The purpose of this study was to evaluate the results of an anatomic open stabilization procedure. Twenty-three consecutive patients with traumatic, recurrent, anterior glenohumeral instability were managed with a modified open procedure. All had a Bankart lesion. A standardized, true anatomic capsulolabral reconstruction was performed with suture anchors. Two patients were lost to follow-up, and twenty-one were evaluated after 36 months [range, 24-49 months] by an independent observer. Shoulder stability and function were the principal outcome measures. None of the patients had subsequent dislocations. All returned to full working capacity, and 19 reached their previous level of sport activities. The mean loss in active abduction and flexion was 1 degrees, in internal rotation, one vertebral level, and in external rotation, 7 degrees (arm at 90 degrees of abduction). Two patients had a positive anterior apprehension test. No sign of shoulder osteoarthritis was observed. The open anatomic capsulolabral reconstruction provides excellent results and allows the restoration of stability with good function.

Glioblastoma multiforme (GBM) is the most malignant variant of human glial tumors. A prominent feature of this tumor is the occurrence of necrosis and vascular proliferation. The regulation of glial neovascularization is still poorly understood and the characterization of factors involved in this process is of major clinical interest. Macrophage migration inhibitory factor (MIF) is a pleiotropic cytokine released by leukocytes and by a variety of cells outside of the immune system. Recent work has shown that MIF may function to regulate cellular differentiation and proliferation in normal and tumor-derived cell lines, and may also contribute to the neovascularization of tumors. Our immunohistological analysis of MIF distribution in GBM tissues revealed the strong MIF protein accumulation in close association with necrotic areas and in tumor cells surrounding blood vessels. In addition, MIF expression was frequently associated with the presence of the tumor-suppressor gene p53. To substantiate the concept that MIF might be involved in the regulation of angiogenesis in GBM, we analyzed the MIF gene and protein expression under hypoxic and hypoglycemic stress conditions in vitro. Northern blot analysis showed a clear increase of MIF mRNA after hypoxia and hypoglycemia. We could also demonstrate that the increase of MIF transcripts on hypoxic stress can be explained by a profound transcriptional activation of the MIF gene. In parallel to the increase of MIF transcripts, we observed a significant rise in extracellular MIF protein on angiogenic stimulation. The data of our preliminary study suggest that the up-regulation of MIF expression during hypoxic and hypoglycemic stress might play a critical role for the neovascularization of glial tumors.

AIMS: The time course of atherosclerosis burden in distinct vascular territories remains poorly understood. We longitudinally evaluated the natural history of atherosclerotic progression in two different arterial territories using high spatial resolution magnetic resonance imaging (HR-MRI), a powerful, safe, and non-invasive tool. METHODS AND RESULTS: We prospectively studied a cohort of 30 patients (mean age 68.3, n = 9 females) with high Framingham general cardiovascular disease 10-year risk score (29.5%) and standard medical therapy with mild-to-moderate atherosclerosis intra-individually at the level of both carotid and femoral arteries. A total of 178 HR-MRI studies of carotid and femoral arteries performed at baseline and at 1- and 2-year follow-up were evaluated in consensus reading by two experienced readers for lumen area (LA), total vessel area (TVA), vessel wall area (VWA = TVA - LA), and normalized wall area index (NWI = VWA/TVA). At the carotid level, LA decreased (-3.19%/year, P = 0.018), VWA increased (+3.83%/year, P = 0.019), and TVA remained unchanged. At the femoral level, LA remained unchanged, VWA and TVA increased (+5.23%/year and +3.11%/year, both P < 0.01), and NWI increased for both carotid and femoral arteries (+2.28%/year, P = 0.01, and +1.8%/year, P = 0.033). CONCLUSION: The atherosclerotic burden increased significantly in both carotid and femoral arteries. However, carotid plaque progression was associated with negative remodelling, whereas the increase in femoral plaque burden was compensated by positive remodelling. This finding could be related to anatomic and flow differences and/or to the distinct degree of obstruction in the two arterial territories.

