El diagnòstic precoç del càncer de pròstata fins a dia d’avui s’ha servit del tacte rectal, i els valors de PSA per establir quins pacients són sospitosos de patir aquesta afecció. Treballs recents estableixen que proves morfològiques com la ressonància magnètica, i funcionals com l’espectroscòpia ajudarien encara més a discriminar aquests pacients dels sans. En el nostre treball pretenem esbrinar; si l’ús de la ressonància magnètica amb espectroscòpia és igual d’eficient en el cas de que l’eventual càncer es localitzi a la glàndula central.

We have previously reported that actin filaments are involved in protein transport from the Golgi complex to the endoplasmic reticulum. Herein, we examined whether myosin motors or actin comets mediate this transport. To address this issue we have used, on one hand, a combination of specific inhibitors such as 2,3-butanedione monoxime (BDM) and 1-[5-isoquinoline sulfonyl]-2-methyl piperazine (ML7), which inhibit myosin and the phosphorylation of myosin II by the myosin light chain kinase, respectively; and a mutant of the nonmuscle myosin II regulatory light chain, which cannot be phosphorylated (MRLC2AA). On the other hand, actin comet tails were induced by the overexpression of phosphatidylinositol phosphate 5-kinase. Cells treated with BDM/ML7 or those that express the MRLC2AA mutant revealed a significant reduction in the brefeldin A (BFA)-induced fusion of Golgi enzymes with the endoplasmic reticulum (ER). This delay was not caused by an alteration in the formation of the BFA-induced tubules from the Golgi complex. In addition, the Shiga toxin fragment B transport from the Golgi complex to the ER was also altered. This impairment in the retrograde protein transport was not due to depletion of intracellular calcium stores or to the activation of Rho kinase. Neither the reassembly of the Golgi complex after BFA removal nor VSV-G transport from ER to the Golgi was altered in cells treated with BDM/ML7 or expressing MRLC2AA. Finally, transport carriers containing Shiga toxin did not move into the cytosol at the tips of comet tails of polymerizing actin. Collectively, the results indicate that 1) myosin motors move to transport carriers from the Golgi complex to the ER along actin filaments; 2) nonmuscle myosin II mediates in this process; and 3) actin comets are not involved in retrograde transport.

To evaluate the efficacy of endorectal Magnetic Resonance Imaging (MRI) and Magnetic Resonance Spetroscopic Imaging (MRSI) combined with total prostate-specific antigen (tPSA) and free prostate-specific antigen (fPSA) in selecting candidates for biopsy. Subjects and Methods: 246 patients with elevated tPSA (median: 7.81 ng/ml) underwent endorectal MRI and MRSI before Transrectal Ultrasound (TRUS) biopsy (10 peripheral + 2 central cores); patients with positive biopsies were treated with radical intention; those with negative biopsies were followed up and underwent MRSI before each additional biopsy if tPSA rose persistently. Mean follow-up: 27.6 months. We compared MRI, MRSI, tPSA, and fPSA with histopathology by sextant and determined the association between the Gleason score and MRI and MRSI. We determined the most accurate combination to detect prostate cancer (PCa) using receiver operating curves; we estimated the odds ratios (OR) and calculated sensitivity, specificity, and positive and negative predictive values. Results: No difference in tPSA was found between patients with and without PCa (p = 0.551). In the peripheral zone, the risk of PCa increased with MRSI grade; patients with high-grade MRSI had the greatest risk of PCa over time (OR = 328.6); the model including MRI, MRSI, tPSA, and fPSA was more accurate (Area under Curve: AUC = 95.7%) than MRI alone (AUC = 85.1%) or fPSA alone (AUC = 78.1%), but not than MRSI alone (94.5%). In the transitional zone, the model was less accurate (AUC = 84.4%). The association (p = 0.005) between MRSI and Gleason score was significant in both zones. Conclusions: MRSI is useful in patients with elevated tPSA. High-grade MRSI lesions call for repeated biopsies. Men with negative MRSI may forgo further biopsies because a significantly high Gleason lesion is very unlikely

Aquest treball té per objectiu mostrar que el joc constitueix un dels trets inherents de la cultura universal i, per extensió, de la tradició occidental. Naturalment, el joc ha estat objecte de diverses consideracions morals, lligades en més d’una ocasió a la religió. En alguns moments –i també des del cristianisme– s’han condemnat amb rigor i severitat les pràctiques lúdiques i, molt especialment, els jocs d’atzar i el món dels espectacles. De fet, la realitat històrica aquí analitzada –a través del cas concret del jeu de paume– confirma l’existència d’una activitat lúdica ben significativa que, malgrat totes les prevencions, va ser tolerada –i fins i tot aprovada– pels humanistes del Renaixement en acceptar la seva vàlua educativa. Sortosament, aquells savis humanistes comptaven amb l’eutrapèlia, una virtut d’arrel aristotèlica que va ser cristianitzada per sant Tomàs d’Aquino i que, com es veurà a continuació, també va arribar a casa nostra a la Baixa Edat Mitjana a través de Francesc Eiximenis. Si algú pensa que la gent d’abans no es divertia, comet un gran error, perquè el nostre passat històric és curull de jocs i activitats lúdiques. Nogensmenys, l’eutrapèlia també es perfila com una virtut social, tal com apareix en diferents autors de l’Edat Moderna, com ara Cervantes, Gracián i Feijoo.
