Stereotactic ablative radiotherapy is a modern cancer treatment strategy able to deliver highly focused radiation in one or a few fractions with a radical intent in several clinical settings. Young radiation oncologists need a constant and tailored update in this context to improve patient care in daily clinical practice. A recent meeting of AIRO Giovani (AIRO - Young Members Working Group) was specifically addressed to this topic, presenting state-of-the-art knowledge, based on the latest evidence in this field. Highlights of the congress are summarized and presented in this report, including thorough contributions of the speakers dealing with the role of stereotactic ablative radiotherapy in both oncological and non-oncological diseases, divided according to anatomical and clinical scenarios: intra-cranial settings (brain malignant primary tumors, metastases, benign tumors and functional disorders) and extra-cranial indications (lung primary tumors and metastases, thoracic re-irradiation, liver, lymph node and bone metastases, prostate cancer). With literature data discussed during the congress as a background, stereotactic ablative radiotherapy has proved to be a consolidated treatment approach in specific oncological and non-oncological scenarios, as well as a promising option in other clinical settings, requiring a further prospective validation in the near future. We herein present an updated overview of stereotactic ablative radiotherapy use in the clinic.

The death-inducing receptor Fas is activated when cross-linked by the type II membrane protein Fas ligand (FasL). When human soluble FasL (sFasL, containing the extracellular portion) was expressed in human embryo kidney 293 cells, the three N-linked glycans of each FasL monomer were found to be essential for efficient secretion. Based on the structure of the closely related lymphotoxin alpha-tumor necrosis factor receptor I complex, a molecular model of the FasL homotrimer bound to three Fas molecules was generated using knowledge-based protein modeling methods. Point mutations of amino acid residues predicted to affect the receptor-ligand interaction were introduced at three sites. The F275L mutant, mimicking the loss of function murine gld mutation, exhibited a high propensity for aggregation and was unable to bind to Fas. Mutants P206R, P206D, and P206F displayed reduced cytotoxicity toward Fas-positive cells with a concomitant decrease in the binding affinity for the recombinant Fas-immunoglobulin Fc fusion proteins. Although the cytotoxic activity of mutant Y218D was unaltered, mutant Y218R was inactive, correlating with the prediction that Tyr-218 of FasL interacts with a cluster of three basic amino acid side chains of Fas. Interestingly, mutant Y218F could induce apoptosis in murine, but not human cells.