PRINCIPLES: This retrospective study analyzes the long-term results of endoscopic and surgical treatment of vesico-ureteral reflux in children. METHODS: A cohort of 130 patients, 67 girls and 63 boys with a mean age of 30 months were treated either by endoscopic subureteral collagen injection (SCIN) in 92 and by Cohen reimplantation surgery in 123 refluxing ureteral units. Mean follow-up was 4.2 years varying from 1 to 8.7 years. Reflux recurrence, urinary tract infection (UTI) and renal function were evaluated. RESULTS: After SCIN reflux was absent in 64% at 6 months. 20% of the initially 92 refluxing ureters were injected twice. After one or two injections reflux was absent in 71%. In 21% recurrent reflux was of grade I or II, not requiring further treatment. UTI was observed in 27%. After Cohen ureteral reimplantation reflux was absent in 96% at 6 months. UTI was observed in 23%. Renal function at diagnosis and follow-up was compared in children with bilateral grade III reflux only. In patients treated with SCIN it was normal in 77% preoperatively and in 90% at follow-up. In patients treated by open surgery it was normal in 47% preoperatively and in 76% at follow-up. CONCLUSION: For high-grade vesico-ureteral reflux re-implantation surgery remains the gold standard. SCIN is indicated for low and medium grade reflux. Recurrent bacteriuria was observed more often after SCIN and pyelonephritis more often after open surgery. The renal function seems to be preserved with both techniques.

In severe forms of Diamond-Blackfan anemia, preimplantation genetic diagnosis (PGD) of histocompatibility leukocyte antigen-compatible embryos for enabling the next sibling in the family to be a stem-cell transplantation donor constitutes the sole lasting cure capable of terminating the enduring need for iterative transfusions. We report here an open collaboration between two renowned institutions to provide a family desiring this treatment even though they resided where the preimplantation genetic diagnosis procedure is banned.

