La prestation court-séjour est proposée dans le canton de Vaud depuis 1984. Elle s'inscrit dans la volonté des services publics de permettre aux personnes, âgées notamment, de vivre à domicile le plus longtemps possible. Il s'agit d'accueillir temporairement en établissement médico-social (EMS) ou en division C d'un hôpital, les personnes momentanément affaiblies à la suite d'une hospitalisation ou d'un problème de santé, mais aussi des personnes durablement atteintes dans leur santé, en particulier pour donner quelques jours ou semaines de répit à leur entourage. En 2013, afin de faire un bilan de cette prestation et démarrer une phase de réflexion quant aux améliorations qui pourraient lui être apportées, le Service des assurances sociales et de l'hébergement (SASH) a mandaté l'Institut universitaire de médecine sociale et préventive (IUMSP) pour la réalisation d'une enquête sur l'expérience des usagers des courts-séjours dans le canton de Vaud.

Cette thèse de doctorat porte sur le vécu et l'adaptation des couples âgés séparés par l'entrée de l'un des conjoints en institution. Notre démarche se fonde sur le paradigme compréhensif en sciences humaines. Nous avons rencontré sept couples dont l'un des conjoints vivait dans un Etablissement Médico-Social (EMS) de Suisse Romande, alors que l'autre continuait de vivre dans la communauté. Pour chaque situation, nous avons mené une interview individuelle de chacun des conjoints ainsi qu'une interview de couple. Nous avons effectué une analyse thématique du discours des interviewés. En outre, adoptant une perspective à la fois scientifique et clinique, nous avons étudié la dynamique conjugale des couples. Enfin, nous avons élaboré une typologie des différentes trajectoires de ces couples, en mettant en évidence les liens entre la dynamique de couple antérieure à l'hébergement, le vécu du moment de l'hébergement et le vécu lors des interviews. Nous avons montré le rôle central de l'ambivalence, vis-à-vis de la relation conjugale ou vis-à-vis de l'hébergement, dans les difficultés d'adaptation des couples à leur nouvelle situation de vie. -- This thesis is about the experience and adaptation of older couples separated by the accommodation of one spouse in a specialised institution. Our approach is based on the comprehensive paradigm in human sciences. We have met seven couples, of which one spouse was living in an institution (EMS) in the French speaking part of Switzerland, as the other spouse was still living in the community. In every situations, we have interviewed each spouse individually and both spouses together. We have carried out a thematic analysis of the discourses. Moreover, taking a scientific as well as a clinical perspective, we have studied the spousal dynamics of the couples. Finally, we have elaborated a typology of couples' trajectories, from earlier spousal dynamics to their experience of the transition and their experience in the time of the interviews. We have showed the crucial role of ambivalence, towards the couple relation or towards the accommodation, in couples' difficulties to adapt to their new living situation.

QUESTION: In the ageing European population, the proportion of interventions by the emergency medical services (EMS) for elderly patients is increasing, but little is known about the recent trend of EMS interventions in nursing homes. The aim of this analysis was to describe the evolution of the incidence of requests for prehospital EMS interventions for nursing home residents aged 65 years and over between 2004 and 2013. METHODS: A prospective population-based register of routinely collected data for each EMS intervention in the Canton of Vaud. Linear time trends of incidence of requests to the EMS in nursing homes were calculated and stratified by age categories. RESULTS: The number of ambulance interventions in nursing homes for people aged 65 years and over (65+) increased by 68.9% (1124‒1898) between 2004 and 2013. A significant linear increase of the annual incidence of requests to EMS per 1,000 nursing home residents was found for people aged 65-79 (10.2, 95% confidence interval [CI] 6.2-14.2), 80-89 (16.5, 95% CI 14.0-19.0) and over 90 (12.1, 95% CI 5.8-18.4). EMS interventions in nursing home residents who required an emergency physician increased during the same period by 205.6% (from 106 to 324), representing an increase from 2% to 7% of all emergency physician interventions in the Canton. CONCLUSIONS: Our results confirmed an important increase in the incidence of EMS interventions in nursing homes during the last decade, far exceeding the actual increase of the nursing home population during the same period. This evolution represents an important opportunity to reconsider the EMS missions in the context of an ageing society.

