Problématique. Le diabete de type 2 est la maladie metabolique la plus frequente, sa prevalence ne cesse de croitre et parallelement, la population vieillissante est en constante augmentation. Le diagnostic de diabete de type 2 accroit le risque de syndrome geriatrique. Pour prevenir d'eventuelles complications, le traitement du diabete de type 2 est constitue de differentes composantes incluant des automesures de la glycemie capillaire, une prise reguliere du traitement pharmacologique souvent multiples de l'activite physique et enfin le suivi de recommandations pour une alimentation equilibree. Pour la personne agee diabetique de type 2, l'apprentissage de l'application de ces composantes du traitement signifie qu'elle doit proceder a des ajustements dans sa vie quotidienne. Buts. Les buts de cette etude sont : 1) de decrire l'ajustement psychosocial de la personne agee au diabete de type 2, et 2) d'explorer la presence de relations entre differentes variables sociodemographiques et de sante et l'ajustement psychosocial de personnes agees diabetiques de type 2. Méthodologie. Il s'agit d'une etude descriptive correlationnelle. L'echantillon de convenance est compose de 57 participants ages de plus de 65 ans connus pour un diagnostic de diabete de type 2. Les mesures ont ete effectuees par l'utilisation du questionnaire « The Psychological Adjustement to Illness Scale (PAIS-SR) » developpe par Derogatis (1986). Les donnees ont ete recoltees dans un centre ambulatoire universitaire de consultation en diabetologie en Suisse Romande. La théorie de l'adaptation cognitive lors d'événements menaçants (Taylor, 1983) a servi de cadre theorique a l'etude. Résultats. La majorite des participants a l'etude est de sexe masculin et l'age moyen des participants a l'etude est de 71 ans. Les participants ont un ajustement psychosocial a la maladie positif. Le score des signes depressifs est faible, tandis que davantage de difficultes apparaissent dans le domaine des relations sexuelles. Tous les participants ont des comorbidites ou ont developpe des complications. La moyenne de l'hemoglobine glyquee (HbA1c) correspond aux normes preconisees par les experts. Aucune correlation significative n'a pu etre etablie entre le niveau d'ajustement psychosocial, et les variables sociodemographiques et de sante.Conclusion : Les participants a l'etude ont un controle glycemique optimal et presentent un ajustement psychosocial positif. Malgre le fait qu'aucune correlation n'a pu etre determinee entre les differentes variables, la problematique reste importante a etudier. Les complications et les comorbidites associees au diagnostic de diabete de type 2 rendent cette population particulierement vulnerable, et a risque de declin fonctionnel. Peu d'etudes ont permis de determiner les composantes favorisant le processus d'adaptation d'un etat de sante vers un etat de maladie, et ce dans un contexte de vieillissement. La recherche dans ce domaine permettrait de determiner plus precisement des axes de prise en charge et de favoriser la prevention du declin fonctionnel.

NlmCategory="UNASSIGNED">Metabolic syndrome after transplantation is a major concern following solid organ transplantation (SOT). The CREB-regulated transcription co-activator 2 (CRTC2) regulates glucose metabolism. The effect of CRTC2 polymorphisms on new-onset diabetes after transplantation (NODAT) was investigated in a discovery sample of SOT recipients (n1=197). Positive results were tested for replication in two samples from the Swiss Transplant Cohort Study (STCS, n2=1294 and n3=759). Obesity and other metabolic traits were also tested. Associations with metabolic traits in population-based samples (n4=46'186, n5=123'865, n6>100,000) were finally analyzed. In the discovery sample, CRTC2 rs8450-AA genotype was associated with NODAT, fasting blood glucose and body mass index (Pcorrected<0.05). CRTC2 rs8450-AA genotype was associated with NODAT in the second STCS replication sample (odd ratio (OR)=2.01, P=0.04). In the combined STCS replication samples, the effect of rs8450-AA genotype on NODAT was observed in patients having received SOT from a deceased donor and treated with tacrolimus (n=395, OR=2.08, P=0.02) and in non-kidney transplant recipients (OR=2.09, P=0.02). Moreover, rs8450-AA genotype was associated with overweight or obesity (n=1215, OR=1.56, P=0.02), new-onset hyperlipidemia (n=1007, OR=1.76, P=0.007), and lower high-density lipoprotein-cholesterol (n=1214, β=-0.08, P=0.001). In the population-based samples, a proxy of rs8450G>A was significantly associated with several metabolic abnormalities. CRTC2 rs8450G>A appears to have an important role in the high prevalence of metabolic traits observed in patients with SOT. Mutations of the Wiskott-Aldrich syndrome gene (WAS) are responsible for Wiskott-Aldrich syndrome (WAS), a disease characterized by thrombocytopenia, eczema, immunodeficiency, and autoimmunity. Mice with conditional deficiency of Was in B lymphocytes (B/WcKO) have revealed a critical role for WAS protein (WASP) expression in B lymphocytes in the maintenance of immune homeostasis. Neural WASP (N-WASP) is a broadly expressed homolog of WASP, and regulates B-cell signaling by modulating B-cell receptor (BCR) clustering and internalization. We have generated a double conditional mouse lacking both WASP and N-WASP selectively in B lymphocytes (B/DcKO). Compared with B/WcKO mice, B/DcKO mice showed defective B-lymphocyte proliferation and impaired antibody responses to T-cell-dependent antigens, associated with decreased autoantibody production and lack of autoimmune kidney disease. These results demonstrate that N-WASP expression in B lymphocytes is required for the development of autoimmunity of WAS and may represent a novel therapeutic target in WAS.

