Contexte: Parmi les complications de toute chirurgie, on retrouve l'embolie pulmonaire, dont l'issue est potentiellement fatale. Peu de travaux cependant ont été effectués pour étudier les facteurs de risques associés à une embolie pulmonaire après une chirurgie hépato-biliaire en particulier. Objectif: Le but de notre travail est d'étudier et comparer aux rares résultats existants les facteurs de risque ainsi que les conséquences d'une embolie pulmonaire post- hépatectomie. Les données pourront éventuellement être utilisées pour cibler les patients à risque et améliorer la prévention. Résultats: Le diagnostic d'embolie pulmonaire a été posé chez 24 patients (8%) au CT-scan dans la période post-opératoire. Le délai moyen entre l'opération et le diagnostic était de 5 jours (1-15j). La comparaison des patients du groupe EP (n = 24) au groupe Non-EP (n = 272) montre un BMI médian plus élevé (27 vs 24 kg/m2, p=0.006), un taux plus élevé de résections majeures (71 vs 43%, p=0.01) ainsi qu'une durée opératoire plus longue (310 vs 260 min, p=0.001). La durée totale d'hospitalisation était également supérieure dans le groupe EP (22 vs 11j, p<0.05), de même que la durée de séjour aux soins intensifs (3 vs 1j, p<0.05). La mortalité globale à 90j post-opératoires était de 3.3%, la mortalité du groupe EP étant nulle. Conclusion: Les résultats de cette étude rétrospective ont démontré l'association des embolies pulmonaires post-hépatectomie avec l'obésité, les hépatectomies majeures, ainsi qu'une longue durée d'opération. L'incidence non négligeable des embolies prouve la nécessité d'une prophylaxie thrombo-embolique.

OBJECTIVE: To examine the relationship of early serum procalcitonin (PCT) levels with the severity of post-cardiac arrest syndrome (PCAS), long-term neurological recovery and the risk of early-onset infections in patients with coma after cardiac arrest (CA) treated with therapeutic hypothermia (TH). METHODS: A prospective cohort of adult comatose CA patients treated with TH (33°C, for 24h) admitted to the medical/surgical intensive care unit, Lausanne University Hospital, was studied. Serum PCT was measured early after CA, at two time-points (days 1 and 2). The SOFA score was used to quantify the severity of PCAS. Diagnosis of early-onset infections (within the first 7 days of ICU stay) was made after review of clinical, radiological and microbiological data. Neurological recovery at 3 months was assessed with Cerebral Performance Categories (CPC), and was dichotomized as favorable (CPC 1-2) vs. unfavorable (CPC 3-5). RESULTS: From December 2009 to April 2012, 100 patients (median age 64 [interquartile range 55-73] years, median time from collapse to ROSC 20 [11-30]min) were studied. Peak PCT correlated with SOFA score at day 1 (Spearman's R=0.44, p<0.0001) and was associated with neurological recovery at 3 months (peak PCT 1.08 [0.35-4.45]ng/ml in patients with CPC 1-2 vs. 3.07 [0.89-9.99] ng/ml in those with CPC 3-5, p=0.01). Peak PCT did not differ significantly between patients with early-onset vs. no infections (2.14 [0.49-6.74] vs. 1.53 [0.46-5.38]ng/ml, p=0.49). CONCLUSIONS: Early elevations of serum PCT levels correlate with the severity of PCAS and are associated with worse neurological recovery after CA and TH. In contrast, elevated serum PCT did not correlate with early-onset infections in this setting.

Post-thrombotic syndrome (PTS) is the most frequent chronic complication of deep vein thrombosis (DVT) with an estimated prevalence of 30-50%. PTS is a significant cause of disability, especially when complicated by venous ulcers. Therefore, PTS has important socio-economic consequences for both the patient and the health care system. Actually, the efficacy of PTS treatment is very limited; therefore, best treatment remains prevention. Compression therapy, particularly by graduated compression stockings (GCS) has a pivotal role in PTS prophylaxis. Aim of this article is to resume state of the art literature on this subject. Recommendations on PTS prevention have even been reported.

