Les questions de mal-être au travail et de risques psychosociaux sont centrales dans le monde du travail actuel. L'ergonomie nous enseigne qu'une analyse des formes d'organisation du travail est nécessaire pour comprendre les origines des plaintes des travailleurs. Dans cette recherche, nous avons étudié une population spécifique, les employés du siège d'une organisation non gouvernementale humanitaire en Suisse (150 personnes environ). Partant du constat de l'expression de nombreuses plaintes de fatigue et de mal-être au travail par ces employés, nous avons cherché à objectiver ces plaintes et avons étudié les éléments d'organisation susceptibles d'y être liés.Dans le cadre d'une démarche inductive, nous avons utilisé conjointement des méthodes qualitatives (entretiens semi-directifs, observation non participante) et quantitatives (questionnaires) et avons appliqué le principe de triangulation méthodologique pour l'analyse des données.Trois thématiques relatives au mode de fonctionnement de l'organisation ont émergé des analyses : le fonctionnement dans l'urgence en permanence, la professionnalisation de l'organisation et le fort engagement des employés dans leur activité.L'organisation fonctionne selon une logique d'urgence en permanence. La nature de ses interventions, qui sont considérées de la plus haute importance, et la forte valorisation des activités « de terrain » conduisent à l'enfermement de l'activité dans la gestion des tâches urgentes, ce qui ne laisse pas de place à l'anticipation et à la planification. Ensuite, cette organisation a connu une forte croissance au cours des dernières décennies : d'une petite association, elle est devenue une véritable institution. La logique de fonctionnement institutionnelle n'est cependant pas venue remplacer totalement la logique associative, mais toutes deux cohabitent. L'organisation continue à demander à ses employés un engagement associatif et militant total mais applique parallèlement un management de type entrepreneurial, ce qui génère des messages paradoxaux pour les employés. Finalement, le fonctionnement de l'organisation dans son ensemble repose sur l'engagement de toute la chaîne des acteurs. Ceci est clairement intégré et repris par les employés du siège : ils relèvent que ce qu'ils font représente bien plus qu'un travail et qu'il est nécessaire de « tout donner », car les besoins sont immenses et la charge de travail énorme. Cependant, la limite entre engagement et surengagement conduisant à l'épuisement est très facilement franchie.Ces éléments participent à générer un environnement de travail potentiellement pathogène, où il est peu légitime pour l'employé de se préoccuper de sa propre santé, et où la charge de travail et le rythme ne lui laissent pas le temps de mettre en place des stratégies de préservation de sa santé. Ce mode de fonctionnement spécifique véhicule des messages paradoxaux et soumet les employés à des exigences contradictoires.

The Western Cordillera of Ecuador consists of Cretaceous crustal fragments of oceanic plateaux and superimposed insular arcs, which were accreted to the northwestern South American margin during the Late Cretaceous and Paleocene. Slices of high-grade metabasites, ultramafic rocks, gabbros and basalts, unmetamorphosed radiolarian cherts and scarce garnet-bearing metasediments were randomly exhumed along Miocene to Recent transcurrent faults crosscutting the Western Cordillera. The basalts show geochemical characteristics of oceanic plateau basalts (flat REE patterns, La/Nb = 0.85). The gabbros differ from the basalts in having lower REE levels, positive Eu anomalies, and negative Nb and Ta anomalies; they are interpreted as resulting from arc magmatism. The amphibolites and banded amphibolites have major and trace element chemistry similar to that of oceanic plateau basalts (flat REE patterns, La/Nb = 0.86) or to cumulate gabbros. The granulite shares with oceanic plateaus similar trace element chemistry (flat REE patterns, La/Nb < 1) and epsilon(Ndi) values (+7.6). Continent-derived metasediments are depleted in heavy REE (La/Y = 4.8) and have a negative Eu anomaly. Foliated Iherzolites, melagabbronorites and pyroxenites consist of serpentinized olivine + cpx + opx +/- Ca-plagioclase. Lherzolites, melagabbronorites and pyroxenites are LREE depleted with positive Eu anomalies, while the harzburgite displays a U-shaped REE pattern. The trace element abundances of the ultramafic rocks are very low (0.1 to 1 times the chonctritic and primitive mantle values). The ultramafic rocks represent fragments of depleted mantle, deformed cpx-rich cumulate, and continental lithospheric mantle or mantle contaminated by subduction-fluid. Except the scarce quartz-rich metasediments, all these rocks likely represent remnants of accreted oceanic crustal fragments and associated depleted mantle. Since these samples were randomly sampled at depth by the fault, we propose that the Western Cordillera and its crustal root are mainly of oceanic nature.

