From the classical gold standard up to the current ERM2 arrangement of the European Union, target zones have been a widely used exchange regime in contemporary history. This paper presents a benchmark model that rationalizes the choice of target zones over the rest of regimes: the fixed rate, the free float and the managed float. It is shown that the monetary authority may gain efficiency by reducing volatility of both the exchange rate and the interest rate at the same time. Furthermore, the model is consistent with some known stylized facts in the empirical literature that previous models were not able to produce, namely, the positive relation between the exchange rate and the interest rate differential, the degree of non-linearity of the function linking the exchage rate to fundamentals and the shape of the exchange rate stochastic distribution.

Les lesions iatrogèniques de la via biliar són causades principalment durant una colecistectomia, una intervenció molt comuna a la nostra societat. Des de la introducció de la laparoscòpia a la dècada dels 80, aquesta tècnica ha esdevingut la tècnica gold standard. Però tot i els anys d’ experiència, existeix una notable problemàtica en relació a la iatrogènia. La seva incidència, la detecció, la clínica, els criteris diagnòstics, el maneig terapèutic, la evolució i el pronòstic a llarg termini, fan que sigui un tema controvertit, objecte d’estudi en aquest treball.

Bladder transitional cell carcinoma (TCC) is the most frequent malignancy of the urinary tract, and its incidence is rising. The gold standard treatment of invasive TCC is radical cystectomy with configuration of urinary diversion with bowel; the need for bowel has been universally considered the prime cause of complications. Since 1960’s urologists, scientists and industry have been trying, unsuccessfully, to obviate the use of bowel with alternative materials. We provide an analysis of problems deriving from using bowel as bladder substitute, a comprehensive review of literature on previous artificial models, an analysis of benefits deriving from the ideal prosthesis.

L’adaptació a l’entrenament aeròbic prolongat provoca a nivell cardíac modificacions estructurals i electrocardiogràfiques que es poden considerar parafisiològiques. Una planificació incorrecta de las càrregues de treball i factors externs estressants poden provocar una desadaptació a l’entrenament i de forma crònica en la Síndrome de Sobreentrenament. Aquesta síndrome es reflex d’un desequilibri del sistema autònom, podent presentar en les seves formes més desenvolupades manifestacions cardiovasculars greus. No es disposen actualment d’un marcador diagnòstic definitiu, sent la anamnesis dirigida el “gold standard. El tractament consisteix en un descans actiu la durada del qual dependrà de la gravetat clínica.

La manometría convencional es el “gold standard para diagnosticar trastornos motores del esófago, pero la información que da sobre la repercusión funcional de estas alteraciones es escasa. La manometría de alta resolución permite estudiar con detalle la motilidad esofagogástrica mediante la generación de mapas topográficos de presiones desde la orofaringe hasta el estómago. Hipótesis: Si la generación de mapas topográficos de presiones en el esófago mediante manometría de alta resolución demuestra la resistencia al flujo esofagogástrico. Objetivo: Comprobar si un test con sobrecarga de agua puede mostrar la presencia de resistencia al flujo esofagogástrico en pacientes con acalasia. Método: Estudiamos 2 grupos de pacientes con alteración de la motilidad esofágica. Un grupo de 8 pacientes que cumplen criterios manométricos de acalasia y, como grupo control, 8 pacientes con disfunción del peristaltismo esofágico ( DPE). A cada paciente se le realizó un test de sobrecarga que consistía en la ingesta rápida de 200 ml de agua mientras se registraban las presiones esofágicas. Resultados: Los pacientes con acalasia ingirieron el agua más lentamente que los pacientes con DPE (82± 14 seg vs 34± 6 seg, p&0,05). Mientras la unión gastroesofágica (UGE) permaneció contraída en el grupo de pacientes con acalasia (46,6± 6 mm Hg; p&0,05), permaneció relajada durante el tiempo del ingesta en pacientes con DPE ( 16,6 ± 4,7 mm Hg). La unión esófago-gástrica (UGE) experimentó una migración proximal en pacientes con acalasia de 1,3 ± 0 cm mientras que en el grupo control no migró (0 cm; p&0,05). La ingesta de agua se asoció a un incremento de la presión del esófago distal (2 cm por encima de la UGE) significativamente mayor en los pacientes con acalasia que en los pacientes con DPE (42,2 ± 20 vs 9,5± 7,9 mm Hg respectivamente, p&0,05) lo que produjo un incremento del gradiente de presión esófago-gástrico en pacientes con acalasia (16± 0,9 mmHg) que no se observó en los pacientes con DPE (0,1 ± 0,4 mmHg ; p&0,05). Conclusión: Un test con sobrecarga de agua durante la medición de la topografía y de las presiones esofágicas demuestra obstrucción al flujo esofagogástrico en los pacientes con acalasia. Este test podría contribuir a valorar la repercusión funcional en pacientes con acalasia y podría servir para el seguimiento de pacientes con acalasia tratados.

