BACKGROUND AND OBJECTIVES: The suprascapular nerve (SSN) block is frequently performed for different shoulder pain conditions and for perioperative and postoperative pain control after shoulder surgery. Blind and image-guided techniques have been described, all of which target the nerve within the supraspinous fossa or at the suprascapular notch. This classic target point is not always ideal when ultrasound (US) is used because it is located deep under the muscles, and hence the nerve is not always visible. Blocking the nerve in the supraclavicular region, where it passes underneath the omohyoid muscle, could be an attractive alternative. METHODS: In the first step, 60 volunteers were scanned with US, both in the supraclavicular and the classic target area. The visibility of the SSN in both regions was compared. In the second step, 20 needles were placed into or immediately next to the SSN in the supraclavicular region of 10 cadavers. The accuracy of needle placement was determined by injection of dye and following dissection. RESULTS: In the supraclavicular region of volunteers, the nerve was identified in 81% of examinations (95% confidence interval [CI], 74%-88%) and located at a median depth of 8 mm (interquartile range, 6-9 mm). Near the suprascapular notch (supraspinous fossa), the nerve was unambiguously identified in 36% of examinations (95% CI, 28%-44%) (P < 0.001) and located at a median depth of 35 mm (interquartile range, 31-38 mm; P < 0.001). In the cadaver investigation, the rate of correct needle placement of the supraclavicular approach was 95% (95% CI, 86%-100%). CONCLUSIONS: Visualization of the SSN with US is better in the supraclavicular region as compared with the supraspinous fossa. The anatomic dissections confirmed that our novel supraclavicular SSN block technique is accurate.

An essential step of the life cycle of retroviruses is the stable insertion of a copy of their DNA genome into the host cell genome, and lentiviruses are no exception. This integration step, catalyzed by the viral-encoded integrase, ensures long-term expression of the viral genes, thus allowing a productive viral replication and rendering retroviral vectors also attractive for the field of gene therapy. At the same time, this ability to integrate into the host genome raises safety concerns regarding the use of retroviral-based gene therapy vectors, due to the genomic locations of integration sites. The availability of the human genome sequence made possible the analysis of the integration site preferences, which revealed to be nonrandom and retrovirus-specific, i.e. all lentiviruses studied so far favor integration in active transcription units, while other retroviruses have a different integration site distribution. Several mechanisms have been proposed that may influence integration targeting, which include (i) chromatin accessibility, (ii) cell cycle effects, and (iii) tethering proteins. Recent data provide evidence that integration site selection can occur via a tethering mechanism, through the recruitment of the lentiviral integrase by the cellular LEDGF/p75 protein, both proteins being the two major players in lentiviral integration targeting.

This dissertation aims to investigate empirical evidence on the importance and influence of attractiveness of nations in global competition. The notion of country attractiveness, which has been widely developed in the research areas of international business, tourism and migration, is a multi-dimensional construct to measure a country's characteristics with regard to its market or destination that attract international investors, tourists and migrants. This analytical concept provides an account of the mechanism as to how potential stakeholders evaluate more attractive countries based on certain criteria. Thus, in the field of international sport-event bidding, do international sport event owners also have specific country attractiveness for their sport event hosts? The dissertation attempts to address this research question by statistically assessing the effects of country attractiveness on the success of strategy for hosting international sports events. Based on theories of signaling and soft power, country attractiveness is defined and measured as the three dimensions of sustainable development: economic, social, and environmental attractiveness. This thesis proceeds to examine the concept of sport-event-hosting strategy and explore multi-level factors affecting the success in international sport-event bidding. By exploring past history of the Olympic Movement from theoretical perspectives, the thesis proposes and tests the hypotheses that economic, social and environmental attractiveness of a country may be correlated with its bid wins or the success of sport-event-hosting strategy. Quantitative analytical methods with various robustness checks are employed with using collected data on bidding results of major events in Olympic sports during the period from 1990 to 2012. The analysis results reveal that event owners of international Olympic sports are likely to prefer countries that have higher economic, social, and environmental attractiveness. The empirical assessment of this thesis suggests that high country attractiveness can be an essential element of prerequisites for a city/country to secure in order to bid with an increased chance of success.