OBJECTIVE: To determine the sensitivity of ultrasonography in screening for foetal malformations in the pregnant women of the Swiss Canton of Vaud. STUDY DESIGN: Retrospective study over a period of five years. METHOD: We focused our study on 512 major or minor clinically relevant malformations detectable by ultrasonography. We analysed the global sensitivity of the screening and compared the performance of the tertiary centre with that of practitioners working in private practice or regional hospitals. RESULTS: Among the 512 malformations, 181 (35%) involved the renal and urinary tract system, 137 (27%) the heart, 71 (14%) the central nervous system, 50 (10%) the digestive system, 42 (8%) the face and 31 (6%) the limbs. Global sensitivity was 54.5%. The lowest detection rate was observed for cardiac anomalies, with only 23% correct diagnoses. The tertiary centre achieved a 75% detection rate in its outpatient clinic and 83% in referred patients. Outside the referral centre, the diagnostic rate attained 47%. CONCLUSIONS: Routine foetal examination by ultrasonography in a low-risk population can detect foetal structural abnormalities. Apart from the diagnosis of cardiac abnormalities, the results in the Canton of Vaud are satisfactory and justify routine screening for malformations in a low-risk population. A prerequisite is continuing improvement in the skills of ultrasonographers through medical education.

Ce rapport présente plusieurs axes relatifs aux jeux de hasard et d'argent et au jeu problématique. Comment comprendre l'évolution de ce type de problématique dans une société? C'est au travers d'une analyse sur les représentations sociales en Suisse romande que ce travail propose une piste de réflexion et ceci dans le but de dresser un état des lieux de ces thématiques telles qu'elles existent dans les consciences collectives. Les représentations sociales ont fait l'objet de nombreuses études en Amérique du Nord mais sont par contre plutôt rares en Suisse. Nous avons pu constater au fil de ce travail que les représentations sont teintées de prudence; la population générale sollicitée ne semble pas ignorer quels sont les écueils pouvant être générés par ces pratiques; mais, en revanche, d'autres résultats nous ont permis de comprendre que le jeu problématique n'est pas encore intégré par l'ensemble de la population puisque 42.6% n'en ont jamais entendu parler. Il y a donc une brèche à exploiter en la matière tant d'un point de vue préventif que scientifique. Afin d'apporter une réalité supplémentaire et concrète à ces considérations d'ordre subjectif, nous avons agrémenté ce rapport d'analyses propres à l'épidémiologie du jeu en Suisse, et ceci au travers des données relatives aux Enquêtes suisses sur la santé 2002 et 2007. Finalement, un certain nombre d'outils de mesure et de dépistage à l'égard du jeu pathologique ont été sélectionnés dans la littérature internationale, et ceci dans le but de mieux comprendre quels sont les critères privilégiés permettant de déterminer ou diagnostiquer un joueur à risque. Le présent rapport offre donc un éventail de ce que sont les jeux et de ce qu'ils représentent dans notre société et délivre des informations diverses sur leurs appréhensions tant au niveau individuel que médical et social.

Anorectal malformation (ARM) can be divided in high, intermediate, and low forms according to the level of termination of the rectum in relation to the pubococcygeal and ischiatic lines. Patients with Down's syndrome have a high incidence of gastrointestinal anomalies, such as tracheoesophageal fistula, duodenal obstruction, annular pancreas, Hirschsprung's disease, and ARM. In these children, ARM is generally low with or without a fistula. The mode of inheritance of ARM and its genetic relation with Down's syndrome is not known, even if the association (ARM-Down's syndrome) seems not to be coincidental. We describe here a very rare case of monozygotic twins born with the association of ARM and Down's syndrome.

