The geochemical compositions of biogenic carbonates are increasingly used for palaeoenvironmental reconstructions. The skeletal delta O-18 temperature relationship is dependent on water salinity, so many recent studies have focused on the Mg/Ca and Sr/Ca ratios because those ratios in water do not change significantly on short time scales. Thus, those elemental ratios are considered to be good palaeotemperature proxies in many biominerals, although their use remains ambiguous in bivalve shells. Here, we present the high-resolution Mg/Ca ratios of two modern species of juvenile and adult oyster shells, Crassostrea gigas and Ostrea edulis. These specimens were grown in controlled conditions for over one year in two different locations. In situ monthly Mn-marking of the shells has been used for day calibration. The daily Mg/Ca.ratios in the shell have been measured with an electron microprobe. The high frequency Mg/Ca variation of all specimens displays good synchronism with lunar cycles, suggesting that tides strongly influence the incorporation of Mg/Ca into the shells. Highly significant correlation coefficients (0.70<R<0.83, p<0.0001) between the Mg/Ca ratios and the seawater temperature are obtained only for juvenile C. gigas samples, while metabolic control of Mg/Ca incorporation and lower shell growth rates preclude the use of the Mg/Ca ratio in adult shells as a palaeothermometer. Data from three juvenile C. gigas shells from the two study sites are selected to establish a relationship: T = 3.77Mg/Ca + 1.88, where T is in degrees C and Mg/Ca in mmol/mol. (c) 2012 Elsevier B.V. The aim of this pilot study is to analyse the discourse of fathers of toddlers concerning fatherhood and the link between some particularities in the discourse and family alliance. The sample consists of 13 Swiss first time fathers (5 fathers of girls and 8 of boys). In order to evaluate the paternal discourse, the fathers were given a semi-structured interview, which was later analysed using the research package Alceste. The family alliance, i.e., the degree of coordination among the partners when executing a task together, was assessed through the Lausanne Trilogue Play (Fivaz-Depeursinge & Corboz-Warnery, 1999). The main results indicated an interesting link between classes of paternal discourse grouped around the following themes "affective relationship", "daily routine" and "educational goals", and the family alliance (defined in two major categories; functional and problematic alliances). Finally, clinical perspectives on links between paternal representations and family functioning at an interactive level are discussed

Introduction :¦L'hyperthyroïdie est la dysfonction thyroïdienne la plus fréquemment symptomatique et diagnostiquée. Son étiologie la plus fréquente chez l'adulte est la maladie de Basedow. Dans sa pratique le médecin se concentre sur les variables spécifiques à la maladie qui sont liées à une augmentation de la mortalité chez les patients. D'autres variables, comme la perte de poids accompagnant la maladie et le risque de reprise de ce poids suite au traitement, semblent peu prises en compte. Ce travail est une étude rétrospective¦portant sur 31 patients hyperthyroïdiens suivis ambulatoirement dans le service d'endocrinologie, diabétologie et métabolisme (EDM) du centre hospitalier universitaire vaudois (CHUV) durant 18 à 24 mois.¦Objectifs :¦Déterminer la variation de poids chez ces patients, la relation qui existe entre l'évolution des résultats des examens de laboratoire et la variation de poids, évaluer les facteurs de risque pour une prise pondérale excessive suite au traitement et comparer la variation de poids en fonction du poids de forme.¦Limites :¦Le caractère rétrospectif de l'étude, le petit nombre de patients inclus le fait que les statistiques n'ont pas été réalisées par un statisticien expérimenté limitent l'interprétation des résultats.¦Résultats et discussion :¦Il existe une perte de poids accompagnant un épisode inaugural dans 71% des cas. Les patients ont tendance à prendre du poids suite au traitement (médiane : + 7.3 kg). Le poids pris consiste en une compensation du poids perdu avant le diagnostic, avec un surplus de reprise pondérale (médiane : + 0.150 kg). Le poids de forme manque dans 41.9% des cas. Après 18 à 24 mois de suivi, la catégorie d'IMC dans¦laquelle les patients se trouvent correspond dans 73.3% des cas à la catégorie d'IMC de forme. Ni le sexe ni l'âge influencent la variation pondérale initiale (sexe : p=0.429 ; âge : p=0.241). L'importance de la perturbation des concentrations hormonales n'influe pas sur l'importance de la perte pondérale au diagnostic (TSH : R2=0.001 ; T4l : R2=0.0037). Les patients avec 6 symptômes et plus présents au moment du diagnostic ont tendance à avoir une T4 libre plus élevée (médiane : 51.26 pmol/l) que ceux qui ont moins de 6 symptômes (médiane : 40.00 pmol/l). Des facteurs prédisposants à un surplus de reprise n'ont¦pas pu être déterminés, mais des tendances peuvent être décrites. Une sur-correction iatrogène¦de l'hyperthyroïdie, avec un passage en hypothyroïdie après 18-24 mois de suivi est possiblement liée à une reprise de poids nette élevée. La reprise de poids est moindre si un suivi diététique a eu lieu (médiane : 0 kg versus 1.8 kg sans suivi diététique).

