An adaptation technique based on the synoptic atmospheric circulation to forecast local precipitation, namely the analogue method, has been implemented for the western Swiss Alps. During the calibration procedure, relevance maps were established for the geopotential height data. These maps highlight the locations were the synoptic circulation was found of interest for the precipitation forecasting at two rain gauge stations (Binn and Les Marécottes) that are located both in the alpine Rhône catchment, at a distance of about 100 km from each other. These two stations are sensitive to different atmospheric circulations. We have observed that the most relevant data for the analogue method can be found where specific atmospheric circulation patterns appear concomitantly with heavy precipitation events. Those skilled regions are coherent with the atmospheric flows illustrated, for example, by means of the back trajectories of air masses. Indeed, the circulation recurrently diverges from the climatology during days with strong precipitation on the southern part of the alpine Rhône catchment. We have found that for over 152 days with precipitation amount above 50 mm at the Binn station, only 3 did not show a trajectory of a southerly flow, meaning that such a circulation was present for 98% of the events. Time evolution of the relevance maps confirms that the atmospheric circulation variables have significantly better forecasting skills close to the precipitation period, and that it seems pointless for the analogue method to consider circulation information days before a precipitation event as a primary predictor. Even though the occurrence of some critical circulation patterns leading to heavy precipitation events can be detected by precursors at remote locations and 1 week ahead (Grazzini, 2007; Martius et al., 2008), time extrapolation by the analogue method seems to be rather poor. This would suggest, in accordance with previous studies (Obled et al., 2002; Bontron and Obled, 2005), that time extrapolation should be done by the Global Circulation Model, which can process atmospheric variables that can be used by the adaptation method.

Background: The current data comparing posterior and anterior circulation strokes with regards to clinical, etiological, radiological and outcome factors are conflicting. We searched for distinguishing features between both territories in 1'449 consecutive acute ischemic stroke patients. Methods: All consecutive patients with acute ischemic stroke admitted to a single stroke unit from January 2003 to July 2008 were included in a prospective registry. Territory of acute stroke was determined by a combination of neuroimaging (MRI and / CT / CTP) and clinical symptoms and signs. Patients with uncertain localisation and patients with simultaneous strokes in the anterior and posterior circulation were excluded from this analysis. Results: Of a total of 1728 patients, 466 (17.0%) had had posterior, 983 (56.8%) anterior, 136 (7.9%) unknown territory, and 43 (2.5%) simultaneous posterior and anterior territory stroke. Of 39 variables that were compared, 29 differed significantly in univariate analysis, including less dependency (OR_0.50) and mortality (OR_0.56) at 3 months in posterior stroke. In multivariate analysis (see table), male gender, lacunar mechanism, arterial dissection and endovascular recanalisation were more frequent in posterior stroke, and admission NIHSS and IV-thrombolysis rate were lower. Significant acute arterial pathology (_50% stenosis) was less frequently found on acute imaging in posterior stroke (OR_0.33). Of 633 patients with significant arterial pathology, it was more frequently present intracranially in posterior (OR_1.62) and extracranially in anterior stroke (OR _ 0.87). In 610 patients where recanalisation was assessed at 24 hours, intracranial (OR_0.26), extracranial (OR_0.25) and overall recanalisation (OR_0.34) was less frequent in the posterior circulation. Conclusions: Acute posterior strokes are less severe and recover better, despite lower IV thrombolysis and recanalisation rates. They are more frequently due to lacunes and dissections and have less arterial pathology burden then anterior circulation strokes.

The history of most cutaneous squamous cell carcinomas (CSCC) is limited to the skin. However, about 4% of these malignancies are at risk of metastasis and can be life-threatening. This risk is determined by clinical and histological elements which are individually recognized, but so far staging systems allow us neither to assess a risk score, nor to adopt a standardized therapeutical approach. This article reviews prognostic factors for CSCC, and underlines the need for the clinician to have all clinical and histological elements available, in order to try to define the best therapeutical strategy for each case, following up-to-date recommendations.

The gene encoding type I signal peptidase (Lmjsp) has been cloned from Leishmania major. Lmjsp encodes a protein of 180 amino residues with a predicted molecular mass of 20.5 kDa. Comparison of the protein sequence with those of known type I signal peptidases indicates homology in five conserved domains A-E which are known to be important, or essential, for catalytic activity. Southern blot hybridisation analysis indicates that there is a single copy of the Lmjsp gene. A recombinant SPase protein and a synthetic peptide of the L. major signal peptidase were used to examine the presence of specific antibodies in sera from either recovered or active individuals of both cutaneous and visceral leishmaniasis. This evaluation demonstrated that sera from cutaneous and visceral forms of leishmaniasis are highly reactive to both the recombinant and synthetic signal peptidase antigens. Therefore, the Leishmania signal peptidase, albeit localised intracellularly, is a significant target of the Leishmania specific immune response and highlights its potential use for serodiagnosis of cutaneous and visceral leishmaniasis.