The prevalence of type 2 diabetes mellitus and of the metabolic syndrome is rising worldwide and reaching epidemic proportions. These pathologies are associated with significant morbidity and mortality, in particular with an excess of cardiovascular deaths. Type 2 diabetes mellitus and the cluster of pathologies including insulin resistance, central obesity, high blood pressure, and hypertriglyceridemia that constitute the metabolic syndrome are associated with low levels of HDL cholesterol and the presence of dysfunctional HDLs. We here review the epidemiological evidence and the potential underlying mechanisms of this association. We first discuss the well-established association of type 2 diabetes mellitus and insulin resistance with alterations of lipid metabolism and how these alterations may lead to low levels of HDL cholesterol and the occurrence of dysfunctional HDLs. We then present and discuss the evidence showing that HDL modulates insulin sensitivity, insulin-independent glucose uptake, insulin secretion, and beta cell survival. A dysfunction in these actions could play a direct role in the pathogenesis of type 2 diabetes mellitus.

Plusieurs études populationnelles ont montré l'existence d'une association entre des taux sanguins élevés de transferrine et le syndrome métabolique (SM). Bien que cette association soit bien établie, restent encore à être décrites les associations entre le SM et les autres marqueurs sanguins du métabolisme du fer, tels que le fer, la transferrine (Tsf), la capacité totale de fixation de la transferrine (CTF) ou la saturation de la transferrine (SaTsf) sanguins. Le but de notre étude a été d'identifier les associations entre les différents marqueurs du métabolisme du fer (fer, ferritine, Tsf, CTF et SaTsf) et le SM. Les données de l'étude CoLaus, récoltées entre 2003 et 2006, ont été utilisées. Le SM était défini selon les critères du National Cholesterol Education Program Adult Panel III. L'analyse statistique a été faite en stratifiant selon le genre ainsi que le status ménopausal chez les femmes. Des 6733 participants, 1235 (18%) ont été exclus de fait d'absence de données concernant les variables qui nous intéressaient, ou chez qui nous avons soupçonné une possible hémochromatose non diagnostiquée (SaTsf> 50%). Des 5498 participants restant (âge moyen ± écart-type: 53 ± 11 ans), 2596 étaient des hommes, 1285 des femmes pré- et 1617 des femmes postménopausées. La prévalence du SM était de 29,4% chez les hommes, 8,3% et 25,5% chez les femmes pré- et postménopausées, respectivement. Dans les trois groupes, la prévalence du SM était la plus haute dans les quartiles les plus élevés de ferritine, Tsf et CTF, ainsi que dans le quartile le plus bas de SaTsf. Après ajustement sur l'âge, l'indice de masse corporelle, la protéine C réactive, la consommation de tabac et/ou d'alcool, la prise de suppléments en fer et les marqueurs hépatiques, l'appartenance au quartile le plus élevé de ferritine, Tsf ou CTF était associée à un risque plus important de SM chez les hommes et les femmes postménopausées : Odds ratio (OR) et [intervalle de confiance à 95%] pour la ferritine 1.44 [1.07-1.94] et 1.47 [0.99-2.17]; pour la Tsf et la CTF, OR=1.43 [1.06-1.91] et 2.13 [1.44-3.15] pour les hommes et les femmes postménopausées, respectivement. Au contraire, l'appartenance au quartile le plus élevé de la SaTsf était associé à un risque moins important de SM: OR=0.77 [0.57-1.05] et 0.59 [0.39-0.90] pour les hommes et les femmes postménopausées, respectivement. Il n'y avait aucune association entre les marqueurs sanguins du métabolisme du fer et le SM chez les femmes préménopausées, ni entre le fer sanguin et le SM chez les trois groupes. En conclusion, la majorité des marqueurs sanguins du métabolisme du fer, mais pas le fer lui-même, sont associés de manière indépendante au SM chez les hommes et les femmes postménopausées. -- Context: Excessive iron storage has been associated with metabolic syndrome (MS). Objective: To assess the association between markers of iron metabolism and MS in a healthy population. Design: Cross-sectional study conducted between 2003 and 2006. Setting: Population-based study in Lausanne, Switzerland. Patients: 5,498 participants aged 35-75 years, stratified by sex and menopausal status. Participants with transferrin saturation (TSAT) >50% were excluded. Intervention: None. Main Outcome Measures: serum iron, ferritin, transferrin, total iron binding capacity (TIBC) and TSAT. MS was defined according to ATP-III criteria. Results: Prevalence of MS was 29.4% in men, 8.3% in premenopausal and 25.5% in postmenopausal women. On bivariate analysis, the highest prevalence of MS occurred in the highest quartiles of serum ferritin, transferrin and TIBC, and in the lowest quartile of TSAT. After multivariate adjustment for age, body mass index, C-reactive protein, smoking, alcohol, liver markers and iron supplementation, men and postmenopausal women in the highest quartile of serum ferritin, transferrin and TIBC had a higher risk of presenting with MS: for ferritin, Odds ratio and [95% CI]=1.44 [1.07-1.94] for men and 1.47 [0.99-2.17] for postmenopausal women; for transferrin and TIBC, OR=1.43 [1.06-1.91] and 2.13 [1.44-3.15], Participants in the highest quartile of TSAT had a lower risk of MS: OR=0.77 [0.57-1.05] for men and 0.59 [0.39-0.90] for postmenopausal women. No association was found between iron and MS and between markers of iron metabolism and MS in premenopausal women. Conclusion: Ferritin, transferrin, TIBC are positively and TSAT is negatively associated with MS in men and postmenopausal women.

