A continuum of carbon, from atmospheric CO2 to secondary calcium carbonate, has been studied in a soil associ- ated with scree slope deposits in the Jura Mountains of Switzerland. This approach is based on former studies conducted in other environments. This C continuum includes atmospheric CO2, soil organic matter (SOM), soil CO2, dissolved inorganic carbon (DIC) in soil solutions, and secondary pedogenic carbonate. Soil parameters (pCO2, temperature, pH, Cmin and Corg contents), soil solution chemistry, and isotopic compositions of soil CO2, DIC, carbonate and soil organic matter (δ13CCO2, δ13CDIC, δ13Ccar and δ13CSOM values) have been monitored at different depths (from 20 to 140 cm) over one year. Results demonstrated that the carbon source in secondary carbonate (mainly needle fiber calcite) is related to the dissolved inorganic carbon, which is strongly dependent on soil respiration. The heterotrophic respiration, rather than the limestone parent material, seems to control the pedogenic carbon cycle. The correlation of δ13Corg values with Rock-Eval HI and OI indices demonstrates that, in a soil associated to scree slope deposits, the main process responsible for 13C-enrichment in SOM is related to bac- terial oxidative decarboxylation. Finally, precipitation of secondary calcium carbonate is enhanced by changes in soil pCO2 associated to the convective movement of air masses induced by temperature gradients (heat pump effect) in the highly porous scree slope deposits. The exportation of soil C-leachates from systems such as the one studied in this paper could partially explain the "gap in the European carbon budget" reported by recent studies.

Plasma-cell neoplasms are classically categorized into four groups as: multiple myeloma (MM), plasma-cell leukemias, solitary plasmacytomas (SP) of the bone (SPB), and extramedullary plasmacytomas (EMP). These tumors may be described as localized or diffuse in presentation. Localized plasma-cell neoplasms are rare, and include SP of the skeletal system, accounting for 2-5% of all plasma-cell neoplasms, and EMP of soft tissue, accounting for approximately 3% of all such neoplasms. SP is defined as a solitary mass of neoplastic plasma cells either in the bone marrow or in various soft tissue sites. There appears to be a continuum in which SP often progresses to MM. The main treatment modality for SP is radiation therapy (RT). However, there are no conclusive data in the literature on the optimal AT dose for SP. This review describes the interrelationship of plasma-cell neoplasms, and attempts to determine the minimal RT dose required to obtain local control.

Résumé: Relevant du domaine des représentations, cette thèse porte sur la reconstitution du processus relatif à l'invention des notions liées au «coeur du continent » asiatique, distinguant deux phases: avant et après l'apparition de la dénomination Asie centrale. Partant de l'idée que dans le processus de création de cet objet d'investigation deux actes restent primordiaux - le découpage d'un continuum géographique et culturel et la nomination des parties extraites cette étude cherche à reconstituer selon quels critères et en fonction de quelles justifications et de quels arguments, il a été possible de parvenir, via des mots, à créer les notions centre-asiatiques, afin de faire valoir des «choses» en les représentant verbalement ou graphiquement comme réelles. Cette optique invite, dans une perspective d'histoire des sciences, à travers des images changeantes, à interroger l'Asie centrale en tant qu'objet d'étude, tel qu'il a été historiquement inventé, construit et représenté par les voyageurs et les scientifiques, sans négliger l'analyse du processus politique qui a réussi à insérer des lignes-frontières dans cet espace. Cette investigation aidera à mieux comprendre le sens épistémologique des expressions relatives à l'Asie centrale, souvent imprégnées d'une vision eurocentriste, lors de la progression des connaissances sur la région, en faisant revivre des courants de pensées multiples afin de pouvoir saisir la logique du développement des idées et comprendre quel rôle ces limites centre-asiatiques inventées, qui ne sont que la codification temporelle de certaines données, jouent dans les investigations scientifiques. Abstract: This dissertation proposes a reconstitution of the process of construction of the concepts relating to the Asia's «Heart of the Continent». It is divided into two parts: I: before the invention of the Central. Asia concept, from Antiquity until nineteenth century; II: the notion after this event. PURPOSE OF REVIEW: This article presents an overview of the common and various kinds of pupillary disorders that can be encountered in an outpatient setting. RECENT FINDINGS: The dorsal midbrain is a site where lesions may produce either an afferent or an efferent pupillary defect. The classic pupillary syndrome secondary to a dorsal midbrain lesion is bilateral light-near dissociation. Another recognized deficit is bilateral mydriasis. Recent reports have documented unilateral mydriasis, unilateral light-near dissociation, and a relative afferent pupillary defect without visual loss in association with lesions of the dorsal midbrain. These are rare syndromes. SUMMARY: Careful history and examination can often identify and localize the pupillary disorder, as well as guide appropriate evaluation.

