Resumo:
Le cas d'une épicondylite, atteinte musculosquelettique au coude, a été reconnu comme maladie professionnelle par le Tribunal fédéral et le Tribunal cantonal des Assurances, suite à une longue bataille juridique. Cette victoire, très encourageante pour les spécialistes de la santé au travail, a été possible grâce à une expertise qui a comparé deux situations professionnelles. Cela a permis la mise en évidence de facteurs de risque liés "de manière nettement prépondérante" aux contitions de travail.

Unspezifische muskuloskelettale Beschwerden wie chronische Rücken- oder Nackenbeschwerden werden als multifaktoriell bedingt verstanden, wobei die Arbeit respektive die Arbeitsbedingungen einen mehr oder weniger bestimmenden Faktor darstellen. Es ist daher wesentlich, speziell auch im Hinblick auf die Wiedereingliederung, dass sowohl die Arbeit wie auch die Arbeitsbedingungen evaluiert werden. [Auteure]

Les médecins de premiers recours (MPR) sont parmi les spécialistes les plus souvent confrontés aux problèmes de santé liés au travail. Les atteintes de l'appareil locomoteur (rachis ou extrémités) et les problèmes psychiques sont les principales pathologies liées au travail rencontrées par leur patient. Dans ce contexte, il est essentiel pour le MPR de pouvoir identifier le lien entre l'atteinte à la santé et le contexte professionnel afin de conduire une prise en charge adaptée. Cet article aborde, par deux vignettes cliniques, les aspects sémiologiques et thérapeutiques du syndrome d'épuisement professionnel (burnout) et des troubles musculosquelettiques du membre supérieur. Il propose des repères au MPR pour orienter sa pratique dans une approche multidimensionnelle tournée aussi bien vers le patient que sur son environnement professionnel.

Little is known about the health of ambulance personnel, especially in Switzerland. This lack of knowledge is particularly striking in the specific field of occupational health. This study aims to identify and better understand protective and risk factors affecting the health of ambulance personnel. Both mental and physical health are considered. The approach used comprised two steps. The first step began in July 2008 and consisted in a qualitative study of real work activities performed by ambulance crews involved in pre-hospital emergency interventions. Researchers shadowed ambulance personnel for the duration of their entire work shift, in average for one week. The paper-pen technique was used to note dialogues, interactions, postural aspects, etc. When the situation allowed it, interventions were filmed. Some selected video sequences were used as a support for selfconfrontation interviews. Observations were performed by three researchers and took place in eleven services, for a total of 416 hours of observations (including 72 interventions + waiting time). Analysis, conducted by a multidisciplinary team (an ergonomist, an occupational therapist and a health psychologist), focused on individual and collective strategies used by ambulance personnel to protect their health. The second step, which is currently ongoing, aims to assess global health of ambulance personnel. A questionnaire is used to gather information about musculoskeletal complaints (Nordic questionnaire), mental health (GHQ-12), stress (Effort-Reward imbalance questionnaire), strategies implemented to cope with stress (Brief COPE), and working conditions. Specific items on strategies were developed based on observational data. It will be sent to all ambulance personnel employed in the French-speaking part of Switzerland. Preliminary analyses show different types of strategies used by ambulance personnel to preserve their health. These strategies involve postural aspects (e.g. use doorframe as a support to ease delicate manipulations), work environment adaptations (e.g. move furniture to avoid awkward postures), coping strategies (e.g. humor), as well as organisational (e.g. formal and informal debriefing) and collective (e.g. cooperation) mechanisms. In-depth analysis is still ongoing. However, patient safety and comfort, work environment and available resources appear to influence the choice of strategies ambulance personnel use. As far as possible, the strategies identified will be transformed into educational materials for professional ambulance personnel.

En quoi et comment l'étude des stratégies représente-t-elle un intérêt pour la prévention des troubles musculosquelettiques (TMS) ? Qu'en est-il des retombées d'une telle étude pour l'entreprise et pour les travailleurs ? Cet article se propose d'évaluer les diverses stratégies utilisées par les travailleurs pour gérer leur douleur face aux TMS. Résultats d'une étude qui s'est déroulée au sein de deux usines de transformation du crabe.

On suppose que les troubles musculo-squelettiques non spécifiques tels que les douleurs chroniques au dos ou à la nuque ont des causes multifactorielles. Toutefois, le travail et les conditions de travail représentent un facteur plus ou moins déterminant. Extraintestinal manifestations (EIM) in inflammatory bowel disease (IBD) are frequent and may occur before or after IBD diagnosis. EIM may impact the quality of life for patients with IBD significantly requiring specific treatment depending on the affected organ(s). They most frequently affect joints, skin, or eyes, but can also less frequently involve other organs such as liver, lungs, or pancreas. Certain EIM, such as peripheral arthritis, oral aphthous ulcers, episcleritis, or erythema nodosum, are frequently associated with active intestinal inflammation and usually improve by treatment of the intestinal activity. Other EIM, such as uveitis or ankylosing spondylitis, usually occur independent of intestinal inflammatory activity. For other not so rare EIM, such as pyoderma gangrenosum and primary sclerosing cholangitis, the association with the activity of the underlying IBD is unclear. Successful therapy of EIM is essential for improving quality of life of patients with IBD. Besides other options, tumor necrosis factor antibody therapy is an important therapy for EIM in patients with IBD.

The 2013 survey addressed the following objectives: Primary objectives : a) Distribution of health behaviors related to NCDs, particularly tobacco use, alcohol drinking, and physical activity ; b) Distribution of the main modifiable risk factors of NCDs, particularly blood pressure, adiposity markers, diabetes and blood lipids ; c) Rates of awareness, treatment and control of hypertension, diabetes and dyslipidemia ; d) Comparison of findings in the survey 2013 with results in previous similar NCD surveys in 1989, 1994, 2004 ; e) Dietary patterns ; f) Knowledge, attitudes and practices related to NCDs and NCD risk factors. Secondary objectives : g) Assessment of indicators of quality of health (e.g. SF‐12) ; h) Assessment of psychological stress and relation with NCD ; i) Assessment of several indicators of frailty (e.g. handgrip strength test, chair strand test, functional limitations) ; j) Assessment of knowledge and level of agreement with current policies on tobacco control ; k) Use of public and private health care services, particularly for NCDs ; l) Exposure to advice on health behaviors given by health professionals at health care level ; m) Burden of chronic diseases not related to the main NCDs (e.g. musculoskeletal, mental health, etc) ; n) Screening of selected cancers ; o) Assessment of the kidney function ; p) Frequency of heart arrhythmias (one‐lead ECG) and heart murmurs (auscultation) ; q) Assessment of bone mineral density (ultrasound of calcaneus) ; r) Exposure of the population to mass media, particularly in relation to health programs, and use by the population of new communication technologies ; s) Assessment of a number of social variables and their association with the variables measured in the survey ; t) More generally, the survey provides broad information (medical, social, environment, etc) that can be useful for tailoring NCD prevention and control programs.

In 2008, several publications have highlighted the role of climate change and globalization on the epidemiology of infectious diseases. Studies have shown the extension towards Europe of diseases such as Crimea-Congo fever (Kosovo, Turkey and Bulgaria), leismaniosis (Cyprus) and chikungunya virus infection (Italy). The article also contains comments on Plasmodium knowlesi, a newly identified cause of severe malaria in humans, as well as an update on human transmission of the H5NI avian influenza virus. It also mentions new data on Bell's palsy as well as two vaccines (varicella-zoster and pneumococcus), and provides a list of recent guidelines for the treatment of common infectious diseases.