El objetivo del presente estudio es comparar la prueba de la tuberculina y los IGRAS (concretamente la prueba del Quantiferón) en pacientes candidatos a recibir tratamiento inmunodepresor. Se evalúan 139 pacientes y se obtienen un mayor número de resultados positivos para la prueba de la tuberculina. La conclusión inicial, por tanto, es que ésta es la prueba más sensible, teniendo en cuenta el sesgo provocado por la ausencia de una prueba “gold standard en la detección de infección tuberculosa latente. No se pueden obviar sin embargo los casos en que resultados negativos de la prueba de la tuberculina que fueron positivos en la prueba del Quantiferón , por lo que se concluye que ambas pruebas son útiles y necesarias en la detección de la infección tuberculosa latente en pacientes candidatos a inmunodepresión.

Purpose: To evaluate the suitability of an improved version of an automatic segmentation method based on geodesic active regions (GAR) for segmenting cerebral vasculature with aneurysms from 3D X-ray reconstruc-tion angiography (3DRA) and time of °ight magnetic resonance angiography (TOF-MRA) images available in the clinical routine.Methods: Three aspects of the GAR method have been improved: execution time, robustness to variability in imaging protocols and robustness to variability in image spatial resolutions. The improved GAR was retrospectively evaluated on images from patients containing intracranial aneurysms in the area of the Circle of Willis and imaged with two modalities: 3DRA and TOF-MRA. Images were obtained from two clinical centers, each using di®erent imaging equipment. Evaluation included qualitative and quantitative analyses ofthe segmentation results on 20 images from 10 patients. The gold standard was built from 660 cross-sections (33 per image) of vessels and aneurysms, manually measured by interventional neuroradiologists. GAR has also been compared to an interactive segmentation method: iso-intensity surface extraction (ISE). In addition, since patients had been imaged with the two modalities, we performed an inter-modality agreement analysis with respect to both the manual measurements and each of the two segmentation methods. Results: Both GAR and ISE di®ered from the gold standard within acceptable limits compared to the imaging resolution. GAR (ISE, respectively) had an average accuracy of 0.20 (0.24) mm for 3DRA and 0.27 (0.30) mm for TOF-MRA, and had a repeatability of 0.05 (0.20) mm. Compared to ISE, GAR had a lower qualitative error in the vessel region and a lower quantitative error in the aneurysm region. The repeatabilityof GAR was superior to manual measurements and ISE. The inter-modality agreement was similar between GAR and the manual measurements. Conclusions: The improved GAR method outperformed ISE qualitatively as well as quantitatively and is suitable for segmenting 3DRA and TOF-MRA images from clinical routine.

