Lorsqu'il est amené à témoigner de sa conversion, le membre d'une assemblée pentecôtiste insiste avant tout sur la dimension personnelle de son choix de vie et sur les manifestations divines qui l'ont encouragées et qui l'encouragent encore chaque jour dans cette voie. Avoir fait l'expérience personnelle de l'agir divin est un élément constitutif de tout récit de conversion. Cet article, basée sur des recherches doctorales menées auprès de deux assemblées pentecôtistes de l'Est de la France, cherche à mettre en évidence que cette relation personnelle et intime entre le fidèle et son créateur se construit dans le cadre communautaire : à travers l'harmonisation des récits de conversion et la structuration des réunions de prières assurant par les manifestations charismatiques la mise en présence de l'Esprit Saint au sein de la communauté des fidèles. C'est en effet sur une validation communautaire que reposent en partie les convictions individuelles. When he has to testify of his conversion, the member of a Pentecostal assembly insists above all on the personal dimension of his choice of life and on the divine intervention which has encouraged him and which will encourage him in his everyday life in this way. The personal experience of God's action is a constituent element of any narrative of conversion. This article, based on my PhD researches carried on with two Pentecostal assemblies located in the east of France, tries to highlight that this personal and intimate relation between the believer and his creator is built in the community framework : harmonization of the narratives of conversion, structuralization of the meetings of prayers and charismatic interventions of the Holy Spirit within the community of the believers. Individual convictions are partly based and validated by the community.

BACKGROUND: Although methicillin-susceptible Staphylococcus aureus (MSSA) native bone and joint infection (BJI) constitutes the more frequent clinical entity of BJI, prognostic studies mostly focused on methicillin-resistant S. aureus prosthetic joint infection. We aimed to assess the determinants of native MSSA BJI outcomes. METHODS: Retrospective cohort study (2001-2011) of patients admitted in a reference hospital centre for native MSSA BJI. Treatment failure determinants were assessed using Kaplan-Meier curves and binary logistic regression. RESULTS: Sixty-six patients (42 males [63.6%]; median age 61.2 years; interquartile range [IQR] 45.9-71.9) presented an acute (n = 38; 57.6%) or chronic (n = 28; 42.4%) native MSSA arthritis (n = 15; 22.7%), osteomyelitis (n = 19; 28.8%) or spondylodiscitis (n = 32; 48.5%), considered as "difficult-to-treat" in 61 cases (92.4%). All received a prolonged (27.1 weeks; IQR, 16.9-36.1) combined antimicrobial therapy, after surgical management in 37 cases (56.1%). Sixteen treatment failures (24.2%) were observed during a median follow-up period of 63.3 weeks (IQR, 44.7-103.1), including 13 persisting infections, 1 relapse after treatment disruption, and 2 super-infections. Independent determinants of treatment failure were the existence of a sinus tract (odds ratio [OR], 5.300; 95% confidence interval [CI], 1.166-24.103) and a prolonged delay to infectious disease specialist referral (OR, 1.134; 95% CI 1.013-1.271). CONCLUSIONS: The important treatment failure rate pinpointed the difficulty of cure encountered in complicated native MSSA BJI. An early infectious disease specialist referral is essential, especially in debilitated patients or in presence of sinus tract.

A partir du cas clinique d'une adolescente de 15 ans prise en charge à l'hôpital de jour, nous nous proposons d'observer certaines fonctions et modes d'expression d'une violence explosive. Repérée comme " violente " dès sa petite enfance, cette jeune fille a grandi dans un milieu peu contenant qui n'a cessé de reculer devant sa violence et qui n'a opposé que peu de limites à sa toute-puissance. Son admission à 1 'hôpital de jour fait suite à une situation familiale de crise et une recrudescence des passages à l'acte violents sur sa petite soeur. Les déterminants institutionnels ont alors suscité un redéploiement de la violence, avec des circuits complexes d'expression alternant entre l'auto et l'hétéroagressivité. A la fois répétition et repère iclentitaire, cette violence est également une forme de communication pour l'adolescente qui exprime ainsi divers indicibles, dont sa souffrance, et qui module les orientations de la prise en charge. Les réponses institutionnelles à la violence de cette adolescente, perçue comme extrêmement dangereuse et impulsive, ont la lourde tâche de s'inscrire dans une visée à la fois compréhensive (sur le plan psychopathologique) et répressive-éducative.

INTRODUCTION: Delirium is a highly prevalent disorder, with serious consequences for the hospitalised patient. Nevertheless, it remains under-diagnosed and under-treated. We developed evidence-based clinical practice guidelines (CPGs) focusing on prevention, screening, diagnosis, and treatment of delirium in a general hospital. This article presents the implementation process of these CPGs and a before-after study assessing their impact on healthcare professionals' knowledge and on clinical practice. METHODS: CPGs on delirium were first implemented in two wards (Neurology and Neurosurgery) of the Lausanne university hospital. Interactive one-hour educational sessions for small groups of nurses and physicians were organised. Participants received a summary of the guidelines and completed a multiple choice questionnaire, assessing putative changes in knowledge, before and three months after the educational session. Other indicators such as "diagnosis of delirium" reported in the discharge letters, and mean duration of patients' hospital stay before and after implementation were compared. RESULTS: Eighty percent of the nurses and physicians from the Neurology and Neurosurgery wards attended the educational sessions. Both nurses and physicians significantly improved their knowledge after the implementation (+9 percentage-points). Other indicators were not modified by the intervention. CONCLUSION: A single interactive intervention improved both nurses' and physicians' knowledge on delirium. Sustained and repeated interventions are probably needed to demonstrate changes in clinical practice.

