Cell therapy for nucleus pulposus (NP) regeneration is an attractive treatment for early disc degeneration as shown by studies using autologous NP cells or stem cells. Another potential source of cells is foetal cells. We investigated the feasibility of isolating foetal cells from human foetal spine tissues and assessed their chondrogenic potential in alginate bead cultures. Histology and immunohistochemistry of foetal tissues showed that the structure and the matrix composition (aggrecan, type I and II collagen) of foetal intervertebral disc (IVD) were similar to adult IVD. Isolated foetal cells were cultured in monolayer in basic media supplemented with 10% Fetal Bovine Serum (FBS) and from each foetal tissue donation, a cell bank of foetal spine cells at passage 2 was established and was composed of around 2000 vials of 5 million cells. Gene expression and immunohistochemistry of foetal spine cells cultured in alginate beads during 28 days showed that cells were able to produce aggrecan and type II collagen and very low level of type I and type X collagen, indicating chondrogenic differentiation. However variability in matrix synthesis was observed between donors. In conclusion, foetal cells could be isolated from human foetal spine tissues and since these cells showed chondrogenic potential, they could be a potential cell source for IVD regeneration.

Extracranial carotid aneurysm is a rare vascular manifestation of Behçet disease. To our knowledge, only 32 cases have been reported. This article presents a complex case of a 28-year-old man who was first treated by vein graft reconstruction. At 12 months of follow-up, a nonanastomotic false aneurysm of the vein graft occurred and was treated by interposition of prosthetic graft. Two months later, an anastomotic pseudoaneurysm between the two grafts was excluded by two stent grafts. Based on our experience and a review of the literature, we compared the outcomes of prosthetic and autologous vein reconstructions and discussed the role of carotid ligation and immunosuppressive treatment.

Objectives: Patients with autoimmune polyendocrinopathy-candidiasis-ectodermal dystrophy (APECED, APS-I) suffer from chronic candidosis caused mainly by Candida albicans, and repeated courses of azole antifungals have led to the development of resistance in the APECED patient population in Finland. The aim of our study was to address whether the patients are persistently colonized with the same or genetically closely related strains, whether epidemic strains are present and which molecular mechanisms account for azole resistance. Methods: Sets of C. albicans (n?=?19) isolates from nine APECED patients reported with decreased susceptibility to fluconazole isolated up to 9 years apart were included. The strains were typed by multilocus sequence typing. CDR1/2, MDR1 and ERG11 mRNA expression was analysed by northern blotting and Cdr1, Cdr2 and Mdr1 protein expression by western blotting, and TAC1 and ERG11 genes were sequenced. Results: All seven patients with multiple C. albicans isolates analysed were persistently colonized with the same or a genetically closely related strain for a mean of 5 years. All patients were colonized with different strains and no epidemic strains were found. The major molecular mechanisms behind the azole resistance were mutations in TAC1 contributing to overexpression of CDR1 and CDR2. Six new TAC1 mutations were found, one of which (N740S) is likely to be a gain-of-function mutation. Most isolates were found to have gained multiple TAC1 and ERG11 point mutations. Conclusions: Despite clinically successful treatment leading to relief of symptoms, colonization by C. albicans strains is persistent within APECED patients. Microevolution and point mutations occur within strains, leading to the development of azole-resistant isolates.

Résumé L'arrivée en force de l'imagerie numérique de bonne qualité à un prix abordable m'a fait réfléchir à la meilleure manière de l'intégrer dans la pratique courante de l'enseignement de la dermatologie, spécialité très visuelle. Comment mettre à profit la richesse des images et les nombreuses possibilités pédagogiques que l'informatique offre. J'ai étudié quelques produits existant sur le marché; je constate que les possibilités offertes par l'informatique restent souvent sous exploitées. Les réalisations manquent de liens hypertextes et la facilité d'accès aux images que permet l'informatique n'est pas appliquée. Les images sont trop souvent présentées avec une légende trop brève, ne soulignant pas les points importants pour le diagnostic. Certains outils ne proposent même pas de diagnostics différentiels. Ma réflexion me pousse à croire que l'apprentissage doit se faire par l'image. L'étudiant doit y apprendre les bases du diagnostic morphologique, trouver ce qui lui permet de poser le diagnostic. Compte tenu de mes observations, j'ai développé à Lausanne mon propre atlas interactif de diagnostics différentiels, basé sur la comparaison d'images. Mon projet n'a donc pas pour but de remplacer un livre ou un atlas, mais je souhaite compléter les moyens d'apprentissage basés sur l'image. Sa particularité tient dans la manière dont on a sélectionné les diagnostics différentiels; mon critère principal n'a pas été un choix théorique, mais la ressemblance entre deux images de ma bibliothèque. Cette manière de procéder m'a forcé à résoudre quelques questions fondamentales à propos des diagnostics différentiels. J'ai prêté une attention particulière à ce que l'utilisateur replace aisément les 850 images dans une structure que j'ai voulue claire. Cela m'a poussé à réfléchir sur la manière dont on aborde la dermatologie: par localisation, d'après les lésions, selon l'âge ou d'après des critères de physiopathologie ? Chaque image est accessible par la table des matières originale, soit par un module de recherche multicritère. Mon produit est personnalisable grâce à la présence de plusieurs outils. "Le Petit Rouvé", première version, est maintenant disponible pour une phase de test. Dans un second temps, l'atlas sera distribué aux étudiants de 4ème et 6ème année de la Faculté de médecine de Lausanne pour la rentrée de 2004-2005.