Despite the long tradition for asking about the negative social and health consequences of alcohol consumption in surveys, little is known about the dimensionality of these consequences. Analysing cross-sectional and longitudinal data from the Nordic Taxation Study collected for Sweden, Finland, and Denmark in two waves in 2003 and 2004 by means of an explorative principal component analysis for categorical data (CATPCA), it is tested whether consequences have a single underlying dimension across cultures. It further tests the reliability, replicability, concurrent and predictive validity of the consequence scales. A one-dimensional solution was commonly preferable. Whereas the two-dimensional solution was unable to distinguish clearly between different concepts of consequences, the one-dimensional solution resulted in interpretable, generally very stable scales within countries across different samples and time.

Objective: Although initially developed to assess spiritual well-being,the FACIT-Sp is increasingly used to assess the other end of the spectrum,i.e. spiritual distress. This study intends to investigate whether theFACIT-Sp could really contribute to this aim in older patients. Method:Patients (N=135, 81.47.1 years, 68.3% women) aged 65 years and over,with MMSE score>19, admitted consecutively in post-acute rehabilitationwere enrolled. The FACIT-Sp (12 items, score 0 to 48, high spiritualwell-being defined as score_36) was administered and commentswere systematically retrieved. Results: Overall, 32(23.7%) patients hadhigh spiritual well-being. FACIT-Sp internal consistency was good(Cronbach's 0.85) and a confirmatory factorial analysis was consistentwith Meaning and Faith proposed subscales. Qualitative analysisshowed that negative answers (score=0) to "My illness has strengthenedmy faith or spiritual beliefs" (N=76/135) could equally reflect theabsence of impact (49/76, 64.5%) or a negative impact (religious struggle,27/76, 35.5%) of illness on faith. However, former patients had significantlyhigher FACIT-Sp scores than the latter (30.35.6 vs 20.97.9,P<.001). Similarly, among patients (N=73/135) with negative answers(score<3) to "I feel a sense of purpose in my life" those mentioning their"old age" to explain their answer (N=34/73, 46.6%) had higher FACITSpscores than those who did not (26.47.7 vs 22.58.1,P=.02). Conclusion:The FACIT-Sp identifies older people with high spiritual wellbeingbut could underestimate well-being in some older patients. Lowscores on some items could have very different meanings and interpretationof FACIT-Sp global scores below the usual cut-off should becautious.

An ab initio structure prediction approach adapted to the peptide-major histocompatibility complex (MHC) class I system is presented. Based on structure comparisons of a large set of peptide-MHC class I complexes, a molecular dynamics protocol is proposed using simulated annealing (SA) cycles to sample the conformational space of the peptide in its fixed MHC environment. A set of 14 peptide-human leukocyte antigen (HLA) A0201 and 27 peptide-non-HLA A0201 complexes for which X-ray structures are available is used to test the accuracy of the prediction method. For each complex, 1000 peptide conformers are obtained from the SA sampling. A graph theory clustering algorithm based on heavy atom root-mean-square deviation (RMSD) values is applied to the sampled conformers. The clusters are ranked using cluster size, mean effective or conformational free energies, with solvation free energies computed using Generalized Born MV 2 (GB-MV2) and Poisson-Boltzmann (PB) continuum models. The final conformation is chosen as the center of the best-ranked cluster. With conformational free energies, the overall prediction success is 83% using a 1.00 Angstroms crystal RMSD criterion for main-chain atoms, and 76% using a 1.50 Angstroms RMSD criterion for heavy atoms. The prediction success is even higher for the set of 14 peptide-HLA A0201 complexes: 100% of the peptides have main-chain RMSD values &lt; or =1.00 Angstroms and 93% of the peptides have heavy atom RMSD values &lt; or =1.50 Angstroms. This structure prediction method can be applied to complexes of natural or modified antigenic peptides in their MHC environment with the aim to perform rational structure-based optimizations of tumor vaccines.