Hair follicles are spaced apart from one another at regular intervals through the skin. Although follicles are predominantly epidermal structures, classical tissue recombination experiments indicated that the underlying dermis defines their location during development. Although many molecules involved in hair follicle formation have been identified, the molecular interactions that determine the emergent property of pattern formation have remained elusive. We have used embryonic skin cultures to dissect signaling responses and patterning outcomes as the skin spatially organizes itself. We find that ectodysplasin receptor (Edar)-bone morphogenetic protein (BMP) signaling and transcriptional interactions are central to generation of the primary hair follicle pattern, with restriction of responsiveness, rather than localization of an inducing ligand, being the key driver in this process. The crux of this patterning mechanism is rapid Edar-positive feedback in the epidermis coupled with induction of dermal BMP4/7. The BMPs in turn repress epidermal Edar and hence follicle fate. Edar activation also induces connective tissue growth factor, an inhibitor of BMP signaling, allowing BMP action only at a distance from their site of synthesis. Consistent with this model, transgenic hyperactivation of Edar signaling leads to widespread overproduction of hair follicles. This Edar-BMP activation-inhibition mechanism appears to operate alongside a labile prepattern, suggesting that Edar-mediated stabilization of beta-catenin active foci is a key event in determining definitive follicle locations.

The drug discovery process has been deeply transformed recently by the use of computational ligand-based or structure-based methods, helping the lead compounds identification and optimization, and finally the delivery of new drug candidates more quickly and at lower cost. Structure-based computational methods for drug discovery mainly involve ligand-protein docking and rapid binding free energy estimation, both of which require force field parameterization for many drug candidates. Here, we present a fast force field generation tool, called SwissParam, able to generate, for arbitrary small organic molecule, topologies, and parameters based on the Merck molecular force field, but in a functional form that is compatible with the CHARMM force field. Output files can be used with CHARMM or GROMACS. The topologies and parameters generated by SwissParam are used by the docking software EADock2 and EADock DSS to describe the small molecules to be docked, whereas the protein is described by the CHARMM force field, and allow them to reach success rates ranging from 56 to 78%. We have also developed a rapid binding free energy estimation approach, using SwissParam for ligands and CHARMM22/27 for proteins, which requires only a short minimization to reproduce the experimental binding free energy of 214 ligand-protein complexes involving 62 different proteins, with a standard error of 2.0 kcal mol(-1), and a correlation coefficient of 0.74. Together, these results demonstrate the relevance of using SwissParam topologies and parameters to describe small organic molecules in computer-aided drug design applications, together with a CHARMM22/27 description of the target protein. Until recently, the hard X-ray, phase-sensitive imaging technique called grating interferometry was thought to provide information only in real space. However, by utilizing an alternative approach to data analysis we demonstrated that the angular resolved ultra-small angle X-ray scattering distribution can be retrieved from experimental data. Thus, reciprocal space information is accessible by grating interferometry in addition to real space. Naturally, the quality of the retrieved data strongly depends on the performance of the employed analysis procedure, which involves deconvolution of periodic and noisy data in this context. The aim of this article is to compare several deconvolution algorithms to retrieve the ultra-small angle X-ray scattering distribution in grating interferometry. We quantitatively compare the performance of three deconvolution procedures (i.e., Wiener, iterative Wiener and Lucy-Richardson) in case of realistically modeled, noisy and periodic input data. The simulations showed that the algorithm of Lucy-Richardson is the more reliable and more efficient as a function of the characteristics of the signals in the given context. The availability of a reliable data analysis procedure is essential for future developments in grating interferometry.

Les troubles de l'humeur sont très fréquents dans la période périnatale. Ils se manifestent sous forme d'une humeur dépressive « sub-clinique » voire sous la forme d'une dépression post-partum avérée. Cette « dépressivité » provoque des perturbations des comportements de parentage, des troubles de la relation à l'enfant et débouchent sur des difficultés émotionnelles, cognitives et comportementales de l'enfant. Plusieurs facteurs peuvent néanmoins tempérer cet enchaînement. L'étude présentée dans cet article évalue dans quelle mesure l'alliance familiale et la satisfaction conjugale modèrent le lien entre dépressivité et développement de l'enfant. Cinquante-sept familles ont participé à l'étude avec leur bébé de trois mois. La dépressivité maternelle a été évaluée par entretien et par questionnaire. L'alliance familiale a été évaluée dans le Jeu Trilogique de Lausanne. La satisfaction conjugale et les symptômes de l'enfant sont rapportés par questionnaires par la mère. Les résultats montrent que (i) le niveau de dépressivité dans notre population est conforme à celui rapporté dans la littérature, (ii) il y a un lien entre dépressivité et difficultés de l'enfant, (iii) ces liens sont effectivement modérés par la satisfaction conjugale, qui joue le rôle de facteur protecteur, et par l'alliance familiale, qui joue le rôle de facteur aggravant. Ces résultats montrent qu'il est indispensable de tenir compte du contexte relationnel dans lequel la mère évolue, afin de pouvoir comprendre sous quelles conditions des troubles de l'humeur maternels vont affecter le développement de l'enfant.
