A partir de l'analyse des effets de l'origine sociale et des niveaux d'études sur la destination sociale de trois cohortes, cet article présente l'évolution de l'inégalité des chances en Suisse. En se positionnant dans le paradigme des parcours de vie, il s'agit de voir comment l'évolution du contexte historique, le développement du système éducatif et la progressive égalisation entre les hommes et les femmes ont pu avoir des effets sur la réduction de l'inégalité des chances en Suisse. Pour percevoir le changement social, douze bases de données de 1975 à 2009 sont agrégées et des modèles log-linéaires et log-multiplicatifs sont appliqués à des tableaux croisant respectivement, l'origine sociale et la destination sociale, l'origine sociale et le niveau d'étude, le niveau d'étude et la destination sociale, avec la cohorte. Les résultats montrent qu'au cours du 20ème siècle, quel que soit la relation étudiée, l'inégalité des chances est restée constante en Suisse.

Context: Both biallelic and monoallelic mutations in PROK2 or PROKR2 have been found in Kallmann syndrome (KS). Objective: The objective of the study was to compare the phenotypes of KS patients harboring monoallelic and biallelic mutations in these genes. Design and Patients: We studied clinical and endocrine features that reflect the functioning of the pituitary-gonadal axis, and the nonreproductive phenotype, in 55 adult KS patients (42 men and 13 women), of whom 41 had monoallelic mutations and 14 biallelic mutations in PROK2 or PROKR2. Results: Biallelic mutations were associated with more frequent cryptorchidism (70% vs. 34%, P < 0.05) and microphallus (90% vs. 28%, P < 0.001) and lower mean testicular volume (1.2 +/- 0.4 vs. 4.5 +/- 6.0 ml; P < 0.01) in male patients. Likewise, the testosterone level as well as the basal FSH level and peak LH level under GnRH-stimulation were lower in males with biallelic mutations (0.2 +/- 0.1 vs. 0.7 +/- 0.8 ng/ml; P = 0.05, 0.3 +/- 0.1 vs. 1.8 +/- 3.0 IU/liter; P < 0.05, and 0.8 +/- 0.8 vs. 5.2 +/- 5.5 IU/liter; P < 0.05, respectively). Nonreproductive, nonolfactory anomalies were rare in both sexes and were never found in patients with biallelic mutations. The mean body mass index of the patients (23.9 +/- 4.2 kg/m(2) in males and 26.3 +/- 6.6 kg/m(2) in females) did not differ significantly from that of gender-, age-, and treatment-matched KS individuals who did not carry a mutation in PROK2 or PROKR2. Finally, circadian cortisol levels evaluated in five patients, including one with biallelic PROKR2 mutations, were normal in all cases. Conclusion: Male patients carrying biallelic mutations in PROK2 or PROKR2 have a less variable and on average a more severe reproductive phenotype than patients carrying monoallelic mutations in these genes. Nonreproductive, nonolfactory clinical anomalies associated with KS seem to be restricted to patients with monoallelic mutations.

Si des enjeux, des configurations d'acteurs, ou la définition de certains problems peuvent être propres au niveau local, ceux-ci sont fréquemment influencés par des paramètres définis à d'autres niveaux de gouvernement. En particulier, il est nécessaire que les dispositifs de recherche centrés sur la comparaison de politiques infranationales abordent directement la question de l'intégration et de l'impact du cadre national. Comment faire la part du national dans les comparaisons de politiques infranationales ? À partir d'un ensemble de travaux de sociologie de l'action publique, cet article montre que dominent deux manières d'articuler national et infranational. Certains travaux privilégient le niveau national, et prennent en compte ce qui se joue à d'autres niveaux pour observer des déclinaisons ou des réappropriations de politiques mises en place au niveau national. D'autres travaux, en particulier la littérature centrée sur la gouvernance territoriale ou urbaine, accordent un primat au local tout en prenant acte de la multiplication des niveaux de gouvernement. Enfin, l'accent est placé sur les tentatives visant à concilier ces perspectives. À l'appui de nos propres travaux, nous montrons comment des approches centrées sur les acteurs permettent d'ouvrir des pistes stimulantes pour articuler plusieurs niveaux d'analyse.

Communication des résultats du projet "Matériau Pathos". Avec la collaboration de la HEAD (Haute Ecole d'Art et de Design de Genève), un ensemble de textes et d'objets ont été conçus pour être réunis dans une «boîte». La première conception de cette boîte et de son contenu a réuni Danielle Chaperon, Christian Geoffroy Schlittler et Julie-Kasuko Rahir. Le résultat de cette première élaboration a été soumis, pendant le semestre de printemps 2012 aux étudiants en Communication visuelle de la Haute Ecole d'Art et de Design de Genève (HEAD), sous la direction de Jérôme Baratelli, Benjamin Stroun et Paul Viaccoz. La production matérielle des exemplaires de la boîte Matériau Pathos est soutenue par la Société Académique Vaudoise.