Although rare, popliteal artery aneurysms are the most common peripheral aneurysms and are frequently associated with abdominal aorta aneurysms. They are often bilateral. One third of patients are asymptomatic at diagnosis, with an insidious evolution. Symptomatic patients may present with symptoms of either acute ischemia or chronic ischemia, or rarely compression or rupture. Surgical exclusion of aneurysm followed by venous bypass remains the treatment of choice. Endovascular treatment is an attractive alternative currently reserved for patients at high risk, with good anatomical criteria. Elective treatment before symptoms onset is preferable given the best results in terms of patency and complications. A conservative approach is allowed for small aneurysms without major embolic risk provided careful monitoring by ultrasound.

A contribution to the knowledge of necrophagous flies of the Jorat (Vaud, Switzerlan). A survey of necrophagous flires has been realized from spring to fall (april-october) in a meadow and a forest using selective traps. Seventeen species of carrion flies were collected belonging to the Callophoridae (10 species), Sarcophagidae (3 species) and Muscidae (4 species). Among these species, 14 were found in the meadow, 11 in the forest with 8 common to both habitats. seasonal distribution of the dominant species for each habitat is presented.

The authors give the first description of evangelicalism in Switzerland using representative data. It is shown that evangelicalism can be conceived of as a "milieu" which is characterized by certain structural and cultural traits, boundaries and high internal communication. The relative success of the evangelical milieu compared to other religious milieus is explained by its remarkable ability to retain its own numerous offspring, while on the other hand providing a "religious product" that is also attractive to people without an evangelical familial background. Les auteurs utilisent des données représentatives afin de faire, pour la première fois, une description de l'évangélisme en Suisse. Ils montrent qu'on peut parler de l'évangélisme comme d'un "milieu'' qui se caractérise par certains attributs structurels et culturels, des frontières et une communication interne élevée. Le succès relatif du milieu évangélique comparé à d'autres milieux s'explique par sa capacité remarquable à retenir les enfants de ses membres dans le mouvement tout en offrant un "produit religieux'' attractif aux personnes sans arrière-fond évangélique.

Thirty-nine patients with various types of hypertension were treated by chronic blockage of the angiotensin converting enzyme, i.e. by twice daily administration of captopril, 50 to 200 mg p.o. The blood pressure reduction observed 1 hour following administration of the inhibitor was directly related to the baseline plasma renin activity (r=- 0.67, p < 0.001). Whenever blockade of the renin system alone did not lower blood pressure to normal levels additional sodium subtraction brought it under control. With the renin system neutralized, blood pressure becomes exquisitely sensitive to changes in sodium balance. Diuretics seem to preserve optimal natriuretic efficacy despite blood pressure reduction, probably because aldosterone levels are reduced and renal blood flow increases. Blockade of the renin system together with individually tailored salt subtraction provides an attractive new approach to long-term treatment of clinical hypertension.

Adolescent health surveys, like those for other segments of the population, tend to remain in the hands of researchers, where they can have no real impact on the way critical health issues are dealt with by policy makers or other professionals directly connected to young people in their everyday work. This paper reviews important issues concerning the dissemination of survey results among professionals from various fields. The content, length and wording of the messages should be tailored to the audience one wants to reach as well as the type of channels used for their diffusion. Survey data sets can be used to select priorities for interventions: ad hoc presentations, attractive summaries and brochures, or even films expressing young peoples' opinions have been used by European public health professionals to make data sets usable in various local, regional and national contexts. CONCLUSION: The impact of these diffusion strategies is, however, difficult to assess and needs to be refined. The adequate delivery of survey findings as well as advocacy and lobbying activities require specific skills which can be endorsed by specialized professionals. Ultimately, it is the researchers' responsibility to ensure that such tasks are effectively performed.

Intravenous silibinin (SIL) is an approved therapeutic that has recently been applied to patients with chronic hepatitis C, successfully clearing hepatitis C virus (HCV) infection in some patients even in monotherapy. Previous studies suggested multiple antiviral mechanisms of SIL; however, the dominant mode of action has not been determined. We first analyzed the impact of SIL on replication of subgenomic replicons from different HCV genotypes in vitro and found a strong inhibition of RNA replication for genotype 1a and genotype 1b. In contrast, RNA replication and infection of genotype 2a were minimally affected by SIL. To identify the viral target of SIL we analyzed resistance to SIL in vitro and in vivo. Selection for drug resistance in cell culture identified a mutation in HCV nonstructural protein (NS) 4B conferring partial resistance to SIL. This was corroborated by sequence analyses of HCV from a liver transplant recipient experiencing viral breakthrough under SIL monotherapy. Again, we identified distinct mutations affecting highly conserved amino acid residues within NS4B, which mediated phenotypic SIL resistance also in vitro. Analyses of chimeric viral genomes suggest that SIL might target an interaction between NS4B and NS3/4A. Ultrastructural studies revealed changes in the morphology of viral membrane alterations upon SIL treatment of a susceptible genotype 1b isolate, but not of a resistant NS4B mutant or genotype 2a, indicating that SIL might interfere with the formation of HCV replication sites. CONCLUSION: Mutations conferring partial resistance to SIL treatment in vivo and in cell culture argue for a mechanism involving NS4B. This novel mode of action renders SIL an attractive candidate for combination therapies with other directly acting antiviral drugs, particularly in difficult-to-treat patient cohorts.