INTRODUCTION: As it might lead to less discomfort, magnetic nerve stimulation (MNS) is increasingly used as an alternative to electrical stimulation methods. Yet, MNS and electrical nerve stimulation (ENS) and electrical muscle stimulation (EMS) have not been formally compared for the evaluation of plantar flexor neuromuscular function. METHODS: We quantified plantar flexor neuromuscular function with ENS, EMS and MNS in 10 volunteers in fresh and fatigued muscles. Central alterations were assessed through changes in voluntary activation level (VAL) and peripheral function through changes in M-wave, twitch and doublet (PS100) amplitudes. Discomfort associated with 100-Hz paired stimuli delivered with each method was evaluated on a 10-cm visual analog scale. RESULTS: VAL, agonist and antagonist M-wave amplitudes and PS100 were similar between the different methods in both fresh and fatigued states. Potentiated peak twitch was lower in EMS compared to ENS, whereas no difference was found between ENS and MNS for any parameter. Discomfort associated with MNS (1.5 ± 1.4 cm) was significantly less compared to ENS (5.5 ± 1.9 cm) and EMS (4.2 ± 2.6 cm) (p < 0.05). CONCLUSION: When PS100 is used to evaluate neuromuscular properties, MNS, EMS and ENS can be used interchangeably for plantar flexor neuromuscular function assessment as they provide similar evaluation of central and peripheral factors in unfatigued and fatigued states. Importantly, electrical current spread to antagonist muscles was similar between the three methods while discomfort from MNS was much less compared to ENS and EMS. MNS may be potentially employed to assess neuromuscular function of plantar flexor muscles in fragile populations.

Background: Microparticles are small phospholipid vesicles of <1 lm shed in blood flow by various cell types including red blood cells. Erythrocyte-derived microparticles (EMPs) accumulate in erythrocyte concentrates (ECs) during their storage time. EMPs are considered as part of storage lesion and as their exact role is not elucidated, they could be involved in these clinical outcomes. Aims: The aim of this study is to evaluate the impact and implication of EMPs isolate from ECs on coagulation. Methods: EMPs were first isolated from erythrocyte concentrates by centrifugation and counted by flow cytometry. Using a calibrated automated thrombogram, EMPs were then added to different type of plasmas in order to evaluate the potential of thrombin generation. Results: We demonstrate that EMPs isolated from ECs are capable to accelerate and amplify thrombin generation in presence of a low exogenous tissue factor concentration, thanks to their negatively charged membrane necessary for the assembly of coagulation complexes. Interestingly, in the absence of exogenous tissue factor, EMPs are also able to trigger thrombin generation. In addition, thrombin generation induced by EMPs is not affected by the presence of anti-TF antibodies. Finally, thrombin generation induced by EMPs is not affected by using plasma samples deficient in factor VII, XI or XII. However, thrombin generation is reduced in plasma deficient in factor VIII or IX and is completely abolished in plasma deficient in factor X, V or II. No thrombin generation was observed in plasma samples without EMPs. Summary/conclusion: Several studies have shown a link between storage time of blood products and post transfusion complications. We provide evidence that EMPs accumulated during storage of erythrocyte concentrates were not only able to accelerate and support thrombin generation in plasma in presence of a low exogenous tissue-factor concentration, but also to trigger thrombin generation in absence of exogenous TF. The impact of those transfused EMs is unknown on recipients, nevertheless it could be hypothesized that under certain circumstances, transfused EMPs could be involved in thrombin generation and could be linked to adverse clinical outcome. Further work is needed to determine whether procoagulant EMPs transfused with erythrocyte concentrate may account for some of the complications occurring after red blood cell transfusion, and more particularly after transfusion of ''older''stored blood, rich in EMPs.

L'environnement construit contribue à influencer la santé des individus, selon l'Organisation Mondiale de la Santé. L'architecture des établissements médico-sociaux (EMS) peut, elle aussi, produire un impact sur le bien-être et la satisfaction des résident-e-s et des autres usagers. Dans le canton de Vaud, des normes architecturales de construction des EMS (les Directives et recommandations architecturales des établissements médico-sociaux, DAEMS) ont été créées en 2003 afin d'optimiser la conception des EMS par rapport aux besoins des usagers. Notre étude a pour but de vérifier si les EMS construits selon les DAEMS sont associés avec une satisfaction accrue des usagers par rapport aux EMS qui n'ont pas été construits selon ces directives. "Le but de la présente étude est de vérifier si le fait de construire les EMS selon les DAEMS est associé avec une plus grande satisfaction chez les usagers par rapport aux EMS qui n'ont pas été construits selon les DAEMS. Nous avons effectué une comparaison de la satisfaction auprès des trois-publics qui sont les principaux usagers des EMS, à savoir les résident-e-s, leurs proches de référence, et les professionnel-le-s en EMS. Trente EMS ont participé à l'étude, à savoir dix construits selon les DAEMS (EMS « DAEMS »), et vingt qui n'ont pas été construits selon les DAEMS (EMS « non-DAEMS »). Ce rapport a été défini pour garantir une bonne validité statistique des résultats"