A 77-year-old man with a 5-year history of mycosis fungoides (MF) who had received several lines of therapy, including intravenous courses of Methotrexate (MTX) for the past 2 years, went on to develop several ulcerated cutaneous nodules on the left leg. Biopsy revealed diffuse sheets of EBV-positive large B cells (CD20+ CD30 ± IgM Lambda), with an angiocentric distribution and a monoclonal IGH gene rearrangement. Although the pathological features were diagnostic for an EBV-positive diffuse large B-cell lymphoma (DLBCL), several possibilities could be considered for assignment to a specific entity: EBV-positive DLBCL of the elderly, methotrexate-induced lymphoproliferative disorder (LPD), lymphomatoid granulomatosis, or the more recently described EBV-positive mucocutaneous ulcer. The development of EBV+ lymphoproliferations has been reported in two other patients with MF under MTX, and occurred as skin lesions of the leg in one of these and in the current case, which may question the relatedness to primary cutaneous DLCBL, leg-type.

Melanoma is the most common lethal cutaneous neoplasm. In order to harmonize treatment and follow-up of melanoma patients, guidelines for the management of melanoma in Switzerland have been inaugurated in 2001. These have been approved by all Swiss medical societies involved in the care of melanoma patients. New data necessitated changes concerning the safety margins (reduction to maximally 2 cm) and modifications of the recommendations of follow-up.

Primary cutaneous posttransplant lymphoproliferative disorders (PTLD) are rare. This retrospective, multicenter study of 35 cases aimed to better describe this entity. Cases were (re)-classified according to the WHO-EORTC or the WHO 2008 classifications of lymphomas. Median interval between first transplantation and diagnosis was 85 months. Fifty-seven percent of patients had a kidney transplant. Twenty-four cases (68.6%) were classified as primary cutaneous T cell lymphoma (CTCL) and 11 (31.4%) as primary cutaneous B cell PTLD. Mycosis fungoides (MF) was the most common (50%) CTCL subtype. Ten (90.9%) cutaneous B cell PTLD cases were classified as EBV-associated B cell lymphoproliferations (including one plasmablastic lymphoma and one lymphomatoid granulomatosis) and one as diffuse large B cell lymphoma, other, that was EBV-negative. Sixteen (45.7%) patients died after a median follow-up of 19.5 months (11 [68.8%] with CTCL [6 of whom had CD30(+) lymphoproliferative disorders (LPD)] and 5 [31.2%] with cutaneous B cell PTLD. Median survival times for all patients, CTCL and cutaneous B cell PTLD subgroups were 93, 93, and 112 months, respectively. Survival rates for MF were higher than those for CD30(+) LPD. The spectrum of primary CTCL in organ transplant recipients (OTR) is similar to that in the general population. The prognosis of posttransplant primary cutaneous CD30(+) LPD is worse than posttransplant MF and than its counterpart in the immunocompetent population. EBV-associated cutaneous B cell LPD predominates in OTR.

Objective: Reconstruction of alar structures of the nose remains difficult. The result has to be not only functional but also aesthetic. Different solutions to reconstruct alar defects are feasible. A good result that meets the specific demands on stability, aesthetics, and stable architecture without shrinkage of the area is not easily achieved. Method: A perichondrial cutaneous graft (PCCG), a graft consisting of a perichondral layer, fatty tissue, and skin that is harvested retroauriculary, is combined with an attached cartilage strip. Case Result: A 72-year-old patient suffering from basal cell carcinoma of the ala of the nose underwent the reconstructive procedure with a good result in 1 year in terms of stability, color match, and graft take. Conclusion: First, a strip of cartilage had been included in a PCCG where tumor resection required sacrifice of more than 50% of the alar rim. The case shows that one can consider a cartilage strip-enhanced PCCG graft to reconstruct alar defects.

Genetic tumor syndromes reflect an inherited predisposition to develop benign and malignant tumors. Increased frequency of neoplasms within the family or occurring at an early age are clinical clues for a possible underlying genetic susceptibility. Awareness of their associated cutaneous manifestations can facilitate early detection of risk for tumors. The goal of this article is to review clinical and molecular features of some genetic tumor syndrome which present with skin involvement at birth or during childhood.