Résumé Etant une importante source d'énergie, les plantes sont constamment attaquées par des pathogènes. Ne pouvant se mouvoir, elles ont développé des systèmes de défense sophistiqués afin de lutter contre ces prédateurs. Parmi ces systèmes, les voies de signalisation mettant en jeu des éliciteurs endog8nes tels que les jasmonates permettent d'induire la production de protéines de défense telles que les protéines dites "liées à la pathogénèse". Les gènes codant pour ces protéines appartiennent à des familles multigéniques. Le premier but de cette thèse est d'évaluer le nombre de ces gènes dans le génome d'Arabidopsis thaliana et d'estimer la part de ce système de défense, dépendant de la voie de signalisation des jasmonates. Nous avons défini un cluster de seulement 1S gènes sur 266, "liés à la pathogénèse", exclusivement régulés par les jasmonates. De multiples membres des familles des lectines de type jacaline et des inhibiteurs de trypsines semblent dépendre du jasmonate. Présente dans tous les systèmes immunitaires des eucaryotes, la famille des défensines est une famille très intéressante. Chez Arabidopsis thaliana, 317 protéines similaires aux défensines ont été définies, cependant seulement 15 défensines (PDF) sont bien annotées. Ces 15 défensines sont séparées en deux groupes dont un semble avoir évolué plus récemment. Le second but de cette thèse est d'étudier ce groupe de défensines à l'aide de la bioinformatique et des techniques de biologie moléculaire (gêne rapporteur, PCR en temps réel). Nous avons montré que ce groupe contenait une défensine acide intéressante, PDF1.5, qui semblait avoir subi une sélection positive. Cette protéine n'avait encore jamais été étudiée. Contrairement à ce que nous pensions, nous avons établi que cette protéine pouvait avoir une activité biologique liée à la défense. Ce travail de thèse a permis de préciser le nombre de gènes "liées à la pathogénèse" induits par la voie des jasmonates et d'apporter des éléments de réponse sur la question de la redondance des gènes de défense. En conclusion, même si de nombreuses familles de gènes intervenant dans la défense sont bien définies chez Arabidopsis, il reste encore de nombreuses études à faire sur chacun de ces membres. Abstract Being an important source of energy, plants are constantly attacked by herbivores and pathogens. As sessile organisms, they have developed sophisticated defense responses to cope with attack. Among these responses, signalling pathways, using endogenous elicitors including jasmonates (JA), allow the plant to induce the production of defense proteins such as pathogenesis-related (PR) proteins. The genes encoding these proteins belong to multigenic families. The first goal of this thesis was to evaluate the number of PR genes in the genome of Arabidopsis thaliana and estimate how much of this plant defense system was dependent on the jasmonate signaling pathway in leaves. Surprisingly a cluster of only 1S genes out of 2ó6 PR genes was exclusively regulated by JA. Multiple members of the jacalin lectin and trypsin inhibitor gene families were shown to be regulated by JA. Present in all eukaryotic immune systems, defensins are an attractive PR family to study. In Arabidopsis thaliana, 317 defensin-related proteins have been found but just 1S defensins (i.e. PDF family) are well annotated. These defensins are split into 2 groups. One of these groups may have appeared and diversified recently. The second goal of this thesis was to study this defensin gene group combining bioinformatic, reporter gene and quantitative PCR techniques. We have shown that this group contains an interesting acidic defensin, PDF1.S, which seems to have undergone positive selection. No information was known on this protein. We have established that this protein may have a biological activity in plant defense. This thesis allowed us to define the number of PR genes induced by the jasmonate pathway and gave initial leads to explain the redundancy of the PR genes in the genome of Arabidopsis. In conclusion, even if many defense gene families are already defined in the Arabidopsis genome, much work remains to be done on individual members.

