BACKGROUND: Rivaroxaban has become an alternative to vitamin-K antagonists (VKA) for stroke prevention in non-valvular atrial fibrillation (AF) patients due to its favourable risk-benefit profile in the restrictive setting of a large randomized trial. However in the primary care setting, physician's motivation to begin with rivaroxaban, treatment satisfaction and the clinical event rate after the initiation of rivaroxaban are not known. METHODS: Prospective data collection by 115 primary care physicians in Switzerland on consecutive nonvalvular AF patients with newly established rivaroxaban anticoagulation with 3-month follow-up. RESULTS: We enrolled 537 patients (73±11years, 57% men) with mean CHADS2 and HAS-BLED-scores of 2.2±1.3 and 2.4±1.1, respectively: 301(56%) were switched from VKA to rivaroxaban (STR-group) and 236(44%) were VKA-naïve (VN-group). Absence of routine coagulation monitoring (68%) and fixed-dose once-daily treatment (58%) were the most frequent criteria for physicians to initiate rivaroxaban. In the STR-group, patient's satisfaction increased from 3.6±1.4 under VKA to 5.5±0.8 points (P<0.001), and overall physician satisfaction from 3.9±1.3 to 5.4±0.9 points (P<0.001) at 3months of rivaroxaban therapy (score from 1 to 6 with higher scores indicating greater satisfaction). In the VN-group, both patient's (5.4±0.9) and physician's satisfaction (5.5±0.7) at follow-up were comparable to the STR-group. During follow-up, 1(0.19%; 95%CI, 0.01-1.03%) ischemic stroke, 2(0.37%; 95%CI, 0.05-1.34%) major non-fatal bleeding and 11(2.05%; 95%CI, 1.03-3.64%) minor bleeding complications occurred. Rivaroxaban was stopped in 30(5.6%) patients, with side effects being the most frequent reason. CONCLUSION: Initiation of rivaroxaban for patients with nonvalvular AF by primary care physicians was associated with a low clinical event rate and with high overall patient's and physician's satisfaction.

UNLABELLED: Trabecular bone score (TBS) is a DXA-based tool that assesses bone texture and reflects microarchitecture. It has been shown to independently predict the risk of osteoporotic fracture in the elderly. In this study, we investigated the determinants of TBS in adolescents. INTRODUCTION: TBS is a gray-level textural measurement derived from lumbar spine DXA images. It appears to be an index of bone microarchitecture that provides skeletal information additional to the standard BMD measurement and clinical risk factors. Our objectives were to characterize the relationship between TBS and both age and pubertal stages and identify other predictors in adolescents. METHODS: We assessed TBS by reanalyzing spine DXA scan images obtained from 170 boys and 168 girls, age range 10-17 years, gathered at study entry and at 1 year, using TBS software. The results are from post hoc analyses obtained using data gathered from a prospective randomized vitamin D trial. Predictors of TBS were assessed using t test or Pearson's correlation and adjusted using regression analyses, as applicable. RESULTS: The mean age of the study population was 13.2 ± 2.1 years, similar between boys and girls. Age, height, weight, sun exposure, spine BMC and BMD, body BMC and BMD, and lean and fat mass are all significantly correlated with TBS at baseline (r = 0.20-0.75, p < 0.035). Correlations mostly noted in late-pubertal stages. However, after adjustment for BMC, age remained an independent predictor only in girls. CONCLUSIONS: In univariate exploratory analyses, age and pubertal stages were determinants of TBS in adolescents. Studies to investigate predictors of TBS and to investigate its value as a prognostic tool of bone fragility in the pediatric population are needed.

Hyperpolarization by dissolution dynamic nuclear polarization (DNP) is a versatile technique to dramatically enhance the nuclear magnetic resonance (NMR) signal intensity of insensitive long-T1 nuclear spins such as (6) Li. The (6) Li longitudinal relaxation of lithium ions in aqueous solutions strongly depends on the concentration of paramagnetic species, even if they are present in minute amounts. We herein demonstrate that blood oxygenation can be readily detected by taking advantage of the (6) Li signal enhancement provided by dissolution DNP, together with the more than 10% decrease in (6) Li longitudinal relaxation as a consequence of the presence of paramagnetic deoxyhemoglobin. The aim of the study is to understand how the family influences the choice of becoming a psychologist and how an occupational choice is repeated in the family, via intergenerational transmission. We interviewed seven female students in a Master of Science in Psychology : first, they filled in a genosociogramm including data about occupations of their ancestors on about four generations ; then, they took part into a semi-structured qualitative enquiry. Our results have shown that a little bit less than half of the subjects have a parent who have social or care jobs, but more than half if we add the grand-parents. In a conscious level, subjects tend to deny any kind of family influence, in the majority ; afterwards, they discover influences they didn't notice. Secondly, the content analysis reveals five categories of family influence : the educational path (doubts, choices), the choice of psychology via the development of self-efficacy (interest, personality and soft skills), the exploration of occupations and activities during childhood and adulthood (leisure activities, professional world, suggestions, advice, education), the transmission of values (immaterial and material) and the family relationships during childhood and teenage years (relationship issues and difficulties, confidences and secrets, relationships and role in the brotherhood and/or sisterhood). The importance for the career counselor to investigate the relational context of his/her consultant is discussed, as much as the need for him to think about his own motivations to help others, linked with his family background.