Background Accurate automatic segmentation of the caudate nucleus in magnetic resonance images (MRI) of the brain is of great interest in the analysis of developmental disorders. Segmentation methods based on a single atlas or on multiple atlases have been shown to suitably localize caudate structure. However, the atlas prior information may not represent the structure of interest correctly. It may therefore be useful to introduce a more flexible technique for accurate segmentations. Method We present Cau-dateCut: a new fully-automatic method of segmenting the caudate nucleus in MRI. CaudateCut combines an atlas-based segmentation strategy with the Graph Cut energy-minimization framework. We adapt the Graph Cut model to make it suitable for segmenting small, low-contrast structures, such as the caudate nucleus, by defining new energy function data and boundary potentials. In particular, we exploit information concerning the intensity and geometry, and we add supervised energies based on contextual brain structures. Furthermore, we reinforce boundary detection using a new multi-scale edgeness measure. Results We apply the novel CaudateCut method to the segmentation of the caudate nucleus to a new set of 39 pediatric attention-deficit/hyperactivity disorder (ADHD) patients and 40 control children, as well as to a public database of 18 subjects. We evaluate the quality of the segmentation using several volumetric and voxel by voxel measures. Our results show improved performance in terms of segmentation compared to state-of-the-art approaches, obtaining a mean overlap of 80.75%. Moreover, we present a quantitative volumetric analysis of caudate abnormalities in pediatric ADHD, the results of which show strong correlation with expert manual analysis. Conclusion CaudateCut generates segmentation results that are comparable to gold-standard segmentations and which are reliable in the analysis of differentiating neuroanatomical abnormalities between healthy controls and pediatric ADHD.

This study aimed to investigate the behaviour of two indicators of influenza activity in the area of Barcelona and to evaluate the usefulness of modelling them to improve the detection of influenza epidemics. DESIGN: Descriptive time series study using the number of deaths due to all causes registered by funeral services and reported cases of influenza-like illness. The study concentrated on five influenza seasons, from week 45 of 1988 to week 44 of 1993. The weekly number of deaths and cases of influenza-like illness registered were processed using identification of a time series ARIMA model. SETTING: Six large towns in the Barcelona province which have more than 60,000 inhabitants and funeral services in all of them. MAIN RESULTS: For mortality, the proposed model was an autoregressive one of order 2 (ARIMA (2,0,0)) and for morbidity it was one of order 3 (ARIMA (3,0,0)). Finally, the two time series were analysed together to facilitate the detection of possible implications between them. The joint study of the two series shows that the mortality series can be modelled separately from the reported morbidity series, but the morbidity series is influenced as much by the number of previous cases of influenza reported as by the previous mortality registered. CONCLUSIONS: The model based on general mortality is useful for detecting epidemic activity of influenza. However, because there is not an absolute gold standard that allows definition of the beginning of the epidemic, the final decision of when it is considered an epidemic and control measures recommended should be taken after evaluating all the indicators included in the influenza surveillance programme.

Objectives: Benign Oral Vascular Lesions (BOVLs) are a group of vascular diseases characterized by congenital, inflammatory or neoplastic vascular dilations clinically evidenced as more or less wide masses of commonly dark bluish color. If traumatized BOVLs are characterized by a great risk of hemorrhage and their treatment usually requires great caution to prevent massive bleeding. In the last decades lasers have dramatically changed the way of treatment of BOVLs permitting the application of even peculiar techniques that gave interesting advantages in their management reducing hemorrhage risks. The aim of this study was to evaluate the capabilities and disadvantages of three laser assisted techniques in the management of BOVLs. Study design: In this study 13 BOVLs were treated by three different laser techniques: the traditional excisional biopsy (EB), and two less invasive techniques, the transmucosal thermocoagulation (TMT) and the intralesional photocoagulation (ILP). Two different laser devices were adopted in the study: a KTP laser (DEKA, Florence, Italy, 532nm) and a GaAlAs laser (Laser Innovation, Castelgandolfo, Italy, 808nm) selected since their great effectiveness on hemoglobin. Results: In each case, lasers permitted safe treatments of BOVLs without hemorrhages, both during the intervention and in the post-operative period. The minimally invasive techniques (TMT and ILP) permitted even the safe resolution of big lesions without tissue loss. Conclusions: Laser devices confirm to be the gold standard in BOVLs treatment, permitting even the introduction of minimal invasive surgery principles and reducing the risks of hemorrhage typical of these neoplasms. As usual in laser surgery, it is necessary a clear knowledge of the devices and of the laser-tissue interaction to optimize the results reducing risks and disadvantages

