Background: The use of complementary and alternative medicine (CAM) and complementary and alternative therapies (CAT) during pregnancy is increasing. Scientific evidence for CAM and CAT in the field of obstetrics mainly covers pain relief in labor. Midwives are responsible for labor and delivery care: hence, their knowledge of CAM and CAT is important. The aims of this study are to describe the professional profile of midwives who provide care for natural childbirth in Catalan hospitals accredited as centers for normal birth, to assess midwives" level of training in CAT and their use of these therapies, and to identify specific resources for CAT in labor wards. Methods: A descriptive, cross-sectional, quantitative method was used to assess the level of training and use of CAT by midwives working at 28 hospitals in Catalonia, Spain, accredited as public normal birth centers. Results: Just under a third of midwives (30.4%) trained in CAT after completion of basic training. They trained in an average of 5.97 therapies (SD 3.56). The number of CAT in which the midwives were trained correlated negatively with age (r = - 0.284; p < 0.001) and with their time working at the hospital in years (r = - 0.136; p = 0.036). Midwives trained in CAT considered that the following therapies were useful or very useful for pain relief during labor and delivery: relaxation techniques (64.3%), hydrotherapy (84.8%) and the application of compresses to the perineum (75.9%). The availability of resources for providing CAT during normal birth care varied widely from center to center. Conclusions: Age may influence attitudes towards training. It is important to increase the number of midwives trained in CAM for pain relief during childbirth, in order to promote the use of CAT and ensure efficiency and safety. CAT resources at accredited hospitals providing normal childbirth care should also be standardized.

Para asesorar a las gestantes es necesario tener unos conocimientos adecuados al respecto. El objetivo de este estudio es conocer la oferta formativa de las matronas en TCA para la atención al parto mediante una revisión bibliográfica y la consulta de fuentes documentales. El actual programa de formación no las contempla y su aprendizaje depende de motivaciones personales. Existe poca información al respecto. Para recomendarlas y utilizarlas con eficacia y seguridad, es necesario dedicar más recursos a la investigación e incorporarlas en los planes convencionales de estudio.

Objetivo: Describir la situación actual de las Terapias Complementarias en la formación enfermera en las escuelas y facultades de enfermería en España. Método: estudio observacional descriptivo transversal. Población de estudio todas las facultades, escuelas públicas y adscritas de España. Recolección de datos mediante una ficha de observación. Unidades de análisis los Planes de Estudio. Variables (créditos, tipo de asignatura, ubicación, tipo de terapia). Análisis descriptivo de los datos relativos y absolutos mediante hoja de Excel. Resultados:en la mayoría de las facultades y escuelas escrutadas la asignatura de Terapias Complementarias ha desaparecido y en aquellas que aparece es una asignatura optativa.Conclusión: la formación en Terapias Complementarias en España es deficiente, debido a que no se recoge como asignatura troncal y/o obligatoria. La ausencia de las Terapias Complementarias en el currículum, plantea interrogantes tales cómo el valor de éstas en la formación, conceptualización que tienen los docentes, la repercusión en la calidad de los cuidados, la formación de los docentes encargados de impartir la asignatura, así como el número de créditos y el curso a impartir en la titulación del Grado.

The design and synthesis of two Janus-type heterocycles with the capacity to simultaneously recognize guanine and uracyl in G-U mismatched pairs through complementary hydrogen bond pairing is described. Both compounds were conveniently functionalized with a carboxylic function and efficiently attached to a tripeptide sequence by using solid-phase methodologies. Ligands based on the derivatization of such Janus compounds with a small aminoglycoside, neamine, and its guanidinylated analogue have been synthesized, and their interaction with Tau RNA has been investigated by using several biophysical techniques, including UV-monitored melting curves, fluorescence titration experiments, and 1H NMR. The overall results indicated that Janus-neamine/guanidinoneamine showed some preference for the +3 mutated RNA sequence associated with the development of some tauopathies, although preliminary NMR studies have not confirmed binding to G-U pairs. Moreover, a good correlation has been found between the RNA binding affinity of such Janus-containing ligands and their ability to stabilize this secondary structure upon complexation.