The gap junction protein connexin37 (Cx37) plays an important role in cell-cell communication in the vasculature. Cx37 is expressed in endothelial cells, platelets and megakaryocytes. We have recently shown that Cx37 limits thrombus propensity by permitting intercellular signaling between aggregating platelets. Here, we have performed high throughput phage display to identify potential binding partners for the regulatory intracellular C-terminus of Cx37 (Cx37CT). We retrieved 2 consensus binding motifs for Cx37CT: WHK...[K,R]XP... and FH-K...[K,R]XXP.... Sequence alignment against the NCBI protein database indicated 66% homology of one the selected peptides with FVIII B-domain. We performed cross-linking reactions using BS3 and confirmed that an 11-mer peptide of the FVIII B-domain sequence linked to recombinant Cx37CT. In vitro binding of this peptide to Cx37CT was also confirmed by surface plasmon resonance. The dissociation constant of FVIII B-domain peptides to Cx37CT was ~20 uM. Other peptide sequences, designed upstream or downstream of the FVIII B-domain sequence, showed very low or no affinity for Cx37CT. Finally, in vivo studies revealed that thrombin generation in platelet-poor plasma from Cx37-/- mice (endogenous thrombin potential: 634±11 nM min, mean±SEM) was increased compared to Cx37+/+ mice (427±12, P<0.001). Moreover, partial activated thromboplastin time (aPTT) was shorter in Cx37-/- (39.7±1.5 s) than in Cx37+/+ mice (45.9±1.8, P=0.03), whereas prothrombin time was comparable. The shorter aPTT in Cx37-/- mice correlated with higher circulating FVIII activity (46.0±0.7 vs. 53.5±2.7 s for Cx37+/+, P=0.03). Overall, our data show for the first time a functional interaction between FVIII and Cx37. This interaction may be relevant for the control of FVIII secretion and, thereby, in the regulation of levels of FVIII circulating in blood. In addition, these results may open new perspectives to improve the efficiency of recombinant FVIII manufacturing.

OBJECTIVE: Binge eating disorder represents a significant public health problem, with up to 50% of weight loss program participants displaying this disorder. In previous studies with orlistat, patients with binge eating disorder were excluded. The goal of this study was to assess the efficacy of orlistat in obese patients with binge eating disorder. RESEARCH METHODS AND PROCEDURES: Eighty-nine patients with clinically diagnosed binge eating disorder and a BMI > or = 30 kg/m2 were randomized in double-blind fashion to 24 weeks of treatment with 120 mg of orlistat or placebo three times daily, in combination with a mildly reduced-calorie diet. RESULTS: After 24 weeks, the mean weight loss from baseline for orlistat-treated patients was significantly greater than for patients receiving placebo (-7.4% vs. -2.3%; p = 0.0001) (intent-to-treat analysis). The overall Eating Disorder Inventory 2 score at week 24 was significantly lower in patients treated with orlistat than in those in the placebo group (p = 0.011). DISCUSSION: Orlistat may be considered as part of the management for patients with obesity and binge eating disorder.

Le présent rapport rend compte d'une partie du travail déjà réalisé sur l'inventaire des données disponibles dans la littérature scientifique sur l'exposition des travailleurs agricoles aux pesticides en France. La possibilité de tirer parti de la littérature produite sur d'autres pays que la France est également explorée dans deux études de cas (réentrée en arboriculture et insecticides en élevages ovins). Les résultats seront exposés par ailleurs car le recueil et l'analyse de données produites par ces études sur les situations d'exposition des personnes travaillant en milieu agricole dans des pays étrangers posent des problèmes méthodologiques spécifiques si on veut pouvoir en tirer des enseignements pour la France. En matière de revue de la littérature scientifique, l'idéal de l'exhaustivité est généralement impossible à atteindre pour des raisons qui tiennent à la croissance exponentielle du nombre de publications scientifiques, à la pluralité des langues de publication, à la fragmentation et à l'accessibilité limitée des bases documentaires ... Nous pensons avoir recueilli tous les documents accessibles, mais peut-être certains nous ont-il échappé. C'est pourquoi le présent rapport est à la fois un exposé de résultats et un appel à contributions complémentaires. Il explicite les détails de la démarche adoptée (critères de sélection des articles, traçabilité des étapes de l'analyse ...) sous une forme qui permet à chacun d'avoir prise sur les résultats mais aussi de proposer des compléments au recensement réalisé. Il vise à être la première brique d'une base de connaissances partagée qui permette de capitaliser les données disponibles et qui puisse être mise à jour régulièrement.

La première partie de cet article vise à soutenir, à l'aide d'une perspective biologique, l'entreprise interdisciplinaire de la nouvelle sociologie décrite et défendue par Laurence Kaufmann et Laurent Cordonier dans leur article « Vers un naturalisme social » paru dans SociologieS en octobre 2011. La seconde partie est consacrée à la question de l'évolution de la morale ; l'idée d'un module de « morale naïve » est remise en question.

Dendritic cells (DCs) are the most potent antigen-presenting cells in the human lung and are now recognized as crucial initiators of immune responses in general. They are arranged as sentinels in a dense surveillance network inside and below the epithelium of the airways and alveoli, where thet are ideally situated to sample inhaled antigen. DCs are known to play a pivotal role in maintaining the balance between tolerance and active immune response in the respiratory system. It is no surprise that the lungs became a main focus of DC-related investigations as this organ provides a large interface for interactions of inhaled antigens with the human body. During recent years there has been a constantly growing body of lung DC-related publications that draw their data from in vitro models, animal models and human studies. This review focuses on the biology and functions of different DC populations in the lung and highlights the advantages and drawbacks of different models with which to study the role of lung DCs. Furthermore, we present a number of up-to-date visualization techniques to characterize DC-related cell interactions in vitro and/or in vivo.