OBJECTIVE: To evaluate antenatal surveillance strategies and the optimal timing of delivery for monoamniotic twin pregnancies. METHODS: Obstetric and perinatal outcomes were retrospectively retrieved for 193 monoamniotic twin pregnancies. Fetal and neonatal outcomes were compared between fetuses followed in an inpatient setting and those undergoing intensive outpatient follow-up from 26 to 28 weeks of gestation until planned cesarean delivery between 32 and 35 weeks of gestation. The risk of fetal death was compared with the risk of neonatal complications. RESULTS: Fetal deaths occurred in 18.1% of fetuses (70/386). Two hundred ninety-five neonates from 153 pregnancies were born alive after 23 weeks of gestation. There were 17 neonatal deaths (5.8%), five of whom had major congenital anomalies. The prospective risk of a nonrespiratory neonatal complication was lower than the prospective risk of fetal death after 32 4/7 weeks of gestation (95% confidence interval 32 0/7-33 4/7). The incidence of death or a nonrespiratory neonatal complication was not significantly different between fetuses managed as outpatients (14/106 [13.2%]) or inpatients (15/142 [10.5%]; P=.55). Our statistical power to detect a difference in outcomes between these groups was low. CONCLUSIONS: The in utero risk of a monoamniotic twin fetus exceeds the risk of a postnatal nonrespiratory complication at 32 4/7 weeks of gestation. If close fetal surveillance is instituted after 26-28 weeks of gestation and delivery takes place at approximately 33 weeks of gestation, the risk of fetal or neonatal death is low, no matter the surveillance setting. OBJECTIVE: To identify pregnancy-related risk factors for different manifestations of congenital anorectal malformations (ARMs). DESIGN: A population-based case-control study. SETTING: Seventeen EUROCAT (European Surveillance of Congenital Anomalies) registries, 1980-2008. POPULATION: The study population consisted of 1417 cases with ARM, including 648 cases of isolated ARM, 601 cases of ARM with additional congenital anomalies, and 168 cases of ARM-VACTERL (vertebral, anal, cardiac, tracheo-esophageal, renal, and limb defects), along with 13 371 controls with recognised syndromes or chromosomal abnormalities. METHODS: Multiple logistic regression analyses were used to calculate adjusted odds ratios (ORs) for potential risk factors for ARM, such as fertility treatment, multiple pregnancy, primiparity, maternal illnesses during pregnancy, and pregnancy-related complications. MAIN OUTCOME MEASURES: Adjusted ORs for pregnancy-related risk factors for ARM. RESULTS: The ARM cases were more likely to be firstborn than the controls (OR 1.6, 95% CI 1.4-1.8). Fertility treatment and being one of twins or triplets seemed to increase the risk of ARM in cases with additional congenital anomalies or VACTERL (ORs ranging from 1.6 to 2.5). Maternal fever during pregnancy and pre-eclampsia were only associated with ARM when additional congenital anomalies were present (OR 3.9, 95% CI 1.3-11.6; OR 3.4, 95% CI 1.6-7.1, respectively), whereas maternal epilepsy during pregnancy resulted in a five-fold elevated risk of all manifestations of ARM (OR 5.1, 95% CI 1.7-15.6). CONCLUSIONS: This large European study identified maternal epilepsy, fertility treatment, multiple pregnancy, primiparity, pre-eclampsia, and maternal fever during pregnancy as potential risk factors primarily for complex manifestations of ARM with additional congenital anomalies and VACTERL.

Abnormal development can lead to deficits in adult brain function, a trajectory likely underlying adolescent-onset psychiatric conditions such as schizophrenia. Developmental manipulations yielding adult deficits in rodents provide an opportunity to explore mechanisms involved in a delayed emergence of anomalies driven by developmental alterations. Here we assessed whether oxidative stress during presymptomatic stages causes adult anomalies in rats with a neonatal ventral hippocampal lesion, a developmental rodent model useful for schizophrenia research. Juvenile and adolescent treatment with the antioxidant N-acetyl cysteine prevented the reduction of prefrontal parvalbumin interneuron activity observed in this model, as well as electrophysiological and behavioral deficits relevant to schizophrenia. Adolescent treatment with the glutathione peroxidase mimic ebselen also reversed behavioral deficits in this animal model. These findings suggest that presymptomatic oxidative stress yields abnormal adult brain function in a developmentally compromised brain, and highlight redox modulation as a potential target for early intervention.

