Artemether-lumefantrine (AL) is the first-line treatment for uncomplicated malaria in the second and third trimesters of pregnancy. Its efficacy during pregnancy has recently been challenged due to altered pharmacokinetic (PK) properties in this vulnerable group. The aim of this study was to determine the PK profile of AL in pregnant and nonpregnant women and assess their therapeutic outcome. Thirty-three pregnant women and 22 nonpregnant women with malaria were treated with AL (80/480 mg) twice daily for 3 days. All patients provided five venous plasma samples for drug quantification at random times over 7 days. Inter- and intraindividual variability was assessed, and the effects of covariates were quantified using a nonlinear mixed-effects modeling approach (NONMEM). A one-compartment model with first-order absorption and elimination with linear metabolism from drug to metabolite fitted the data best for both arthemether (AM) and lumefantrine (LF) and their metabolites. Pregnancy status and diarrhea showed a significant influence on LF PK. The relative bioavailability of lumefantrine and its metabolism rate into desmethyl-lumefantrine were, respectively, 34% lower and 78% higher in pregnant women than in nonpregnant patients. The overall PCR-uncorrected treatment failure rates were 18% in pregnant women and 5% in nonpregnant women (odds ratio [OR] = 4.04; P value of 0.22). A high median day 7 lumefantrine concentration was significantly associated with adequate clinical and parasitological response (P = 0.03). The observed reduction in the relative bioavailability of lumefantrine in pregnant women may explain the higher treatment failure in this group, mostly due to lower posttreatment prophylaxis. Hence, a modified treatment regimen of malaria in pregnancy should be considered.

The aim of the present article was to present a specific activity approach called the 'life course of a practice' that could be used for the analysis of the long-term dynamic of learning within diverse social practices. The cases of unethical actions (doping) within a population of high-level athletes were examined, considering they could be fruitful exam- 10 ples for this type of analysis. After describing the theoretical construct 'life course of a practice', the places of these unethical actions in the sporting life courses and elements that characterize their emergence in the long-term dynamics are described. Last, targeted prevention measures of unethical actions over a lifetime are presented.

L'échec des tentatives de juguler les émissions globales de gaz à effet de serre a mené au renforcement d'un discours sur la nécessité de l'adaptation au changement climatique. Il est de plus souvent considéré qu'une transformation de l'action publique serait nécessaire afin de réduire les impacts de grande magnitude qui découlent du changement climatique. Depuis les années 2000, on constate ainsi un développement des activités étatiques dans le domaine de l'adaptation. Cette évolution suscite une question centrale: quels sont les effets réels que provoque l'intégration de l'adaptation au changement climatique dans la conduite des politiques publiques et comment expliquer les éventuelles limites à ces processus? La littérature spécialisée sur l'adaptation fait preuve d'un certain réductionnisme à l'égard de la complexité de l'action publique en considérant que lorsque la capacité d'agir contre le problème de la vulnérabilité au changement climatique existe, toute forme d'inaction publique serait un déficit de mise en oeuvre provoqué par des barrières à l'action qu'il faudrait lever. A contrario, cette thèse considère l'adaptation comme une norme de politique publique dont l'intégration est susceptible de provoquer des changements institutionnels dans la forme et le contenu des politiques publiques, et des effets concrets sur les acteurs vulnérables au changement climatique. Afin de mesurer les effets de l'intégration politique de l'adaptation, un concept est formulé sur la base d'un idéal-type du régime institutionnel adaptatif, soit une construction théorique des régulations qu'impliquerait une gestion cohérente du système socioécologique complexe dans lequel s'insère le système climatique. L'auteur propose un cadre d'analyse empirique des processus publics d'adaptation qu'il applique ensuite à deux États: l'Inde (politique agricole) et la Suisse (politique du tourisme hivernal), avec pour objectif de repérer des mécanismes explicatifs communs aux effets de l'intégration dans ces deux contextes extrêmement dissemblables. L'étude débouche sur six résultats qui constituent une contribution appréciable à la littérature. L'auteur montre: premièrement, que l'intégration politique de l'adaptation conduit à une production d'actes symboliques, qui s'explique notamment par le manque de pression publique à agir sur le problème de la vulnérabilité au changement climatique; deuxièmement, que le processus d'intégration agit sur la réalité sociale par le biais d'un mécanisme d'attribution de ressources publiques à certains acteurs du champ politique; troisièmement, que les groupes sociaux qui bénéficient de ces ressources publiques ne sont pas nécessairement les plus vulnérables, mais ceux détenant les ressources d'action nécessaires à l'activation de l'intervention publique; quatrièmement, que l'absence de clefs de répartition des coûts de l'adaptation, de procédures de monitoring et d'instruments d'action contraignants limitent les effets des programmes publics d'adaptation; cinquièmement, que la faible articulation institutionnelle entre objectif d'adaptation et objectifs de protection de l'environnement accentue le risque de processus d'adaptation produisant des externalités environnementales; sixièmement, que les transformations dont la littérature souligne la nécessité ne sont pas nécessairement faisables ni désirables. Sur la base de cette analyse, cette thèse s'écarte de la littérature dominante en concluant que les processus publics d'adaptation ne souffrent pas d'un déficit de mise en oeuvre, mais d'un déficit de légitimité lié à leurs effets pernicieux. L'auteur formule alors trois recommandations pour améliorer la légitimité de ces politiques et trois questions ouvertes sur le futur politique de l'adaptation.

