La définition des interfaces pour améliorer la qualité: l'exemple de l'anorexie en hospitalisation somatique [Defining interfaces to improve quality of care: the example of anorexia nervosa in acute somatic hospitalisation]

Specialisation in medicine requires multidisciplinary approaches, and hence coordination in collaborations of the different partners involved. These integrated approaches, sometimes called "disease management", fit particularly well to chronic diseases. Our institution introduced an integrated approach for taking care of the acute somatic hospitalisation of patients suffering from anorexia nervosa. Interfaces with the different partners were defined, specifying tasks, rights, and duties of each person, care givers or patients. This initiative allows now to identify any deviation occurring in the process of care or hole in the care system, so that it can be corrected and recurrence prevented. This model will be extended to other complex and multidisciplinary care processes and other services in our institution.

Poumon et grossesse [Lung and pregnancy].

During pregnancy several adaptations develop in response to the enhanced maternal and fetal metabolic needs. This review summarizes the major cardiorespiratory modifications of pregnancy as well as their consequences in chronic respiratory diseases such as restrictive ventilatory defects (post-tuberculosis pneumonectomy, kyphoscoliosis, neuromuscular disorders), asthma, cystic fibrosis, and pulmonary hypertension. It is important to recognize early the cardiorespiratory situations for which pregnancy is contraindicated or associated with a high risk of respiratory complications. Clinical management by an expert and often pluridisciplinary team is recommended.

Pharmacovigilance [Pharmacovigilance update]

The observations of pharmacovigilance reported during 2007 reflect an increasing attention towards drug-induced augmentation of the incidence of common disorders. New substances are thus to be added to the list of risk factors susceptible to favour cardiovascular events (tegaserod, rosiglitazone, erythropoïetin, aprotinine) or psychiatric disorders (dopaminergic agonists, rimonabant). The evaluation of the security profile of new medicines remains challenging. Besides biological investigations of questionable relevance and clinical trial of inconstant efficiency towards safety outcomes, the role of pharmacovigilance notifications by practitioners remains of paramount importance.

IRM cardiaque dans le suivi des cardiopathies congénitales à l'âge adulte [Cardiac MRI in the follow-up of adult congenital cardiomyopathy patients].

Magnetic resonance imaging is a rapidly developing modality in cardiology. It offers an excellent image definition and a large field of view, allowing a more accurate morphological assessment of cardiac malformations. Due to its unique versatility and its ability to provide myocardial tissue characterization, cardiac magnetic resonance (CMR) is now recognized as a central imaging modality for a wide range of congenital heart diseases, including assessment of post-surgical cardiac anatomy, quantification of valvular disease and detection of myocardial ischemia. CMR provides useful diagnostic information without any radiation exposure, and improves the global management of patients with congenital heart disease.

Tabagisme et rein [Smoking and the kidney]

Tobacco consumption is a major public health problem. More than 20 years ago smoking has been identified to contribute substantially to the degradation of renal function in patients suffering from diabetic nephropathy. Recently it has been shown that smoking alters renal hemodynamics and contributes to albuminuria. Smoking increases the risk of progression of renal failure in patients suffering from IgA nephropathy and polycystic kidney disease. Furthermore smoking has a deleterious effect on patients on hemodialysis and on the transplanted kidney. Nonetheless, it is important to realize that smoking not only is deleterious for the progression of vascular and pulmonary diseases, but also has a strong negative effect on kidney function.

Etudes coût-efficacité : ce que devraient retenir les médecins

Les ressources de notre système de santé sont limitées. Des choix dans l'attribution de ressources sont nécessaires pour assurer son équilibre. Une analyse coût-efficacité compare les effets d'une intervention de santé à une autre, en prenant en compte les coûts (y compris ceux économisés) et les années de vie gagnées, ajustées pour la qualité de vie (coût-utilité). Une analyse coût-efficacité devrait se placer dans la perspective de la société et l'intervention étudiée doit être comparée à une intervention actuelle pertinente. Les médecins, situés à l'interface entre les patients et les payeurs, sont dans une position idéale pour interpréter, voire effectuer des analyses coût-efficacité, et pour promouvoir les interventions les plus efficaces ayant des coûts raisonnables. Pédiatrie: nouveautés en médecine 2007 [Pediatrics]

Cette année, nous avons résumé les avances dans deux pathologies très fréquentes et deux plus rares : 1) la cryptorchidie est opérée plus précocement sans utiliser des hormones. Ce traitement vise à assurer la fertilité et à éviter le développement de tumeurs testiculaires. 2) Dans le traitement de la pharyngite streptococcique, les céphalosporines orales durant quatre à cinq jours, peuvent remplacer la pénicilline. 3) Le carvédilol (bêtabloquant), le lévosimendan (sensibilisateur du calcium) et le nésiritide (analogue du peptide natriurétique) permettent une approche neurohormonale à l'insuffisance cardiaque. 4) Les glucocorticoïdes administrés tôt et une prise en charge multidisciplinaire ralentissent le cours de la dystrophie musculaire de Duchenne. This article summarizes the medical progress achieved in 2 frequent and 2 rare pathologies: 1. Cryptorchidism should be operated around 12 months of age and hormonal treatment abandoned in order to maintain fertility and avoid development of testicular tumors. 2. For the treatment of streptococcal pharyngitis oral cephalosporins for 4 to 5 days are equivalent to a Penicillin treatment of 10 days. 3. Thanks to carvedilol (a beta-blocker agent), levosimendan (a calcium sensibiliser) and nesiritide (an analog to the natriuretic peptide) a new hormonal approach to cardiac failure is possible. 4. Molecular medicine, as a result of the human genome project, also modifies the field of dermatology. Bioinformatics and biotechnology revolutionize the daily clinical practice in dermatology. A change of paradigm occurs notably in infectious diseases.
