Rapport de synthèseObjectif: Evaluer les données sociales, économiques et médicales concernant les enfants sans permis de séjour pris en charge à l'Hôpital de l'Enfance afin de pouvoir mieux comprendre leurs besoins spécifiques.Conclusions: La majorité des enfants ayant participé à l'étude sont originaires d'Amérique Latine et vivent dans des conditions de vie très précaires. Leur état de santé global est satisfaisant et la plupart bénéficient d'un suivi médical régulier. La prévention ciblée sur une meilleure hygiène de vie est particulièrement importante en raison de l'incidence élevée de sur poids et d'obésité dans cette population. Ce qui est connu et ce que l'étude apporte de nouveau: Cette étude est la 1ère qui analyse la situation socio-économique et l'état de santé d'enfants sans?papiers en Suisse. Les points forts de cette étude sont son caractère prospectif et le suivi de plus de la moitié de ces enfants à 1 an malgré une population particulièrement vulnérable et difficile à monitorer.Méthode: Etude exploratoire prospective par le biais d'un questionnaire incluant des données socio-démographiques, médicales et scolaires de 103 enfants sans permis de séjour ayant consulté pour la 1ère fois l'Hôpital de l'Enfance entre août 2003 et mars 2006. Ces enfants étaient ensuite reconvoqués pour une deuxième consultation 1 année plus tard afin d'obtenir un suivi médical.Résultats principaux: 87% des enfants sont originaires d'Amérique Latine, 36% ont moins de 2 ans. Cette population vit dans des conditions précaires avec un revenu familial sous le seuil de pauvreté (89% des familles vivent avec moins de 3100.- CHF/mois). Les raisons principales de consultation étaient des maladies infectieuses, un bilan de santé demandé par l'école ou un contrôle du nourrisson. La plupart des enfants étaient en bonne santé ou présentaient les mêmes pathologies retrouvées chez des enfants d'un âge similaire. 13% des enfants entre 2 et 16 ans sont obèses et 27% souffrent de surpoids. Tous les enfants en âge d'être scolarisés fréquentent l'école dans l'année suivant le 1er contrôle médical et 48% sont assurés auprès d'une assurance maladie.Biais: Au contrôle medical à 1 an, nous n'avions pas de donnée supplémentaire concernant 43% des enfants qui avaient été perdus de vue (courrier envoyé revenu en retour).

Applications of genetic constructs with multiple promoters, which are fused with reporter genes and simultaneous monitoring of various events in cells, have gained special attention in recent years. Lentiviral vectors, with their distinctive characteristics, have been considered to monitor the developmental changes of cells in vitro. In this study, we constructed a novel lentiviral vector (FUM-M), containing two germ cell-specific promoters (Stra8 and c-kit), fused with ZsGreen and DsRed2 reporter genes, and evaluated its efficiency in different cells following treatments with retinoic acid and DMSO. Several cell lines (P19, GC-1 spg and HEK293T) were transduced with this vector, and functional capabilities of the promoters were verified by flow cytometry and quantitative RT-PCR. Our results indicate that FUM-M shows dynamic behavior in the presence and absence of extrinsic factors. A correlation was also observed between the function of promoters, present in the lentiviral construct and the endogenous level of the Stra8 and c-kit mRNAs in the cells. In conclusion, we recommend this strategy, which needs further optimization of the constructs, as a beneficial and practical way to screen chemical inducers involved in cellular differentiation toward germ-like cells.

A questionnaire was developed by the members of WG12 of EURADOS in order to establish an overview of the current status of eye lens radiation dose monitoring in hospitals. The questionnaire was sent to medical physicists and radiation protection officers in hospitals across Europe. Specific topics were addressed in the questionnaire such as: knowledge of the proposed eye lens dose limit; monitoring and dosimetry issues; training and radiation protection measures. The results of the survey highlighted that the new eye lens dose limit can be exceeded in interventional radiology procedures and that eye lens protection is crucial. Personnel should be properly trained in how to use protective equipment in order to keep eye lens doses as low as reasonably achievable. Finally, the results also highlighted the need to improve the design of eye dosemeters in order to ensure satisfactory use by workers.

Background: Recent data have suggested that a population of CD4+ CD25high T cells, phenotypically characterized by the expression of CD45RO and CD127, is significantly expanded in stable liver and kidney transplant recipients and represents alloreactive T cells. We analyzed this putative new alloreactive cellular marker in various groups of kidney transplant recipients. Patients and methods: Flow cytometry was used to analyze the expression of CD25, CD45RO and CD127 on peripheral CD4+ T cells. Of 73 kidney recipients, 59 had a stable graft function under standard immunosuppressive therapy (IS), 5 had biopsy-proven chronic humoral rejection (CHR), 8 were stable under minimal IS and one was an operationally "tolerant" patient who had discontinued IS for more than 3 years. Sixty-six healthy subjects (HS) were studied as controls. Results: Overall, the alloreactive T cell population was found to be significantly increased in the 73 kidney recipients (mean ± SE: 15.03 ± 1.04% of CD4+ CD25high T cells) compared to HS (5.93 ± 0.39%) (p <0.001). In the 5 patients with CHR, this population was highly expanded (31.33 ± 4.16%), whereas it was comparable to HS in the 8 stable recipients receiving minimal IS (6.12 ± 0.86%), in 4 patients who had been switched to sirolimus (4.21 ± 0.53%) as well as in the unique "tolerant" recipient (4.69%). Intermediate levels (15.84 ± 0.93%) were found in the 55 recipients with stable graft function on standard CNI-based IS. Regulatory T cells, defined as CD4+ CD25high FoxP3+ CD127low, were found to be significantly reduced in all recipients except in those with minimal or no IS, and this reduction was particularly striking in recipients with CHR. Conclusion: After kidney transplantation, an alloreactive T cell population was found to be significantly expanded and it correlates with the clinical status of the recipients. Interestingly, in stable patients with minimal (or no) IS as well as in patients on sirolimus, alloreactive T cells were comparable the healthy controls. Measuring circulating CD4+ CD25high CD45RO+ CD127high T cells may become a useful monitoring tool after transplantation.

With an approach, based on chromosomal, biochemical and morphological analysis, we show that the animals wich are classically attributed to C. r. gueldenstaedti and/or to C. r. monacha belong in fact to the taxon C. suaveolens. The oriental forms, formely seen as C. russula, are of the same karyotype as C. suaveolens and are biochemically close to C. suaveolens in Central Europe. The origin of the taxonomical confusion might stem from the large morphological variation in and between populations of C. suaveolens in the Near East. Based on our results C. russula monacha Thomas, 1906 belongs to the polymorphic species C. suaveolens Pallas, 1811 and we propose to consider C. (russula) gueldenstaedti Pallas, 1881 as "incertae sedis".