To assess the preferred methods to quit smoking among current smokers. Cross-sectional, population-based study conducted in Lausanne between 2003 and 2006 including 988 current smokers. Preference was assessed by questionnaire. Evidence-based (EB) methods were nicotine replacement, bupropion, physician or group consultations; non-EB-based methods were acupuncture, hypnosis and autogenic training. EB methods were frequently (physician consultation: 48%, 95% confidence interval (45-51); nicotine replacement therapy: 35% (32-38)) or rarely (bupropion and group consultations: 13% (11-15)) preferred by the participants. Non-EB methods were preferred by a third (acupuncture: 33% (30-36)), a quarter (hypnosis: 26% (23-29)) or a seventh (autogenic training: 13% (11-15)) of responders. On multivariate analysis, women preferred both EB and non-EB methods more frequently than men (odds ratio and 95% confidence interval: 1.46 (1.10-1.93) and 2.26 (1.72-2.96) for any EB and non-EB method, respectively). Preference for non-EB methods was higher among highly educated participants, while no such relationship was found for EB methods. Many smokers are unaware of the full variety of methods to quit smoking. Better information regarding these methods is necessary.

OBJECTIVE: In general population survey instruments that measure volume of drinking, additional questions and shorter reference periods yield higher volumes. Comparison studies have focused on volume but not on associations between volume and consequences. METHOD: From a cohort study on substance use risk factors (Cohort Study on Substance Use Risk Factors [C-SURF]), baseline data were analyzed for 5,074 young (approximately 20-year-old) men who were drinkers in the past 12 months. Volume of drinking was measured by a generic quantity-frequency (QF) instrument, an extended QF (separately for weekends and weekdays) instrument with 12-months recall, and a retrospective past-week diary. Associations of consequences with and without attribution of alcohol as a cause, Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition (DSM-IV), criteria for dependence, and DSM-5 alcohol use disorder in the past 12 months were analyzed. RESULTS: The generic QF measure resulted in lower volume compared with either the extended QF measure (more questions) or the retrospective diary (the most questions and the shortest recall period). For outcomes, however, the extended QF assessment performed the best and the diary the worst. CONCLUSIONS: Higher volume yields are not always better regarding associations with outcomes. The extended QF instrument better captures the variability of drinking. The retrospective diary performs poorly for associations because of the mismatch with the recall period for past-12-months consequences and the potential for misclassification of past-week abstainers and heavy drinkers because of an uncommon past week. Diaries are not recommended for research investigating individual associations between exposure and outcomes in young populations if consequences are measured with a sufficiently long interval to capture rare consequences. Protection from reactivation of persistent herpes virus infection is mediated by Ag-specific CD8 T cell responses, which are highly regulated by still poorly understood mechanisms. In this study, we analyzed differentiation and clonotypic dynamics of EBV- and CMV-specific T cells from healthy adults. Although these T lymphocytes included all subsets, from early-differentiated (EM/CD28(pos)) to late-differentiated (EMRA/CD28(neg)) stages, they varied in the sizes/proportions of these subsets. In-depth clonal composition analyses revealed TCR repertoires, which were highly restricted for CMV- and relatively diverse for EBV-specific cells. Virtually all virus-specific clonotypes identified in the EMRA/CD28(neg) subset were also found within the pool of less differentiated "memory" cells. However, striking differences in the patterns of dominance were observed among these subsets, because some clonotypes were selected with differentiation while others were not. Late-differentiated CMV-specific clonotypes were mostly characterized by TCR with lower dependency on CD8 coreceptor interaction. Yet all clonotypes displayed similar functional avidities, suggesting a compensatory role of CD8 in the clonotypes of lower TCR avidity. Importantly, clonotype selection and composition of each virus-specific subset upon differentiation was highly preserved over time, with the presence of the same dominant clonotypes at specific differentiation stages within a period of 4 years. Remarkably, clonotypic distribution was stable not only in late-differentiated but also in less-differentiated T cell subsets. Thus, T cell clonotypes segregate with differentiation, but the clonal composition once established is kept constant for at least several years. These findings reveal novel features of the highly sophisticated control of steady state protective T cell activity in healthy adults.

Inbreeding depression is one of the main forces opposing the evolution of self-fertilization. Of central importance is the hypothesis that inbreeding depression and selfing coevolve antagonistically, generating either low selfing rate and high inbreeding depression or vice versa. However, there is limited evidence for this coevolution within species. We investigated this topic in the hermaphroditic snail Physa acuta. In this species, isolated individuals delay the onset of egg laying compared to individuals having access to mates. Longer delays (''waiting times'') indicate more intense selfing avoidance. We measured inbreeding depression and waiting time in a large quantitative-genetic experiment (281 outbred families derived from 26 natural populations). We observed large genetic variance for both traits and a strong positive genetic covariance between them, most of which resided within rather than among populations. It means that, within populations, individuals with higher mutation load avoided selfing more strongly on average. This genetic covariance may result from pleiotropy and/or linkage disequilibrium. Whatever its genetic architecture, the fact it emerges specifically when individuals are deprived of mates suggests it is not fortuitous and rather reflects the action of natural selection. We conclude that a diversity of mating strategies can arise within populations subjected to variation in inbreeding depression.
