[...] L'objectif de ce travail est d'exposer les notions de "compétence sanitaire" (Health Literacy) et d' "autonomisation" (Empowerment) dans un contexte de prise en charge pédiatrique des enfants de migrants. Nous sommes convaincus que ces deux concepts émergeants dans le domaine de la santé et appliqués au contexte migratoire constituent, sur les plans économiques et de management, des éléments qui peuvent mener à terme à : i) une réduction mesurable des coûts de la santé et ii) une porte d'entrée efficace dans un processus d'intégration - dont l'importance est reconnue sur le plan économique - des migrants dans leur pays d'accueil. Tout au long de ce travail, il apparaîtra comme évident que des données propres à la prise en charge sanitaire pédiatrique des enfants de migrants font actuellement souvent défaut, a fortiori l'application des concepts de compétence sanitaire et d'autonomisation pour cette part de la société. Nous le percevons comme une opportunité de mettre en place les conditions cadres pour combler ce vide. Nous poserons tout d'abord le cadre de ce mémoire au travers de la présentation des concepts de compétence sanitaire et d'autonomisation, en mettant en avant leurs aspects économiques dans les systèmes de santé actuels, en particulier en Suisse. Nous présenterons dans les chapitres 2 et 3 d'une part des données concernant l'état de santé de la population migrante en Suisse et d'autre part la stratégie mise en place au niveau fédéral dans le domaine " migration et santé 2008-2013 ". Le chapitre 4 nous permettra de présenter un "case study" que nous avons intégré à notre travail afin d'apporter des éléments plus concrets aux notions théoriques abordées dans ce mémoire. Nous avons choisi de nous concentrer sur l'Hôpital de l'Enfance de Lausanne (HEL) parce que l'HEL nous semble présenter un cas de structure hospitalière qui fait un effort remarquable de prise en charge médico-psycho-sociale des enfants de migrants. Le chapitre 5, en conclusion, aura pour objectif principal de soutenir que l'inclusion d'une dimension de management/gestion de la santé (représenté ici par l'autonomisation et la compétence sanitaire) introduirait une valeur ajoutée remarquable à l'effort constructif consenti sur le plan médico-social décrit au chapitre précédent. We argue that attitudes about immigration can be better understood by paying closer attention to the various ways in which national group boundaries are demarcated. We describe two related lines of work that address this. The first deals with national group definitions and, based on evidence from studies carried out in England and analyses of international survey data, argues that the relationship between national identification and prejudice toward immigrants is contingent on the extent to which ethnic or civic definitions of nationality are endorsed. The second, which uses European survey data, examines support for ascribed and acquired criteria that can be applied when determining who is permitted to migrate to one's country, and the various forms of national and individual threat that affect support for these criteria. We explain how the research benefits from a multilevel approach and also suggest how these findings relate to some current policy debates.

BACKGROUND: Migration is considered a depression risk factor when associated with psychosocial adversity, but its impact on depression's clinical characteristics has not been specifically studied. We compared 85 migrants to 34 controls, examining depression's severity, symptomatology, comorbidity profile and clinical course. METHOD: A MINI interview modified to assess course characteristics was used to assign DSM-IV axis I diagnoses; medical files were used for Somatoform Disorders. Severity was assessed with the Montgomery-Asberg scale. Wherever possible, we adjusted comparisons for age and gender using logistic and linear regressions. RESULTS: Depression in migrants was characterized by higher comorbidity (mostly somatoform and anxiety disorders), higher severity, and a non-recurrent, chronic course. LIMITATIONS: Our sample comes from a single center, and should be replicated in other health care facilities and other countries. Somatoform disorder diagnoses were solely based on file-content. CONCLUSION: Depression in migrants presented as a complex, chronic clinical picture. Most of our migrant patients experienced significant psychosocial adversity before and after migration: beyond cultural issues, our results suggest that psychosocial adversity impacts on the clinical expression of depression. Our study also suggests that migration associated with psychosocial adversity might play a specific etiological role, resulting in a distinct clinical picture, questioning the DSM-IV unitarian model of depression. The chronic course might indicate a resistance to standard therapeutic regimen and hints at the necessity of developing specific treatment strategies, adapted to the individual patients and their specific context.

Using a social identity theory approach, we theorized that recruiters might be particularly biased against skilled immigrant applicants. We refer to this phenomenon as a skill paradox, according to which immigrants are more likely to be targets of employment discrimination the more skilled they are. Furthermore, building on the common ingroup identity model, we proposed that this paradox can be resolved through human resource management (HRM) strategies that promote inclusive hiring practices (e.g., by emphasizing fit with a diverse clientele). The results from a laboratory experiment were consistent with our predictions: Local recruiters preferred skilled local applicants over skilled immigrant applicants, but only when these applicants were qualified for a specific job. This bias against qualified and skilled immigrant applicants was attenuated when fit with a diverse clientele was emphasized, but not when fit with a homogeneous clientele was emphasized or when the hiring strategy was not explained. We discuss the implications of our findings for research on employment discrimination against skilled immigrants, including the role of inclusiveness for reducing discriminatory biases.