BACKGROUND: Artemisinin-resistant Plasmodium falciparum has emerged in the Greater Mekong sub-region and poses a major global public health threat. Slow parasite clearance is a key clinical manifestation of reduced susceptibility to artemisinin. This study was designed to establish the baseline values for clearance in patients from Sub-Saharan African countries with uncomplicated malaria treated with artemisinin-based combination therapies (ACTs). METHODS: A literature review in PubMed was conducted in March 2013 to identify all prospective clinical trials (uncontrolled trials, controlled trials and randomized controlled trials), including ACTs conducted in Sub-Saharan Africa, between 1960 and 2012. Individual patient data from these studies were shared with the WorldWide Antimalarial Resistance Network (WWARN) and pooled using an a priori statistical analytical plan. Factors affecting early parasitological response were investigated using logistic regression with study sites fitted as a random effect. The risk of bias in included studies was evaluated based on study design, methodology and missing data. RESULTS: In total, 29,493 patients from 84 clinical trials were included in the analysis, treated with artemether-lumefantrine (n = 13,664), artesunate-amodiaquine (n = 11,337) and dihydroartemisinin-piperaquine (n = 4,492). The overall parasite clearance rate was rapid. The parasite positivity rate (PPR) decreased from 59.7 % (95 % CI: 54.5-64.9) on day 1 to 6.7 % (95 % CI: 4.8-8.7) on day 2 and 0.9 % (95 % CI: 0.5-1.2) on day 3. The 95th percentile of observed day 3 PPR was 5.3 %. Independent risk factors predictive of day 3 positivity were: high baseline parasitaemia (adjusted odds ratio (AOR) = 1.16 (95 % CI: 1.08-1.25); per 2-fold increase in parasite density, P <0.001); fever (>37.5 °C) (AOR = 1.50 (95 % CI: 1.06-2.13), P = 0.022); severe anaemia (AOR = 2.04 (95 % CI: 1.21-3.44), P = 0.008); areas of low/moderate transmission setting (AOR = 2.71 (95 % CI: 1.38-5.36), P = 0.004); and treatment with the loose formulation of artesunate-amodiaquine (AOR = 2.27 (95 % CI: 1.14-4.51), P = 0.020, compared to dihydroartemisinin-piperaquine). CONCLUSIONS: The three ACTs assessed in this analysis continue to achieve rapid early parasitological clearance across the sites assessed in Sub-Saharan Africa. A threshold of 5 % day 3 parasite positivity from a minimum sample size of 50 patients provides a more sensitive benchmark in Sub-Saharan Africa compared to the current recommended threshold of 10 % to trigger further investigation of artemisinin susceptibility.

OBJECTIVE: Several smaller single-center studies have reported a prognostic role for Ki-67 labeling index in prostate cancer. Our aim was to test whether Ki-67 is an independent prognostic marker of biochemical recurrence (BCR) in a large international cohort of patients treated with radical prostatectomy (RP). METHODS: Ki-67 immunohistochemical staining on prostatectomy specimens from 3,123 patients who underwent RP for prostate cancer was retrospectively performed. Univariable and multivariable Cox regression models were used to assess the association of Ki-67 status with BCR. RESULTS: Ki-67 positive status was observed in 762 (24.4 %) patients and was associated with lymph node involvement (LNI) (p = 0.039). Six hundred and twenty-one (19.9 %) patients experienced BCR. The estimated 3-year biochemical-free survivals were 85 % for patients with negative Ki-67 status and 82.1 % for patients with positive Ki-67 status (log-rank test, p = 0.014). In multivariable analysis that adjusted for the effects of age, preoperative PSA, RP Gleason sum, seminal vesicle invasion, extracapsular extension, positive surgical margins, lymphovascular invasion, and LNI, Ki-67 was significantly associated with BCR (HR = 1.19; p = 0.019). Subgroup analysis revealed that Ki-67 is associated with BCR in patients without LNI (p = 0.004), those with RP Gleason sum 7 (p = 0.015), and those with negative surgical margins (p = 0.047). CONCLUSION: We confirmed Ki-67 as an independent predictor of BCR after RP. Ki-67 could be particularly informative in patients with favorable pathologic characteristics to help in the clinical decision-making regarding adjuvant therapy and optimized follow-up scheduling.

The present talk aims to examine the heart-rate variability (HRV) indexes of two populations known to be particularly sensitive to stress condition: violent offenders and intellectually impaired patients. For both, difficulties in managing stressful and emotional events could lead to unpredictable and hostile acting. Results in favour of specific HRV baseline patterns and emotional control will be developed, as the proposition of precursors of agitation in the physiological fluctuations preceding high tensed episodes. The improvement offered by coherent breathing trainings will be discussed for both groups.