A high-resolution U-Pb zircon geochronological study of plutonic units along the south Peruvian margin between 17 degrees and 18 degrees S allows the integration of the geochemical, geodynamic and tectonic evolution of this part of the Andean margin. This study focuses on the composite Jurassic-early Cretaceous Ilo Batholith that was emplaced along the southern Peruvian coast during two episodes of intrusive magmatism; a first period between 173 and 152 Ma (with a peak in magmatic activity between roughly 168 and 162 Ma) and a second period between 110 and 106 Ma. Emplacement of the Jurassic part of the composite Ilo Batholith shortly post-dated the accumulation of the volcanosedimentary succession it intruded (Chocolate formation), which allows to estimate a subsidence rate for this unit of similar to 3.5 km/Ma. The emplacement of the main peak of Jurassic plutonism of the Ilo Batholith was also closely coeval with widespread and repeated slumping (during deposition of the Cachios Formation) in the back-arc region, suggesting a common causal link between these phenomena, which is discussed in the context of an observed 100 km trenchward arc migration at similar to 175 Ma, and the relation with extensional tectonics that prevailed along the Central Andean margin during Pangaea break-up. Résumé : La première partie de ce travail de thèse est consacrée au canal à sodium épithélial (ENaC), l'élément clé du transport transépithélial de Na+ dans le néphron distal, le colon et les voies aériennes. Ce canal est impliqué dans certaines formes génétiques d'hypo- et d'hypertension (PHA I, syndrome de Liddle), mais aussi, indirectement, dans la mucoviscidose. La réabsorption transépithéliale de Na+ est principalement régulée par des hormones (aldostérone, vasopressine), mais aussi directement par le Na+, via deux phénomènes distincts, la « feedback inhibition » et la « self-inhibition » (SI). Ce second phénomène est dépendant de la concentration de Na+ extracellulaire, et montre une cinétique rapide (constante de temps d'environ 3 s). Son rôle physiologique serait d'assurer l'homogénéité de la réabsorption de Na+ et d'empêcher que celle-ci soit excessive lorsque les concentrations de Na+ sont élevées. Différents éléments appuient l'hypothèse de la présence d'un site de détection de la concentration du Na+ extracellulaire sur ENaC, gouvernant la SI. L'objectif de ce premier projet est de démontrer l'existence du site de détection impliqué dans la SI et de déterminer ses propriétés physiologiques et sa localisation. Nous avons montré que les caractéristiques de la SI (en termes de sélectivité et affinité ionique) sont différentes des propriétés de conduction du canal. Ainsi, nos résultats confirment l'hypothèse de l'existence d'un site de détection du Na+ (responsable de la transmission de l'information au mécanisme de contrôle de l'ouverture du canal), différent du site de conduction. Par ailleurs, ce site présente une affinité basse et indépendante du voltage pour le Na+ et le Li+ extracellulaires. Le site semble donc être localisé dans le domaine extracellulaire, plutôt que transmembranaire, de la protéine. L'étape suivante consiste alors à localiser précisément le site sur le canal. Des études précédentes, ainsi que des résultats préliminaires récemment obtenus, mettent en avant le rôle dans la self-inhibition du premiers tiers des boucles extracellulaires des sous-unités α et γ du canal. Le second projet tire son origine des limitations de la méthode classique pour l'étude des canaux ioniques, après expression dans les ovocytes de Xenopus laevis, par la méthode du voltage-clamp à deux électrodes, en particulier les limitations dues à la lenteur des échanges de solutions. En outre, cette méthode souffre de nombreux désavantages (manipulations délicates et peu rapides, grands volumes de solution requis). Plusieurs systèmes améliorés ont été élaborés, mais aucun ne corrige tous les désavantages de la méthode classique Ainsi, l'objectif ici est le développement d'un système, pour l'étude électrophysiologique sur ovocytes, présentant les caractéristiques suivantes : manipulation des cellules facilitée et réduite, volumes de solution de perfusion faibles et vitesse rapide d'échange de la perfusion. Un microsystème intégré sur une puce a été élaboré. Ces capacités de mesure ont été testées en utilisant des ovocytes exprimant ENaC. Des résultats similaires (courbes IV, courbes dose-réponse au benzamil) à ceux obtenus avec le système traditionnel ont été enregistrés avec le microsystème. Le temps d'échange de solution a été estimé à ~20 ms et des temps effectifs de changement ont été déterminés comme étant 8 fois plus court avec le nouveau système comparé au classique. Finalement, la SI a été étudiée et il apparaît que sa cinétique est 3 fois plus rapide que ce qui a été estimé précédemment avec le système traditionnel et son amplitude de 10 à 20 % plus importante. Le nouveau microsystème intégré apparaît donc comme adapté à la mesure électrophysiologique sur ovocytes de Xenopus, et possèdent des caractéristiques appropriées à l'étude de phénomènes à cinétique rapide, mais aussi à des applications de type « high throughput screening ». Summary : The first part of the thesis is related to the Epithelial Sodium Channel (ENaC), which is a key component of the transepithelial Na+ transport in the distal nephron, colon and airways. This channel is involved in hypo- and hypertensive syndrome (PHA I, Liddle syndrome), but also indirectly in cystic fibrosis. The transepithelial reabsorption of Na+ is mainly regulated by hormones (aldosterone, vasopressin), but also directly by Na+ itself, via two distinct phenomena, feedback inhibition and self-inhibition. This latter phenomenon is dependant on the extracellular Na+ concentration and has rapid kinetics (time constant of about 3 s). Its physiological role would be to prevent excessive Na+ reabsorption and ensure this reabsorption is homogenous. Several pieces of evidence enable to propose the hypothesis of an extracellular Na+ sensing site on ENaC, governing self-inhibition. The aim