Les maladies rhumatismales sont fréquemment observées chez les personnes âgées et ont un impact considérable sur la qualité de vie des personnes en souffrant. Peu d'études suisses sur la situation épidémiologique et sur l'impact de ce type de maladies sur la consommation des services de santé existent. Cette thèse a pour but d'étudier les connaissances actuelles à partir de la littérature suisse et étrangère et d'établir un bilan en Suisse au moyen d'une enquête de population effectuée en 1997. Une revue systématique de littérature a été effectuée. En dépit d'une grande variété des approches méthodologiques rendant délicates les comparaisons géographiques et temporelles, la prévalence des maladies rhumatismales chez les personnes de 65 ans et plus semble être homogène entre pays et stable temporellement. particulièrement dès 1980. Celle-ci est élevée et augmente rapidement avec le très grand âge. De plus, les femmes sont plus fréquemment atteintes que les hommes. Notre revue montre que le recours aux soins ambulatoires (médecins, chiropraticiens, traitements ambulatoires des hôpitaux) lié aux maladies rhumatismales est important. L'impact de ces maladies sur les hospitalisations est, par contre, moins clairement établi. Les nouvelles générations de personnes en souffrant semblent avoir plus recours aux services de santé que les précédentes. Ces maladies sont aussi à l'origine d'une forte consommation d'anti-inflammatoires non-stéroïdiens avec comme conséquence une multitude de complications. En dépit de son efficacité et de son utilité, le recours à l'arthroplastie est sous- utilisé. Notre analyse se base sur une enquête réalisée auprès d'un échantillon représentatif des individus âgés de 15 ans et plus résidant de manière permanente en Suisse en 1997 : la prévalence des maladies rhumatismales en Suisse s'élève à 41 % chez les personnes âgées de 65 ans et plus, dont 48 % chez les femmes et 31 % chez les hommes. Ces prévalences sont inférieures à celles relevées dans la littérature probablement en raison de notre définition relativement restrictive des maladies rhumatismales. Ces dernières augmentent de 50 % le nombre attendu de consultations chez un médecin ou un chiropraticien et de 30 % le nombre attendu d'hospitalisations. Les personnes souffrantes ont. en outre, une probabilité de recours aux services de Soins à domicile 1,7 fois plus élevé que les autres. Aucun impact sur le nombre de traitements ambulatoires en milieu hospitalier n'a été trouvé. Nos résultats sont comparables à ceux relevés dans la littérature internationale et suisse, sauf pour les traitements ambulatoires des hôpitaux. En 1990, sur les 983'400 personnes de 65 ans et plus (recensement fédéral de la population de 1990), 403'200 personnes souffraient de maladies rhumatismales. Quelque 5'334'900 consultations chez un médecin ou un chiropraticien, 4'959'300 consultations chez un médecin et 216'800 hospitalisations étaient imputables aux personnes de 65 ans et plus toutes causes de consultations confondues, dont 1'008'000 consultations chez un médecin/chiropraticien, 927'300 chez un médecin et 98'500 hospitalisations imputables aux maladies rhumatismales. Selon ie scénario (( tendance )) des projections démographiques publiées par l'Office Fédéral de la Statistique. d'ici 2040, le nombre de personnes souffrant de maladies rhumatismales en Suisse risque d'augmenter de 80 % (en supposant que la prévalence reste stable), affectant 726'500 sur 1'772'000 personnes de 65 ans et plus. Cette augmentation est la conséquence de l'accroissement prévu de la population de 65 ans et plus dans la population générale. Le nombre global de consultatiordhospitalisations risque d'augmenter dans les mêmes proportions si le recours aux services de santé reste stable. En effet. en 2040, quelque 9'613'100 consultations chez un médecinichiropraticien, 8'936'200 consultations chez un médecin et 390'700 hospitalisations pourraient être imputables aux personnes de 65 ans et plus. dont 1'8 16'300 consultations chez un médecin/chiropraticien, 1'67 1'000 consultations chez un médecin et 1 90'600 hospitalisations en raison de maladies rhumatismales. Une légère diminution du nombre de personnes atteintes de maladies rhumatismales. ainsi que du recours aux services de santé engendré par ces maladies. est attendue dès 3040. Le nombre de personnes souffrant de maladies rhumatismales et le nombre de consultations/ hospitalisations associées risquant d'augmenter de façon considérable, il est nécessaire de freiner cette progression. Des mesures préventives primaires, secondaires ou tertiaires peuvent diminuer la prévalence des maladies rhumatismales et l'impact de celles-ci sur la consommation des services de santé.<br/><br/>Rheumatic diseases are frequently observed in elderly people and have an important impact on tlieir life qurlity. This thesis aims at studying the current knowledge based on Swiss and international literature and at establishing the situation in Switzerland from a population survey conducted in 1997. A systeinatic literature review lias been carried out. Despite a large range of methods making a comparisoii diffcult, the prevalence of rheumatic diseases seems to be homogeneous in different countries and stable. especially since 1980. It is high and increases rapidly with age. Furthermore, \niorneil suffer more frequently thaii men. Our review shows that the use of ambulatory care linked to rheumatic diseases is important. On the contrary, the impact of such diseases on hospitalization is less clearly established. New generations seem to consult more. Rheumatic diseases are also at the origin of a strong consumptioii of non-steroidal anti- inflammatory drugs \vitIl potential severe consequences. Despite its effectiveness and efficiency, arthroplasty is underused. Our analysis is based 011 a survey of Swiss permanent residents aged 15 or more in 1997. Based on Our analysis, the prevalence of rheumatic diseases in Switzerland is 41 % for elderly people (48 96 for women and 31 % for men). Theses prevalences are smaller than those found in the literature because of our relatively strict definition of rheumatic diseases. The latter diseases increase of about 50 o/o the expected number of consultations (chiropractor included or not) and of about 30 960 the expected number of hospitalizations. The affected persons have a probability of home care use 1.7 times higlier than the others. No impact on the number of outpatient care provided by hospitals has been found. Our results are comparable to those found in the international and Swiss literature, except for hospital outpatient care. According to the "tendance" scenario of demographic projections published by the Swiss Federal Office of Statistics, until 2040 the number of persons suffering from rheumatic diseases will increase of 80 % if the prevalence stays stable, affecting 736,500 of 1,772,000 perçons of 65 and older. This increase is due to the increase of the percentage of persons 65 and older in the population. The global number of consultationshospitalizations will increase similarly if the use of health services stays stable. In 2040, 9,613,l 00 consultations by a physiciaidchiropractor, 8,936,200 Consultations by a physician and 390,700 hospitalizations could be attributed to the persons aged 65 and older, of which 1,816,300 consultations by a physician, 1,671,000 consultations by a physician/chiropractor and 109,600 hospitalizations will be due to the rheumatic diseases. However a small decrease of the number of affected perçons and of the subsequent use of health services is expected after 2040. The number of affected elderly people and the volume of conçultations/hospitalizations are expected to increase and it ir necessx-y to slow down this progression. Preventive interventions, primary, secondary or tertiary, can decrease the prevalence of rheumatic diseases and the impaci on the consumption of health services.