We build a theoretical framework that allows for endogenous conflict behaviour (i.e., fighting efforts) and for endogenous natural resource exploitation (i.e., speed, ownership, and investments). While depletion is spread in a balanced Hotelling fashion during peace, the presence of conflict creates incentives for rapacious extraction, as this lowers the stakes of future contest. This voracious extraction depresses total oil revenue, especially if world oil demand is relatively elastic and the government's weapon advantage is weak. Some of these political distortions can be overcome by bribing rebels or by government investment in weapons. The shadow of conflict can also make less efficient nationalized oil extraction more attractive than private extraction, as insecure property rights create a holdup problem for the private firm and lead to a lower license fee. Furthermore, the government fights less intensely than the rebels under private exploitation, which leads to more government turnover. Without credible commitment to future fighting efforts, private oil depletion is only lucrative if the government's non-oil office rents are large and weaponry powerful, which guarantees the government a stronger grip on office and makes the holdup problem less severe.

BACKGROUND: Positron emission tomography (PET) during the cold pressor test (CPT) has been used to assess endothelium-dependent coronary vasoreactivity, a surrogate marker of cardiovascular events. However, its use remains limited by cardiac PET availability. As multidetector computed tomography (MDCT) is more widely available, we aimed to develop a measurement of endothelium-dependent coronary vasoreactivity with MDCT and similar radiation burden as with PET. METHODS AND RESULTS: A study group of 18 participants without known cardiovascular risk factor (9F/9M; age 60±6 years) underwent cardiac PET with (82)Rb and unenhanced ECG-gated MDCT within 4h, each time at rest and during CPT. The relation between absolute myocardial blood flow (MBF) response to CPT by PET (ml·min(-1)·g(1)) and relative changes in MDCT-measured coronary artery surface were assessed using linear regression analysis and Spearman's correlation. MDCT and PET/CT were analyzed in all participants. Hemodynamic conditions during CPT at MDCT and PET were similar (P>0.3). Relative changes in coronary artery surface because of CPT (2.0-21.2%) correlated to changes in MBF (-0.10-0.52ml·min(-1)·g(1)) (ρ=0.68, P=0.02). Effective dose was 1.3±0.2mSv for MDCT and 3.1mSv for PET/CT. CONCLUSIONS: Assessment of endothelium-dependent coronary vasoreactivity using MDCT CPT appears feasible. Because of its wider availability, shorter examination time and similar radiation burden, MDCT could be attractive in clinical research for coronary status assessment.

PURPOSE OF REVIEW: To review and summarize current knowledge on gender differences and sex steroid hormones in nonmuscle invasive bladder cancer. RECENT FINDINGS: Beyond the proven role of gender as a risk factor for the development of bladder cancer, recent studies indicate that women present with more advanced bladder cancer tumor stages than men, which may be due to differences in both bladder cancer care and biology. In addition, female gender has been identified as an independent prognostic factor for both recurrence and progression and may be associated with worse response to Bacillus Calmette-Guérin instillation therapy. Overall, sex steroid hormones and their receptors impact bladder carcinogenesis, recurrence and progression. Basic and transitional research evidence suggests that estrogens may initially protect against bladder cancer development, but later promote bladder cancer progression. Androgens, in contrast, seem to initiate and drive bladder cancer with its receptor playing a central role. Promising novel research shows a potential role of sex steroid hormones as therapeutic targets. SUMMARY: Whereas men are more likely to develop bladder cancer, women present generally with more advanced disease and have worse oncologic outcomes even after adjusting for tumor stage. Sex steroid hormones and their receptors play an active role in bladder cancer development and progression and represent attractive therapeutic targets for gender-specific care.