We present a novel steered molecular dynamics scheme to induce the dissociation of large protein-protein complexes. We apply this scheme to study the interaction of a T cell receptor (TCR) with a major histocompatibility complex (MHC) presenting a peptide (p). Two TCR-pMHC complexes are considered, which only differ by the mutation of a single amino acid on the peptide; one is a strong agonist that produces T cell activation in vivo, while the other is an antagonist. We investigate the interaction mechanism from a large number of unbinding trajectories by analyzing van der Waals and electrostatic interactions and by computing energy changes in proteins and solvent. In addition, dissociation potentials of mean force are calculated with the Jarzynski identity, using an averaging method developed for our steering scheme. We analyze the convergence of the Jarzynski exponential average, which is hampered by the large amount of dissipative work involved and the complexity of the system. The resulting dissociation free energies largely underestimate experimental values, but the simulations are able to clearly differentiate between wild-type and mutated TCR-pMHC and give insights into the dissociation mechanism.

Objectives:To investigate the associations between falls before hospital¦admission, falls during hospitalization, and length of stay in elderly¦people admitted to post-acute geriatric rehabilitation. Method: History¦of falling in the previous 12 months before admission was recorded¦among 249 older persons (mean age 82.3±7.4 years, 69.1% women)¦consecutively admitted to post-acute rehabilitation. Data on medical,¦functional and cognitive status were collected upon admission. Falls¦during hospitalization and length of stay were recorded at discharge.¦Results: Overall, 92 (40.4%) patients reported no fall in the 12 months¦before admission; 63(27.6%) reported 1 fall, and 73(32.0%) reported¦multiple falls. Previous falls occurrence (one or more falls) was significantly¦associated with in-stay falls (19.9% of previous fallers fell¦during the stay vs 7.6% in patients without history of falling, P=.01),¦and with a longer length of stay (22.4 ± 10.1 days vs 27.1 ± 14.3 days,¦P=.01). In multivariate robust regression controlling for gender, age,¦functional and cognitive status, history of falling remained significantly¦associated with longer rehabilitation stay (2.8 days more in single fallers,¦p=.05, and 3.3 days more in multiple fallers, p=.0.1, compared to¦non-fallers). Conclusion: History of falling in the 12 months prior to¦post acute geriatric rehabilitation is independently associated with a¦longer rehabilitation length of stay. Previous fallers have also an¦increased risk of falling during rehabilitation stay. This suggests that¦hospital fall prevention measures should particularly target these high¦riskpatients.

We study the interaction between nonprice public rationing and prices in the private market. Under a limited budget, the public supplier uses a rationing policy. A private firm may supply the good to those consumers who are rationed by the public system. Consumers have different amounts of wealth, and costs of providing the good to them vary. We consider two regimes. First, the public supplier observes consumers' wealth information; second, the public supplier observes both wealth and cost information. The public supplier chooses a rationing policy, and, simultaneously, the private firm, observing only cost but not wealth information, chooses a pricing policy. In the first regime, there is a continuum of equilibria. The Pareto dominant equilibrium is a means-test equilibrium: poor consumers are supplied while rich consumers are rationed. Prices in the private market increase with the budget. In the second regime, there is a unique equilibrium. This exhibits a cost-effectiveness rationing rule; consumers are supplied if and only if their costbenefit ratios are low. Prices in the private market do not change with the budget. Equilibrium consumer utility is higher in the cost-effectiveness equilibrium than the means-test equilibrium [Authors]