Objetivo general: Analizar la formación de las enfermeras y la aplicación de las terapias complementarias (TC) en los cuidados de enfermería al paciente oncológico. Ámbito de estudio: El estudio se realizó en el hospital Duran i Reynals de l"Hospitalet de Llobregat, en las unidades de: hematología clínica, terapia intensiva, tratamiento programado, hospital de día, oncología médica, cuidados paliativos, consultas externas, soporte de la atención continuada, servicio de oncología radioterápica y en el equipo de soporte hospitalario. Diseño: Estudio descriptivo transversal. Método: Aplicación de un cuestionario elaborado que consta de dos apartados, uno de datos demográficos y otro apartado con 8 preguntas especificas. Participantes: Enfermeras del área asistencial de todos los turnos que trabajen en las unidades mencionadas. Análisis de datos: Para el análisis de los datos se ha utilizado el paquete estadístico SPSS15.0 para Windows y se ha realizado un análisis descriptivo para todas las variables. Variables cualitativas: frecuencias y porcentajes. Variables cuantitativas: medidas de tendencia central y de dispersión. Resultados: El 58,8% de las enfermeras ha realizado algún tipo de formación en TC. Las intervenciones mente-cuerpo son las más efectuadas en cuanto a formación, seguidas de las terapias manuales y las terapias de base energética. La relajación-visualización es la terapia que más aplican las enfermeras oncológicas. Conclusiones: La formación de las enfermeras oncológicas en las TC es fundamental para poder informar y asesorar a sus pacientes y poder cuidarlos de una forma más holística. La falta de tiempo y de disposición del hospital al reconocimiento y valor de las TC son los principales factores de dificultad que se encuentran las enfermeras oncológicas.

Besley (1988) uses a scaling approach to model merit good arguments in commodity tax policy. In this paper, I question this approach on the grounds that it produces 'wrong' recommendations--taxation (subsidisation) of merit (demerit) goods--whenever the demand for the (de)merit good is inelastic. I propose an alternative approach that does not suffer from this deficiency, and derive the ensuing first and second best tax rules, as well as the marginal cost expressions to perform tax reform analysis.

The objective of this paper is to analyse the economic impacts of alternative water policies implemented in the Spanish production system. The methodology uses two versions of the input-output price model: a competitive formulation and a mark-up formulation. The input-output framework evaluates the impact of water policy measures on production prices, consumption prices, intermediate water demand and private welfare. Our results show that a tax on the water used by sectors considerably reduces the intermediate water demand, and increases the production and consumption prices. On the other hand, according to Jevons' paradox, an improvement in technical efficiency, which leads to a reduction in the water requirements of all sectors and an increase in water production, increases the amount of water consumed. The combination of a tax on water and improved technical efficiency takes the pressure off prices and significantly reduces intermediate water demand. JEL Classification: C67 ; D57 ; Q25. The spread of milk consumption was a significant change in the diet of Europeans, however it is one that has not been greatly studied with regard to the populations of Mediterranean Europe. In this article we shall analyse the ain circumstances that conditioned that process in Catalonia between the middle of the 19th century and 1936. In our study we shall argue that the consumption of milk in this area was only relevant in the 19th century in situations of illness or old age, and that it subsequently increased and acquired a new significance as a result of various factors. In particular, we shall emphasise: (a) the scientific advances in microbiology and nutrition, (b) the activities carried out by doctors and various public institutions to promote the consumption of fresh milk, and (c) the technological innovations in the milk producing sector. In Appendix 1 we show two maps representing the main territorial references that we shall mention.

The aim of the paper is to analyse the economic impact of alternative policies implemented on the energy activities of the Catalan production system. Specifically, we analyse the effects of a tax on intermediate energy uses, a reduction in the final production of energy, and a reduction in intermediate energy uses. The methodology involves two versions of the input-output price model: a competitive price formulation and a mark-up price formulation. The input-output price framework will make it possible to evaluate how the alternative measures modify production prices, consumption prices, private welfare, and intermediate energy uses. The empirical application is for the Catalan economy and uses economic data for the year 2001.

