In the context of lay mobilization in health-related areas, this article addresses the role and activities of patients' associations in connection with organ donation, on the basis of interviews carried out with thirty members of transplant patients' associations in the French-speaking part of Switzerland. First, we describe the three main categories of activities conducted by these associations. While self-help and public awareness activities are predominant, policy-oriented actions are marginal. Then, we examine the factors likely to explain why these associations have a limited capacity to be active, especially in the public sphere. Such a lack of social visibility is all the more important in the current political context, characterized by the implementation of a national action plan designed to improve organ donation.

Les antipsychotiques atypiques, de deuxième génération, ont largement contribué à améliorer le traitement des patients souffrant de schizophrénie. Cependant, leur mécanisme d'action reste mal compris et leurs effets secondaires sont importants, notamment la prise de poids. CRTC1 (CREB-regulated transcription coactivator 1), aussi appelé TORC1 (transducers of regulated CREB activity 1), est un coactivateur de CREB. Il régule la transcription de Bdnf1 qui joue un rôle essentiel dans le contrôle de la balance énergétique dépendant du VMH 2, 3, 4. Nous pensons que CRTC1 est impliqué dans la prise de poids induite par certains antipsychotiques. En effet, il a été démontré que les souris Crtc1-/- devenaient hyperphagiques et obèses 5, 6, que la régulation de l'activité de CRTC1 se faisait par l'AMPK et que les antipsychotiques atypiques activaient cette kinase dans l'hypothalamus.7 L'AMPK de l'hypothalamus est liée à la régulation de la prise alimentaire, elle inverse l'action de la leptine, hormone anorexigène. Suite à ces constatations, nous proposons de suivre l'hypothèse de travail suivante : l'activation de l'AMPK par les antipsychotiques atypiques dans l'hypothalamus peut maintenir la phosphorylation de CRTC1 et le bloquer dans le cytoplasme, l'empêchant ainsi d'activer les gènes anorexigènes, comme Bdnf par exemple. En effet, la forme phosphorylée inactive de CRTC1 est séquestrée dans le cytoplasme et sa migration dans le noyau nécessite en même temps l'activation de la phosphatase calcineurine et l'inactivation des kinases de la famille de l'AMPK. Dans le travail que nous avons entamé, nous cherchons donc, par western blot et par immunohistochimie, à comprendre si les antipsychotiques atypiques inactivent CRTC1 en induisant sa phosphorylation par l'AMPK et sa rétention dans le cytoplasme.

Introduction: The Violence Medical Unit (VMU), a specialised forensic medical consultation, was created at the Lausanne university Hospital in 2006. All patients consulting at the ED for interpersonal violencerelated injury are referred to the VMU, which provides forensic documentation of the injury and referral to the relevant community based victim-support organisations within 48 hours of the ED visit. This frees the ED medical staff from forensic injury documentation and legal/social referral, tasks for which they lack both time and training. Among community violence, assaults by nightclub security agents against patrons have increased from 6% to 10% between 2007 and 2009. We set out to characterise the demographics, assault mechanisms, subsequent injuries, prior alcohol intake and ED & VMU costs incurred by this group of patients. Methods: We retrospectively included all patients consulting at the VMU due to assault by nightclub security agents from January 2007 to December 2009. Data was obtained from ED & VMU medical, nursing and administrative records. Results: Our sample included 70 patients, of which 64 were referred by the CHUV ED. The victims were typically young (median age 29) males (93%). 77% of assaults occurred on the weekend between 12 PM and 4 AM, and 73% of the victims were under the influence of alcohol. 83% of the patients were punched, kicked and/or head-butted; 9% had been struck with a blunt instrument. 80% of the injuries were in the head and neck area and 19% of the victims sustained fractures. 21% of the victims were prescribed medical leave. Total ED & VMU costs averaged 1048 SFr. Conclusion: Medical staff treating this population of assault victims must be aware of the assault mechanisms and injury patterns, in particular the high probability of fractures, in order to provide adequate diagnosis and care. Associated inebriation mandates liberal use of radiology, as delayed or missed diagnosis may have medical, medicolegal and legal implications. Emergency medical services play an important role in detecting and reporting of such incidents. Centralised management of the forensic documentation facilitates referral to victim support organisations and epidemiological data collection. Magnitudes and trends of the different types of violence can be determined, and this information can be then impact public safety management policies.

Un certain nombre de malades ne sont toujours pas soulagés de manière satisfaisante par le traitement médical de la maladie de de reflux gastro-oesophagien (MR). Ce travail rapport notre expérience de 55 malades ayant bénéficié d'un montage anti-reflux réalisé par laparoscopie entre février 1992 et février 1997. La durée opératoire moyenne a été de 181 minutes, et tend à diminuer. La médiane de s&jour postopératoire a été de 4 jours. Quatre-vingt-neuf pour-cent des malades ont eu un suivi complet de 2 à 62 mois. Nonante-deux pour-cent d'entre eux étaient satisfaits ou très satisfaits du résultat. L'enthousiasme lié au traitement laparoscopique de la MR est justifié. Les résultats dépendent de la maîtrise technique du chirurgien, de sa connaissance de la MR, et de ses capacités à adapter le type de montage anti-reflux en fonction de l'évaluation préopératoire. L'intervention donne alors un résultat prévisible, ce qui incite à la proposer aux malades mal contrôlés médicalement, ou qui préfèrent éviter les contraintes d'un traitement de maintien à long terme.

The period of adolescence is not only marked by important growth and pubertal events, but is also characterized by important psychosocial changes driven by a search for autonomy and the construction of one's identity. It can thus be easily understood that puberty disorders interfere heavily with these process, requiring from the endocrinologist not only medical knowledge, but also a great deal of emotional and psychological skills. They must progressively move from an educational approach that heavily involves the parents to one of shared information and decision making that places the young patient at the center of the therapeutic process. This can be achieved in several ways: respecting the affective and cognitive development of the adolescent; securing his privacy and (if requested by him) confidentiality; exploring his self-image and self-esteem and adapting the therapeutic process to the patient's expectations; reviewing the teenager's lifestyle, including the issue of sexuality and sexual behavior, and involving him in any therapeutic choice that has to be made, even if it does not match with the parents' expectations. The skills required for this respectful and holistic follow-up often exceed the abilities of any physician; it is thus suggested that a team approach involving a clinical nurse and/or a psychologist and/or social worker(s) be set up whenever possible.
