During their lifetime, 20% of men will suffer from a fracture secondary to osteoporosis, and morbidity and mortality of a hip fracture in men are more severe than in women. Despite these facts, there are only few studies on osteoporosis in men. Hyopgonadism is a known risk factor for bone mineral density decrease. Hypogonadism can be found in patients diagnosed with prostate cancer who are receiving androgen deprivation therapy, but can also be discovered in patients with male infertility or erectile dysfunction. Urologists have central role in men's health aftercare, and therefore have key role in the screening and in the multidisciplinary treatment of osteoporosis and osteopenia.

KRAS mutation testing for predicting response to anti-EGFR therapy for colorectal carcinoma: proposal for an European quality assurance program.

Novel therapeutic agents targeting the epidermal growth factor receptor (EGFR) have improved outcomes for patients with colorectal carcinoma. However, these therapies are effective only in a subset of patients. Activating mutations in the KRAS gene are found in 30-40% of colorectal tumors and are associated with poor response to anti-EGFR therapies. Thus, KRAS mutation status can predict which patient may or may not benefit from anti-EGFR therapy. Although many diagnostic tools have been developed for KRAS mutation analysis, validated methods and standardized testing procedures are lacking. This poses a challenge for the optimal use of anti-EGFR therapies in the management of colorectal carcinoma. Here we review the molecular basis of EGFR-targeted therapies and the resistance to treatment conferred by KRAS mutations. We also present guideline recommendations and a proposal for a European quality assurance program to help ensure accuracy and proficiency in KRAS mutation testing across the European Union.

Les inflammasomes et les maladies humaines

The understanding of the innate immunity, the first line of the host defence, was significantly modified following the sequential discovery of innate immune receptors such as the Toll-like receptors (TLRs) and the NOD-like receptors (NLRs). In response to recognition of microbial patterns or danger signals, some NLRs assemble a multimolecular platform termed as the inflammasome. Inflammasome assembly leads to the activation of the proinflammatory caspase-1. Consequently, an inflammatory immune response is mounted along with a programmed cell death, called pyroptosis. This review summarizes recent advances in the knowledge of the inflammasome and its role in auto-inflammatory diseases, autoimmune diseases, and most common metabolic, cardiovascular or rheumatic diseases.

Adhésion thérapeutique et maladies chroniques: l'exemple de l'ostéoporose [Adherence and chronic disease: what's about osteoporosis].

Osteoporosis is a silent chronic disease. The human, medical and economic impacts, as well as the easy access to the screening and specific treatments incite us to introduce a treatment as soon as this one is justified. The maximal efficiency of treatments is possible only if the therapeutic adherence (which includes compliance and persistence) is good. Regrettably, as frequently in the chronic diseases, this one is often bad. It can be strengthened by a better patient's education and information, a better follow-up, but also by a today available variety of treatments.

Le dispositif mythico-rituel comme opérateur efficace (techniques magico-religieuses d'arrêt, figuration et résolution des états critiques : analyse de cas)

Dans une première partie, l'ouvrage propose des éclairages sur le corps à partir de ce qui met en question sa vision strictement rationnelle. Puis, trois auteurs évoquent les différentes manières dont la Bible, la philosophie et la littérature contemporaine mettent en scène les corps. Dans une troisième partie, sont abordées des questions plus spécifiquement reliées à la tradition catholique, au christianisme primitif et à la pratique de l'ascèse. Enfin, quatre contributions explorent le défi posé par la déréalisation du corps dans nos sociétés d'aujourd'hui, avec, pour clore l'ensemble, une réflexion sur le dualisme qui traverse le questionnement sur le corps.

Maladies vasculaires rares [Rare vascular diseases].

Scarce knowledge of vascular rare diseases, defined by prevalence lower than 1/2000, is accompanied by increased patients mis-management and impaired quality of life. Recent advances in clinical knowledge, molecular biology, and genetic evaluation of certain vascular rare diseases allows designing new management strategies. A tight coordinated collaboration between angiologists and other specialists is therefore necessary to optimize patient's care.

La place des registres dans la surveillance des maladies transmissibles. [The role of registries in the surveillance of transmissible diseases].

Registries are among the oldest methods used in public health for epidemiological surveillance and decision making in the area of communicable diseases. Although other sources of data are now available in many developed countries, registries still provide important information. This article reviews the main aims and characteristics of modern registries, providing several examples of current epidemiological problems. Practical advantages and disadvantages of registries are also discussed, as well as some developmental perspectives in this area.