BACKGROUND: Previous published studies have shown significant variations in colonoscopy performance, even when medical factors are taken into account. This study aimed to examine the role of nonmedical factors (ie, embodied in health care system design) as possible contributors to variations in colonoscopy performance. METHODS: Patient data from a multicenter observational study conducted between 2000 and 2002 in 21 centers in 11 western countries were used. Variability was captured through 2 performance outcomes (diagnostic yield and colonoscopy withdrawal time), jointly studied as dependent variables, using a multilevel 2-equation system. RESULTS: Results showed that open-access systems and high-volume colonoscopy centers were independently associated with a higher likelihood of detecting significant lesions and longer withdrawal durations. Fee for service (FFS) payment was associated with shorter withdrawal durations, and so had an indirect negative impact on the diagnostic yield. Teaching centers exhibited lower detection rates and longer withdrawal times. CONCLUSIONS: Our results suggest that gatekeeping colonoscopy is likely to miss patients with significant lesions and that developing specialized colonoscopy units is important to improve performance. Results also suggest that FFS may result in a lower quality of care in colonoscopy practice and highlight the fact that longer withdrawal times do not necessarily indicate higher quality in teaching centers.

The postsynaptic density protein PSD-95 is a major element of synapses. PSD-95 is involved in aging, Alzheimer's disease (AD) and numerous psychiatric disorders. However, contradictory data about PSD-95 expression in aging and AD have been reported. Indeed in AD versus control brains PSD-95 varies according to regions, increasing in the frontal cortex, at least in a primary stage, and decreasing in the temporal cortex. In contrast, in transgenic mouse models of aging and AD PSD-95 expression is decreased, in behaviorally aged impaired versus unimpaired rodents it can decrease or increase and finally, it is increased in rodents grown in enriched environments. Different factors explain these contradictory results in both animals and humans, among others concomitant psychiatric endophenotypes, such as depression. The possible involvement of PSD-95 in reactive and/or compensatory mechanisms during AD progression is underscored, at least before the occurrence of important synaptic elimination. Thus, in AD but not in AD transgenic mice, enhanced expression might precede the diminution commonly observed in advanced aging. A two-compartments cell model, separating events taking place in cell bodies and synapses, is presented. Overall these data suggest that AD research will progress by untangling pathological from protective events, a prerequisite for effective therapeutic strategies.

BACKGROUND: Several parameters of cardiovascular physiology and pathophysiology exhibit circadian rhythms. Recently, a relation between infarct size and the time of day at which it occurs has been suggested in experimental models of myocardial infarction. The aim of this study is to investigate whether circadian rhythms could cause differences in ischemic burden in patients with ST-elevation myocardial infarction (STEMI) undergoing primary percutaneous coronary intervention (PPCI).¦METHODS: In 353 consecutive patients with STEMI treated by PPCI, time of symptom onset, peak creatine kinase (CK), and follow-up at 30 days were obtained. We divided 24 hours into 4 time groups based on time of symptom onset (00:00-05:59, 06:00-11:59, 12:00-17:59, and 18:00-23:59).¦RESULTS: There was no difference between the groups regarding baseline patients and management's characteristics. At multivariable analysis, there was a statistically significant difference between peak CK levels among patients with symptom onset between 00:00 and 05:59 when compared with peak CK levels of patients with symptom onset in any other time group (mean increase 38.4%, P < .05). Thirty-day mortality for STEMI patients with symptom onset occurring between 00:00 and 05:59 was significantly higher than any other time group (P < .05).¦CONCLUSION: This study demonstrates an independent correlation between the infarct size of STEMI patients treated by PPCI and the time of the day at which symptoms occurred. These results suggest that time of the day should be a critical issue to look at when assessing prognosis of patients with myocardial infarction.

The basic photosynthetic unit containing the reaction centre and the light-harvesting I complex (RC-LHI) of the purple non-sulphur bacterium Rhodospirillum rubrum was purified and reconstituted into two-dimensional (2D) membrane crystals. Transmission electron microscopy using conventional techniques and cryoelectron microscopy of the purified single particles and of 2D crystals yielded a projection of the RC-LHI complex at a resolution of at least 1.6 nm. In this projection the LHI ring appears to have a square symmetry and packs in a square crystal lattice. The square geometry of the LHI ring was observed also in images of single isolated particles of the RC-LHI complex. However, although the LHI units are packed identically within the crystal lattice, a new rotational analysis developed here showed that the reaction centres take up one of four possible orientations within the ring. This fourfold disorder supports our interpretation of a square ring symmetry and suggests that a hitherto undetected component may be present within the photosynthetic unit.

Contexte : Hermaphrodisme, pseudohermaphrodisme, intersexe ou encore troubles du développement du sexe (Disorders of Sex Development) : autant de termes désignant des pathologies concernant une proportion non-négligeable de la population. Dans une société où le développement génital et l'identité sexuelle sont à la fois tabous et mal connus, le corps médical a un rôle clé à jouer dans la prise en charge à long terme ainsi que dans le soutien des patients intersexués et de leur entourage proche. Cette prise en charge commence par une information complète et transparente des parents et des enfants touchés. Objectifs : Elaborer un document capable de décrire de la manière la plus complète et compréhensible possible le thème de l'intersexualité. Il sera à la fois basé sur les connaissances médicales actuelles, mais également sur les témoignages de patients, parents et groupes de soutien aux intersexes. Il sera finalement destiné à l'information des parents ainsi que de leurs enfants intersexués à différentes étapes de leur développement. Méthode : Une compréhension approfondie de la thématique de l'intersexualité sur le plan médical, impliquant une approche multidisciplinaire, est une première étape cruciale. Agrémentée de témoignages de patients, de parents et des divers groupes de soutient actifs sur ce sujet, nous obtiendrons les bases nécessaires pour concevoir un document d'information à la fois le plus complet et le plus proche possible des attentes des patients et des parents. Résultat escompté : Répondre aux attentes formulées par les différents acteurs de la thématique de l'intersexualité et ainsi promouvoir une meilleure compréhension des mécanismes et des enjeux qui la caractérisent. Obtenir un feedback positif des groupes de soutien aux intersexes sur la qualité de l'information donnée dans ce document.