Though rare, anomalous coronary artery disease is a well-known cause of myocardial ischemia and sudden death among children and young adults. The projectional nature of conventional x-ray angiography often leads to difficulty in the definition of anomalous vessels. Studies have now documented the high accuracy of coronary magnetic resonance angiography (MRA) for the noninvasive detection and definition of anomalous coronary arteries among patients with suspected anomalous coronary arteries of congenital conditions associated with anomalous coronary arteries. With increasing clinical experience, coronary MRA will likely emerge as the gold standard for the diagnosis of this condition.

ABSTRACT : Research in empirical asset pricing has pointed out several anomalies both in the cross section and time series of asset prices, as well as in investors' portfolio choice. This dissertation aims to discover the forces driving some of these "puzzling" asset pricing dynamics and portfolio decisions observed in the financial market. Through the dissertation I construct and study dynamic general equilibrium models of heterogeneous investors in the presence of frictions and evaluate quantitatively their implications for financial-market asset prices and portfolio choice. I also explore the potential roots of puzzles in international finance. Chapter 1 shows that, by introducing jointly endogenous no-default type of borrowing constraints and heterogeneous beliefs in a dynamic general-equilibrium economy, many empirical features of stock return volatility can be reproduced. While most of the research on stock return volatility is empirical, this paper provides a theoretical framework that is able to reproduce simultaneously the cross section and time series stylized facts concerning stock returns and their volatility. In contrast to the existing theoretical literature related to stock return volatility, I don't impose persistence or regimes in any of the exogenous state variables or in preferences. Volatility clustering, asymmetry in the stock return-volatility relationship, and pricing of multi-factor volatility components in the cross section all arise endogenously as a consequence of the feedback between the binding of no-default constraints and heterogeneous beliefs. Chapters 2 and 3 explore the implications of differences of opinion across investors in different countries for international asset pricing anomalies. Chapter 2 demonstrates that several international finance "puzzles" can be reproduced by a single risk factor which captures heterogeneous beliefs across international investors. These puzzles include: (i) home equity preference; (ii) the dependence of firm returns on local and foreign factors; (iii) the co-movement of returns and international capital flows; and (iv) abnormal returns around foreign firm cross-listing events in the local market. These are reproduced in a setup with symmetric information and in a perfectly integrated world with multiple countries and independent processes producing the same good. Chapter 3 shows that by extending this framework to multiple goods and correlated production processes; the "forward premium puzzle" arises naturally as a compensation for the heterogeneous expectations about the depreciation of the exchange rate held by international investors. Chapters 2 and 3 propose differences of opinion across international investors as the potential resolution of several international finance `puzzles'. In a globalized world where both capital and information flow freely across countries, this explanation seems more appealing than existing asymmetric information or segmented markets theories aiming to explain international finance puzzles.

The Polochic and Motagua faults define the active plate boundary between the North American and Caribbean plates in central Guatemala. A splay of the Polochic Fault traverses the rapidly growing city of San Miguel Uspantan that is periodically affected by destructive earthquakes. This fault splay was located using a 2D electrical resistivity tomography (ERT) survey that also characterized the fault damage zone and evaluated the thickness and nature of recent deposits upon which most of the city is built. ERT images show the fault as a similar to 50 m wide, near-vertical low-resistivity anomaly, bounded within a few meters by high resistivity anomalies. Forward modeling reproduces the key aspects of the observed electrical resistivity data with remarkable fidelity thus defining the overall location, geometry, and internal structure of the fault zone as well as the affected lithologies. Our results indicate that the city is constructed on a similar to 20 m thick surficial layer consisting of poorly consolidated, highly porous, water-logged pumice. This soft layer is likely to amplify seismic waves and to liquefy upon moderate to strong ground shaking. The electrical conductivity as well as the major element chemistry of the groundwater provides evidence to suggest that the local aquifer might, at least in part, be fed by water rising along the fault. Therefore, the potential threat posed by this fault splay may not be limited to its seismic activity per se, but could be compounded its potential propensity to enhance seismic site effects by injecting water into the soft surficial sediments. The results of this study provide the basis for a rigorous analysis of seismic hazard and sustainable development of San Miguel Uspantan and illustrate the potential of ERT surveying for paleoseismic studies.