This paper presents an initial challenge to tackle the every so "tricky" points encountered when dealing with energy accounting, and thereafter illustrates how such a system of accounting can be used when assessing for the metabolic changes in societies. The paper is divided in four main sections. The first three, present a general discussion on the main issues encountered when conducting energy analyses. The last section, subsequently, combines this heuristic approach to the actual formalization of it, in quantitative terms, for the analysis of possible energy scenarios. Section one covers the broader issue of how to account for the relevant categories used when accounting for Joules of energy; emphasizing on the clear distinction between Primary Energy Sources (PES) (which are the physical exploited entities that are used to derive useable energy forms (energy carriers)) and Energy Carriers (EC) (the actual useful energy that is transmitted for the appropriate end uses within a society). Section two sheds light on the concept of Energy Return on Investment (EROI). Here, it is emphasized that, there must already be a certain amount of energy carriers available to be able to extract/exploit Primary Energy Sources to thereafter generate a net supply of energy carriers. It is pointed out that this current trend of intense energy supply has only been possible to the great use and dependence on fossil energy. Section three follows up on the discussion of EROI, indicating that a single numeric indicator such as an output/input ratio is not sufficient in assessing for the performance of energetic systems. Rather an integrated approach that incorporates (i) how big the net supply of Joules of EC can be, given an amount of extracted PES (the external constraints); (ii) how much EC needs to be invested to extract an amount of PES; and (iii) the power level that it takes for both processes to succeed, is underlined. Section four, ultimately, puts the theoretical concepts at play, assessing for how the metabolic performances of societies can be accounted for within this analytical framework.

Aquest projecte de MQD ha permès realitzar una sèrie d’experiències diverses i valuoses que han apuntat formes alternatives en la docència de la Biologia Cel·lular i l’aprenentatge de l'estudiant de la llicenciatura o del grau de medicina. En base a la valoració qualitativa del grau de satisfacció dels usuaris i en un estudi antitatiu de la seva opinió, considerem que ha estat experiències molt vàlides que afavoreixen que l’alumne participi de forma més activa en la seva formació. La principal dificultat observada ha estat o millor dit serà, l’aplicació de les experiències desenvolupades, de forma obligatòria i generalitzada, a tots els alumnes, sobretot als de 1er curs, donat que es tracta d’un grup molt nombrós (més de 300 alumnes). De totes maneres l’experiència adquirida ha estat molt favorable. Les activitats desenvolupades es continuen aplicant i es continuaran aplicant en el nou pla de medicina. Una aplicació perllongada en cursos successius afavorirà poder extreure’n conclusions de tipus més quantitatiu a més de les apreciacions qualitatives.
Mejora diagnóstica de hepatopatías de afectación difusa mediante técnicas de inteligencia artificial

The automatic diagnostic discrimination is an application of artificial intelligence techniques that can solve clinical cases based on imaging. Diffuse liver diseases are diseases of wide prominence in the population and insidious course, yet early in its progression. Early and effective diagnosis is necessary because many of these diseases progress to cirrhosis and liver cancer. The usual technique of choice for accurate diagnosis is liver biopsy, an invasive and not without incompatibilities one. It is proposed in this project an alternative non-invasive and free of contraindications method based on liver ultrasonography. The images are digitized and then analyzed using statistical techniques and analysis of texture. The results are validated from the pathology report. Finally, we apply artificial intelligence techniques as Fuzzy k-Means or Support Vector Machines and compare its significance to the analysis Statistics and the report of the clinician. The results show that this technique is significantly valid and a promising alternative as a noninvasive diagnostic chronic liver disease from diffuse involvement. Artificial Intelligence classifying techniques significantly improve the diagnosing discrimination compared to other statistics.
