Background:The direct-acting platelet P2Y receptor antagonist ticagrelor can reduce the incidence of major adverse cardiovascular events when administered at hospital admission to patients with ST-segment elevation myocardial infarction (STEMI). Whether prehospital administration of ticagrelor can improve coronary reperfusion and the clinical outcome is unknown. Methods: We conducted an international, multicenter, randomized, double-blind study involving 1862 patients with ongoing STEMI of less than 6 hours' duration, comparing prehospital (in the ambulance) versus in-hospital (in the catheterization laboratory) treatment with ticagrelor. The coprimary end points were the proportion of patients who did not have a 70% or greater resolution of ST-segment elevation before percutaneous coronary intervention (PCI) and the proportion of patients who did not have Thrombolysis in Myocardial Infarction flow grade 3 in the infarct-related artery at initial angiography. Secondary end points included the rates of major adverse cardiovascular events and definite stent thrombosis at 30 days. Results: The median time from randomization to angiography was 48 minutes, and the median time difference between the two treatment strategies was 31 minutes. The two coprimary end points did not differ significantly between the prehospital and in-hospital groups. The absence of ST-segment elevation resolution of 70% or greater after PCI (a secondary end point) was reported for 42.5% and 47.5% of the patients, respectively. The rates of major adverse cardiovascular events did not differ significantly between the two study groups. The rates of definite stent thrombosis were lower in the prehospital group than in the in-hospital group (0% vs. 0.8% in the first 24 hours; 0.2% vs. 1.2% at 30 days). Rates of major bleeding events were low and virtually identical in the two groups, regardless of the bleeding definition used

Background: Vorapaxar is a new oral protease-activatedreceptor 1 (PAR-1) antagonist that inhibits thrombin-induced platelet activation. Methods: In this multinational, double-blind, randomized trial, we compared vorapaxar with placebo in 12,944 patients who had acute coronary syndromes without ST-segment elevation. The primary end point was a composite of death from cardiovascular causes, myocardial infarction, stroke, recurrent ischemia with rehospitalization, or urgent coronary revascularization. RESULTS: Follow-up in the trial was terminated early after a safety review. After a median follow-up of 502 days (interquartile range, 349 to 667), the primary end point occurred in 1031 of 6473 patients receiving vorapaxar versus 1102 of 6471 patients receiving placebo (KaplanMeier 2-year rate, 18.5% vs. 19.9%; hazard ratio, 0.92; 95% confidence interval [CI], 0.85 to 1.01; P = 0.07). A composite of death from cardiovascular causes, myocardial infarction, or stroke occurred in 822 patients in the vorapaxar group versus 910 in the placebo group (14.7% and 16.4%, respectively; hazard ratio, 0.89; 95% CI, 0.81 to 0.98; P = 0.02). Rates of moderate and severe bleeding were 7.2% in the vorapaxar group and 5.2% in the placebo group (hazard ratio, 1.35; 95% CI, 1.16 to 1.58; P<0.001). Intracranial hemorrhage rates were 1.1% and 0.2%, respectively (hazard ratio, 3.39; 95% CI, 1.78 to 6.45; P<0.001). Rates of nonhemorrhagic adverse events were similar in the two groups. Conclusions: In patients with acute coronary syndromes, the addition of vorapaxar to standard therapy did not significantly reduce the primary composite end point but significantly increased the risk of major bleeding, including intracranial hemorrhage. La hiperglicèmia elevada en la fase aguda de l’Infart Agut de Miocardi amb Elevació del segment ST, està estretament relacionada amb l’elevat risc de complicacions i mal pronòstic dels pacients, independentment que presentin Diabetis Mellitus prèviament coneguda. Hi ha evidències que en pacients que han rebut teràpia de reperfusió coronària, la hiperglicèmia major de 140mg/dL en el moment de l’ingrés està associada a esdeveniments tant perjudicials com és el desenvolupament d’una Diabetis Mellitus post Infart Agut de Miocardi. El control glicèmic a llarg termini s’associa a millores en els resultats cardiovasculars. L’objectiu d’aquesta revisió ésdeterminar la influència de la hiperglicèmia en les complicacions d’un malaltamb Infart Agut de Miocardi