Metabolic syndrome (MetS) is a disease composed of different risk factors such as obesity, type 2 diabetes or dyslipidemia. The prevalence of this syndrome is increasing worldwide in parallel with the rise in obesity. Nonalcoholic fatty liver disease (NAFLD) is now the most frequent chronic liver disease in western countries, affecting more than 30% of the general population. NAFLD encompasses a spectrum of liver manifestations ranging from simple steatosis to nonalcoholic steatohepatitis (NASH), fibrosis and cirrhosis, which may ultimately progress to hepatocellular carcinoma. There is accumulating evidence supporting an association between NAFLD and MetS. Indeed, NAFLD is recognized as the liver manifestation of MetS. Insulin resistance is increasingly recognized as a key factor linking MetS and NAFLD. Insulin resistance is associated with excessive fat accumulation in ectopic tissues, such as the liver, and increased circulating free fatty acids, which can further promote inflammation and endoplasmic reticulum stress. This in turn aggravates and maintains the insulin resistant state, constituting a vicious cycle. Importantly, evidence shows that most of the patients developing NAFLD present at least one of the MetS traits. This review will define MetS and NAFLD, provide an overview of the common pathophysiological mechanisms linking MetS and NAFLD, and give a perspective regarding treatment of these ever growing metabolic diseases.

Proteases are important for regulating multiple tumorigenic processes, including angiogenesis, tumor growth, and invasion. Elevated protease expression is associated with poor patient prognosis across numerous tumor types. Several multigene protease families have been implicated in cancer, including cysteine cathepsins. However, whether individual family members have unique roles or are functionally redundant remains poorly understood. Here we demonstrate stage-dependent effects of simultaneously deleting cathepsin B (CtsB) and CtsS in a murine pancreatic neuroendocrine tumor model. Early in tumorigenesis, the double knockout results in an additive reduction in angiogenic switching, whereas at late stages, several tumorigenic phenotypes are unexpectedly restored to wild-type levels. We identified CtsZ, which is predominantly supplied by tumor-associated macrophages, as the compensatory protease that regulates the acquired tumor-promoting functions of lesions deficient in both CtsB and CtsS. Thus, deletion of multiple cathepsins can lead to stage-dependent, compensatory mechanisms in the tumor microenvironment, which has potential implications for the clinical consideration of selective versus pan-family cathepsin inhibitors in cancer.