of this first project is to demonstrate the existence of the sensing site involved in self-inhibition and to determine its physiological properties and localization. We show self-inhibition characteristics (ionic selectivity and affinity) are different from the conducting properties of the channel. Our results support thus the hypothesis that the Na+ sensing site (responsible of the transmission of the information about the extracellular Na+ concentration to the channel gating mechanism), is different from the channel conduction site. Furthermore, the site has a low and voltage-insensitive affinity for extracellular Na+ or Li+. This site appears to be located in the extracellular domain rather than in the transmembrane part of the channel protein. The next step is then to precisely localize the site on the channel. Some previous studies and preliminary results we recently obtained highlight the role of the first third of the extracellular loop of the α and γ subunits of the channel in self-inhibition. The second project originates in the limitation of the classical two-electrode voltageclamp system classically used to study ion channels expressed in Xenopus /aevis oocytes, in particular limitations related to the slow solution exchange time. In addition, this technique undergoes several drawbacks (delicate manipulations, time consumption volumes). Several improved systems have been built up, but none corrected all these detriments. The aim of this second study is thus to develop a system for electrophysiological study on oocytes featuring an easy and reduced cell handling, small necessary perfusion volumes and fast fluidic exchange. This last feature establishes the link with the first project, as it should enable to improve the kinetics analysis of self-inhibition. A PDMS chip-based microsystem has been elaborated. Its electrophysiological measurement abilities have been tested using oocytes expressing ENaC. Similar measurements (IV curves of benzamil-sensitive currents, benzamil dose-response curves) have been obtained with this system, compared to the traditional one. The solution exchange time has been estimated at N20 ms and effective exchange times (on inward currents) have been determined as 8 times faster with the novel system compared to the classical one. Finally, self-inhibition has been studied and it appears its kinetics is 3 times faster and its amplitude 10 to 20 % higher than what has been previously estimated with the traditional system. The novel integrated microsystem appears therefore to be convenient for electrophysiological measurement on Xenopus oocytes, and displays features suitable for the study of fast kinetics phenomenon, but also high throughput screening applications. Résumé destiné large public : Le corps humain est composé d'organes, eux-mêmes constitués d'un très grand nombre de cellules. Chaque cellule possède une paroi appelée membrane cellulaire qui sépare l'intérieur de cette cellule (milieu intracellulaire) du liquide (milieu extracellulaire) dans lequel elle baigne. Le maintien de la composition stable de ce milieu extracellulaire est essentiel pour la survie des cellules et donc de l'organisme. Le sodium est un des composants majeurs du milieu extracellulaire, sa quantité dans celui-ci doit être particulièrement contrôlée. Le sodium joue en effet un rôle important : il conditionne le volume de ce liquide extracellulaire, donc, par la même, du sang. Ainsi, une grande quantité de sodium présente dans ce milieu va de paire avec une augmentation du volume sanguin, ce qui conduit l'organisme à souffrir d'hypertension. On se rend donc compte qu'il est très important de contrôler la quantité de sodium présente dans les différents liquides de l'organisme. Les apports de sodium dans l'organisme se font par l'alimentation, mais la quantité de sodium présente dans le liquide extracellulaire est contrôlée de manière très précise par le rein. Au niveau de cet organe, on appelle urine primaire le liquide résultant de la filtration du sang. Elle contient de nombreuses substances, des petites molécules, dont l'organisme a besoin (sodium, glucose...), qui sont ensuite récupérées dans l'organe. A la sortie du rein, l'urine finale ne contient plus que l'excédent de ces substances, ainsi que des déchets à éliminer. La récupération du sodium est plus ou moins importante, en fonction des ajustements à apporter à la quantité présente dans le liquide extracellulaire. Elle a lieu grâce à la présence de protéines, dans les membranes des cellules du rein, capables de le transporter et de le faire transiter de l'urine primaire vers le liquide extracellulaire, qui assurera ensuite sa distribution dans l'ensemble de l'organisme. Parmi ces protéines « transporteurs de sodium », nous nous intéressons à une protéine en particulier, appelée ENaC. Il a été montré qu'elle jouait un rôle important dans cette récupération de sodium, elle est en effet impliquée dans des maladies génétiques conduisant à l'hypo- ou à l'hypertension. De précédents travaux ont montré que lorsque le sodium est présent en faible quantité dans l'urine primaire, cette protéine permet d'en récupérer une très grande partie. A l'inverse, lorsque cette quantité de sodium dans l'urine primaire est importante, sa récupération par le biais d'ENaC est réduite. On parle alors d'autorégulation : la protéine elle-même est capable d'adapter son activité de transport en fonction des conditions. Ce phénomène d'autorégulation constitue a priori un mécanisme préventif visant à éviter une trop grande récupération de sodium, limitant ainsi les risques d'hypertension. La première partie de ce travail de thèse a ainsi consisté à clarifier le mécanisme d'autorégulation de la protéine ENaC. Ce phénomène se caractérise en particulier par sa grande vitesse, ce qui le rend difficile à étudier par les méthodes traditionnelles. Nous avons donc, dans une deuxième partie, développé un nouveau système permettant de mieux décrire et analyser cette « autorégulation » d'ENaC. Ce second projet a été mené en collaboration avec l'équipe de Martin Gijs de l'EPFL.

