This is the third edition of the compendium. It documents the status of important projects on nanomaterial toxicity and exposure monitoring, integrated risk management, research infrstructure and coordination and support activities. The compendium is not intended to be a guidance document for human health and environmental safety management of nanotechnologies, as such guidance documents already exist and are widely available. Neither is the compendium intended to be a medium for the publication of scientific papers and research results, as this task is covered by scientific conferences and the reviewed press. The compendium aims to bring researchers closer together and show them the potential for synergy in their work. It is a means to establish links and communication between them during the actual research phase and well before the publication of their results. It thus focuses on the communication of projects' strategic aims, extensively covers specific work objectives and the methods used in research, and documents human capacities and available laboratory infrastructure. As such, the compendium supports collaboration on common goals and the joint elaboration of future plans, whilst compromising neither the potential for scientific publication, nor intellectual property rights. Evolution of knowledge and skills in violence risk management and assessment allowed an indisputable progress. Nevertheless instruments used such as HCR-20 still remain source of controversy and gives occurrence to numerous criticisms. In this context, the current high security climate puts added pressure on the medical teams which also have to deal with the violence of patients. A reflection on the possibilities and the limits of care as well as freedom deprivation may allow us to envision what is an acceptable risk.

The recent advances in sequencing technologies have given all microbiology laboratories access to whole genome sequencing. Providing that tools for the automated analysis of sequence data and databases for associated meta-data are developed, whole genome sequencing will become a routine tool for large clinical microbiology laboratories. Indeed, the continuing reduction in sequencing costs and the shortening of the 'time to result' makes it an attractive strategy in both research and diagnostics. Here, we review how high-throughput sequencing is revolutionizing clinical microbiology and the promise that it still holds. We discuss major applications, which include: (i) identification of target DNA sequences and antigens to rapidly develop diagnostic tools; (ii) precise strain identification for epidemiological typing and pathogen monitoring during outbreaks; and (iii) investigation of strain properties, such as the presence of antibiotic resistance or virulence factors. In addition, recent developments in comparative metagenomics and single-cell sequencing offer the prospect of a better understanding of complex microbial communities at the global and individual levels, providing a new perspective for understanding host-pathogen interactions. Being a high-resolution tool, high-throughput sequencing will increasingly influence diagnostics, epidemiology, risk management, and patient care.

- Risk management is often assessed through linear methods which stress positioning and causal logical frameworks: to such events correspond such consequences and such risks accordingly. Consideration of the interrelationships between risks is often overlooked and risks are rarely analyzed in their dynamic and nonlinear components. This work shows what systemic methods, including the study of complex systems, are likely to bring to knowledge, management, anticipation of business risks, both on the conceptual and the practical sides. Based on the definitions of systems and risks in various areas, as well as methods used to manage risk, this work confronts these concepts with approaches of complex systems analysis and modeling. This work highlights the reducing effects of some business risk analysis methods as well as limitations of risk universes caused in particular by unsuitable definitions. As a result this work also provides chief officers with a range of different tools and approaches which allows them a better understanding of complexity and as such a gain in efficiency in their risk management practices. It results in a better fit between strategy and risk management. Ultimately the firm gains in its maturity of risk management.

NanoImpactNet (NIN) is a multidisciplinary European Commission funded network on the environmental, health and safety (EHS) impact of nanomaterials. The 24 founding scientific institutes are leading European research groups active in the fields of nanosafety, nanorisk assessment and nanotoxicology. This 4−year project is the new focal point for information exchange within the research community. Contact with other stakeholders is vital and their needs are being surveyed. NIN is communicating with 100s of stakeholders: businesses; internet platforms; industry associations; regulators; policy makers; national ministries; international agencies; standard−setting bodies and NGOs concerned by labour rights, EHS or animal welfare. To improve this communication, internet research, a questionnaire distributed via partners and targeted phone calls were used to identify stakeholders' interests and needs. Knowledge gaps and the necessity for further data mentioned by representatives of all stakeholder groups in the targeted phone calls concerned: potential toxic and safety hazards of nanomaterials throughout their lifecycles; fate and persistence of nanoparticles in humans, animals and the environment; risks associated to nanoparticle exposure; participation in the preparation of nomenclature, standards, methodologies, protocols and benchmarks; development of best practice guidelines; voluntary schemes on responsibility; databases of materials, research topics and themes. Findings show that stakeholders and NIN researchers share very similar knowledge needs, and that open communication and free movement of knowledge will benefit both researchers and industry. Consequently NIN will encourage stakeholders to be active members. These survey findings will be used to improve NIN's communication tools to further build on interdisciplinary relationships towards a healthy future with nanotechnology.

