The non-invasive evaluation of myocardial ischemia is a priority in cardiology. The preferred initial non-invasive test is exercise ECG, because of its high accessibility and its low cost. Stress radionuclide myocardial perfusion imaging or stress echocardiography are now routinely performed, and new non-invasive techniques such as perfusion-MRI, dobutamine stress-MRI or 82rubidium perfusion PET have recently gained acceptance in clinical practice. In the same time, an increasing attention has been accorded to the concept of myocardial viability in the decisional processes in case of ischemic heart failure. In this indication, MRI with late enhancement after intravenous injection of gadolinium and 18F-FDG PET showed an excellent diagnostic accuracy. This article will present these new imaging modalities and their accepted indications.

Jusqu'à aujourd'hui la recherche en psychothérapie a essentiellement exploré quatre questions clés : 1) la psychothérapie est-elle efficace?; 2) quelle forme de psychothérapie est la plus efficace ? ; 3) quelle forme de psychothérapie est efficace pour quel patient? et 4) qu'est-ce qui permet à la psychothérapie d'être efficace? Si l'efficacité de la psychothérapie est désormais reconnue, la recherche empirique n'a jamais permis de désigner une approche psychothérapeutique comme étant la plus efficace de toutes. Les questions de l'indication différentielle des formes de psychothérapie selon les pathologies, et celles concernant les processus de la thérapie, qui permettraient d'en prédire le résultat, n'ont pas trouvé à ce jour de réponse définitive.

Internists must regularly adjust their patients care according to recent relevant publications. The chief residents from the Department of Internal Medicine of a university hospital present some major themes of internal medicine treated during the year 2009. Emphasis will be placed primarily on changes in the daily hospital practice induced by these recent studies. This variety of topics illustrates both the broad spectrum of the current internal medicine, and the many uncertainties associated with modern medical practice based on evidence.

Onset of rheumatoid arthritis in an elderly patient is common, in fact more common than in younger subject. If the reality of true differences in clinical presentation between younger and older subjects remained discussed, the central point remains that the prognosis is not better in older patients. Finally, conventional disease-modifying antirheumatic drugs (DMARD) and biological treatments are as efficient and safe than in younger patients, and the same strict management objectives used for younger subjects must be applied in the elderly.

The year 2011 was full of significant advances in all areas of medicine. Whether small or large issues, they all have an impact on daily practice in general internal medicine. For example, intravenous administration of diuretics in heart failure shows no benefit. But double dose may improve symptoms faster. Direct Xa inhibitors are emerging as alternative to anti-vitamin K. beta-blockers reduce overall mortality in COPD and do not worsen lung function significantly. Each year, the chief residents from the Department of internal medicine at the University hospital of Lausanne meet to share their readings. Twelve new therapeutic considerations of 2011 are reviewed here.

We know very little about the importance of history and physical examination compared to the importance of paraclinical tests in the diagnostic process in primary care. To answer this question, we examined prospectively 672 consecutive patients with chest pain in primary care. We recorded the timing and the clinical characteristics of the most frequent diagnosis. The resort to laboratory or other clinical tests and reference to specialist were influenced by: emergency consultation, potentially life-threatening aetiology, personal characteristics of the general practitioners' (GP) and patients' anxiety. GPs attributed the diagnosis to history and physical examination alone in 66% and to the association of history, physical examination and tests in 31% cases. This, clinical strategy remains the most important factor in the diagnostic process; even when they are insufficient, they allowed to generate hypotheses and guide investigations.

Plusieurs infirmières pratiquant dans les centres d'hébergement se questionnent sur la pertinence d'utiliser systématiquement certaines échelles de mesure. Elles sont d'avis que l'utilisation ou les modalités entourant l'usage de ces échelles augmentent surtout leur charge de travail et ne contribuent pas toujours à l'amélioration de la qualité des soins. Face à cette préoccupation, la communauté de pratique de la Faculté des sciences infirmières de l'Université Laval sur les soins à l'aîné en centre d'hébergement (CP-FSI) a voulu explorer les fondements à la base des recommandations de ces différents instruments de mesure en l'occurrence l'échelle du risque de plaie de pression de Braden et les échelles d'évaluation des risques de chute. Un examen de la littérature scientifique et l'analyse de données empiriques amènent à la conclusion que l'utilisation systématique de ces échelles alourdit en effet la tâche des infirmières. Qui plus est, le jugement clinique infirmier est équivalent ou supérieur à l'utilisation de ces échelles. La CP-FSI conclut à l'importance de modifier les recommandations sur l'usage systématique de ces échelles et de plutôt valoriser le jugement clinique des infirmières.