INTRODUCTION: The importance of the micromovements in the mechanism of aseptic loosening is clinically difficult to evaluate. To complete the analysis of a series of total knee arthroplasties (TKA), we used a tridimensional numerical model to study the micromovements of the tibial implant. MATERIAL AND METHODS: Fifty one patients (with 57 cemented Porous Coated Anatomic TKAs) were reviewed (mean follow-up 4.5 year). Radiolucency at the tibial bone-cement interface was sought on the AP radiographs and divided in 7 areas. The distribution of the radiolucency was then correlated with the axis of the lower limb as measured on the orthoradiograms. The tridimensional numerical model is based on the finite element method. It allowed the measurement of the cemented prosthetic tibial implant's displacements and the micromovements generated at bone-ciment interface. A total load (2000 Newton) was applied at first vertically and asymetrically on the tibial plateau, thereby simulating an axial deviation of the lower limbs. The vector's posterior inclination then permitted the addition of a tangential component to the axial load. This type of effort is generated by complex biomechanical phenomena such as knee flexion. RESULTS: 81 per cent of the 57 knees had a radiolucent line of at least 1 mm, at one or more of the tibial cement-epiphysis jonctional areas. The distribution of these lucent lines showed that they came out more frequently at the periphery of the implant. The lucent lines appeared most often under the unloaded margin of the tibial plateau, when axial deviation of lower limbs was present. Numerical simulations showed that asymetrical loading on the tibial plateau induced a subsidence of the loaded margin (0-100 microns) and lifting off at the opposite border (0-70 microns). The postero-anterior tangential component induced an anterior displacement of the tibial implant (160-220 microns), and horizontal micromovements with non homogenous distribution at the bone-ciment interface (28-54 microns). DISCUSSION: Comparison of clinical and numerical results showed a relation between the development of radiolucent lines and the unloading of the tibial implant's margin. The deleterious effect of lower limbs' axial deviation is thereby proven. The irregular distribution of lucent lines under the tibial plateau was similar of the micromovements' repartition at the bone-cement interface when tangential forces were present. A causative relation between the two phenomenaes could not however be established. Numerical simulation is a truly useful method of study; it permits to calculate micromovements which are relative, non homogenous and of very low amplitude. However, comparative clinical studies remain as essential to ensure the credibility of results.