Decision situations are often characterized by uncertainty: we do not know the values of the different options on all attributes and have to rely on information stored in our memory to decide. Several strategies have been proposed to describe how people make inferences based on knowledge used as cues. The present research shows how declarative memory of ACT-R models could be populated based on internet statistics. This will allow to simulate the performance of decision strategies operating on declarative knowledge based on occurrences and co-occurrences of objects and cues in the environment.

Abstract Since its creation, the Internet has permeated our daily life. The web is omnipresent for communication, research and organization. This exploitation has resulted in the rapid development of the Internet. Nowadays, the Internet is the biggest container of resources. Information databases such as Wikipedia, Dmoz and the open data available on the net are a great informational potentiality for mankind. The easy and free web access is one of the major feature characterizing the Internet culture. Ten years earlier, the web was completely dominated by English. Today, the web community is no longer only English speaking but it is becoming a genuinely multilingual community. The availability of content is intertwined with the availability of logical organizations (ontologies) for which multilinguality plays a fundamental role. In this work we introduce a very high-level logical organization fully based on semiotic assumptions. We thus present the theoretical foundations as well as the ontology itself, named Linguistic Meta-Model. The most important feature of Linguistic Meta-Model is its ability to support the representation of different knowledge sources developed according to different underlying semiotic theories. This is possible because mast knowledge representation schemata, either formal or informal, can be put into the context of the so-called semiotic triangle. In order to show the main characteristics of Linguistic Meta-Model from a practical paint of view, we developed VIKI (Virtual Intelligence for Knowledge Induction). VIKI is a work-in-progress system aiming at exploiting the Linguistic Meta-Model structure for knowledge expansion. It is a modular system in which each module accomplishes a natural language processing task, from terminology extraction to knowledge retrieval. VIKI is a supporting system to Linguistic Meta-Model and its main task is to give some empirical evidence regarding the use of Linguistic Meta-Model without claiming to be thorough.

Gestures are the first forms of conventional communication that young children develop in order to intentionally convey a specific message. However, at first, infants rarely communicate successfully with their gestures, prompting caregivers to interpret them. Although the role of caregivers in early communication development has been examined, little is known about how caregivers attribute a specific communicative function to infants' gestures. In this study, we argue that caregivers rely on the knowledge about the referent that is shared with infants in order to interpret what communicative function infants wish to convey with their gestures. We videotaped interactions from six caregiver-infant dyads playing with toys when infants were 8, 10, 12, 14, and 16 months old. We coded infants' gesture production and we determined whether caregivers interpreted those gestures as conveying a clear communicative function or not; we also coded whether infants used objects according to their conventions of use as a measure of shared knowledge about the referent. Results revealed an association between infants' increasing knowledge of object use and maternal interpretations of infants' gestures as conveying a clear communicative function. Our findings emphasize the importance of shared knowledge in shaping infants' emergent communicative skills.