Résumé : Introduction Cette étude est une analyse rétrospective des complications urétérales et de leurs prises en charge à partir d'une série monocentrique de 277 transplantations rénales consécutives. Matériel et méthode De septembre 1979 à juin 1999, 277 transplantations rénales (origine cadavérique) ont été pratiquées chez 241 patients. L'uretère provenant de la greffe rénale a été inséré dans la vessie selon la technique d'implantation extravésicale décrite par Lich-Gregoir et Campos-Freire. L'étude a analysé la date de survenue et le genre de complications observés. Les différentes procédures pour restaurer le tractus urinaire transplanté sont présentées dans cette étude. Résultats Des complications sont survenues chez 43/277 transplantations rénales (15,5%). Les fuites urinaires sur l'anastomose ou les sténoses urétérales étaient les plus fréquentes. La date de survenue de ces complications étaient soit précoce (< 1 mois) soit tardive (> 1 mois) dans un nombre similaire de cas. La plupart des cas ont été pris en charge chirurgicalement 33/43 cas (76,7%). La réparation chirurgicale la plus fréquente a été la réimplantation urétérovésicale (n-13), suivie par : l'anastomose urétérourétérale termino-terminale (uretère natif-uretère greffé, n-5) ; l'anastomose pyélourétérale (uretère natif-bassinet rénal greffé, n=5) ; la simple révision de l'implantation urétérovésicale (n=4) ; la résection et l'anastomose termino-terminale de l'uretère greffé (n=2) ; la calico-vésicostomie (vessie greffée, n=1) ; l'implantation selon Boari (n=1) ; la pyélovésicostomie avec bipartition de la vessie (n-1), et la pyéloiléocystoplastie avec greffe iléale détubularisée (n=1). Aucun décès en relation avec les complications urologiques n'a été rapporté. Cependant, 2 reflux vésico-rénaux consécutifs ont conduit à distance à la perte du greffon. Conclusion Le taux de complications constaté dans cette analyse rétrospective est similaire à celui observé dans d'autres études. Il se situe entre 2 et 20%. Si l'implantation urétérovésicale extravésicale classique reste une technique attractive en raison de sa simplicité, l'équipe chirurgicale dans un centre de formation doit rester attentive à toute mesure de prévention des complications urétérales, comme le choix d'une autre technique d'implantation de l'uretère et/ou de l'insertion transitoire d'un stent urétéral. Abstract Introduction: This study is a retrospective analysis of ureteral complications and their management from a monocenter series of 277 consecutive renal transplantations. Materials and Methods: From September 1979 to June 1999, 277 renal transplantations (cadaveric origin) were performed in 241 patients. The ureter from the kidney graft was inserted into the bladder according to the technique of extravesical implantation described by Lich-Gregoir and Campos-Freire. The study analyzed the time of occurrence and the type of complications observed. The different procedures to restore the transplanted urinary tract are presented. Results: Complications occurred in 431277 renal transplantations (15.5%). Anastomotic urine leakage or ureteral stricture were the most frequent. The time to appearance of these complications was either short (<1 month) or late (>1 month) in a similar number of cases. Most cases were managed surgically: 33/43 cases (76.7%). The most frequent surgi cal repair was ureterovesical reimplantation n =13), Followed by: ureteroureteral end, to end anastomosis (native ureter-ureter transplant, n =, 5); pyeloureteral anastomosis (native ureter-renal pelvis transplant n = 5): simple revision of ureterovesical implantation (n=4): resection and end-to end anastomosis of the transplant ureter (n=2); calico-vesicostomy graft-bladder, n = 1); implantation according to Boari (n= 1); pyelovesicostomy with bipartition of bladder (n = 1), and pyeloileocystoplasty with detubularized ileal graft (n=1). No deaths related to any of the urological complications were reported However, 2 consecutive vesico-renal refluxes led to the loss of the kidney graft in the long-term. Conclusion: The rate of complications observed in this retrospective analysis is similar to the experience of other studies, ranging from 2 to 20% If the classical extravesical ureteral bladder implantation is to remain an attractive technique due to its simplicity, the surgical team at the training center should be aware of all the means to prevent any ureteral complications, such as the choice of another implantation technique and/or insertion of a transient ureteral stent.

The common ectodermal origin of the skin and nervous systems can be expected to predict likely interactions in the adult. Over the last couple of decades much progress has been made to elucidate the nature of these interactions, which provide multidirectional controls between the centrally located brain and the peripherally located skin and immune system. The opioid system is an excellent example of such an interaction and there is growing evidence that opioid receptors (OR) and their endogenous opioid agonists are functional in different skin structures, including peripheral nerve fibres, keratinocytes, melanocytes, hair follicles and immune cells. Greater knowledge of these skin-associated opioid interactions will be important for the treatment of chronic and acute pain and pruritus. Topical treatment of the skin with opioid ligands is particularly attractive as they are active with few side effects, especially if they cannot cross the blood-brain barrier. Moreover, cutaneous activation of the opioid system (e.g. by peripheral nerves, cutaneous and immune cells, especially in inflamed and damaged skin) can influence cell differentiation and apoptosis, and thus may be important for the repair of damaged skin. While many of the pieces of this intriguing puzzle remain to be found, we attempt in this review to weave a thread around available data to discuss how the peripheral opioid system may impact on different key players in skin physiology and pathology.