RESUME GENERAL Au cours de ces dernières années, le monoxyde d'azote (NO) produit par une famille d'enzymes, les NO synthases (NOS), est apparu comme un effecteur central dans la régulation du système cardiovasculaire et du métabolisme énergétique. Chez l'homme, un défaut de production du NO est associé à des maladies cardiovasculaires et métaboliques comme la résistance à l'insuline ou le diabète de type 2. Ces pathologies se retrouvent chez les souris invalidées pour la NO synthase endothéliale (eN0S-/-) qui présentent non seulement une hypertension mais également une résistance à l'insuline et une dyslipidémie (augmentation des triglycérides et des acides gras libres). Ces anomalies sont étroitement associées et impliquées dans le développement du diabète de type 2. Dans cette étude, nous avons essayé de déterminer à partir du modèle de souris eN0S-/-, l'influence de la eNOS et de son produit, le NO, sur la régulation du métabolisme lipidique intracellulaire. Ainsi, nous avons montré que cette enzyme et le NO régulent directement l'activité β-oxydative des mitochondries isolées du muscle squelettique, du muscle cardiaque et du tissu adipeux blanc. Par ailleurs, dans le muscle de ces souris, le contenu des mitochondries et l'expression des gènes impliqués dans leur biogénèse sont diminués, ce qui suggère que la eNOS et/ou le NO contrôlent également la synthèse de ces organelles. Les mitochondries, via la β-oxydation, sont impliquées dans la production d'énergie à partir des acides gras libres. Dans notre modèle animal, la diminution de la β-oxydation dans le muscle, s'accompagne d'une accumulation des triglycérides intramyocellulaires. Cette accumulation prédispose fortement au développement de la résistance à l'insuline. Les anomalies du métabolisme β-oxydatif favorisent donc probablement l'apparition de la dyslipidémie et le développement de la résistance à l'insuline observées chez les souris eN0S-/-. Cette hypothèse est soutenue par différentes études effectuées chez l'homme et l'animal qui suggèrent qu'une dysfonction mitochondriale peut être à l'origine de la résistance à l'insuline. Ces données récentes et les résultats de ce travail apportent un regard nouveau sur le rôle du NO dans le développement des maladies métaboliques que sont la résistance à l'insuline, le diabète de type 2 et l'obésité. Elles placent aux centres de ces mécanismes une organelle, la mitochondrie, située au carrefour des métabolismes glucidiques et lipidiques. SUMMARY Over the last years, nitric oxide (NO), synthesized by a family of enzymes, the NO synthases, has become a central regulator of the cardiovascular system and energy metabolism. In humans, defective NO production is found in cardiovascular and metabolic diseases such as insulin resistance or type 2 diabetes mellitus. These alterations are also found in knockout mice for the endothelial nitric oxide synthase (eN0S-/-), which are not only hypertensive but also display insulin resistance and dyslipidemia (with increased triglyceride and free fatty acid levels). These pathologic features are tightly linked and involved in the pathogenesis of type 2 DM. In this study, using eN0S-/- mice, we determined the role played by this enzyme and its product, NO, on intracellular lipid metabolism. We show that eNOS and NO directly regulate β-oxidation in mitochondria isolated from skeletal and cardiac muscle as well as white adipose tissue. Furthermore, in the skeletal muscle of these mice, the mitochondrial content and the expression of genes involved in mitochondrial biogenesis are decreased, suggesting that eNOS and/or NO also regulate the synthesis of this intracellular organelle. Mitochondria, through β-oxidation, play a role in energy production from free fatty acids. In our animal model, decreased β-oxidation in skeletal muscle is associated with accumulation of intramyocellular lipids. This increased lipid content plays an important role in the pathogenesis of insulin resistance. Defective β-oxidation, therefore, probably favours the development of insulin resistance and dyslipidemia as seen in these animals. This hypothesis is strengthened by studies in humans and animals indicating that mitochondrial dysfunction is associated with insulin resistance. These recent data and the results of this work provide evidence for a role of NO in the development of metabolic diseases such as insulin resistance or type diabetes mellitus. They put as a central player, an organelle, the mitochondria, which lies at the crossway of carbohydrate and lipid metabolism. RESUME DIDACTIQUE Le maintien des fonctions vitales et l'accomplissement d'une activité physique nécessitent, chez l'homme, un apport quotidien d'énergie. Cette énergie est présente, dans l'alimentation, principalement sous forme de graisses (lipides) ou de sucres. La production d'énergie s'effectue en majorité dans le muscle au niveau d'une organelle particulière, la mitochondrie. La régulation du métabolisme énergétique fait intervenir de nombreux facteurs de régulation dont l'un des plus connu est l'insuline. De nombreuses maladies comme le diabète de type 2, l'obésité ou le syndrome métabolique découlent de la dérégulation du métabolisme énergétique. Un mécanisme particulier, la résistance à l'insuline, qui se caractérise par un défaut d'action de l'insuline au niveau de ses tissus cibles (foie, muscle...) est souvent impliqué dans le développement de ces pathologies. L'étude de ces anomalies métaboliques nécessite l'utilisation de modèles, notamment animaux, qui ont la particularité de reproduire partiellement un état pathologique caractéristique de certaines maladies humaines. Dans ce travail, nous avons utilisé un modèle de souris dont la particularité est de ne pas exprimer une enzyme, la monoxyde d'azote (NO) synthase endothéliale (eNOS), responsable de la synthèse d'un gaz, le NO. Ces souris présentent une hypertension artérielle, des anomalies du métabolisme des lipides et une résistance à l'insuline. Or, de récents travaux effectués chez l'homme montrent que des individus insulino-résistants ou diabétiques de type 2 ont une diminution de la production de NO. Lors de nos investigations, nous avons démontré que la quantité et la capacité des mitochondries à utiliser les lipides comme substrat énergétique est diminuée dans les muscles des souris eN0S-/-. Par ailleurs, ces deux anomalies sont associées dans ce tissu à une accumulation des lipides. De façon très intéressante, ce phénomène est décrit dans de nombreuses études effectuées chez l'homme et l'animal comme favorisant le développement de la résistance à l'insuline. Les résultats de ce travail suggèrent donc que la eNOS et/ou le NO joue un rôle important dans l'activité et la synthèse des mitochondries. Le NO pourrait donc constituer une cible thérapeutique dans le traitement des maladies métaboliques.