The description more than 30 years ago of the role of de novo purine synthesis in T and B lymphocytes clonal proliferation opened the possibility for selective immunosuppression by targeting specific enzymatic pathways. Mycophenolic acid (MPA) blocks the key enzyme inosine monophosphate dehydrogenase and the production of guanosine nucleotides required for DNA synthesis. Two MPA formulations are currently used in clinical transplantation as part of the maintenance immunosuppressive regimen. Mycophenolate mofetil (MMF) was the first MPA agent to be approved for the prevention of acute rejection following renal transplantation, in combination with cyclosporine and steroids. Enteric-coated mycophenolate sodium (EC-MPS) is an alternative MPA formulation available in clinical transplantation. In this review, we will discuss the clinical trials that have evaluated the efficacy and safety of MPA in adult kidney transplantation for the prevention of acute rejection and their use in new combination regimens aiming at minimizing calcineurin inhibitor toxicity and chronic allograft nephropathy. We will also discuss MPA pharmacokinetics and the rationale for therapeutic drug monitoring in optimizing the balance between efficacy and safety in individual patients.

The role of socio-political parameters and of knowledge in the management of hydrological risks¦The purpose of the thesis is to analyse (1) the socio-political management of hydrological risks, (2) the knowledge about hydrological risks, and (3) the interaction between risk management and knowledge. Three case studies were carried out, two at a regional level (city of Berne, village of Saillon) and one about a specific stakeholder (the dyke corporations in the canton of Berne). Empirical data were collected by the means of semi-structured interviews and surveys.¦The management of hydrological risks is highly influenced by socio-political issues, i.e. by interests and goals of stakeholders, by the balance of power between stakeholders, as well as by negotiations and decision-making processes. In practice, however, risk management is mainly constrained by physical, technical, and administrative aspects. The neglect of the socio-political dimension may thus be the source of conflicts which significantly delay the planning and implementation of flood protection measures, or even stop them. Furthermore, risk management mostly concentrates on hazard reduction. Discussions on vulnerability issues are less frequent although they are fundamental for treating risks in a holistic manner.¦Because of the importance of the social-political dimension and of vulnerability issues, it is necessary that the common approach of managing hydrological risks is reconsidered and adapted. Moreover, the integration of all stakeholders that are concerned with hydrological risks is essential for finding solutions which are supported by a majority. For instance, the application of appropriate negotiation instruments is insufficient.¦Knowledge about hydrological risks and their management can be classified into four categories (system knowledge, event knowledge, intervention knowledge, socio-political knowledge) which are all influencing the reduction of risks. Among the most important factors that are likely to trigger knowledge transformations, one can point out flood events, studies about risk parameters (hazards, vulnerability, protection measures, etc.), knowledge exchange between stakeholders, and the search for solutions during risk management.¦The characteristics of knowledge vary considerably between stakeholders. The affiliation to a specific group is thus not a sufficient criterion to determine the quality of a stakeholder's knowledge: every stakeholder may have knowledge that is relevant for risk management. However, differences between stakeholders complicate the communication. This problem could be attenuated by mediators which ensure the translation between stakeholders. In practice, such instances are generally lacking.¦The management and knowledge of hydrological risks are highly interdependent. The state and the characteristics of the four categories of knowledge determine directly the quality of flood protection. Gaps and imprecison may thus lead to forms of management which are not adapted to the actual risks. This kind of situation can be avoided by updating regularly the knowledge about hydrological risks and about protection measures. However, knowledge must not be restricted to the experience of past floods. On the contrary, it is indispensable also to involve prospective reflections and new scenarios.¦Risk management does not only require knowledge, it may also generate new knowledge and enlarge existing knowledge. The creation and the transfer of knowledge has thus to be seen as a central task in risk management.