RESUMELes troubles des conduites alimentaires atypiques (TCAA) représentent une catégorie diagnostique émergente, relativement peu décrite. Ils regroupent des tableaux cliniques ne satisfaisant pas entièrement aux critères des troubles alimentaires typiques anorexie et boulimie. Désignés par le terme de EDNOS' dans le DSM-IV2 (American Psychiatrie Association, 1994), ils sont appelés troubles atypiques dans la CIM-103 (World Health Organization, 1993).Les TCAA représentent la majorité des demandes dans les consultations pour troubles alimentaires, concernent une grande proportion de la population non-consultante et constituent un enjeu de santé publique prioritaire.Cette étude se penche sur le parcours de 24 jeunes filles présentant de tels troubles, qui ont été suivies dans le cadre d'un groupe thérapeutique à l'UMSA (Unité Multidisciplinaire de Santé des Adolescents, CHUV, Lausanne). Elle a pour but de mieux connaître ces troubles alimentaires atypiques.Deux axes de questions de recherche organisent ce travail: le premier, orienté autour du trouble alimentaire atypique, de sa définition (avec les questions de classification diagnostique) et de son évolution (avec les questions de passages d'une catégorie à une autre), le second autour du groupe thérapeutique, de son utilité et de ses indications.1. Résultats cliniquesLes résultats de l'analyse thématique décrivent un trouble important, qui induit une souffrance plus intense que ne tendraient à le laisser penser la relative banalité de la présentation clinique en comparaison des troubles typiques ainsi que la catégorisation en tant que troubles résiduels. Des moyens compensatoires -qui visent au maintien d'un poids stable en dépit des crises alimentaires ainsi qu'à la perte de poids, soit par des méthodes non-purgatives (restrictions massives, diètes, jeûnes, pratique du sport à outrance), soit par des méthodes purgatives (vomissements auto-induits, usage de laxatifs, diurétiques) - sont présents chez 15 participantes, sous forme de vomissements chez 6 d'entre elles. Seize participantes ont présenté des troubles des menstruations. Des difficultés de la lignée anxio-dépressive sont relevées chez la moitié des participantes, alliées à un perfectionnisme important. L'estime de soi apparaît globalement basse, excessivement influencée par l'insatisfaction attachée au poids et aux formes corporelles. L'analyse fait clairement apparaître le lien complexe unissant les attitudes face à l'alimentation et le désir de maigrir aux crises de frénésie alimentaire. Les crises permettent aux patientes de moduler leurs émotions, tant .positives que négatives. Le vécu de la maladie et des préoccupations pour le corps et l'alimentation semble généralisable. et «transcatégorique», bien que les troubles alimentaires de l'ordre de l'anorexie atypique restrictive, du fait de l'absence de crises de frénésie, semblent appartenir à un registre différent des troubles avec perte de contrôle sur l'alimentation et crises, notamment en termes d'implications dans la vie quotidienne.Lorsque les participantes sont revues (au minimum un an après la sortie du groupe thérapeutique), 15 d'entre elles sont indemnes de tout symptôme, 5 présentent encore un TCAA type boulimie atypique, et 4 ont parfois des crises résiduelles. Des préoccupations pour l'alimentation et le poids subsistent. Le pronostic d'évolution n'apparaît lié ni à la durée de participation au groupe, ni uniquement à la catégorie diagnostique ou aux seuls éléments de co-morbidité, mais davantage à un investissement rapidement positif du groupe, ainsi qu'à la qualité des prises en charge parallèles en présence d'éléments de co-morbidité importants ou d'antécédents d'autres troubles alimentaires.Notre collectif atteste de passages d'une catégorie de trouble à une autre (anorexie puis boulimie, comme décrit dans la littérature) et de changements d'intensité (trouble typique puis atypique) au cours de la maladie, confirmant un continuum possible, tant entre les troubles typiques, qu'entre les troubles typiques et atypiques. Néanmoins, la multiplicité des parcours possibles dans la maladie, ainsi que l'existence de formes stables, sans passages d'une catégorie à une autre, incite à la prudence et ne permet pas de confirmer en tout point l'hypothèse de la nature dimensionnelle (même nature, intensité différente) des troubles alimentaires.Le trouble atypique représente parfois une étape dans l'évolution à partir d'un trouble typique, pouvant être envisagé comme moment sur le chemin de la guérison, ce qui pose la question des critères de guérison.2. Nature des troubles alimentaires atypiquesDu collectif émergent 4 catégories diagnostiques de troubles alimentaires atypiques: hyperphagie boulimique, boulimie atypique non-purgative, boulimie atypique purgative et anorexie atypique restrictive. Le tableau clinique.de l'hyperphagie boulimique comporte des crises de boulimie, mais peu de préoccupations concernant le poids et les formes corporelles en compá= raison des 3 autres catégories, et pas de moyens de compensation des crises, ce qui induit fréquemment un surpoids voire une obésité pour les patientes concernées. Les moyens de compensation des crises de boulimie demeurent non-purgatives (jeûne, sport) dans la boulimie atypique non-purgative, alors que vomissements et usage de laxatifs suite aux crises caractérisent la boulimie atypique purgative. Le tableau clinique de l'anorexie atypique restrictive ne présente ni crises de boulimie ni moyens de compensation purgatifs type vomissements. Ces catégories sont retrouvées dans la littérature.Nos résultats rejoignent la littérature actuelle qui envisage la nécessité d'une reclassification des troubles alimentaires en vue du DSM-V. Dans l'intervalle, adopter la classification de la CIM et parler d'anorexie atypique, de -type restrictif ou avec crises, de boulimie atypique, purgative ou non, et y ajouter l'hyperphagie boulimique, rendrait mieux compte de la réalité clinique des troubles alimentaires atypiques que ne le fait la classe des EDNOS du DSM, peu différenciée, mêlant des tableaux très divers et au final insatisfaisante.3. Utilité et indication du groupeLe groupe recèle un fort pouvoir thérapeutique pour les patientes qui s'y impliquent. L'étayage sur les autres émerge comme facteur thérapeutique principal, ainsi que le décrit la littérature. Cette prise en charge thérapeutique convient particulièrement à des patientes présentant un trouble alimentaire sur le versant boulimique, comprenant l'hyperphagie boulimique, la boulimie atypique, purgative ou non, ainsi que certaines boulimies de moyenne gravité. Les patientes du collectif qui présentent des difficultés de l'ordre de l'anorexie atypique restrictive bénéficient moins du groupe que les participantes qui expérimentent des pertes de contrôle sur la nourriture.4. ImplicationsL'intensité de la souffrance, la complexité des tableaux symptomatiques, la diversité des parcours et les multiples répercussions sur la vie sociale démontrent l'importance de repérer et prendre en charge ces troubles alimentaires qui demeurent souvent banalisés. Dans cette perspective, la formation au dépistage, à l'évaluation et à la prise en charge des divers intervenants confrontés à ces troubles représente un enjeu majeur. .Enfin, dénoncer les pressions socioculturelles à la minceur et le dictat des régimes, promouvoir une notion de bien-être et une bonne estime de soi qui ne soient pas liées à l'apparence contribuera à prévenir les troubles alimentaires atypiques.