One hundred de novo multiple myeloma patients with t(4;14) treated with double intensive therapy according to IFM99 protocols were retrospectively analyzed. The median overall survival (OS) and event-free survival (EFS) were 41.4 and 21 months, respectively, as compared to 65 and 37 for patients included in the IFM99 trials without t(4;14) (P<10(-7)). We identified a subgroup of patients presenting at diagnosis with both low beta(2)-microglobulin <4 mg/l and high hemoglobin (Hb) >/=10 g/l (46% of the cases) with a median OS of 54.6 months and a median EFS of 26 months, respectively, which benefits from high-dose therapy (HDT); conversely patients with one or both adverse prognostic factor (high beta(2)-microglobulin and/or low Hb) had a poor outcome. The achievement of either complete response or very good partial response after HDT was also a powerful independent prognostic factor for both OS and EFS.

OBJECTIVES: To evaluate the socio-demographic as well as the health and psychiatric profiles of adolescents hospitalised for suicide attempt or overwhelming suicide ideation and to assess repetition of suicide attempt over a period of 18 months. PATIENTS AND METHODS: Between April 2000 and September 2001, all patients aged 16 to 21 years admitted to the University Hospitals of Geneva and Lausanne for suicide attempt or ideation were included in the study. At this time (T0) semi-structured face to face interviews were conducted to identify socio-demographic data, mental health and antecedents regarding suicidal conducts. Current psychiatric status was assessed with the MINI (Mini International Neuropsychiatric Instrument). At T1 and T2, reassessments included psychiatric status (MINI) as well as lifestyles, socio-professional situation and suicidal behaviours. RESULTS: At T0, 269 subjects met the study criteria, among whom 83 subjects (56 girls and 27 boys) left the hospital too quickly to be involved or refused to participate in the study (final sample at T0: 149 girls; 37 boys). The participation rate at T1 and T2 was respectively 66% and 62% of the original sample. The percentage of adolescents meeting the criteria for psychiatric diagnoses (91%) was high: affective disorder (78%); anxiety disorder (64%); substance use disorder (39%); eating disorder (9%); psychotic disorder (11%); antisocial personality (7%) with most subjects (85%) having more than one disorder. Around 90% of the subjects interviewed at T1, and/or T2, had received follow-up care after their hospitalisation, either by a primary care physician or a psychotherapist or both. Two subjects died of violent death and 18% made a further suicide attempt. CONCLUSION: Most adolescents hospitalised for suicidal episodes suffer from psychiatric problems which should be addressed by a careful psychiatric assessment, followed up if needed by a structured after care plan.

Chronic kidney disease (CKD) is an important public health problem with a genetic component. We performed genome-wide association studies in up to 130,600 European ancestry participants overall, and stratified for key CKD risk factors. We uncovered 6 new loci in association with estimated glomerular filtration rate (eGFR), the primary clinical measure of CKD, in or near MPPED2, DDX1, SLC47A1, CDK12, CASP9, and INO80. Morpholino knockdown of mpped2 and casp9 in zebrafish embryos revealed podocyte and tubular abnormalities with altered dextran clearance, suggesting a role for these genes in renal function. By providing new insights into genes that regulate renal function, these results could further our understanding of the pathogenesis of CKD.

The cellular response to fasting and starvation in tissues such as heart, skeletal muscle, and liver requires peroxisome proliferator-activated receptor-alpha (PPARalpha)-dependent up-regulation of energy metabolism toward fatty acid oxidation (FAO). PPARalpha null (PPARalphaKO) mice develop hyperinsulinemic hypoglycemia in the fasting state, and we previously showed that PPARalpha expression is increased in islets at low glucose. On this basis, we hypothesized that enhanced PPARalpha expression and FAO, via depletion of lipid-signaling molecule(s) for insulin exocytosis, are also involved in the normal adaptive response of the islet to fasting. Fasted PPARalphaKO mice compared with wild-type mice had supranormal ip glucose tolerance due to increased plasma insulin levels. Isolated islets from the PPARalpha null mice had a 44% reduction in FAO, normal glucose use and oxidation, and enhanced glucose-induced insulin secretion. In normal rats, fasting for 24 h increased islet PPARalpha, carnitine palmitoyltransferase 1, and uncoupling protein-2 mRNA expression by 60%, 62%, and 82%, respectively. The data are consistent with the view that PPARalpha, via transcriptionally up-regulating islet FAO, can reduce insulin secretion, and that this mechanism is involved in the normal physiological response of the pancreatic islet to fasting such that hypoglycemia is avoided.

Atraumatic osteonecrosis of the talus can be extremely painful and lead to significant functional impairment. Although clinical, radiographic, and demographic characteristics of atraumatic osteonecrosis of the talus have been well documented, the diagnosis is frequently missed or delayed; the most common causes are use of corticosteroids and the presence of immune disorders. Operative treatment of large osteochondral lesions of the talus is difficult because the blood supply is poor in the talar dome. Microvascular reconstruction of the talar dome with iliac crest autografts is a complex but functionally excellent therapeutic option. We present a 48-year-old man, who developed an extensive atraumatic avascular necrosis of the talar dome without collapse. Except for insulin dependent diabetes mellitus no further comorbidities were known. A microvascular iliac crest bone flap was inserted into the talus. A follow-up 16 years postoperatively showed a clinically as well as radiographically stable reconstruction of the talar dome and an excellent mobility of the ankle joint. The AOFAS hindfoot scale had improved from initially 33 points to 100 on the last follow-up. Free microvascular bony reconstruction of the talar dome should not only be considered in younger patients but also for middle aged active patients, since our follow-up shows an excellent long term result. Early reconstruction can prevent collapse of the talar bone.