Background: Although randomized clinical trials (RCTs) are considered the gold standard of evidence, their reporting is often suboptimal. Trial registries have the potential to contribute important methodologic information for critical appraisal of study results. Methods and Findings: The objective of the study was to evaluate the reporting of key methodologic study characteristics in trial registries. We identified a random sample (n = 265) of actively recruiting RCTs using the World Health Organization International Clinical Trials Registry Platform (ICTRP) search portal in 2008. We assessed the reporting of relevant domains from the Cochrane Collaboration’s ‘Risk of bias’ tool and other key methodological aspects. Our primary outcomes were the proportion of registry records with adequate reporting of random sequence generation, allocation concealment, blinding, and trial outcomes. Two reviewers independently assessed each record. Weighted overall proportions in the ICTRP search portal for adequate reporting of sequence generation, allocation concealment, blinding (including and excluding open label RCT) and primary outcomes were 5.7% (95% CI 3.0–8.4%), 1.4% (0–2.8%), 41% (35–47%), 8.4% (4.1–13%), and 66% (60–72%), respectively. The proportion of adequately reported RCTs was higher for registries that used specific methodological fields for describing methods of randomization and allocation concealment compared to registries that did not. Concerning other key methodological aspects, weighted overall proportions of RCTs with adequately reported items were as follows: eligibility criteria (81%), secondary outcomes (46%), harm (5%) follow-up duration (62%), description of the interventions (53%) and sample size calculation (1%). Conclusions: Trial registries currently contain limited methodologic information about registered RCTs. In order to permit adequate critical appraisal of trial results reported in journals and registries, trial registries should consider requesting details on key RCT methods to complement journal publications. Full protocols remain the most comprehensive source of methodologic information and should be made publicly available.

Background Maternal mortality is a major public-health problem in developing countries. Extreme differences in maternal mortality rates between developed and developing countries indicate that most of these deaths are preventable. Most information on the causes of maternal death in these areas is based on clinical records and verbal autopsies. Clinical diagnostic errors may play a significant role in this problem and might also have major implications for the evaluation of current estimations of causes of maternal death. Methods and Findings A retrospective analysis of clinico-pathologic correlation was carried out, using necropsy as the gold standard for diagnosis. All maternal autopsies (n ¼ 139) during the period from October 2002 to December 2004 at the Maputo Central Hospital, Mozambique were included and major diagnostic discrepancies were analyzed (i.e., those involving the cause of death). Major diagnostic errors were detected in 56 (40.3%) maternal deaths. A high rate of false negative diagnoses was observed for infectious diseases, which showed sensitivities under 50%: HIV/AIDS-related conditions (33.3%), pyogenic bronchopneumonia (35.3%), pyogenic meningitis (40.0%), and puerperal septicemia (50.0%). Eclampsia, was the main source of false positive diagnoses, showing a low predictive positive value (42.9%). Conclusions Clinico-pathological discrepancies may have a significant impact on maternal mortality in sub-Saharan Africa and question the validity of reports based on clinical data or verbal autopsies. Increasing clinical awareness of the impact of obstetric and nonobstetric infections with their inclusion in the differential diagnosis, together with a thorough evaluation of cases clinically thought to be eclampsia, could have a significant impact on the reduction of maternal mortality.

Colorectal cancer (CRC) is the third most common cancer and the fourth leading cause of cancer death worldwide. About 85% of the cases of CRC are known to have chromosomal instability, an allelic imbalance at several chromosomal loci, and chromosome amplification and translocation. The aim of this study is to determine the recurrent copy number variant (CNV) regions present in stage II of CRC through whole exome sequencing, a rapidly developing targeted next-generation sequencing (NGS) technology that provides an accurate alternative approach for accessing genomic variations. 42 normal-tumor paired samples were sequenced by Illumina Genome Analyzer. Data was analyzed with Varscan2 and segmentation was performed with R package R-GADA. Summary of the segments across all samples was performed and the result was overlapped with DEG data of the same samples from a previous study in the group1. Major and more recurrent segments of CNV were: gain of chromosome 7pq(13%), 13q(31%) and 20q(75%) and loss of 8p(25%), 17p(23%), and 18pq(27%). This results are coincident with the known literature of CNV in CRC or other cancers, but our methodology should be validated by array comparative genomic hybridisation (aCGH) profiling, which is currently the gold standard for genetic diagnosis of CNV.