The aim of this study is to perform a thorough comparison of quantitative susceptibility mapping (QSM) techniques and their dependence on the assumptions made. The compared methodologies were: two iterative single orientation methodologies minimizing the l2, l1TV norm of the prior knowledge of the edges of the object, one over-determined multiple orientation method (COSMOS) and anewly proposed modulated closed-form solution (MCF). The performance of these methods was compared using a numerical phantom and in-vivo high resolution (0.65mm isotropic) brain data acquired at 7T using a new coil combination method. For all QSM methods, the relevant regularization and prior-knowledge parameters were systematically changed in order to evaluate the optimal reconstruction in the presence and absence of a ground truth. Additionally, the QSM contrast was compared to conventional gradient recalled echo (GRE) magnitude and R2* maps obtained from the same dataset. The QSM reconstruction results of the single orientation methods show comparable performance. The MCF method has the highest correlation (corrMCF=0.95, r(2)MCF =0.97) with the state of the art method (COSMOS) with additional advantage of extreme fast computation time. The l-curve method gave the visually most satisfactory balance between reduction of streaking artifacts and over-regularization with the latter being overemphasized when the using the COSMOS susceptibility maps as ground-truth. R2* and susceptibility maps, when calculated from the same datasets, although based on distinct features of the data, have a comparable ability to distinguish deep gray matter structures.

Doctors must regularly adjust their patients' care according to recent relevant publications. The chief residents from the Department of Internal Medicine of a university hospital present some major themes of internal medicine treated during the year 2008, such as heart failure, diabetes, COPD, and thromboembolic disease. Emphasis will be placed primarily on changes in the daily hospital practice induced by these recent studies. This variety of topics illustrates both the broad spectrum of the current internal medicine, and the many uncertainties associated with modem medical practice based on evidence.

L'année 2007 a été marquée par la publication de plusieurs études internationales concernant directement le quotidien de l'interniste hospitalier. Un résumé de ces travaux ne saurait être qu'un extrait condensé et forcément subjectif d'une croissante et dynamique diversité. Au gré de leurs lectures, de leurs intérêts et de leurs interrogations, les chefs de clinique du Service de médecine interne vous proposent ainsi un parcours original revisitant les thèmes de l'insuffisance cardiaque, du diabète, de l'endocardite, de la BPCO ou de la qualité des soins. Cette variété de sujets illustre à la fois le vaste champ couvert par la médecine interne actuelle, ainsi que les nombreuses incertitudes liées à la pratique médicale moderne basée sur les preuves. In 2007, several international studies brought useful information for the daily work of internists in hospital settings. This summary is of course subjective but reflects the interests and questions of the chief residents of the Department of internal medicine who wrote this article like an original trip in medical literature. This trip will allow you to review some aspects of important fields such as heart failure, diabetes, endocarditis, COPD, and quality of care. Besides the growing diversity of the fields covered by internal medicine, these various topics underline also the uncertainty internists have to face in a practice directed towards evidence.

Aims and objectives This study aimed to determine the discriminant validity and the test-retest reliability of a questionnaire testing the impact of evidence-based medicine (EBM) training on doctors' knowledge and skills. Methods Questionnaires were sent electronically to all doctors working as residents and chief residents in two French speaking hospital networks in Switzerland. Participants completed the questionnaire twice, within a 4-week interval. The discriminant validity was examined in comparing doctors' performance according to their reported EBM previous training. Proportion of agreement between both sessions of the questionnaire, Cohen's kappa and 'uniform kappa' determined its test-retest reliability. Results The participation rate was 9.8%/7.1% to first/second session. Performance increased according to the level of doctors' previous training in EBM. The observed proportion of agreement between both sessions was over 70% for 14/19 questions, and the 'uniform kappa' was superior to 0.60 for 15/19 questions. Conclusion The discriminant validity and test-retest reliability of the questionnaire were satisfying. The low participation rate did not prevent the study from achieving its aims.

The pace of development of new healthcare technologies and related knowledge is very fast. Implementation of high quality evidence-based knowledge is thus mandatory to warrant an effective healthcare system and patient safety. However, even though only a small fraction of the approximate 2500 scientific publication indexed daily in Medline is actually useful to clinical practice, the amountof the new information is much too large to allow busy healthcare professionals to stay aware of possibly important evidence-based information.

More than 60% of neuroendocrine tumours, also called carcinoids, are localised within the gastrointestinal tract. Small bowel neuroendocrine tumours have been diagnosed with increasing frequency over the past 35 years, being the second most frequent tumours of the small intestine. Ileal neuroendocrine tumours diagnosis is late because patients have non-specific symptoms. We have proposed to illustrate as an example the case of a patient, and on its basis, to make a brief review of the literature on small bowel neuroendocrine tumours, resuming several recent changes in the field, concerning classification criteria of these tumours and new recommendations and current advances in diagnosis and treatment. This patient came to our emergency department with a complete bowel obstruction, along with a 2-year history of peristaltic abdominal pain, vomits and diarrhoea episodes. During emergency laparotomy, an ileal stricture was observed, that showed to be a neuroendocrine tumour of the small bowel.