Early repolarization, which is characterized by an elevation of the J-point on 12-lead electrocardiography, is a common finding that has been considered as benign for decades. However, in the last years, it has been related with vulnerability to idiopathic ventricular fibrillation and with cardiac mortality in the general population. Recently, 4 potential ECG predictors that could differentiate the benign from the malignant form of early repolarization have been suggested. Any previous study about early repolarization has been done in Spain. Aim. To ascertain whether the presence of early repolarization pattern in a resting electrocardiogram is associated with a major risk of cardiac death in a Spanish general population and to determine whether the presence of potential predictors of malignancy in a resting electrocardiogram increases the risk of cardiac mortality in patients with early repolarization pattern. Methods. We will analyse the presence of early repolarization and the occurrence of cardiac mortality in a retrospective cohort study of 4,279 participants aged 25 to 74 years in the province of Girona. This cohort has been followed during a mean of 9.8 years. Early repolarization will be stratified according to the degree of J-point elevation (≥0.1 mV or ≥0.2 mV), the morphology of the J-wave (slurring, notching or any of these two), the ST-segment pattern (ascending or descending) and the localization (inferior leads, lateral leads, or both). Association of early repolarization with cardiac death will be assessed by adjusted Cox-proportional hazards models

El nivell d’acompliment de les recomanacions de les GPC de la ESC (European Society of Cardiology) per la SCASEST a l’Hospital de Sant Pau als mesos d’octubre a desembre del 2010 és subòptim, similar al d’altres centres americans i europeus, i roman invariable respecte les dades de 2006-2007. El servei de Cardiologia destaca en les recomanacions d’hàbits i control de factors de risc cardiovascular. Existeixen diferències poc rellevants entre els serveis de Cardiologia i Medicina Interna en les recomanacions farmacològiques. No s’ha objectivat una relació significativa entre la qualitat de la informació dels documents assistencials i l’adherència a les GPC.

Little information is currently available from the various societies of cardiology on primary percutaneous coronary intervention (PCI) for acute myocardial infarction (AMI). Since primary PCI is the main method of reperfusion in AMI in many centres, and since of all cardiac emergencies AMI represents the most urgent situation for PCI, recommendations based on scientific evidence and expert experience would be useful for centres practising primary PCI, or those looking to establish a primary PCI programme. To this aim, a task force for primary PCI in AMI was formed to develop a set of recommendations to complement and assist clinical judgment. This paper represents the product of their recommendations.

En la pràctica diària hi ha síndromes coronàries amb elevació del segment ST que no compleixen de forma estricta els criteris diagnòstics de la definició universal de l'infart. Un 7,2% dels IAM de la nostra mostra presentaven una elevació del ST menor als criteris diagnòstics de l’IAM. El 70,4% d'aquests pacients no van rebre cap tractament de reperfusió en fase aguda. A la coronariografia el 73.4% dels pacients presentaven malaltia dels segments proximals o mitjos. Aquests IAM van presentar una baixa taxa de complicacions intrahospitalàries, però en el seguiment a 26,5 mesos destaca una important necessitat de noves revascularitzacions.

We investigate the role of earnings quality in determining the levels of segment disclosure, and whether and how better quality earnings and segment disclosure influences cost of capital. Using a large US sample for the period 2001-2006, we find a positive relation between earnings quality and levels of segment disclosures. We also find that firms providing better quality segment information, contingent upon good earnings quality, enjoy lower cost of capital. We base our empirical tests on a self created index of segment disclosure. Our results contribute to a better understanding of (1) the incentives for providing segment disclosures, and (2) how accounting quality (quality of segment information and earnings quality) is related to the cost of capital.

Registre per a estudiar l'impacte clínic de l'SCASEST a un hospital de tercer nivell. En un any van ingressar 310 pacients (54% Cardiologia, 46% Semicrítics/Unitat d'Estada Curta). La mortalitat hospitalaria va ser de l'1%; a 6 mesos va haver un 4,6% de morts i un 16% de reingressos. Edat, filtrat glomerular i FE van ser predictors de mortalitat; una troponina positiva i l'ingres a Cardiologia es van associar amb un menor risc de reingrés. Ingressar a Cardiologia es va associar a ser tributari de coronariografia i revascularització quirúrgica. El tipus de Servei pot tenir un impacte adicional en la història de l'SCASEST.

Aquest projecte presenta, per una banda, una revisió del Lean Management i de l'estat actual de la seva aplicació al sector sanitari. Es descriuen els principis i la filosofia en què es basa aquest sistema, les eines i tècniques que li són pròpies, així com les avantatges que aporta cadascuna d’elles a l'empresa en general i, més concretament, al sector sanitari. D'altra banda, també descriu una petita aplicació pràctica de la metodologia Lean per al control de fluxos de pacients al servei d’atenció ambulatòria de l’Hospital Sant Rafael de Barcelona, amb l’objectiu de disminuir el temps total que els pacients tarden en ser atesos pel metge, a través d’eines que alhora aporten valor afegit a tant a pacients com a professionals.