The evaluation of forensic evidence can occur at any level within the hierarchy of propositions depending on the question being asked and the amount and type of information that is taken into account within the evaluation. Commonly DNA evidence is reported given propositions that deal with the sub-source level in the hierarchy, which deals only with the possibility that a nominated individual is a source of DNA in a trace (or contributor to the DNA in the case of a mixed DNA trace). We explore the use of information obtained from examinations, presumptive and discriminating tests for body fluids, DNA concentrations and some case circumstances within a Bayesian network in order to provide assistance to the Courts that have to consider propositions at source level. We use a scenario in which the presence of blood is of interest as an exemplar and consider how DNA profiling results and the potential for laboratory error can be taken into account. We finish with examples of how the results of these reports could be presented in court using either numerical values or verbal descriptions of the results.

We examine entry mode choice and its consequences when a multinational enterprise (MNE) expands into an institutionally different country. We argue that discussions of entry mode should distinguish between informal (e.g., culture) and formal (e.g., laws) institutions, and should take into account not just the home country of the MNE and its distance to the focal host country, but the MNE's overall footprint and experience across the world in general, especially in countries with an institutional structure that is similar to that of the focal host country. Specifically, we argue that firms with experience in countries with different informal institutions will be more likely to enter via acquisitions than firms without such experience, that such experience will not matter as much in the case of formal institutions, and that such firms will exit more quickly when they enter via equity alliances than through full acquisitions. We also distinguish between balanced and unbalanced alliances and argue that balanced alliances will be more enduring, but only when the host country is culturally (not legally) different from the other countries where the MNE has experience. Our arguments suggest that entry mode should be conditioned on a firm's experience in other markets, and that intercountry differences in formal versus informal institutions have distinct influences on entry mode.

Au cours des dernières décennies, les recherches articulant sport et travail se sont beaucoup développées. Elles portent sur un large ensemble de questions comme le fonctionnement des organisations sportives, les carrières des sportifs de haut niveau, la croissance d'un secteur économique et de métiers de l'intervention sportive, les migrations internationales des sportifs, les discriminations sexuelles ou raciales dans l'accès aux marchés du travail sportif, etc. Ici nous mettons l'accent sur une dimension, centrale, des activités sportives : la compétition. Et notre objectif est d'analyser les mécanismes de production de la performance sportive. Nous considérons celle-ci comme le résultat d'un travail qui n'engage pas les seuls sportifs, avec leurs aptitudes, qualités ou capacités individuelles. Nous la définissons comme une activité collective, qui mobilise une pluralité d'acteurs, institutions, organisations. À travers une variété d'opérations de jugement, d'évaluation, de reconnaissance, de qualification, de cotation, de sélection, ces acteurs contribuent, de manière directe et décisive, à produire la performance sportive. En présentant des travaux empiriques qui argumentent cette problématique et la mobilisent dans des domaines variés (cyclisme, rugby, judo, etc.), nous invitons au développement de recherches sur le travail sportif. In recent decades, there has been much development in research connecting sport and work. It covers a wide range of questions such as how sports organisations operate, the careers of top-level athletes, the growth of an economic sector and its specific jobs, the international migrations of athletes, sexual or racial discrimination in access to the labour market in sport, etc. Here, we place the emphasis on one central dimension of sports activities : competition. Our objective is to analyse the mechanisms of production of sports performance. We consider this to be the outcome of work that does not only involve athletes, with their individual skills, qualities or capacities. We define it as a collective activity that marshals multiple actors, institutions, organisations. Through a variety of activities of judgement, evaluation, recognition, qualification, classification and selection, these actors contribute directly and decisively to producing sports performance. By presenting empirical work that discusses this issue and applies it in varied domains (cycling, rugby, judo, etc.), we call for the development of research into work in sport.