We propose a new approach and related indicators for globally distributed software support and development based on a 3-year process improvement project in a globally distributed engineering company. The company develops, delivers and supports a complex software system with tailored hardware components and unique end-customer installations. By applying the domain knowledge from operations management on lead time reduction and its multiple benefits to process performance, the workflows of globally distributed software development and multitier support processes were measured and monitored throughout the company. The results show that the global end-to-end process visibility and centrally managed reporting at all levels of the organization catalyzed a change process toward significantly better performance. Due to the new performance indicators based on lead times and their variation with fixed control procedures, the case company was able to report faster bug-fixing cycle times, improved response times and generally better customer satisfaction in its global operations. In all, lead times to implement new features and to respond to customer issues and requests were reduced by 50%.

In my thesis, I defend the idea that Aristotle's notion of phronêsis (practical wisdom) is best understood as a kind of practical knowledge. I interpret phronêsis as the knowledge we display when we make the correct decision to act. In a particular situation that demands a specific response, we have practical knowledge of what to do when we make the best decision possible. This interpretation of phronêsis involves that it is possible to evaluate our decisions epistemically, that is, to evaluate whether we really know what to do or not. Aristotle provides a tool for the evaluation of our decisions, which is a definite kind of argument and which the tradition has called the 'practical syllogism'. The practical syllogism stands as the explanation of our decisions or actions. We invoke it when we want to explain or justify why we act as we do. My claim is that the components of the practical syllogism enable one to evaluate not only the moral character of our actions, but also the epistemic strength of our decisions. Correspondingly, a decision is morally right, i.e. virtuous, if the agent considers the right moral principle to apply, and if he is aware of the relevant circumstances of the situation (moral evaluation). Moreover, a decision displays practical knowledge if the agent meets three conditions (epistemic evaluation): he must desire the moral principle for its own sake; he must have experience in spotting the relevant circumstances of the situation; and he must be able to closely connect these circumstances with the moral principle. This interpretation of phronêsis differs from other more traditional interpretations in the emphasis it puts on phronêsis as knowledge. Other interpretations focus more on the moral dimension on phronêsis, without taking its epistemic value seriously. By contrast, I raise seriously the question of what it takes to genuinely know what one should do in a particular situation. -- Dans ma thèse, je défends l'idée que la notion aristotélicienne de phronêsis (sagesse pratique) doit être interprétée comme connaissance pratique. Je comprends la phronêsis comme étant la connaissance que nous avons lorsque nous prenons une bonne décision. Dans une situation particulière qui demande une réponse précise, nous avons une connaissance pratique lorsque nous prenons la meilleure décision possible. Cette interprétation de la phronêsis implique qu'il est possible d'évaluer nos décisions de manière épistémique, c'est-à-dire, d'évaluer si nous savons vraiment ce qu'il faut faire ou non. Ma position est qu'Aristote fournit un outil pour évaluer épistémiquement nos décisions, qui consiste en un certain type d'argument et que la tradition a appelé le 'syllogisme pratique'. Le syllogisme pratique correspond à l'explication de nos décisions ou de nos actions. Nous invoquons un syllogisme pratique lorsque nous voulons expliquer ou justifier pourquoi nous agissons comme nous le faisons. Les éléments du syllogisme pratique permettent d'évaluer non seulement le caractère moral de nos actions, mais aussi la force épistémique de nos décisions. Par conséquent, une décision est moralement correcte, i.e. vertueuse, si l'agent considère le bon principe moral, et s'il est attentif aux circonstances pertinentes de la situation (évaluation morale). En outre, une décision inclut la connaissance pratique si l'agent remplit trois conditions (évaluation épistémique) : il doit désirer le principe moral pour lui-même, il doit avoir de l'expérience pour déceler les circonstances pertinentes, et il doit pouvoir lier intimement ces circonstances avec le principe moral. Cette interprétation de la phronêsis diffère d'autres interprétations plus traditionnelles par l'emphase mise sur la phronêsis en tant que connaissance. D'autres interprétations se concentrent plus sur la dimension morale de la phronêsis, sans se préoccuper sérieusement de sa valeur épistémique. Au contraire, je pose sérieusement la question des conditions nécessaires pour réellement savoir ce qu'il faut faire dans une situation donnée.
