Los datos existentes sobre el progresivo incremento de la infección con el virus de inmunodeficiencia humana (VIH) entre los adictos a las drogas por via parenteral (ADVP) y sus parejas e hijos, plantean la necesidad urgente de elaborar programas preventivos con el mayor grado de eficacia posible. En el presente trabajo nos proponemos tres objetivos: 1) Poner de manifiesto algunas insuficiencias observadas en los modelos deprevención que se aplican al caso del SIDA. 2) Conferir un énfasis especial a la influencia sobre los comportamientos preventivos frente al SIDA, de ciertos factores que, en general, no se tienen 10 bastante en cuenta en los modelos actuales como son: la magnitud del reforzamiento contingente a un determinado comportamiento y la demora con la que éste se recibe. 3) Exponer los resultados de una investigación realizada con drogadictos por via parenteral (Planes, 1991), cuyos objetivos eran conocer las relacionesexistentes entre la magnitud y la demora del reforzamiento contingente a los comportamientos sexuales preventivos y la frecuencia de dichos comportamientos

The developments in enteral feeding for Crohn's disease in the past decade are critically reviewed. The advent of amino acid based chemically defined elemental diets signalled the end of 'total bowel rest' in the management of these patients. Subsequently, controlled clinical trials showed that elemental diets were as effective as corticosteroids in inducing clinical remission in patients with acute exacerbations of Crohn's disease. The later use of peptide based elemental diets, in Crohn's disease produced somewhat conflicting results. The initial uncontrolled studies suggest that polymeric whole protein diets might also be effective in the management of acute exacerbations of the disease, casting in turn doubts concerning the role of dietary antigens in the pathogenesis of Crohn's disease. Results of controlled studies comparing the use of elemental and polymeric diets as primary therapy in Crohn's disease have, however, also produced conflicting results. It is well known that hospital malnutrition is a highly prevalent condition associated to increase morbidity and mortality as well as related healthcare costs. Although previous studies have already measured the prevalence and/or costs of hospital nutrition in our country, their local focus (at regional or even hospital level) make that the true prevalence and economic impact of hospital malnutrition for the National Health System remain unknown in Spain. The PREDyCES® (Prevalence of hospital malnutrition and associated costs in Spain) study was aimed to assess the prevalence of hospital malnutrition in Spain and to estimate related costs. Some aspects made this study unique: a) It was the first study in a representative sample of hospitals of Spain; b) different measures to assess hospital malnutrition (NRS2002, MNA as well as anthropometric and biochemical markers) where used both at admission and discharge and, c) the economic consequences of malnutrition where estimated using the perspective of the Spanish National Health System.

Resveratrol has been shown to have beneficial effects on diseases related to oxidant and/or inflammatory processes and extends the lifespan of simple organisms including rodents. The objective of the present study was to estimate the dietary intake of resveratrol and piceid (R&P) present in foods, and to identify the principal dietary sources of these compounds in the Spanish adult population. For this purpose, a food composition database (FCDB) of R&P in Spanish foods was compiled. The study included 40 685 subjects aged 3564 years from northern and southern regions of Spain who were included in the European Prospective Investigation into Cancer and Nutrition (EPIC)-Spain cohort. Usual food intake was assessed by personal interviews using a computerised version of a validated diet history method. An FCDB with 160 items was compiled. The estimated median and mean of R&P intake were 100 and 933 mg/d respectively. Approximately, 32% of the population did not consume RΠ The most abundant of the four stilbenes studied was trans-piceid (53·6 %), followed by trans-resveratrol (20·9 %), cis-piceid (19·3 %) and cis-resveratrol (6·2 %). The most important source of R&P was wines (98·4 %) and grape and grape juices (1·6 %), whereas peanuts, pistachios and berries contributed to less than 0·01 %. For this reason the pattern of intake of R&P was similar to the wine pattern. This is the first time that R&P intake has been estimated in a Mediterranean country.

Este trabajo se realiza con la idea fundamental de analizar la prescripción de la NP en un hospital universitario y más concretamente del servicio ofertado por la unidad de endocrinología y nutrición de dicho centro, mediante un seguimiento de los pacientes con algún tipo de intervención nutricional, en los que se haya recurrido a alguna de las modalidades de la NP, ya sea Nutrición parenteral total (NPT) o Nutrición parenteral periférica (NPP).

FUNDAMENTO: Determinar la prevalencia de la infección tuberculosa y por el VIH, así como los factores asociados, en la población de usuarios del programa de reducción de riesgos de la ciudad de Lleida. MÉTODOS: La muestra la formaron los nuevos usuarios del programa en el período abril-junio de 1996, entre los los cuales se realizó un cuestionario para la recogida de datos de las variables: edad, sexo, resultado de la prueba de la tuberculina, vacunación BCG, conocimiento de la serología frente al VIH, ingreso en prisión y años de consumo de heroína. Se calculó la prevalencia de la infección tuberculosa y por el VIH, con el intervalo de confianza (IC) del 95%. La asociación de ambas variables con el resto de variables del estudio se determinó mediante la odds ratio (OR) y su IC del 95% . RESULTADOS: Acudieron 150 pacientes diferentes, de los cuales 45 eran nuevos usuarios. De ellos, el 80,0% eran varones, con una edad media de 31,1 años. La prevalencia de la coinfección fue del 8,9% (IC 95% 2,8-22,1). La prevalencia de la infección tuberculosa fue de 27,3% (IC 95% 12,4-43,0), siendo superior en los que tenían antecedentes de ingreso en prisión (OR=3,4; IC 95% 0,5-27,4). La prevalencia de la infección por el VIH fue del 36,1% (IC 95% 21,3-53,8), siendo superior en los que tenían una antigüedad, en el consumo de heroína, superior a los 11 años ( OR = 7,3; IC 95% 1,0-65,9). CONCLUSIONES: El antecedente de ingreso en prisión es el principal factor de riesgo de la infección tuberculosa. Los años de consumo se asocian con la infección por el VIH, especialmente a partir de los 11 años. Los programas de reducción de riesgos de nuestro país deberían realizar actividades de control de la infección tuberculosa y por VIH.

Home enteral nutrition (HEN) is a type of enteral nutrition (EN) which is becoming progressively more widespread in pediatrics due to the benefits it affords to patients, their families and to reducing hospital costs. However, the true extent of its use is unknown in Spain as the data-base set up for this purpose is still underused (Registro de Nutrición Enteral Pediátrica Ambulatoria y Domiciliaria -NEPAD-). More thorough registration of patients in the NEPAD online register will provide information about the characteristics of HEN in Spain: prevalence, diagnosis, the population sector being administered HEN, complications and developments. Likewise, forecast and planning of the necessary resources could be made while those in use could be analysed.