BACKGROUND: Animal societies are diverse, ranging from small family-based groups to extraordinarily large social networks in which many unrelated individuals interact. At the extreme of this continuum, some ant species form unicolonial populations in which workers and queens can move among multiple interconnected nests without eliciting aggression. Although unicoloniality has been mostly studied in invasive ants, it also occurs in some native non-invasive species. Unicoloniality is commonly associated with very high queen number, which may result in levels of relatedness among nestmates being so low as to raise the question of the maintenance of altruism by kin selection in such systems. However, the actual relatedness among cooperating individuals critically depends on effective dispersal and the ensuing pattern of genetic structuring. In order to better understand the evolution of unicoloniality in native non-invasive ants, we investigated the fine-scale population genetic structure and gene flow in three unicolonial populations of the wood ant F. paralugubris. RESULTS: The analysis of geo-referenced microsatellite genotypes and mitochondrial haplotypes revealed the presence of cryptic clusters of genetically-differentiated nests in the three populations of F. paralugubris. Because of this spatial genetic heterogeneity, members of the same clusters were moderately but significantly related. The comparison of nuclear (microsatellite) and mitochondrial differentiation indicated that effective gene flow was male-biased in all populations. CONCLUSION: The three unicolonial populations exhibited male-biased and mostly local gene flow. The high number of queens per nest, exchanges among neighbouring nests and restricted long-distance gene flow resulted in large clusters of genetically similar nests. The positive relatedness among clustermates suggests that kin selection may still contribute to the maintenance of altruism in unicolonial populations if competition occurs among clusters.

The rebinding of NO to myoglobin after photolysis is studied using the 'reactive molecular dynamics' method. In this approach the energy of the system is evaluated on two potential energy surfaces that include the heme-ligand interactions which change between liganded and unliganded myoglobin. This makes it possible to take into account in a simple way, the high dimensionality of the transition seam connecting the reactant and product states. The dynamics of the dissociated NO molecules are examined, and the geometrical and energetic properties of the transition seam are studied. Analysis of the frequency of recrossing shows that the height of the effective rebinding barrier is dependent on the time after photodissociation. This effect is due mainly to protein relaxation and may contribute to the experimentally observed non-exponential rebinding rate of NO, as has been suggested previously.

The dynamic properties of helix 12 in the ligand binding domain of nuclear receptors are a major determinant of AF-2 domain activity. We investigated the molecular and structural basis of helix 12 mobility, as well as the involvement of individual residues with regard to peroxisome proliferator-activated receptor alpha (PPARalpha) constitutive and ligand-dependent transcriptional activity. Functional assays of the activity of PPARalpha helix 12 mutants were combined with free energy molecular dynamics simulations. The agreement between the results from these approaches allows us to make robust claims concerning the mechanisms that govern helix 12 functions. Our data support a model in which PPARalpha helix 12 transiently adopts a relatively stable active conformation even in the absence of a ligand. This conformation provides the interface for the recruitment of a coactivator and results in constitutive activity. The receptor agonists stabilize this conformation and increase PPARalpha transcription activation potential. Finally, we disclose important functions of residues in PPARalpha AF-2, which determine the positioning of helix 12 in the active conformation in the absence of a ligand. Substitution of these residues suppresses PPARalpha constitutive activity, without changing PPARalpha ligand-dependent activation potential.