This 4-year project is the new focal point for information exchange within the research community. Contact with other stakeholders is vital and their needs are being surveyed. NIN is communicating with 100s of stakeholders: businesses; internet platforms; industry associations; regulators; policy makers; national ministries; international agencies; standard-setting bodies and NGOs concerned by labour rights, EHS or animal welfare. To improve this communication, internet research, a questionnaire distributed via partners and targeted phone calls were used to identify stakeholders' interests and needs. Knowledge gaps and the necessity for further data mentioned by representatives of all stakeholder groups in the targeted phone calls concerned: • the potential toxic and safety hazards of nanomaterials throughout their lifecycles; • the fate and persistence of nanoparticles in humans, animals and the environment; • the associated risks of nanoparticle exposure; • greater participation in: the preparation of nomenclature, standards, methodologies, protocols and benchmarks; • the development of best practice guidelines; • voluntary schemes on responsibility; • databases of materials, research topics and themes, but also of expertise. These findings suggested that stakeholders and NIN researchers share very similar knowledge needs, and that open communication and free movement of knowledge will benefit both researchers and industry. Subsequently a workshop was organised by NIN focused on building a sustainable multi-stakeholder dialogue. Specific questions were asked to different stakeholder groups to encourage discussions and open communication. 1. What information do stakeholders need from researchers and why? The discussions about this question confirmed the needs identified in the targeted phone calls. 2. How to communicate information? While it was agreed that reporting should be enhanced, commercial confidentiality and economic competition were identified as major obstacles. It was recognised that expertise was needed in the areas of commercial law and economics for a wellinformed treatment of this communication issue. 3. Can engineered nanomaterials be used safely? The idea that nanomaterials are probably safe because some of them have been produced 'for a long time', was questioned, since many materials in common use have been proved to be unsafe. The question of safety is also about whether the public has confidence. New legislation like REACH could help with this issue. Hazards do not materialise if exposure can be avoided or at least significantly reduced. Thus, there is a need for information on what can be regarded as acceptable levels of exposure. Finally, it was noted that there is no such thing as a perfectly safe material but only boundaries. At this moment we do not know where these boundaries lie. The matter of labelling of products containing nanomaterials was raised, as in the public mind safety and labelling are connected. This may need to be addressed since the issue of nanomaterials in food, drink and food packaging may be the first safety issue to attract public and media attention, and this may have an impact on 'nanotechnology as a whole. 4. Do we need more or other regulation? Any decision making process should accommodate the changing level of uncertainty. To address the uncertainties, adaptations of frameworks such as REACH may be indicated for nanomaterials. Regulation is often needed even if voluntary measures are welcome because it mitigates the effects of competition between industries. Data cannot be collected on voluntary bases for example. NIN will continue with an active stakeholder dialogue to further build on interdisciplinary relationships towards a healthy future with nanotechnology.

[Auteurs]

AIM: Patients with non-insulin-dependent diabetes mellitus (NIDDM) are at increased cardiovascular risk due to an accelerated atherosclerotic process. The present study aimed to compare skin microvascular function, pulse wave velocity (PWV), and a variety of hemostatic markers of endothelium injury [von Willebrand factor (vWF), plasminogen activator inhibitor-1 (PAI-1), tissue plasminogen activator (t-PA), tissue factor pathway inhibitor (TFPI), and the soluble form of thrombomodulin (s-TM)] in patients with NIDDM. METHODS: 54 patients with NIDDM and 38 sex- and age-matched controls were studied. 27 diabetics had no overt micro- and/or macrovascular complications, while the remainder had either or both. The forearm skin blood flow was assessed by laser-Doppler imaging, which allowed the measurement of the response to iontophoretically applied acetylcholine (endothelium-dependent vasodilation) and sodium nitroprusside (endothelium-independent vasodilation), as well as the reactive hyperemia triggered by the transient occlusion of the circulation. RESULTS: Both endothelial and non-endothelial reactivity were significantly blunted in diabetics, regardless of the presence or the absence of vascular complications. Plasma vWF, TFPI and s-TM levels were significantly increased compared with controls only in patients exhibiting vascular complications. Concentrations of t-PA and PAI-1 were significantly increased in the two groups of diabetics versus controls. CONCLUSION: In NIDDM, both endothelium-dependent and -independent microvascular skin reactivity are impaired, whether or not underlying vascular complications exist. It also appears that microvascular endothelial dysfunction is not necessarily associated in NIDDM with increased circulating levels of hemostatic markers of endothelial damage known to reflect a hypercoagulable state.

Nanomaterials have properties that are often very different from normal materials made of the same substance, which can be used to create novel products with exciting properties. However, the health and environmental impact of these nanomaterials is also changed and their potential risk needs to be studied. There is evidence that some nanomaterials can pass through tissue barriers (including the blood-brain barrier) and cell membranes. This is interesting for medical applications, but it raises concerns about the impact of non-medical nanomaterials. Current research aims at better coordinating research efforts and at better communication between researchers and involved stakeholders. Many research labs and production sites currently follow strategies that were established for dealing with very toxic chemicals and powders, until future research in this field helps identify the appropriate level of protection. All these efforts will ultimately ensure a safe, healthy and environmental friendly production, use and disposal of nanomaterials.

In 2007, several international studies brought useful information for the daily work of internists in hospital settings. This summary is of course subjective but reflects the interests and questions of the chief residents of the Department of internal medicine who wrote this article like an original trip in medical literature. This trip will allow you to review some aspects of important fields such as heart failure, diabetes, endocarditis, COPD, and quality of care. Besides the growing diversity of the fields covered by internal medicine, these various topics underline also the uncertainty internists have to face in a practice directed towards evidence.