Methods Ten patients with aniridia from 3 families of Egyptian origin underwent full ophthalmologic, general and neurological examination, and blood drawing. Cerebral MRI was performed in the index case of each family. Genomic DNA was prepared from venous leukocytes and direct sequencing of all the exons and intron-exon junctions of the PAX6 gene was performed after PCR amplification. Results Common features observed in the three families included absence of iris tissue, corneal pannus with different degrees of severity and foveal hypoplasia with severely reduced visual acuity. In families 2 and 3, additional findings such as lens dislocation, lens opacities or polar cataract and glaucoma were observed. We identified two novel (c.170-174delTGGGC [p.L57fs17] and c.475delC [p.R159fs47]) and one known (c.718C>T) PAX6 mutations in the affected members of the 3 families. Systemic and neurological examination was normal in all ten affected patients. Cerebral MRI showed absence of the pineal gland in all three index patients. Severe hypoplasia of the brain anterior commissure was associated to the p.L57fs17mutation, absence of the posterior commissure to both p.R159fs47 and p.R240X, and optic chiasma atrophy and almost complete agenesis of the corpus callosum to p.R240X. Conclusions We identified two novel PAX6 mutations in families with severe aniridia from Northern Egypt, an ethnic group which is not well studied. In addition to common phenotype of aniridia and despite normal neurological examination, absence of the pineal gland was observed in all 3 index patients. The heterogeneity of brain anomalies related to PAX6 mutations is underexplored and is highlighted in this study.

Compilation of the recent literature from the Southern Alps and adjacent area confirms the geochemical variations of unusual amplitudes during the Permian-Triassic boundary interval (PTBI). A great attention has been given to the negative δ13C anomaly within the Tesero Member close to the Permian-Triassic boundary. Very detailed geochemical works have been done on the scientific Gartnerkofel core (Gk-1) and on the Slovenian sections. Major minor and rare earth elements (REE) data are reported and show a marked enrichment in alkaline metals and REE of some levels of the boundary interval. But recent studies show that the low Iridium anomalies and the Osmium and Helium isotopes anomalies lack the characteristics of a large extraterrestrial impact.
