliomas, and neuroblastomas. Since all these tumors are known to derive from cells originating embryologically from the neural crest, it can be assumed that the antigens recognized by our monoclonal antibodies are neuroectodermal differentiation antigens. However, absorption with fetal brain homogenates abolished only the binding of monoclonal anti-glioma antibody, but did not modify the binding of monoclonal anti-melanoma antibodies.
