Dans ce texte, nous défendons une approche réfutant tout causalisme et centrée sur les situations révolutionnaires. Une revue de la littérature sur les récentes « révoltes arabes » montre à quel point une telle orientation est rare. Nous proposons, à partir de là, de ne pas nous en tenir à la description des propriétés des situations révolutionnaires pour tenter de retracer les séquences d'action, définies comme des chaînes d'interaction complexes mais observables, conduisant - ou pas - à ces situations. À partir de la mise en regard d'un certain nombre de situations révolutionnaires dans la région, nous tentons d'identifier dans les événements qui y conduisent une série de processus relationnels et cognitifs. L'on peut ce faisant approcher les modes de transformation d'états propres aux situations révolutionnaires.

PURPOSE: An optimal target for glucose control in ICU patients remains unclear. This prospective randomized controlled trial compared the effects on ICU mortality of intensive insulin therapy (IIT) with an intermediate glucose control. METHODS: Adult patients admitted to the 21 participating medico-surgical ICUs were randomized to group 1 (target BG 7.8-10.0 mmol/L) or to group 2 (target BG 4.4-6.1 mmol/L). RESULTS: While the required sample size was 1,750 per group, the trial was stopped early due to a high rate of unintended protocol violations. From 1,101 admissions, the outcomes of 542 patients assigned to group 1 and 536 of group 2 were analysed. The groups were well balanced. BG levels averaged in group 1 8.0 mmol/L (IQR 7.1-9.0) (median of all values) and 7.7 mmol/L (IQR 6.7-8.8) (median of morning BG) versus 6.5 mmol/L (IQR 6.0-7.2) and 6.1 mmol/L (IQR 5.5-6.8) for group 2 (p < 0.0001 for both comparisons). The percentage of patients treated with insulin averaged 66.2 and 96.3%, respectively. Proportion of time spent in target BG was similar, averaging 39.5% and 45.1% (median (IQR) 34.3 (18.5-50.0) and 39.3 (26.2-53.6)%) in the groups 1 and 2, respectively. The rate of hypoglycaemia was higher in the group 2 (8.7%) than in group 1 (2.7%, p < 0.0001). ICU mortality was similar in the two groups (15.3 vs. 17.2%). CONCLUSIONS: In this prematurely stopped and therefore underpowered study, there was a lack of clinical benefit of intensive insulin therapy (target 4.4-6.1 mmol/L), associated with an increased incidence of hypoglycaemia, as compared to a 7.8-10.0 mmol/L target. The flourishing number of publications on the use of isotope ratio mass spectrometry (IRMS) in forensicscience denotes the enthusiasm and the attraction generated by this technology. IRMS has demonstratedits potential to distinguish chemically identical compounds coming from different sources. Despite thenumerous applications of IRMS to a wide range of forensic materials, its implementation in a forensicframework is less straightforward than it appears. In addition, each laboratory has developed its ownstrategy of analysis on calibration, sequence design, standards utilisation and data treatment without aclear consensus.Through the experience acquired from research undertaken in different forensic fields, we propose amethodological framework of the whole process using IRMS methods. We emphasize the importance ofconsidering isotopic results as part of a whole approach, when applying this technology to a particularforensic issue. The process is divided into six different steps, which should be considered for a thoughtfuland relevant application. The dissection of this process into fundamental steps, further detailed, enablesa better understanding of the essential, though not exhaustive, factors that have to be considered in orderto obtain results of quality and sufficiently robust to proceed to retrospective analyses or interlaboratorycomparisons.

L'économiste russe Tugan-Baranovskij (1865-1919) a publié en 1899 une notice sur le développement de la « science économique russe » pour la première encyclopédie universelle russe, le Brockhaus-Efron. Sous la forme d'un catalogue chronologique des économistes russes et de leurs ouvrages tout au long du XIXe siècle, le récit de Tugan-Baranovskij représente une des premières tentatives d'écrire une histoire de la pensée économique russe. L'intérêt du texte se manifeste lorsqu'on s'interroge sur l'existence d'une telle science « nationale » de l'économie politique. Nous présentons ici en traduction française ce récit jamais encore traduit du russe, et nous l'accompagnons de notes sur les auteurs et les ouvrages qui y sont signalés.