Introduction : 6 décembre 1992: le peuple suisse rejette l'Accord sur l'Espace économique européen (EEE). Le Conseil fédéral décide alors de privilégier la voie bilatérale pour réglementer ses relations avec l'Union européenne (UE). Près de sept ans plus tard, le 21 juin 1999, un premier paquet de sept accords bilatéraux sont signés à Luxembourg. L'Accord sur la libre circulation des personnes (ALCP ou Accord) en constitue certainement « la partie la plus importante ». L'ALCP a pour objectif de garantir la libre circulation des travailleurs salariés, des travailleurs indépendants ainsi que de certaines catégories de non-actifs, et de faciliter la prestation de services sur le territoire des parties contractantes. Or la réalisation de ces objectifs dépend directement de la mise en oeuvre de l'interdiction de discrimination en raison de la nationalité. Au coeur même du principe de libre circulation, l'interdiction de discrimination en raison de la nationalité constitue, sans aucun doute, la clé de voûte de l'Accords. Partant de ce constat, il conviendra d'étudier dans un premier temps le principe de libre circulation (Partie I), puis de nous pencher dans un second temps sur l'interdiction de discrimination en raison de la nationalité (Partie II). La première partie de notre étude (consacrée à la libre circulation des personnes en vertu de l'ALCP) débutera par un survol des dispositions essentielles à l'appréhension d'un droit fondamental à la libre circulation, ainsi que par une présentation succincte du régime communautaire de libre circulation. Après avoir rappelé les étapes du cheminement ayant conduit à l'adoption de l'ALCP, et après avoir défini les différentes sources régissant la libre circulation entre la Suisse et les Etats membres de l'UE, nous analyserons ensuite l'ALCP lui-même, en procédant à la détermination de son champ d'application ainsi qu'à l'examen des principes généraux qui le régissent. Cet examen nous amènera alors à déterminer le rôle que devrait jouer la jurisprudence de la Cour de justice des Communautés européennes (CJCE) dans le cadre de l'Accord. Fort de cette analyse, nous conclurons cette première partie par une présentation du régime de libre circulation tel qu'institué par l'ALCP, sans oublier bien sûr d'évoquer, finalement, la portée du concept d'entrave et de la notion de citoyenneté européenne dans le cadre de ce régime. La seconde partie de notre étude (consacrée à l'interdiction de discrimination en raison de la nationalité selon l'ALCP) identifiera, à titre liminaire, les différentes dispositions de l'Accord visant à prohiber les discriminations en raison de la nationalité. Ces dispositions mises en lumière, nous détaillerons alors les étapes de l'examen permettant de relever la présence d'une discrimination en raison de la nationalité, en nous appuyant sur la jurisprudence de la Cour de justice et celle du Tribunal fédéral d'une part, et sur la doctrine développée en matière d'interdiction de discrimination d'autre part. Dans la foulée, nous pourrons ainsi présenter les différentes formes que peut revêtir une discrimination. Nous examinerons, en particulier, le régime prévu par la jurisprudence de la Cour de justice et celle du Tribunal fédéral en matière de discriminations à rebours présentant un élément transfrontalier. Suite de quoi nous serons en mesure - après examen des jurisprudences allemande et autrichienne relatives aux situations internes de discriminations à rebours - de procéder à une appréciation critique de la solution adoptée par le Tribunal fédéral en pareilles situations. A la suite de ces deux chapitres portant sur la notion et sur les formes de discriminations, il y aura lieu de définir le champ d'application matériel et personnel des différentes dispositions de l'Accord visant à interdire les discriminations en raison de la nationalité. Lors de la définition du champ d'application personnel en particulier, il conviendra, en premier lieu, d'identifier les personnes susceptibles d'invoquer ces différentes dispositions (les bénéficiaires). Cet examen débutera par l'identification des conditions générales communes à l'ensemble des dispositions étudiées. Il se poursuivra par l'analyse des conditions spécifiques de chaque norme, et se terminera par une présentation du régime particulier réservé aux membres de la famille et aux travailleurs détachés. En second lieu, il s'agira de déterminer les sujets de droit à l'encontre desquels ces dispositions sont opposables (les destinataires). Plus précisément, il s'agira d'examiner dans quelle mesure les particuliers, en sus des Etats, peuvent être destinataires des différentes obligations de l'ALCP en matière de libre circulation et d'interdiction de discrimination. Les champs d'application ayant été définis, il nous restera alors à examiner les différentes dispositions de l'Accord susceptibles de limiter la portée du principe de non-discrimination. Pour ce faire, nous analyserons en détail l'article 5 Annexe I-ALCP qui permet de limiter les droits consacrés par l'Accord pour des raisons d'ordre public, de sécurité publique ou de santé publique. Nous nous pencherons aussi sur les limitations liées à l'exercice de la puissance publique, consacrées par les articles 10, 16 et 22 al. 1 Annexe I-ALCP. Après un passage en revue des dernières dispositions de l'Accord prévoyant des limitations, nous examinerons ensuite dans quelle mesure certains principes non expressément prévus par l'Accord sont également susceptibles de justifier une différence de traitement en raison de la nationalité. Nous conclurons cette étude en analysant les implications juridiques d'une violation de l'interdiction de discrimination, ceci non seulement dans les relations liant l'Etat à un particulier, mais aussi dans celles liant deux particuliers entre eux. Dans le premier cas de figure nous verrons qu'il y a lieu de différencier l'analyse en fonction du type d'acte en question - les implications d'une violation n'étant pas les mêmes selon que l'on se trouve en présence d'une norme, d'une décision ou d'un contrat (de droit administratif ou de droit privé) présentant un caractère discriminatoire. Dans le second cas de figure, il s'agira cette fois-ci de distinguer les implications des conditions discriminatoires en fonction des différentes phases d'une relation contractuelle - aux stades de la conclusion d'un contrat, de son exécution et de sa résiliation.

Parasites have to survive in their vertebrate host during a sufficiently prolonged period of time to achieve their life cycle through successful transmission via insect vectors. In their vertebrate hosts, parasites are often confronted by vigorous effector immune responses that they have to subvert somehow to be able to outlast and be successfully transmitted.