On the basis of a large populationbased sample who underwent full polysomnography at home (HypnoLaus cohort), we recently reported that 49·7% of men and 23·4% of women aged 40 years or older had an apnoea-hypopnoea index of 15 events per h or more1 according to the American Academy of Sleep Medicine (AASM) 2013 scoring criteria. When excessive daytime sleepiness (ie, Epworth score >10 [maximum score 24]) was included in the definition with an apnoea-hypopnoea index of 5 events per h or more, the prevalence was 12·5% in men and 5·9% in women. This high prevalence of sleep disordered breathing reinforced the idea that the treatment decision should not only be based the apnoeahypopnoea index, but should also take into account associated symptoms and cardiovascular and metabolic comorbidities. After this Article was published, several readers contacted us to ask for the prevalence of sleep apnoea syndrome in our sample according to the International Classification of Sleep Disorders (ICSD-3) criteria. These criteria include either the presence of an apnoea-hypopnoea index of 5 events per h or more associated with obstructive sleep apnoearelated symptoms or cardiovascular and metabolic comorbidities, or an apnoea-hypopnoea index of 15 events per h or more (figure).

In a cohort study of 182 consecutive patients with active endogenous Cushing's syndrome, the only predictor of fracture occurrence after adjustment for age, gender bone mineral density (BMD) and trabecular bone score (TBS) was 24-h urinary free cortisol (24hUFC) levels with a threshold of 1472 nmol/24 h (odds ratio, 3.00 (95 % confidence interval (CI), 1.52-5.92); p = 0.002). INTRODUCTION: The aim was to estimate the risk factors for fracture in subjects with endogenous Cushing's syndrome (CS) and to evaluate the value of the TBS in these patients. METHODS: All enrolled patients with CS (n = 182) were interviewed in relation to low-traumatic fractures and underwent lateral X-ray imaging from T4 to L5. BMD measurements were performed using a DXA Prodigy device (GEHC Lunar, Madison, Wisconsin, USA). The TBS was derived retrospectively from existing BMD scans, blinded to clinical outcome, using TBS iNsight software v2.1 (Medimaps, Merignac, France). Urinary free cortisol (24hUFC) was measured by immunochemiluminescence assay (reference range, 60-413 nmol/24 h). RESULTS: Among enrolled patients with CS (149 females; 33 males; mean age, 37.8 years (95 % confidence interval, 34.2-39.1); 24hUFC, 2370 nmol/24 h (2087-2632), fractures were confirmed in 81 (44.5 %) patients, with 70 suffering from vertebral fractures, which were multiple in 53 cases; 24 patients reported non-vertebral fractures. The mean spine TBS was 1.207 (1.187-1.228), and TBS Z-score was -1.86 (-2.07 to -1.65); area under the curve (AUC) was used to predict fracture (mean spine TBS) = 0.548 (95 % CI, 0.454-0.641)). In the final regression model, the only predictor of fracture occurrence was 24hUFC levels (p = 0.001), with an increase of 1.041 (95 % CI, 1.019-1.063), calculated for every 100 nmol/24-h cortisol elevation (AUC (24hUFC) = 0.705 (95 % CI, 0.629-0.782)). CONCLUSIONS: Young patients with CS have a low TBS. However, the only predictor of low traumatic fracture is the severity of the disease itself, indicated by high 24hUFC levels.

Perinatal asphyxia induces neuronal cell death and brain injury, and is often associated with irreversible neurological deficits in children. There is an urgent need to elucidate the neuronal death mechanisms occurring after neonatal hypoxia-ischemia (HI). We here investigated the selective neuronal deletion of the Atg7 (autophagy related 7) gene on neuronal cell death and brain injury in a mouse model of severe neonatal hypoxia-ischemia. Neuronal deletion of Atg7 prevented HI-induced autophagy, resulted in 42% decrease of tissue loss compared to wild-type mice after the insult, and reduced cell death in multiple brain regions, including apoptosis, as shown by decreased caspase-dependent and -independent cell death. Moreover, we investigated the lentiform nucleus of human newborns who died after severe perinatal asphyxia and found increased neuronal autophagy after severe hypoxic-ischemic encephalopathy compared to control uninjured brains, as indicated by the numbers of MAP1LC3B/LC3B (microtubule-associated protein 1 light chain 3)-, LAMP1 (lysosomal-associated membrane protein 1)-, and CTSD (cathepsin D)-positive cells. These findings reveal that selective neuronal deletion of Atg7 is strongly protective against neuronal death and overall brain injury occurring after HI and suggest that inhibition of HI-enhanced autophagy should be considered as a potential therapeutic target for the treatment of human newborns developing severe hypoxic-ischemic encephalopathy.
